Abstract

Background: Uterine inversion is a rare obstetric emergency but potentially serious complication of labour. This disease characterized by severe pain and can cause life threatening condition. If these not immediately identified, the massive and underestimated blood loss can lead to hypovolemic shock. There for, early diagnosis and management of this condition is desirable.Objective: Report on the management of chronic uterine inversion due to myoma geburtMethods: Case reportCase: A 46 years old patient was admitted to the gynaecology ward of Dr. M. Djamil Central General hospital with slight bleeding from vagina since 1 month ago with 2-3 pieces of underware dark-red colored, with pain. Bleeding from the vagina often recurs. There was a mass revealed from vagina with size as big  as baby’s head. The mass  had pus and necrotic tissue.Conclusion: Uterine inversion is an serious obstetric complication due to life threaten of the patient. Its low incidence leads to scarce experience in solving this kind of situation. There are some risk factors or mechanism as an underlying etiology of uterine inversion. The authors concluded that the presence of myoma geburt can be assosiated with the chronic uterine inversion. Surgical approach by eksterpation of myoma geburt with  total hysterectomy can be the right procedure. Therefore, it is essential to keep in mind this diagnosis in all cases of vaginal bleeding. Abstract

The Risk Malignancy Index (RMI) is one of the simplest assessments that can assist in diagnosing and determining the prognosis of benign and malignant adnexa masses. Epithelial carcinoma is the most common type of about 90% of ovarian cancers.  As many as 35-40% of the epithelial type are serous and 6-10% are musinosum.This study aims to compare the picture of RMI value on the incidence of ovarian cancer serosum and musinosum type. This study was cross sectinal comparative study from medical records of ovarian cancer patients at obstetrics and gynecology section in DR M Djamil Hospital Padang from January 1st, 2017 until December 31st, 2017. The population was found one hundred and forty of patients with ovarian cancer and only one hundred and twenty nine of patients met the inclusion criteria and there were no exclusion criteria. Next RMI value is calculated based on RMI 1 formula, result is described in tabular form and data processing with SPSS program. Conclucion of this study is there were no differences in age distribution, ascites occurrence and age of menopause in serous and musinosum ovarian cancer. There is a difference in Ca, 125 levels in serous with musinosum ovarian cancer which also contribute to the high value of RMI. The mean value of patients‘s RMI in serous type ovarian cancer is higher than the mean value of RMI in patients with type Musinosum ovarian cancer. Abstract

Reported case in a woman aged 30 years, with a history of cesarean scar four years ago, diagnosed with gravid two by one gravid 7-8 weeks with gestation on cesarean scar. The patient had a history of accidental trauma two days prior to the complaint of bleeding from the genitals. Several obstetricians have examined but still obtained different results (intrauterine pregnancy and ectopic pregnancy). An accurate diagnosis is critical to the success of the management of a patient with a pregnancy with this life-threatening cesarean scar. With a precise and accurate transvaginal ultrasound examination, the diagnostic criteria with ultrasound on previous surgical scars have been fulfilled in this patient, including an empty uterus, an empty cervical canal image, growth of a gestational pouch in the anterior lower segment of the uterus, and the absence of myometral image between the bladder wall and the gestational pocket. On B-HCG examination, the result was 58,808.70 mlU / mL. Conservative therapy with metrotrexate (MTX) 50 mg / m2 intramuscularly. After giving metrotrexate (MTX) therapy for four times, the results of B-HCG decreased significantly with 21.530, 12 mlU / mL, 2,949.47 mlU / mL, and 593.61 mlU / mL, respectively. After administration of metrotrexate (MTX) therapy, there was a decrease in the size of the gestational bag. Keywords: Ectopic, Pregnancy, Methotrexate, Cesarean Section

