Abstract

Introduction: Leprosy remains a significant problem in Indonesia as the number of cases has not been eliminated until now. One of the cytokines that plays a role in the disease is interleukin-10 (IL-10), which has a dominant anti-inflammatory effect and influences the immunopathogenesis of leprosy. Genetic factors, including the IL-10 gene promoter polymorphism, affect the amount of IL-10 produced, thereby increasing the risk of leprosy. Therefore, the objective of this study is to gather information on the role of IL-10 gene promoter polymorphism in the development of leprosy.Method: A systematic review method was used to identify, evaluate, and extensively discuss published works through national and international journals such as Google Scholar, Science Direct, Elsevier, EBSCO, Medline, PubMed, Proquest, and Wiley. The study was conducted by searching relevant keywords related to the research topic in these databases. Subsequently, article selection was performed based on predetermined inclusion and exclusion criteria. The selected articles were then evaluated for quality to ensure accuracy and reliability of the presented information. Finally, the results of this systematic review were used to obtain accurate and up-todate information regarding the role of IL-10 gene promoter polymorphism in leprosy.Results: This systematic review analyzed a total of 14 articles and revealed that results varied among study populations. However, there is consistent evidence to suggest that the IL-10 gene promoter polymorphism is associated with leprosy, which was found in the majority of the study populations.Conclusion: IL-10 gene promoter polymorphism tends to be associated with leprosy.

Abstract

Congenital heart disease (CHD) may affect 1% of all pregnancies per year. This means, out of 100 babies there will be 1 baby who will experience CHD.1 Moreover, the latest data from the Global Burden of Disease Study 2017 shows the prevalence reaches 1.6%-1.7% for the Southeast Asia region, including Indonesia.2 On the other hand, Indonesia has a growth rate The population is 1.39% with 4.2-4.8 million babies born per year. 3 Thus, it can be estimated that the number of patients with CHD born in Indonesia is 71,400 to 81,600 babies per year.

Abstract

Introduction: Osteoarthritis (OA) of the knee causes disability due to pain and will affect the functional ability to walk. Low-Level Laser Therapy (LLLT) has been shown to reduce pain in knee OA, while High Intensity Laser Therapy (HILT) is able to reach deeper joint areas. Method: This study aimed to compare the effect differences between LLLT and HILT on pain and functional capacity in knee OA. Methods: This is a double-blind randomized controlled trial with 61 subjects randomized into LLLT (n=31) and HILT (n=30) groups. All the subjects were knee OA patient with Visual Analog Scale (VAS) ≥ 4. The laser therapy and exercise were given 3 times per week for 6 sessions. The pain scale evaluated using VAS and functional ability evaluated using 50-feet walk test. Result: After 6 sessions, both LLLT and HILT group showed reduced VAS score [∆VAS Score LLLT = 3 (2 – 4), HILT = 3 (2 – 5)] and increased walking speed (∆ walking speed for LLLT = 0.23 (0.02 – 1.24) m/s, HILT = 0.22 (0.08 – 0.7) m/s) which were statistically (p less than 0.001) and clinically significant. HILT group had faster walking speed and greater VAS reduction compared to LLLT group (p less than 0.001), but there was no significant difference in walking speed between two groups (p=0.655). Conclusion: HILT and LLLT combined with exercise were effective in reducing pain and increasing functional ability in knee OA after 6 sessions of intervention. Pain and functional ability improvement was faster and greater in HILT group than LLLT group.

Abstract

Berdasarkan data dari Badan Pusat Statistik Tahun 2021, lebih dari 10% penduduk di Indonesia merupakan lansia atau berjumlah 29,3 Juta Jiwa. Jumlah tersebut diprediksi akan mengalami peningkatan. Diketahui sejak tahun 2000, persentase peningkatan populasi lansia di Indonesia melebih 7% sehingga Indonesia dikatakan masuk dalam negara kelompok berstruktur tua (aging populations). Diproyeksikan pada tahun 2045, satu per lima dari total populasi di Indonesia adalah lansia. Di Dunia pun pada tahun 2030 diprediksi akan ada 1 dari 6 orang yang merupakan lansia. Pada lansia, terjadi akumulasi kerusakan molekuler dan seluler yang dilanjutkan dengan penurunan fungsi organ sehingga terjadi peningkatan morbiditas dan mortalitas. Beberapa penyakit yang dialami lansia di antaranya adalah hipertensi, jantung, penyakit sendi, stroke, kanker, alzheimer, hingga penyakit langka seperti Myasthenia gravis (MG). Berdasarkan studi epidemiologi, diketahui terjadi peningkatan insiden Myasthenia gravis (MG) pada lansia.

