Abstract

Quantitative Analysis is a review of certain parts of the contents of medical records to find deficiencies, especially those related to the documenting of medical records. Coronavirus Disease 2019 (Covid-19) is an infectious disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARSCoV-2). The purpose of this study is to find out the implementation of the Covid-19 Isolation Room Patient Medical Record Document Review at RSU Imelda Pekerja Indonesia in 2020. The population is all patients treated in the Covid-19 Isolation Room in 2020 as many as 182 people, the study sample is all patients treated in the Covid-19 Isolation Room in October as many as 48 people. The DRM review was conducted on four variables. From the results of the study obtained that the Review of Timeliness of appropriate DRM Returns as much as 35.41%; Review of the Accuracy of Filling out DRM for the exact Opname Warrant Form (SPO) 31.25%, the appropriate IGD Assessment Form 52.08%, the right Medical Resume Form 100%, and the proper Observation Form 66.67%; DRM Readability Review for Opname Warrant Form (SPO) which reads 100%, IGD Assessment Form that reads 77.08%, Medical Resume Form that reads 100%, and Observation Form that reads 72.92%; Drm Completeness Review for Screening Form found in 68.75% of documents, Triage Form found in 77.08% documents, Internal Transfer Form found on 89.58% of documents, Discharge Planning Form found on 81.25% of documents.For General Consent Forms, Inpatient Assessments, CPPT Forms, Observation Forms, and Information and Education Forms are found in all documents that are 100%. It is recommended that the existing SPO socialization prioritizes the accuracy of filling out medical records qualitatively, not limited to the completeness of the form only.

Abstract

The development of information technology is currently moving very rapidly and rapidly spreading to various fields, one of which is in the health sector. Based on PERMENKES Number 31 Year (2019) concerning Information Systems, Puskesmas must implement an Information System to assist the decision-making process in implementing the management of the Puskesmas to achieve its activity targets. This causes the data collected to be inaccurate, prone to data loss, duplication of data and the reporting takes a long time. To overcome this problem, an electronic information system will be developed so that the workload of officers will be reduced and the procedure for collecting patient data will be better, it will not take time to search and record data, and reporting can be done faster and submitted to the relevant parties. The method used to create this system uses visual basic and MySQL database and uses the DFD (data flow diagram) development method. In this study produced a prototype of the puskesmas information system which includes a login menu, an officer data input menu, patient registration and a report menu. It is recommended that the Tumbuan Health Center consider the results of the SIMPUS design so that it can be implemented immediately to support registration activities.

Abstract

APM is a self registration machine in which contains a touchscreen display, barcode scanner, printer or other additional tools. There are several problems related to the use of APM at Pakuwon Hospital, one of them is the queue number that can’t be printed due to the error of SIMRS network. This situation makes the process of service obtained by patients hampered. The quality of service provided by the hospital to the patient can lead to a sense of patient satisfaction. Patient satisfaction must always be considered because it relates to the subjectivity value to the quality of services provided. The purpose of this was to determine the level of satisfaction of patients using APM at Pakuwon Sumedang General Hospital. Method used in this study is quantitative with descriptive resejiarch design. The measurement of satisfaction using the End User Computing Satisfaction (EUCS) which determines from 5 dimensions. The population were 72,364 patients and the samples of this study were 110 patients. Data collection techniques using a questionnaire. Data analysis using a criterion score. The characteristics of the respondents contained in this stud based on age, education, occupation and membership status. The level of satisfaction obtained based on the content aspect is 89.22%, the accuracy aspect is 87.60%, the format aspect is 88.37%, the timeliness aspect is 86.98%, the ease of use aspect is 88.50% and overall patient satisfaction is 84.30%.

Abstract

a complete medical record reflects the quality of good documentation, the complete of diagnosis writing reduces incorrect and inaccurate code and appropriate reporting to support decision makers. this study aims to measure the completeness and accuracy of the death code and the basic cause of death of patients. This study uses a quantitative descriptive design using a completeness checklist instrument. researchers used secondary data from the medical records of patients who died in 2021, the total sample was 199 using the slovin formula. secondary data analyzed with STATA, presented with tabulations and descriptive narratives. the basic cause of the basic cause of death was highest in code J80 at 33 (17%), J12.8 at 21 (11%) and the third being E11.9 . Based on the category of completeness of filling out of the 199 medical records analyzed, there were 37% (75) death forms that were not filled in completely

Abstract

Background: Motivation is an important factor that can affect employee performance. If the employee feel satisfied with their work, the employee will produce good performance, and vice versa. Based on the result of the preliminary study with the head of the medical record unit, it is known that they never get a reward if they work well, In addition there are still many officer who come late and performance of officers is still not paid attention. This condition resulted in work performed by medical recorders at Nur Hidayah Bantul hospital is not optimal, so it will affect work motivation. Objective: To determine the relationship between work motivation and the performance of medical recorder in the Nur Hidayah Bantul Hospital Method: This is a quantitative research with cross sectional design. The data collection technique used total sampling with a total respondent of 16 medical recorder. Results: From the results of the study, the results of work motivation showed that most medical recorder had sufficient work motivation (63%) and the performance of medical recorder mostly had sufficient performance (81%). From the results of the Spearman’s rho test on the relationship between work motivation and the performance of medical recorder in the Nur Hidayah Bantul hospital obtained r value 0.522 and p value 0.38 which means that there is a significant relationship between work motivation and performance. Conclusion: There is a relationship between work motivation and the performance of medical recorder in the Nur Hidayah Bantul hospital.

