Abstract

Introduction. Cyanotic congenital heart disease (CHD) is an abnormality of the heart that causes blood with low oxygen content to circulate directly to the body.Objective. To evaluate the profile of cyanotic congenital heart disease at Dr. Soetomo General Hospital Surabaya.Methods. This is a descriptive study using medical records of cyanotic CHD patients aged ≤18 years old from June 2018 – May 2020. Gender, age, nutritional status, sign and symptom, type of cyanotic CHD, complication and treatment were taken as the variables.Result. Out of 116 patients in this study, the majority were male (58.62%). Most patients first diagnosed at the age of 0 – less than 1 year (76.72%) with normal nutritional status (51.72%). Tetralogy of Fallot (TOF) is the most common type (42.24%). Murmur was the most common sign and symptom (92.24%). Frequently observed complications were pneumonia (37.07%).Conclusion. Patients with cyanotic CHD are mostly male, diagnosed at the age of 0 – less than 1 year with normal nutritional status. TOF is the most common type. Most patients present with murmurs. Predominant complication is pneumonia.

Abstract

Introduction: At a certain level, excessive anxiety could lead to anxiety disorders and adversely affect daily life. Medical students had a higher prevalence of anxiety disorders than the general population. This anxiety might lead to negative behavioral tendencies and certain mental disorders, such as Compulsive Buying Disorder (CBD). This study aimed to determine whether there was an association between anxiety level and CBD among preclinical students of the Faculty of Medicine.Method: This research was an analytic observational study which was conducted using a cross-sectional study method on 107 preclinical students of Faculty of Medicine and Health Sciences, Atma Jaya Catholic University of Indonesia as respondents by filling out the Depression Anxiety Stress Scale (DASS-42) and Revised Edwards Compulsive Buying Scale (ECBS-R) questionnaire. The data was analyzed using the Chi-square test.Results: The results of statistical tests showed that there was a significant relationship between anxiety levels and CBD with p less than 0.001. The results showed that the prevalence of anxiety disorders in preclinical students was 33.6%, with the highest prevalence being at moderate anxiety levels (16.8%), and the prevalence of CBD was 25.2%.Conclusion : In conclusion, there was an association between the level of anxiety and CBD among preclinical students of the faculty of medicine.

Abstract

Introduction: Purpose, to evaluate patient demographic characteristics, indications and variables related to penetrating keratoplasty (PK) survival. Study design, this study was retrospective case series. Methods: We trace all medical records of PK patients from 2015 to 2018 in Cipto Mangunkusumo Hospital Jakarta. The traced variables were indications of surgery, corneal vascularization, previous failed graft, glaucoma after PK, and other complication. PK survival rate is shown in the Kaplan Meier curve using SPSS v20.0. Results: A total of 214 patients underwent PK (men 67.3%, woman 32,7%) with mean age of 42.11 (0-85) years were included in this study. Three most common indications of keratoplasty were corneal scar (32.7%), infectious corneal ulcer (25.5%), and failed graft (19.2%). The overall graft survival rate for PK is 61.7%. The mean graft survival time was 14.388 ± 0.580 months (95% CI; 13.252-15.524). Conclusion: The graft survival rate for penetrating optic keratoplasty was 61.7%. Establishing an appropriate early diagnosis, reasonable surgical procedures, close monitoring, and early detection of complications with proactive interventions lead to better outcomes. Further research is needed to determine the relationship between the risk factors for corneal graft failure.

