Abstract

Objectives: to analyze the relationship between menopause and depression in PadangMethods : This type of research is an analytical observational with a comparative cross sectional approach that looks at the relationship between menopause and depression incidence. The research was conducted in the working area of the Padang City Health Center from April to June 2020. The instrument used in this study was the Hamilton Depression Rating Scale (HDRS / Ham-D) which was used to measure the level of depression and a questionnaire to ask questions related to the characteristics of the respondents.Results: A study was conducted on 170 people consisting of 85 respondents who had menopause and 85 respondents who had not yet menopause. Conclusion: As many as 50 (58.8%) menopausal respondents experienced depression and 35 (41.2%) respondents did not experience depression. There is a relationship between menopause and depression (p <0.05). There was a relationship between age, current medical history, income, occupation, and education level with the incidence of depression (p <0.05). There is no relationship between menopause duration, marital status, and BMI with depression incidence (p> 0.05)Keywords: depression, Hamilton Depression Rating Scale , menoupause

Abstract

Background: The term tanatophorik comes from the Greek word thanatophorus which means "innate death" or "bearing death". The problem that underlies this disease is the process of bone formation. This disease is associated with an autosomal dominant inherited mutation of the fibroblast growth factor 3 receptor (FGFR3) gene on the arm of chromosome 4 (4p16.3). Because FGFR3 is the main modulator in bone formation, the typical clinical features of this disease include shortening of the extremities, curved femur, clover-like skull and narrowing of the thoracic cavity.Tanatophoric dysplasia is a skeletal disorder that is "lethal" or deadly. The deaths occurred due to respiratory failure caused by reduced chest cavity capacity, hypoplastic lungs and / or brainstem compression.Destination: Reported a case of thanatophoric dysplasiaMethod: Case Report Case Report: Case 33 years old woman, with preterm parturient G1P0A0H0 35-36 weeks 1 latent phase + history of 2x laparotomy + suspected fetal tanatophoric dysplasia. On ultrasound examination, it was found that BPD = 9.14 cm; AC = 30.56 cm; HC = 32.05 cm; FL = 2.55 cm; AFI; 9.06cm; SDAU = 1.72 cm. The presence of frontal bosing, saddle nose and micromilia (proximal, distal, phalanges) was found. The patient was planned for vaginal delivery and the progress of labor was followed. Patients provided informed consent regarding the possibility of fetal death during labor and after birth. During the active phase of the labor process, hypotony uterine innersia occurs and oxytocin drip is performed to accelerate labor. The baby was born male, weight 2175 grams, body length 34 cm and A / S: 1/0. Postmortem physical examination revealed macroscopic findings of tanatophoric dysplasia infants such as hypertelorism, low nasal bridge, cranio-facial disproportion. Narrow chest with protruding abdomen and short, bent limbs.Conclusion: Tanatophoric dysplasia is "lethal" skeletal dysplasia. Abstract

Antibiotics have been proven to be beneficial for human life since their discovery. This type of drug is the most widely used in infections caused by bacteria. the function is to inhibit growth or kill microorganisms. Therefore, more than 30-80% of patients in developed countries who are hospitalized receive antibiotics, either alone or in combination. Evaluating the implementation of PPRA at RSUP. Dr. M. Djamil by comparing the evaluation of antibiotic use before and after the application of PPRA in inpatients in the Obgyn.We conducted this research including a type of correlational analytical research with a retrospective approach. The research sample was taken by the Medical Record room of the RSUP. Dr. M. Djamil Padang. The research was carried out for 1 month, from May 1 to May 31, 2020. The population in this research was all inpatients of Obstetrics and Gynecology in Dr. M. Djamil Padang.There was an increase in the percentage of antibiotic use in 2019 in line with the higher percentage of surgery in 2019 compared to 2018. As regulated by the guidelines for the use of antibiotics in Dr. M. Djamil, for surgery, prophylactic antibiotics are given 30 minutes before the procedure to prevent infection due to the invasive action. The use of prophylactic antibiotics whether to continue is determined by the type of operation, the length of the operation, the amount of bleeding during the operation, the clinical patient, the laboratory value.There were no significant changes in the implementation of PPRA's work on the qualitative analysis of antibiotic use.Keywords: Antibiotics, PPRA, Evaluation

