Abstract

The concept and Philosophy of Functional Endoscopic Sinus Surgery (FESS) need to disseminate in the era of evidence-based medicine and technology development. Following lifelong learning, the previous indication of FESS as a surgical intervention of chronic rhinosinusitis not response to appropriate medical treatment, broaden into other indications of congenital anomaly, degenerative, infectious, inflammation, neoplasm, trauma even endocrine-underlying diseases. This literature review explains updated understanding from an immunological point of view related to FESS management for the integrity of the sinonasal barrier and further deeper and more specific literature reviews. Fourteen published manuscripts were selected based on FESS’s historical aspect, scientific philosophy, and applied life analogy for good deeds. Maintaining, preserving, and restoring the function of the nose and paranasal sinuses achieved through irrigation and hydration using physiologic saline solution and good sleep hygiene and quality. Surgical intervention of FESS in chronic mucosal inflammation may treat nasal dysbiosis by increasing the microbiome diversity and stability of Corynebacterium and Dolosigranulum. The competence of a skillful ENT Specialist and Surgeon in FESS needs a deeper understanding of anatomy, anatomy, and anatomy. Recommendation for Rhinology Consultant and Surgeon is adding interest and competence in immunology.

Abstract

Stress fractures are a common overused injury in both the general population, as well as the specialized population of athletes and military members.1,2 The incidence of stress fracture injuries is 21.1% in the athlete population, with higher rates in female athletes (9-13%) than male athletes (6.5%).3,4 Studies show 40% of athletes have experienced a stress fracture at some point in their career.3 Stress fracture injuries also account for 10% of orthopedic injuries and up to 20% of cases seen in sports medicine clinics. As many as 80-95% of stress fracture injuries occur in the lower extremities, with the commonest sites being the tibia, tarsal, metatarsal, and pelvis.3

Abstract

Introduction: Cardiac sarcoidosis (CS) is a rare granulomatous disorder in which white blood cells form clusters on the myocardium. The clinical presentations of CS are varied, as is its etiology. Multiple diagnostic approaches to determine the cause of persistent dyspnea may fail, as cardiac sarcoidosis can mimic many other diseases.Case Report: A 47-year-old woman presented with a sudden onset of dyspnea and a history of chronic thrombo-embolic pulmonary hypertension (CTEPH). Multiple diagnostic approaches were employed, including a CT scan of the thorax with contrast, CT pulmonary angiography, CT coronary angiography, and a Ventilation/Perfusion test, but the results were inconclusive. A Cardiac MRI was ultimately performed, which led to a diagnosis of cardiac sarcoidosis.Case Discussion: The patient had been stable and asymptomatic over the years, despite her history of CTEPH, until she developed a sudden onset of dyspnea. The presence of pitting edema, ECG results, and echocardiography data initially suggested a diagnosis of congestive heart failure. However, tests from the pulmonary system and cardiac coronary were normal. This highlights the elusive nature of cardiac sarcoidosis, which can often go unnoticed and be frequently misdiagnosed. Given the inconclusive results from previous tests, a Cardiac MRI was performed to further characterize the pathology. This led to the findings of Late Gadolinium Enhancement (LGE) and the diagnosis of CS.Conclusion: Cardiac sarcoidosis is a master imitator of many diseases, with patients sometimes presenting with only dyspnea. In such cases, Cardiac MRI plays a pivotal role as a diagnostic test, especially when results from other diagnostic workups are nonspecific.

Abstract

Introduction: The high incidence of death in patients with COVID-19 requires a risk stratification tool to assess the patient's condition and determine appropriate medical action. The 4C mortality score is a stratification tool that uses patient demographics, clinical observations and laboratory parameters to describe the patient's risk of death. Method: This study used a nested case-control method with secondary data on patients with confirmed COVID-19. The sample with 71 confirmed COVID-19 patients was categorized into groups of patients who died (cases) and patients who recovered (controls). Data analysis using the independent T test and bivariate using the chi-square method to see the relationship between the 4C mortality score and the incidence of patient death.Results: It was found that the 4C mortality score in the group of patients who died was higher compared to patients who recovered (mean = 8.20 vs 4.05). A significant relationship was found between the 4C mortality score and the incidence of death in COVID-19 patients (OR = 22.696; 95% CI = 2.82-182.9; p = 0.0034)Conclusion: It can be concluded that the 4C mortality score of medium and high COVID-19 patients is associated with the risk of patient death.