Abstract

Introduction: Hyperthyroidism might lead to rhabdomyolysis. Rhabdomyolysis in thyroid crisis is very rare, currently there are only 7 cases in the world. This is the eight cases in the world that had been reported.Cases: We reported a case of a 46-year-old man with Graves’ Disease who presented with thyroid crisis and rhabdomyolysis.Discussion: The patient came with shortness of breath and palpitations for 10 hours before admission. Tachycardia, tachypnea, thyroid enlargement, motoric weakness, and bilateral lung crackles were noted. The Burch-Wartofsky Point Scale was 60 and the Japan Thyroid Association grade was TS2 first combination. Laboratory showed hyperkalemia (7.7 meq/L), increase in urea (144 mg/dl), creatinine (1.92 mg/dl), fT4 ( greater than 5.0), TSHs (0.06 uIU/ml) creatine kinase (3645 U/L), positive TRAb and Anti-TPO. The patient was treated with thyroid crisis management (propylthiouracil, lugol, dexamethasone) and supportive treatment (dobutamine, digoxin, furosemide, antibiotics, hyperkalemia therapy). After hospitalized for 11 days, the patient was discharged with resolution clinical symptom and levels of CK, urea, and creatinine. Conclusion: Hyperthyroidism might lead to rhabdomyolysis. This condition needs to be recognized and becomes a differential diagnosis in non-traumatic rhabdomyolysis accompanied by acute kidney injury. Therefore, can lead to appropriate and prompt management.

Abstract

Coronavirus disease 2019 (COVID-19) is one of the leading health problems globally, including in Indonesia. The first case of COVID-19 in Indonesia was discovered on March 2nd 2020. From then until August 7th 2022, 6.244.978 cases of COVID-19 and 157.095 COVID-19-related death have been recorded. A high virus transmission rate, various clinical manifestations, morbidity and mortality caused by COVID-19 alerts clinician to notice some factors which can make a person vulnerable to COVID-19 and which factors contribute to severe COVID-19. Indonesia’s COVID-19 case fatality rate (CFR) is higher than the global CFR. COVID-19 mortality in Indonesia mainly occurs in patients with comorbidity, of which the most common is diabetes mellitus.

Abstract

Rheumatoid arthritis (RA) is a systemic and chronic autoimmune disease with joint manifestations. Delays in RA treatment will pose a risk of permanent joint damage and increased mortality risk. Diagnosis as early as possible, followed by appropriate therapy, will prevent various complications due to RA, aiming to achieve a condition of remission or minimal low disease activity. The treatment choice for RA is currently a group of disease-modifying anti-rheumatic drugs (DMARDs), with methotrexate (MTX) as the first choice. Various studies have reported various factors that play a role in the success of MTX treatment, including younger age, male sex, normal body mass index, not smoking, early treatment, number of joints involved, inflammatory factors (LED/CRP) and negative serological factors (RF/anti-CCP).

Abstract

Thyroidectomy is one of the frequently performed surgery worldwide with indications of malignant or benign tumors, or functional abnormalities in thyroid gland. Surgery on the neck is actually a high-risk procedure because it is performed on structures attached to blood vessels, nerves and airways, so adequate perioperative management is warranted. This literature review aims to summarize the latest perioperative management of thyroid surgery. Preoperative management includes control of signs and symptoms of hyperthyroidism or hypothyroidism, fasting, administration of prophylactic antibiotic, and airway management. During surgery, special attentions are needed related the use of anesthetic drugs, hemodynamic monitoring, and efforts to prevent nerve injury. In addition, several postoperative complications that need to be detected and treated early, including hypocalcemia, hematoma, injury of recurrent laryngeal nerve, postoperative pain, nausea, and vomiting, and tracheomalacia. All those perioperative management requires multidisciplinary collaboration.

Abstract

Diabetes is a chronic disease which included in the 10 major leading causes of death globally. Pathogenesis of DM 2 involves complex mechanisms in various organs. Psychological conditions including stress, anxiety, and depression considered to be reciprocally related to impaired glucose regulation. On the other hand, hyperglycaemia in DM 2 may triggers some psychological disorder. Chronic psychological disorders activate the HPA axis, causing cortisol level elevation and disrupt its negative feedback mechanism. Other factors involved in disturbance of glucose regulation due to psychological distress were sympathetic nervous system, inflammatory and oxidative stress pathways, tryptophan metabolism and gut microbiota dysbiosis. These factors were interrelated to form a vicious cycle causing progression and complications in DM 2.