Abstract

Introduction: The clinical manifestations of COVID-19 vary from asymptomatic to symptomatic. COVID-19 patients with severe symptoms often have coagulation disorders similar to systemic coagulopathy. This is associated with a significant increase in mortality. The purpose of this study was to determine the relationship between hypercoagulation and death in hospitalized patients with confirmed COVID-19.Method: This research is an analytic observational, quantitative approach with a cross sectional research design. The method of collecting secondary data comes from medical record data on inpatients who have confirmed COVID-19 at the Islamic Hospital in Jakarta Sukapura, by total sampling.Result: There are 625 research subjects. Most of the subjects were women (52.3%) at the age of 50-59 years (26.6%). The status of living patients was (98.9 %). The value of D dimer tends to increase, the value of PT is normal, and the value of APTT is normal. The Case Fatality Rate (CFR) in the subject was 0.0112% (CFR less than 4.35% = low CFR). Conclusion: There is a low CFR in hospitalized patients with confirmed COVID-19 at RSIJ Sukapura. The results of the examination on the parameters of coagulant agents based on patient status in hospitalized patients who were confirmed to be COVID-19 at RSIJ Sukapura, on the D-dimer and APTT parameters there was no significant relationship, while on the PT parameter there was a significant relationship.

Abstract

Introduction: Ankle sprain injury is an injury that often occurs in sports activities with an incidence up to 40%. The main management is by doing conservative management in the form of RICE and strengthening exercises. However, over time, many doctors and athletes use analgesics and anti-inflammatory drugs to treat the perception of pain and swelling. This article aims to find the latest evidence-based medicine on the use of analgesics and anti-inflammatory drugs relevant to ankle sprain injuries. Methods: The study was conducted by Evidence Base Medicine approach. Literature search from two digital databases; PubMed and Cochrane and selection based on Prism Flow Charts with inclusion and exclusion criteria. No statistical analysis was performed in this study. Results: The investigation was obtained six studies, consisting of 5 systematic review studies discussing the use of NSAIDs, Opioids, Acetaminophen and Cox-inhibitors in ankle sprain injury patients. The use of NSAIDs was less edible than the use of placebo and there was no significant difference between the use of NSAIDs and other analgesics. Topical use has a perceived better effectiveness than oral NSAID use. Conclusion: The use of analgesics in case of ankle sprain injury still needs further research; the main consideration is based on the patient’s pain perception. Conservative therapy remains the mainstay.

Abstract

Coronavirus disease (COVID-19) caused by infection of severe respiratory syndrome coronavirus 2 (SARS-CoV-2) was first detected in December 2019. Since then, many countries have started reporting a series of cases in children post COVID-19 infection, with various clinical manifestations, i.e: persistent fever, cardiovascular shock, and hyperinflammation similar with Kawasaki disease and/or toxic shock syndrome. The terminology Pediatric Inflammatory Multisystem Syndrome Temporarily Associated with SARS-CoV-2 (PIMS-TS) was given by the Royal College of Paediatrics and Child Health (RCPH), hence commonly known as Multisystem Inflammatory Syndrome in Children (MIS-C) as named by the Centers for Disease Control (CDC) and the World Health Organization (WHO). Multisystem Inflammatory Syndrome in Children has a very wide spectrum of clinical signs and symptoms, with the most common symptoms reported is persistant fever, along with gastrointestinal, cardiovascular and dermatological symptoms. Fever is reported in 97-100% of patients. Gastrointestinal symptoms are the second most common symptom occurring in 70% of patients. Until now, the pathogenesis underlying the occurrence of MIS-C is still unknown. Multisystem Inflammatory Syndrome in Children case management requires multidisciplinary knowledge. Consensus or standard guideline for the management of MIS-C is a work in progress, but in general, the initial management includes intravenous immunoglobulin (IVIG) and corticosteroids.