Abstract

Introduction: The Coronavirus Disease 2019 (COVID-19) pandemic has led to changes in the methods of teaching and assessing medical students, shifting from face-to-face to online interactions. Various modifications have been made to the Objective Structured Clinical Examination (OSCE) for achieving competency in preclinical students. However, the impact of these teaching and assessment methods during the pandemic has not been widely reported. This study aims to compare comprehensive OSCE scores before and during the COVID-19 pandemic in final-year preclinical medical students.Methods: This cross-sectional study involved data from final-year preclinical students at the Faculty of Medicine Universitas Islam Negeri Syarif Hidayatullah Jakarta (FMUINSHJ) who underwent OSCE in 2019, 2021, and 2022. The OSCE data included seven clinical competency areas. The data were then analyzed to compare the mean OSCE scores among the three student cohorts.Results: Among the three student cohorts (with respective sample sizes for 2019, 2021, and 2022: 82, 91, and 88), the highest mean score was obtained by students in the 2021 OSCE (score 63.75), while the lowest score was observed in the 2022 OSCE (score 60.20). Significant statistical differences were found among the seven competency areas in aspects such as history-taking ability, further diagnostic examination and data interpretation, diagnosis, and professionalism. Conclusion: There are differences in OSCE scores among final-year preclinical students before and during the COVID-19 pandemic. Among the seven competency areas, online learning and assessment can be applied to skills related to history-taking, communication, non-pharmacological management, while other areas require direct hands-on experience.

Abstract

Introduction: Military pilots require good cardiorespiratory fitness due to the need to perform tasks in difficult and varied environmental conditions. Cardiorespiratory fitness is usually expressed in terms of maximal oxygen consumption (V̇O2 max). It is important to know the factors associated with V̇O2 max in order to maintain the fitness of Indonesian Air Force pilots. This study was conducted to assess the association between body fat percentage with V̇O2 max of Indonesian Air Force pilots.Methods: A cross-sectional study was conducted on active Indonesian Air Force pilots who carried out periodic medical examinations from October to November 2022 at Lakespra dr. Saryanto, Jakarta. The research subjects underwent body composition examinations using Bioelectrical Impedance Analysis (BIA), and V̇O2 max assessment was conducted using the Bruce protocol. The association between body fat percentage and V̇O2 max value was assessed by Pearson’s correlation test.Results: Of the 64 subjects analysed, the average value of V̇O2 max was 44.6±6.2 ml/kg/min and the average body fat percentage was 23.4±5.0 %. There is a significant correlation between body fat percentage and V̇O2 max. (r=-0.334; p=0.007)Conclusion: There is a significant negative correlation with moderate strength between the body fat percentage and the V̇O2 max of Indonesian Air Force pilots who carried out medical examination at Lakespra dr. Saryanto.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a condition which airflow limitation occurs due to an abnormality of the airways. The cause of COPD is usually exposure to noxious particles or gases. COPD is often accompanied by metabolic syndrome and leads to impaired physical activity, leading to increased morbidity and mortality. According to the international recommendation, COPD patients should receive a 30-minute daily step program. Pulmonary rehabilitation program with daily step count could be prescribed to all COPD patients who have no exacerbations within 4 weeks and able to ambulate. The pedometer can be used for ≥ 8 hours/day, ≥ 3 days/week, and a minimum of ≥ 100 steps/day. Increasing activity that can be assessed by calculating daily steps in patients with COPD can reduce the number of exacerbations, reduces the risk of death, increases exercise capacity, and improves the quality of life.

Abstract

Besides their underlying diseases, patients in the Pediatric Intensive Care Unit (PICU), also have a high risk of developing secondary infections infections due to various procedures and the long length of stay. It has become a culture to give antibiotics to almost all patients who come to the PICU. The aim of this study is to assess the pattern and impact of the irrational use of antibiotics (both as empiric therapy, prophylaxis and definitive therapy), as well as to determine the role of biomarkers as a consideration for administering antibiotics to pediatric patients in the PICU. Irrational use of antibiotics in PICU still occurs all over the world, both as empiric therapy, prophylaxis and definitive therapy. This can have an impact on increasing the incidence of antibiotic resistance, extending the length of stay, increasing the economic burden on society, and worst of all, increasing the death rate due to infection. PCT and CRP, can help to differentiate the etiology, so they are useful guides for the initiation, de-escalation, or discontinuation of antibiotics. It is important to provide regular education to medical personnel regarding the protocol for administering antibiotics, including indications, types, doses and duration of administration. A written form can also be provided to assess the need for therapy using the Gyssens flowchart. In addition, cooperation from various parties is needed to carry out a comprehensive, sustainable management strategy to resolve this problem.

Abstract

Indonesia is an archipelagic country located in an area crossed by the equator and only knows the summer and rainy seasons. Between these seasons there is a transition season. Throughout the year Indonesia experiences the heat of the sun and on some islands the exposure to sunlight is greater. This can accelerate the process of skin aging or skin aging which is usually characterized by wrinkles, the appearance of black spots or lentigo, the skin becomes dry and other signs.

Abstract

In 2022, there will be 10.6 million cases of tuberculosis, which is a serious problem worldwide. Thailand's cohort research after the COVID-19 pandemic found a 7-fold risk of tuberculosis infection in COVID-19 patients, pointing to a probable rise in TB cases. In order to completely eradicate TB, care must be given to patients both before and after infection, guaranteeing public health against TB infection.1,2