Abstract

Objective: To report the case of ovarian clear cell carcinoma with involvement of both ovaries and metastatic to ascitic fluid and the label mass in the bladderMethod: Case Report  Case: A 51 years old female presented with enlarging abdominal with gradual pain. The result of transabdominal sonography were multiple cysts with solid mass, suspected solid cystic ovarian neoplasm and ascites. The patient prepared for laparotomy; optimal debulking surgery, mass resection from bladder. Cytology examination was performed from ascitic fluid and it was confirmed by histopathology examination.Result: Microscopic features on cytology examination of ascitic fluid smear was suggest carcinoma. Histological examination was confirmed the diagnosis and the result was ovarian clear cell carcinoma. Discussion: Ovarian clear cell carcinoma is a rare subtype of epithelial ovarian cancer and comprises about 5-10% of ovarian carcinomas. Clear cell carcinoma tends to occur in the fifth to seventh decades. Cytology examination showed the cellular smear consists of groups of epithelial cells with large nucleus, hyperchromatic, pale-staining, vacuolated cytoplasm. There is also eosinophilic, extracellular substance. The presence of a tumor in ascitic fluid and the label mass in the bladder can categorize become IIB. This determined based on the FIGO’s ovarian tumor staging system.Keywords: ovarian clear cell carcinoma; ascitic fluid.

Abstract

Iron deficiency anemia that followed by low serum ferritin levels in early pregnancy has an indirect impact on decreasing the expression of Brain Derived Neurotrophic Factor (BDNF) in the maternal hippocampus. BDNF together with its receptor tyrosine kinase B (TrkB) induced the expression of metalloproteinase-9 (MMP-9) which plays an important role in blastocyst implantation, trophoblast invasion and placental development. Decreasing BDNF levels can interfere with those process which caused imbalance of pro-angiogenic and anti-angiogenic factors such as VEGF, PIGF, sFlt1 and sEng that leads to preeclampsia. Abstract

Introduction : Albumin main function is maintaining the plasma colloid oncotic pressure. Preeclampsia cause a decrease in serum albumin in the pregnant woman. Low albumin levels increase mortality, longer wound healing and longer hospital stays. The intake of oral albumin is expected to be effective in increasing the level of albumin in the blood.Objective : To determine the effect of freeze dryer method intake compared to liquid snakehead fish extract in patients with preeclampsia and hypoalbuminemia.Material and methods : Using pre and post-test control group design.The total sample was 38 patients with severe preeclampsia with hypoalbuminemia, divided into control and intervention groups. Control was given the intake of liquid snakehead fish extract, in the intervention was given the Freeze dryer method. The research data were analyzed using the Mann Whitney test method and the Wilcoxon test.Results : In the intervention group the mean pretest albumin level was 2.66 ± 0.17 gr / dl and on the 4th day it was 3.13 ± 0.16 gr / dl (p = 0.000). In the control group the mean pretest albumin level was 2.72 ± 0.15 gr / dl and on the 4th day it became 3.06 ± 0.14 gr / dl (p = 0.000). On the 4th day after treatment, the increase in serum albumin levels in the intervention group was 0.47 ± 0.19 gr / dl; whereas in the control group was 0.34 ± 0.10 gr / dl with a value of p = 0.031 (p <0.05).Conclusion : The freeze dryer method (freeze dryer method ) for the Cork Fish Extract resulted in a higher increase in albumin levels than the liquid snakehead fish extract.Keywords: Preeclampsia, Hypoalbuminemia, freeze dryer method of Snakehead Fish Extract