Abstract

Objective : This article objective is to describe a woman with extra gastrointestinal stromal tumor (extra GIST) in ovary; Method : A case report and literature review; The author reports a woman 54 years old with complaints of swelling in the abdomen. Tumors suspected originate from ovary with an extension to abdominal wall. The patient had history of previous ovarian tumor surgery with pathological anatomy diagnosis was thecoma. Working diagnosis of the patient was residif ovarian malignancy and then suboptimal debulking was performed. Microscopically, ovarian tumors appear cellular and diffuse, partially arranged fascicles. Cells with rounded-spindle nuclei, mitosis ≥ 4 per 10 HPF. The conclusion was malignant thecoma which metastasizes to peritoneum and omentum. Differential diagnosis were GIST and leiomyosarcoma. Immunohistochemistry examination was performed with  Calretinin and CD117 to rule out the differential diagnosis. Calretinin were negative  and CD117 were strongly positively smeared. Based on the morphology and positive CD117 results, diagnosis was extra GIST of ovary.; Conclusion: Extra GIST in the ovary is an unusual location so that it can be misdiagnosed as a gynecological disorder. The differential diagnosis at this location is quite limited including thecoma, fibrothecoma and leiomyosarcoma.Keywords: extra gastrointestinal stromal tumor, ovary, thecoma

Abstract

Background: Hyperthyroid is a hypermetabolic condition caused by abnormal thyroid gland function resulting in overproduction and overexpression of thyroid hormone. The prevalence of hyperthyroid during pregnancy is 0.1-0.4%, where 85% of case are presented as grave’s disease. Objective: To report the treatment of uncontrolled hyperthyroid during pregnancy.Method: Case Report Case:  Ms. S, Female, 33 years old, presenting with brethlessness since 5 days before admission. Breathlessness persist and aggravated by lying down position. The patient has history of hyperthyroid since 1 years before admission. The blood pressure was 120/80 mmHg, respiration rate 28 times/min, and body temperature 36,7oC. Uterus fundal height 26 cm, cephalic presentation, fetal heart rate 130 times/min, single fetus intrauterine and alive. Laboratoric test for leukocyte: 21,300/ul, T4 level 22.8 mg/dl dan T3 level 2.9 mg/dl. The patient diagnosed with G3P2A0 31-week gestational age single alive fetus intrauterine with uncontrolled hyperthyroid and bilateral pleural effusion. Treatment consist of propylthiouracil as the drug of choice for anti-thyroidal drug, nifedipine for gestational hypertension and furosemide to treat the pleural effusion.  Conclusion: History taking, physical examination, thyroid function test, and maintaining euthyroidism during pregnancy is a key to reduce the risk of maternal and fetal complication.Keywords: hyperthyroid, pregnancy , IUGR

Abstract

Maternal cardiac arrest or maternal collaps is defined as an acute event involving the cardiorespiratory systems and/or brain, resulting in a reduced or absent consciousness level (and potentially death), at any stage in pregnancy and up to six weeks after delivery. Perimortem Cesarean Section (PCS) is performed either during maternal cardiac arrest or during impending maternal cardiac arrest toresuscitate mother and fetal. Current recommendations for maternal resuscitation include performance of the procedure following five minutes of unsuccessful cardiopulmonary resuscitation. The most common aetiology of maternal collaps was know as “4 H and  4 T” (Hypovolemia, Hypoxia, Hypo/Hyperkalemia,Hypothermia; Tromboembolism, Toxicity, Tension pneumothorax, Tamponade). Resuscitation in maternal cardiac arrest is mostly similar with non-pregnant patient resuscitation. There are several considerations need to be addressed in primary survey such as endotracheal tube 1 size smaller, supplemental O2 regardless of peripheral saturation, aggressive volume resuscitation, and uterine displacement to relieve compression of the IVC.Keywords: Maternal cardiac arrest; non-pregnant patient resuscitation