Abstract

Introduction: The role of proton pump inhibitor (PPI) in Coronavirus Disease-19 (COVID-19) is still debatable. PPI is commonly used to prevent or treat upper gastrointestinal bleeding and/or dyspeptic symptoms. However, previous studies showed that PPI may lead to adverse outcomes in COVID-19 patients, thus the dose of PPI may play an important role. This study aimed to compare the risk of mortality between high vs low dose PPI in hospitalized COVID-19 patients. Methods: We performed retrospective cohort study from two COVID-19 referral centers in Jakarta between June 2021 and September 2021. We included hospitalized COVID-19 patients, moderate-critically ill cases, who had been given intravenous PPI for more than 7 days. We defined high dose PPI for omeprazole greater than 40 mg/day or pantoprazole greater than 40 mg/day, and low dose PPI for omeprazole ≤40 mg/day or pantoprazole ≤40 mg/day. Results: Of the total 365 patients (median age[Q1–Q3] 55 [45–64] years old), 216 subjects were given high dose PPI. Subjects with high dose PPI had a significantly higher mortality rate than the low dose PPI according to bivariate analysis (54.2% vs 26.1%; p less than 0.001), but quite similar length of stay (median[Q1–Q3] 12.5 [9–16] vs 13 [9–18] days). After conducting a multivariate analysis to control the confounders, we found that high dose PPI still led to higher mortality (aOR 3.04; 95%CI 1.22–7.60; p=0.017). Conclusion: High dose PPI may increase risk of mortality in hospitalized COVID-19 patients.

Abstract

Sejak dinyatakan sebagai pandemi di Maret 2020, sampai dengan saat ini kita menghadapi ancaman gelombang ketiga wabah COVID-19 di Indonesia, adanya penyakit yang menyerang sistem respirasi ini mengakibatkan dampak fungsional yang cukup tinggi. Pada sistem respirasi yang terbesar menyebabkan disfungsi respirasi tipe restriksi sejak awitan masa infeksi dan bervariasi tergantung area yang terdampak, bahkan sampai saat pasca infeksi yang dikenal sebagai sindrom long COVID-19.

Abstract

Introduction: Rehabilitation program has been proven to be able to restore the cardiorespiratory function of Covid-19 survivors. Reciting Holy Qur’an are associated with a more relaxed and lowered heart rate of the reciter. The present study aims to compare the regular rehabilitation program with addition of Reciting Holy Qur’an on cardiorespiratory fitness among moslem Covid-19 survivors. Method: This quantitative experimental study involved 18 moslem patients who were assigned into two groups. Eight patients in the interventional group received additional Qur’an recitation every day at least 20 pages, divided into several sessions according to the participants’ ability and opportunity. Meanwhile, ten patients in the control group received regular rehabilitation programs with mild to moderate intensity. The cardiorespiratory fitness level was evaluated by a count test (CT), incentive spirometry test (IST), and a 6-minute walk test (6MWT). The measurement was performed three times: before intervention, the first week, and the second week after treatment. Result: The participants were eight males and ten females with an age average of 51.9 ± 9.5 years. Both groups showed significant differences before and after treatment in the first and second week in terms of CT, IST, and 6MWT (p less than 0.05). There was a significant difference between the two groups in the first week in the IST result (p less than 0.05), no significant difference in the second week was noticed. Conclusion: The Covid-19 survivors receiving additional Holy Qur’an recitation exhibit faster cardiorespiratory fitness improvement than the control group.

Abstract

Introduction: The Mallampati score and thyromental distance (TMD) are frequently used to identify challenging laryngoscopies, but their reliability in predicting difficulty is uncertain. This study aims to assess the effectiveness of using the ratio of height to thyromental distance (RHTMD) in predicting difficult visualization of the larynx (DVL) when compared to the Mallampati score and TMD.Method: To achieve this goal, 277 patients who received general anesthesia during elective surgery were evaluated using the Mallampati score, TMD, and RHTMD. The Cormack and Lehane (CL) classification was used to grade the laryngeal view, with CL grade 3 and 4 indicating difficult visualization. The study then determined and compared the area under the curve (AUC), sensitivity, and specificity for each airway predictor.Results: The AUC of RHTMD (85.5%) was better than TMD (82.7%) and significantly better than the Mallampati score (61.4%).Conclusion: RHTMD is more accurate in predicting difficult laryngoscopy than both the Mallampati score and TMD.