Abstract

Objective: To report a case of Ectopic pregnancy with implant receptorsMaterials and Methods: This article describes the case of a 41-year-old woman, diagnosed with an Ectopic Pregnancy on the gravid G5P4A0H4 11-12 weeks. Patients are using implant contraception, which has been used since 2009-2018 (3 installations). The patient came to the obstetrics and gynecology clinic Dr. M. Djamil Padang. The ultrasound gives the impression of Ectopic pregnancy in the infindibule tube. Analysis Ectopic pregnancy can also occur due to the influence of hormonal contraceptive use (progesterone).Results: Patient gets intervention performed laparotomy After the peritoneum is opened it appears fresh red blood amounting to ± 500 cc filling the abdominal cavity. Blood evacuation and exploration are performed, it appears that the source of bleeding originates from the left tubal rupture (Infundibulum) Impression of the left tubal rupture (Infundibulum) ecectic ectopic pregnancy is impaired. Sinistal salpingectomy was performed.Conclusion: Ectopic pregnancy Ectopic pregnancy is all pregnancies where the ovum fertilized by spermatozoa implant and grows outside the uterine cavity endometrial.Keywords: Ectopic Pregnancy, Acceptor Implant

Abstract

Introduction : According to WHO, the prevalence of reproductive health problems in women has reached 33% from all types of diseases. One of the most frequent reproductive health problems is fluor albus. The incidence of candidiasis in pregnant women has doubly increased, especially at third trimester, compared to women who were not pregnant. Candida should be considered a dangerous pathogen in early pregnancy.Objective : This study compared the number of colonies before and after the administration of virgin coconut oil (VCO) to address vaginal discharge caused by C. albicans infection in pregnant women.Material and methods : This is an experimental study on 36 pregnant women. Examination of fluor albus is done by applying vaginal swab before and after administration of 1 tablespoon of VCO orally three times a day dan 5cc of VCO was applied on the vagina and vulva for intervention group, meanwhile the control group was not given anything. Statistical analysis to test the significance is done by using T-test.Results : There was a difference on average number of colonies with higher number of colonies prior to administration (137.17 ± 16.35 vs 122.17 ± 14.42). The average number of colonies at the first examination was 96.06 ± 8.85 colonies and increased at the second examination to 112.89 ± 12.49 colonies.Conclusion : There was an effect of VCO administration on decreasing the number of colonies in the intervention group (p=0.000), while in control group, there was increasing average number of colonies in the second examination (p=0.000).Keywords: Candida albicans, fluor albus, pregnant women, virgin coconut oil

Abstract

Objective: To report a case of Turner syndrome diagnosed in prenatal care.Method: A case report.Case: Case of Mrs. Y 26-year-old woman G2P1A0L1 diagnosed on 19-20 weeks of gestation with Turner syndrome. Ultrasound examination findings were hydrops fetalis on the neck region, multiple septated cystic masses, subcutaneous edema, ascites, and hydrothorax. Subsequently, amniocentesis was performed and the chromosome analysis result showed Turner syndrome (45, X0). The patient was induced vaginal delivery on 22-23 weeks of pregnancy due to intrauterine fetal death indication. The baby was born with ambiguous genitalia, birth weight 500 grams, birth length 22 centimeters, and Apgar’s score was 0/0. The congenital anomalies discovered include subcutaneous edema, ascites, hydromacolly, and hydrops fetalis.Conclusion: Turner syndrome can be diagnosed at a prenatal period by ultrasound examination. Abstract

The aim of this study is to describe the knowledge level and accuracy of reproductive health among students at SMA N 1 Padang. The study design was analytical survey with cross-sectional approach. The population were all SMA N 1 students while cluster random sampling was applied to get 267 students as respondents which represented each major (natural science or social science) and class (X, XI, and XII). The questionnaires were divided into several sections consisting the general knowledge of reproductive health, contraception, and sexually transmitted disease (STD). Result of the study showed that students in natural science major (scoring 71.9%) were having better knowledge than students in social science major (scoring 51.8%). It was also found that students in grade XII (scoring 67.7%) were getting highest result compared to students in grade XI (scoring 61.4%) and grade X (scoring 67.1%). Nevertheless, the result was not satisfying, considering that the average score of all students were only 65.6%. Therefore, guidance from parents, teachers, as well as counselling unit are crucial to help adolescent in Padang, in the context of reproductive health knowledge.