Abstract

Contraception is a very important issue in women with epilepsy, since very broad questions taken in applying it to prevent unplanned pregnancy. The choice of contraceptive methods must be related to how the mechanism of action of antiepileptic drugs (AEDs), the contraceptive itself, and both interactions. Hormonal contraception can induce metabolism of AEDs and lead to increasing of seizure frequency, and in the other hand AEDs may decrease the blood levels of hormonal contraception. Therefore, physician must consider the type of contraception being used before prescribing AEDs. Additionally, they should emphasize the interaction between them in counseling, to assure that women with epilepsy have time to decide which type will be best use for them. This challenging matter needs better recommendation for practice, both neurologist and gynecologist.Keywords: antiepileptic drug, contraception, women with epilepsy

Abstract

The purpose of this study is to find the correlation between leptin levels and the onset of preeclampsi. This study used a cross sectional comparative study design that conducted in May 2018 - April 2019 in the SMF / Obstetrics and Gynecology department of RSUP dr. M. Djamil Padang, RSUD Achmad Mochtar, RSUD Solok, RST Reksodiwiryo. We used consecutive sampling method which consists of 69 pregnant women who fulfill the inclusion and exclusion criteria. Leptin level tests were done using ELISA method. The average level of leptin in early-onset preeclampsia is found to be the highest when compared to the late-onset preeclampsia and normal pregnancy, 64.07 ± 78.27 vs. 30.46 ± 31.99 vs. 16.61 ± 24.49. This differentiation is highly significant with the ANOVA statistical test (p <0.05). There is a significant correlation between leptin levels with the onset of preeclampsia.Keywords: preeclampsia early onset, preeclampsia late onset, leptin levels

Abstract

Cervical cancer is an abnormal growth or cell changes in the cervical wall, which is the 4th rank in the most common cancer among female and the 2nd most common cancer in female aged fifteen until fourty four years old worldwide. Neovascularization becomes an important step in determining the onset and progressive cancer. Neoadjuvant chemotherapy has been evaluated as a treatment strategy in the stages IB2 and IIA2. The use of chemotherapy drug becomes more effective in adjacent and regular vascularization, and it allows the blood flow to the organs more quickly. This study aims to determine the correlation of tumor vascularization using Spectral Pulse Wave Doppler with neoadjuvant chemotherapy response in IB2 and IIA2 stages of cervical cancer. This research is a cohort analytic study which involves thirty six of patient in IB2 and IIA2 stages. The sampling is selected by using consecutive sampling technique. The data includes the result of ultrasound examination and were analyzed by using the Chi-square test. The results of the research on good and poor tumor vascularization had a good response to thirty of patient (83.3%) who were given neoadjuvant chemotherapy. There was no significant effect between tumor vascularization using Spectral Pulse Wave Doppler to the responses of neoadjuvant chemotherapy in IB2 and IIA2 stages of cervical cancer.Keywords: tumor vascularization using Spectral Pulse Wave Doppler, responses of neoadjuvant chemotherapy, IB2 and IIA2 stages of cervical cancer

Abstract

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasm of gastrointestinal tract that originate from Interstitial Cell of Cajal (ICC). The correct diagnosis of GISTs is important for therapeutic reasons of imatinib. Recently, DOG-1 has been introduced as an important diagnostic marker with high sensitivity and specificity. We report a caseof 40 years old woman with pain and swollen stomach in left lower quadrant since four month before come to the hospital. Ultrasound examination showed hipoechoic lession in the posterior of uterus and ascites. Paraovarian mass attached to the transverse colon and ascites was found on surgery. Histopathology diagnosis was a mesenchymal stromal tumour suggestive malignant GIST. Citological examination of asites fluid show a malignancy metastases. Immunohistochemistry showed negative for c-kit and positive for DOG-1, the conclusion is a GIST. Immunohistochemistry examinations are important to make defenitive diagnosis for GIST.  C-kit-negative GIST sare still rare, but defenitive diagnosis must be made because related to target therapy. DOG 1 has been proven in 89% of c-kit-negative GIST and claimed more sensitive and spesific.Keywords: GIST, c-kit, DOG-1, ICC, paraovary
