Abstract

Background: COVID-19 patients are reported suffering from anxiety, depression, and stress which culminates in the form of insomnia, and becomes a typical comorbid in hospitalized patients. Sedative-hypnotic drugs usually used to treat insomnia may not provide a viable solution as it has inhibitory effects on the respiratory system. Previous research suggested that one-week Cognitive Behavioral Therapy improved sleep latency, night sleep time, and sleep efficiency of COVID-19 patients with insomnia. Case: A case of 53-year-old female patient with moderate symptoms of COVID-19 suffered from sleeping problems was diagnosed with insomnia due to adjustment disorder. She was given medication for three days and was scheduled for one hour of cognitive behavioral therapy intervention everyday. CBT intervention also included scheduling a regular daily activity and sleep hygiene programme. Result: There was no improvement from medication administered to help in relieving the insomnia symptoms. However, the patient was more able to sleep well on the sixth day of treatment, after five daily sessions of CBT, which was three days after stopping the medication. Conclusion: These results confirmed the effectiveness of daily CBT in improving the patient’s sleep quality and efficiency after one week, as suggested by previous studies.

Abstract

A peptide known as antimicrobial peptide (AMP) possesses antimicrobial effects against bacteria, fungus and viruses. AMP, also known as Host Defense Protein (HDP), is one of the body's defense mechanisms since it has the capacity to actively kill viruses. When it comes to respiratory cells, which are the major target of the SARS CoV-2 virus that produces COVID-19, cathelicidin, a kind of AMP in humans, is known to have protective properties. Cathelicidin AMP performance represents the first-line action of the natural immune system, with cathelicidin synthesis linked to appropriate vitamin D levels. Numerous earlier studies have discovered a connection between low cathelicidin levels and low vitamin D levels; this underlines the significance of vitamin D levels in strengthening the immune system. Naturally, particularly for the production of cathelicidin, which serves as the body's primary line of defense during the COVID-19 pandemic.

Abstract

Introduction: Biliary atresia is the most common cause of progressive liver disease in children. Studies have shown that biliary atresia causes impairment of cognitive and motor development in children. This study was conducted to establish a systematic review that examines the relationship between biliary atresia and developmental disorders in children.Methods: Literature searching from electronic databases (PubMed, Cochrane, and SCOPUS) and hand searching were performed. Inclusion and exclusion criteria were applied to the selected studies. The selected studies were assessed for the risk of biases of each study. The results of each study were summarized and compared in a summary table.Result: A total of nine relevant studies were obtained from a systematic search through the electronic databases and external resources using specific keywords. Seven of nine studies reported that biliary atresia affects both children’s cognitive and motor development.Conclusion: Children with biliary atresia are generally suspected of having impaired cognitive and motor development. The relationship between surgical intervention in children with biliary atresia with cognitive and motor development in children is still unclear. Further studies are needed to determine the cognitive and motor development of children with biliary atresia before and after surgical intervention.

Abstract

Intorduction: Congestive heart failure is a complex clinical syndrome that may result from disturbances in myocardial function (systolic and diastolic function) leading to decreased perfusion of cardiac blood flow. The main goals in treating patients with heart failure are to improve functional capacity and quality of life, as well as to reduce mortality and morbidity. Cardiac rehabilitation is one of many forms of treatment recommended by the American Heart Association and has been proven safe and effective in patients with heart failure. Case: This case report consists of three patients undergoing phase two cardiac rehabilitation. Case Discussion: Each patient underwent several examinations consisting of recollection of previous medical history and current complaints. Vital signs and Borg Scale were checked before and after the walking test. The 6-Minute Walk Test was carried out according to the standard protocol on a 30-meter flat track. Conclusion: From a series of 6-minute walking trials conducted on three patients, there was a significant increase in mileage since cardiac rehabilitation phase I, early phase II, and four weeks of phase II exercise.

Abstract

Carotid-cavernous fistula (CCF) is a vascular abnormality that occurs between the internal or external carotid artery and veins within the cavernous sinus. CCFs can be classified as direct (Barrow type A) or indirect CCFs (Barrow types B, C, and D). The causes of direct CCFs are trauma and vascular rupture, whereas indirect CCFs are commonly found in patients with hypertension, connective tissue disorders, pregnancy, and dissection of the internal carotid artery. Signs and symptoms of CCF vary widely, ranging from acute-onset headache, proptosis, diplopia, ophthalmoplegia, conjunctival chemosis, orbital bruit, and blindness, but it can solely manifest as conjunctival redness. Patients with suspected CCF should undergo neuroradiology imaging, followed by endovascular intervention to close the fistula and keep the carotid artery open. With appropriate treatment, complete resolution is expected to occur in all patients.

