Abstract

Introduction: The COVID-19 pandemic is a disease outbreak caused by SARS-CoV-2; as of July 20, 2021, there were 2,950,058 positive cases and 76,200 deaths in Indonesia. COVID-19 patients with comorbidities are mostly hypertension (50%), diabetes mellitus (37.2%), and heart disease (16.7%). This study aims to prove how comorbidities affect the mortality of COVID-19 patients at Pertamina Jaya Hospital, Central Jakarta.Method: This study is a retrospective cohort analysis. The sample comprised 500 patients, 250 comorbid groups, and 250 without comorbidities. The secondary data source is medical records of COVID-19 patients at Pertamina Jaya Hospital from April 2020 to September 2021. Data analysis using univariate, bivariate, and multivariate.Result: Multivariate analysis with logistic regression resulted in a value of (32.197 95% CI 9.714 - 106.718) so that there is an influence on the risk of death with a comorbid history in COVID-19 patients. The relative risk value proves that hypertension has a 3-fold risk of death (3.124 95% CI 1.441 - 6.773), diabetes mellitus is 2.5 times (2.521 95% CI 1.055 - 6.024), while heart disease is 2.3 times (2.325 95% CI 1.169 - 4.622).Conclusion: Hypertension, diabetes mellitus, and heart disease are comorbidities that are predictors of mortality for COVID-19 patients due to a higher risk of death.

Abstract

Introduction: Schizophrenic patients with a bereaved history of close family members can develop depressive affect which is often misinterpreted as depressive episodes or even schizoaffective.Case: A 45-year-old woman was diagnosed as a paranoid schizophrenic (F20.02). During the last month, the patient also often felt tired and unenthusiastic. He thought it was only a matter of time before he could catch up with his parents who had died. The patient had no suicidal intentions and had never attempted suicide. There were two differential diagnoses in this patient, such as schizoaffective depressive type (F25.1) and major depressive episode with psychotic symptoms (F32.3).Discussion: The topic of conversation in this patient’s affective disorder was always related to the mood of mourning: the death of both parents. The response to a significant loss, such as bereavement, might very well resemble a depressive episode.Conclusion: Bereavement can be distinguished from depressive episodes by looking at some of the characteristic symptoms. The correct diagnosis aims to provide appropriate management so that it can help patients reach their best state.

Abstract

Introduction: There is an increasing number of suicides in Indonesia. A high risk of suicide is often associated with borderline personality disorder (BPD). To address this issue, dialectical behavior therapy (DBT) has been specifically designed for patients with frequent suicidal behavior and BPD. This study aims to develop DBT skills modules in Indonesian language for groups therapy and implement in clinical practice.Methods: This research was conducted in three stages. Stage 1 involved preparing the DBT skills module for 13 weeks, including the adding of TIPP (Temperature, Intense exercise, Paced Breathing, Paired Muscle Relaxation) distress tolerance skills and chain analysis. Stage 2 focused on module validation using face validity and content validity assessment conducted by two psychotherapy consultants. Stage 3 involved modules trials, both of whom had completed DBT training, practicing the prepared modules with two groups of 10 patients.Result: The face validity scores from the two experts for the DBT module on BPD in Indonesian was 3.188, indicating a high level of correctness. The content validity results from the two experts for the DBT module on BPD in Indonesian language were 84.06, indicating that almost everything was done correctly. After practicing DBT group skills training, two psychotherapists achieved a very good interrater score of 0.969 (p=0.003; 95% CI 0.746-0.997).Conclusion: Based on the face validity and content validity scores by the two experts, it was concluded that the DBT module for BPD in Indonesian language is suitable for use in clinical services.

Abstract

Cough is one of the innate primitive reflexes that is part of the immune system to protect the body from foreign substances. Cough is a manifestation of symptom found in various lung diseases, both infectious and non-infectious lung diseases. Cough variant asthma is a phenotype of asthma with the only symptom of cough. In cough variant asthma, airway (bronchial) hyperresponsiveness is found without abnormalities in pulmonary function tests. Airway hyperresponsiveness is assessed from the bronchial provocation test by measuring provocation dose (PD20) or provocation concentration (PC20). Management of cough variant asthma is no different from classic asthma, which are inhalation of bronchodilators and corticosteroids. Partial response to inhalation therapy can be reversed by changing the treatment regimen, increasing the inhaled dose, or adding other drugs such as long-acting β2 agonists, sustained-release theophylline or leukotriene receptor antagonist (LTRA).