Abstract

Based on a preliminary survey at RSJD Surakarta, the completeness of filling out the electronic-based and paper-based Integrated Patient Development Record (CPPT) form with a total of 10 samples taken at random has 40% completeness on the electronic-based and 60% paper-based CPPT form. This type of research is comparative. The population in this study were all medical record documents of inpatients with a diagnosis of paranoid schizophrenia at the Surakarta Hospital in the first quarter of 2019 totaling 59 electronic patient medical record documents and 63 paper patient medical record documents in the first quarter of 2020. The sample size was obtained by the sample formula. a minimum of unpaired numerical comparative research and with a simple random sampling technique, the results obtained were 31 electronic medical record documents and 31 paper medical record documents. The results of the research on electronic-based CPPT forms on identification reviews are 100%, 100% authentication reviews, and 88% important report reviews. Meanwhile, in the paper-based CPPT form, the identification review is 97%, the authentication review is 90.5% and the important report review is 100%. In this study, the Mann-Whitney test was used as an alternative to the independent t-test which did not meet the requirements. Mann Whitney test results p-value <0.05 so ha is accepted which means there is a difference in the completeness of the electronic-based and paper-based CPPT forms. Abstract

Management is a process consisting of regulatory, planning, organizing, directing and controlling activities carried out to determine and achieve goals through the use of human resources and other resources to achieve organizational goals effectively and efficiently. Elements of management include 5M including (Man, Method, Material, Machine, Money). The purpose of this research is to determine the 5 element of The Timely Return of Inpatient Medical Record Files at Bina Kasih Hospital in 2021. This type of research is qualitative with a case study approach. The research was conducted in August-September 2021 which coincided with Bina Kasih Hospital in Medan with 7 subjects and head of medical record informants, the sampling technique used was purposive sampling. The timeliness of returning inpatient medical record files in August 2021 totaled 270 files and the number of inaccuracies in inpatient medical record files was 60.74% machine and money elements.

Abstract

Analysis of the medical record workload is very necessary in improving health services in a hospital. This aims to determine which workloads need to be streamlined. In addition, with the analysis or measurement of workloads, scientific evidence-based decision making can be made. Seeing the large role of medical records in health services in hospitals, it is necessary what things are needed to improve the best quality in medical record installations. The purpose of this study was to determine the need for medical record personnel based on the workload in the filling section of Mitra Sejati General Hospital Medan. This type of research is descriptive with a cross sectional approach with a population of 4 medical record officers. The place where the research was conducted was at Mitra Sejati General Hospital Medan because the hospital had problems regarding work requirements in the filling section. The available working time is based on the number of effective working hours in the medical record section, which is 2200 hours/year, in 1 day working hours are 8 hours. The above regulation is based on the number of working hours of medical record officers at Mitra Sejati Hospital Medan, the number of outpatient visits is 6098 patients/year and for inpatients is 14789 patients/year while the standard time allowance for officers is 0.01 minutes. Based on the results of the calculation of the need for filling officers using the WISN method, the total human resource requirements in the outpatient filling unit were 2.89 or 3 officers while the outpatient filling unit officers at Mitra Sejati General Hospital Medan were 2 people. Meanwhile, the total human resource requirements for the inpatient filling unit are 3.11 or 3 officers, while the inpatient filling staff at Mitra Sejati General Hospital Medan are 2 people, so there is still a need for additional medical records officers specifically in the filling section.

Abstract

Storage (Filling) is an activity of storing, structuring, or storing medical record files facilitate retrieval. The aim of this research is to determine the overview of the storage system with the length of time for retrieval of medical record files at RSIA Artha Mahinrus Medan. This research is descriptive qualitative to see by describing the results as they are and comparing them with the theory which is then drawn conclusions. The time of the study was carried out in May at the Artha Mahinrus Mother and Child Hospital, Medan. The population and sample used are medical record employees in the registration and storage division (filling). The instruments used in data collection were observation and interviews. Based on the results of this study, it shows that the storage system at the Artha Mahinrus Mother and Child Hospital Medan is a centralized storage system, by combining medical record files in one room. The time of taking medical record files is very influential on education, age, years of service and storage shelves. The education of medical record file storage officers is not all D3 medical record graduates. But the storage process continues to run well. for the medical record retrieval process, it is hoped that the retrieval process from registration to the polyclinic is expected to be improved again with a time of 10 minutes.

Abstract

Misfiled is an error in the storage of medical records, or the non-discovery of medical records on the storage shelf. This study aims to determine the description of misfiled events in the medical record storage shelf of RSUD Dr. Adjidarmo Kab. Lebak in 2022. This study uses a descriptive analysis method with a quantitative approach by conducting observations and interviews. The results of the study with 99 samples found that 9 (9.1%) of the misfiled and the non-misfiled were 90 (90,9%). Most of the medical records that occurred were misfiled because it did not fit on the proper shelf or medical record documents located on another shelf. The storage system used is a centralized system and its alignment system uses a digit filing terminal. The first factor causing the misfiled in the hospital is that the medical record officer is not careful in storing medical records, because there are still often errors when reading the numbers written. Another factor is in the facilities and infrastructure of the storage room, because there is no tracer as a substitute for medical records that come out of the shelves. The advice is always to use tracers to replace medical records that come out, so as not to misfiled the alignment of medical records.