Abstract

Introduction : Metabolic syndrome (MetS) has emerged as one of the world’s major public health issues and the prevalence of this syndrome varies among population. There is a little of information on the epidemiology of MetS in male workers in Indonesia. Therefore, this study aims to determine the prevalence, its component and risk factors of MetS in Indonesian’s male working population. Methods : This was a cross sectional study of 1298 workers (median age, IQR 41,37-48 years) who underwent a routine Medical Check Up (MCU). The metabolic syndrome is diagnosed using the criteria established by the revised National Cholesterol Education Program-Adult Treatment Panel-III definition (2005 ATP III). Results : Metabolic syndrome prevalence was 13.9% in male workers. The most common abnormalities among all participants were hypertriglyceridemia (94.5%) and abdominal obesity (80.6%) [p less than 0.001]. Multivariable logistic regression analysis revealed that obesity (OR: 9.29; 95% CI: 5.56-15.54), overweight (OR: 2.15; 95% CI: 1.11-4.18), increasing age (OR: 2.39; 95% CI: 1.36-4.21), white blood cells/WBC or leukocyte (OR: 1.13; 95% CI: 1.04-1.24) and exercise (OR: 1.51; 95% CI: 1.06-2.15), were associated with a higher risk of developing MetS in the working population. Conclusion : The most common component of MetS in workers is hypertriglyceridemia followed by abdominal obesity. As a result, it is possible that these are the first detectable component of MetS in the working population. Early detection of MetS components, especially in obese workers, could be effective way to prevent the development of the syndrome.

Abstract

Introduction: The COVID-19 pandemic problem was followed by the development of an infodemic supported by advance technologies. This development has an impact on how quickly information spreads throughout society. Different types of COVID-19 information are very accessible to the public. However, some of the information received by the public contained hoaxes. The purpose of this research is to know the correlation between the level of knowledge COVID-19 vaccine hoax and people’s attitudes toward the vaccination program.Methods: This research was conducted online in Samarinda using a cross-sectional research design with voluntary response sampling. The minimal sample was 147 respondents, which was distributed through social media. This research using Spearman Rank correlation test, Mann-Whitney test, and Kruskal-Wallis test with 95% level of confidence.Result: The results showed there was a significant correlation between people’s attitudes about the vaccination program and their level of knowledge COVID-19 vaccine hoax (p less than 0,001). Furthermore, it was discovered that people who were greater than 35 years old (p = 0.011), had a recent education history at college (p less than 0.001), and were married (p = 0.001) were disagree with the COVID-19 vaccination program. There was no significant differences on gender (p = 0.076) and a positive history of COVID-19 (p = 0.702).Conclusion: There is a significant correlation between the level of knowledge COVID-19 vaccine hoax and people’s attitudes toward the vaccination program.

Abstract

The prevalence of obesity is increasing worldwide and is considered as epidemic. According to the 2018 Riskesdas data, the obesity rate in Indonesia has increased to 35.4% compared to 2007. Obesity is a risk factor for metabolic diseases due to chronic inflammatory conditions. This is associated with increased permeability of the intestinal wall which is characterized by increased levels of zonulin. Probiotics are known to reduce zonulin levels and improve intestinal integrity in obesity.

Abstract

Introduction: Globally, cases of pulmonary tuberculosis (PTB) are rising by 4%, while new cases have declined in many nations, most notably by 14% in Indonesia. During the COVID-19 pandemic, the number of undiagnosed and untreated PTB patients could rise and infect more people. In particular, CD8+ lymphocytes were decreased in COVID-19. Perhaps this is the cause of the rise in cases, but there is no data to show it. Aim: Determine current data about the PTB with COVID-19 coinfection and possible reactivation of PTB after COVID-19 infection in LTB patient. Methods: This systematic review is based on PRISMA statements using Pubmed, EBSCOHost, Science Direct and Cochrane from December 2019 to July 2022. Inclusion and exclusion criteria were set to select studies that included. We analyze risk of bias of all included studies with JBI critical appraisal tools. Results: This study had 107,425 patients in total from 20 studies. All studies were deemed to have low overall author bias risks. A critical component of PTB defense is CD8+. By reducing lymphocytes, COVID-19 can encourage the growth of PTB, which might therefore result in coinfection or the reactivation of existing PTB. Conclusion: COVID-19 possibly increase case of PTB in form of co-infection or reactivation of PTB in LTB patients. This needs special attention for patients that have infected by COVID-19 to followed up for risk developing PTB.

Abstract

Introduction: Rheumatoid arthritis (RA) is a rheumatic disease that often results in functional disability and decreased health-related quality of life (HRQoL). This study aims to evaluate the HRQoL and related factors in patients with RA.Method: This study enrolled 152 patients from Rheumatology polyclinic at RSUPN Dr. Cipto Mangunkusumo Jakarta. Sosiodemographic, clinical, and laboratory data related to disease activity, functional status, psychological problems, and number of comorbidities were collected. Disease activity was assessed with cumulative Disease Activity Score-28 with erythrocyte sedimentation rate (DAS28 ESR) for the last 6 months. HRQoL was assessed using the Indonesian EuroQol five-dimensional questionnaire (EQ-5D) and EQ global health visual analogue scale (VAS). Univariate analysis, bivariate and multivariate analysis were employed to identify factors related to HRQoL.Result: Of the enrolled participants, 90.8% were female with a mean age of 49.41 ± 12.31 years. Majority of subjects had moderate disease activity and an independent functional status. Median duration of illness was 3 (0 – 34) years. The study also identified psychological disorders such as anxiety (11.2%) and depression (20.4%). The median EQ-5D index value was 0.84 (0.170 – 1.000), and the median EQ VAS was 70 (40 – 100). Functional disability, disease activity, and depression were independently associated with both the index EQ-5D score and EQ VAS, while anxiety and number of comorbidities were only independently associated with EQ VAS.Conclusion: Functional disability, disease activity, psychological disorders and number of comorbidities negatively impact the HRQoL of RA patients. Hence, evaluation of these factors should be considered in the standard of care for RA patients.