Abstract

Recently, the cases of early menarche are increasing. Early menarche has a negative impact on a woman’s physical and mental health. It is necessary to study the latest theory about early menarche as a promotive and preventive effort for early menarche. This paper aims to present the latest findings on early menarche, including its causes, impacts, and management strategies. This article summarized the last five years of research presented in the PubMed database. The author used the keyword “causes early menarche”, and found 182 articles published from 2018 - 2023. Furthermore, the author used the keyword “effect early menarche”, and obtained 153 articles. From a total of 335 articles found, the authors conducted a screening and found 74 articles that were relevant to the topic of this paper. The entire contents of the article were reviewed, summarized and presented thematically. The results of cross-sectional studies, cohort studies, and systematic reviews confirmed that early menarche is when a young woman experiences her first menstruation at the age of less than 10 years. Some factors that cause early menarche are hormonal factors, body weight, environment, pollution, lifestyle, activity patterns, nutritional patterns and prenatal factors. The long-term effects of early menarche include cancer, melanoma, obesity, cardiovascular disease, high blood pressure, metabolic system diseases (such as diabetes), less physical activity, asthma, pain, menopausal complaints, and psychosocial effects such as anxiety and self-injury.

Abstract

Introduction: Nowadays, mental health literacy through internet videos, such as YouTube®, plays an important role for individuals experiencing depression. However, the majority of YouTube® videos related to depression are generally in English. This study aims to provide an overview of Indonesian-language YouTube® videos related to depression.Methods: The keyword “depression” is entered into the YouTube® search engine, and based on the highest number of views, the top 100 videos are selected for analysis regarding the characteristics of the videos and various information related to depression. This includes causes of depression, symptoms of depression, prevention/therapy of depression, recovery from depression, and sources of information about depression. Each characteristic will be compared based on the number of views, likes or dislikes, and number of comments. Results: Only 10% of Indonesian-language videos with the keyword “depression” are uploaded by professional channels, and 6.57% of the videos are sourced from professionals. Most videos do not provide information about prevention or therapy (57%) and the recovery from depression (78%). The majority of the videos mention that depression is caused by life stress (49%) with symptoms of suicidal thoughts (39%) and sadness (25%). However, only 16% of the videos state that depression can be treated or prevented with professional consultation. Conclusion: Depression literacy through Indonesian-language YouTube® videos is still lacking. There is a need for YouTube® videos that provide information about depression, come from professional sources, and include encouragement to seek professional help.

Abstract

“Senam Asma Indonesia” (SAI) is a series of medical knowledge-based structured physical exercises invented by a multidisciplinary team incorporated in the Indonesian Asthma Foundation (YAI) in 1994, taking into account the characteristics of asthma patients adjusted to the severity of the asthma diagnosis with different loads at each stage. The principles for treating asthma are avoidance of triggers, using medication, and maintaining physical fitness. The quality of life (QoL) of asthma patients decreases due to limited daily life activities caused by the respiratory symptoms they suffer. Multifactorial symptoms of asthma such as limited ventilation, gas transfer abnormalities, pulmonary and cardiac blood vessel dysfunction, and dysfunction of extremity muscles, will cause shortness of breath during physical activity or exercise, which is known as Exercise-induced Bronchoconstriction (EIB). Oral steroid therapy given to treat acute exacerbations can cause steroid-induced myopathy and skeletal muscle remodeling, resulting in decreased muscle endurance. SAI consists of a series of movements designed to improve lung function, strengthen respiratory muscles, and improve the quality of life of asthma patients. From a medical rehabilitation perspective, SAI can contribute to optimizing thoracic wall mobility, respiratory control with pursed-lip breathing, relaxation, and improving cardiorespiratory fitness.

Abstract

Introduction: Choledocholithiasis, the presence of stones in the common bile duct (CBD), is a challenge in the field of digestive surgery due to the wide range of cases with varying levels of difficulty. Until now, the options for diagnostic and therapeutic modalities have been increasingly evolving, ranging from non-invasive to invasive procedures. Therefore, the Indonesian Society of Digestive Surgeons provides recommendations for the management of CBD stone cases in Indonesia.Method: The consensus was developed using the Delphi survey method involving digestive surgery experts from various cities in Indonesia. Several issues related to the diagnosis and management of CBD stones were formulated and answered based on current research, while also considering the opinions of the experts.Result: This consensus consists of two parts: 15 recommendations related to the selection of diagnostic modalities and 10 recommendations related to management options. Recommendations regarding diagnosis include the use of ultrasonography, abdominal CT-scan, and magnetic resonance cholangiopancreatography (MRCP). In addition to standard therapeutic procedures, this consensus also provides recommendations regarding contemporary procedures such as endoscopic retrograde cholangiopancreatography (ERCP), laparoscopic common bile duct exploration (LCBDE), and open common bile duct exploration (OCBDE).Conclusion: These recommendations are aimed to assist digestive surgery experts in providing the best management for CBD stone cases.

Abstract

Stewardship atau dalam Bahasa Indonesia diterjemahkan sebagai penatalayanan. Kata tersebut seringkali digunakan dalam rangkaian kata antimicrobial stewardship atau dengan terjemahan yang disesuaikan, yaitu penatagunaan antimikroba. World Health Organization (WHO) telah menerbitkan pedoman diagnostic stewardship pada tahun 2016 sebagai pelengkap pedoman antimicrobial stewardship. Diagnostic stewardship diartikan sebagai suatu panduan untuk mengoptimalkan atau meningkatkan kesesuaian penggunaan pemeriksaan mikrobiologi sebagai acuan penentuan terapi. Peningkatan kesesuaian dalam diagnostic stewardship meliputi semua tahap proses diagnosis mikrobiologi klinis dan tata kelola laboratorium, yaitu tahap pra-analisis, analisis, dan pasca-analisis. Tahap pra-analisis meliputi pemilihan, penanganan, dan pengiriman spesimen ke laboratorium disertai data pasien yang penting untuk menentukan jenis pemeriksaan mikrobiologi dan interpretasi hasilnya. Tahap analisis meliputi penentuan kualitas spesimen dan proses pengujian, sedangkan tahap pasca-analisis meliputi pelaporan hasil, interpretasi sesuai kondisi pasien, dan kemudian hasil digunakan oleh klinisi untuk tata laksana pasien. Keberhasilan setiap tahap tersebut juga sangat tergantung pada kualitas serta efektivitas pemanfaatan sumber daya yang tersedia.1

Abstract

Introduction: The need for arterio-venous fistula (AVF) procedures continues to increase along with the rising incidence of chronic kidney disease. However, AVF maturation remains a challenge in clinical applications due to its susceptibility to various factors with complex mechanisms. Mesenchymal stem cells (MSCSs) have the potential to stimulate tissue regeneration, particularly in vascular injuries. This study aims to assess the effect of in situ and intravascular MSCSs administration on AVF maturation.Methods: This study employed an experimental design utilizing an animal model, specifically the Lepus domestica rabbit. Vascular maturation was assessed through parameters such as diameter, hyperplasia, and flow using Doppler Ultrasonography (USG) over a 14-days period. The production of stem cells was conducted using fluids and umbilical cord membranes. Statistically analysis involved one-way ANOVA and Kruskal Wallis test, followed by a post Hoc, with a confidence level of 95%.Results: A total of 28 Lepus domesticas were utilized, distributed across three groups: P1-negative control (n=9), P2-MSCSs in situ (n=9), and P3-MSCSs intravenous (n=10). Group P2 showed the widest vascular diameter (p less than 0.001) and the least formation of hyperplasia (p=0.014), with mean values of 4.5 mm and 1.2 mm, respectively. The P3 group demonstrated the fastest flow compared to the other groups (p=0.02), with an average flow of 144.6 mL/min.Conclusion: The administration of MSCSs in situ enhances vascular maturation following AVF procedures by increasing diameter size and reducing the formation of vascular hyperplasia.
