Abstract

The COVID-19 pandemic accelerated the adoption of digital mental health services (DMHS) in Indonesia as an alternative approach to addressing limited access to conventional psychological services. However, the rapid implementation of these services also revealed various structural, technological, and regulatory challenges within Indonesia’s mental health system. This study aimed to explore stakeholders’ experiences in implementing DMHS during the COVID-19 era, including the challenges encountered, adaptive strategies employed, and recommendations for future service development. This research employed a qualitative exploratory design using semi-structured interviews involving 14 stakeholders, consisting of psychologists, platform managers, founders, and representatives of professional organizations in Indonesia. Data were collected through online interviews and analyzed using inductive thematic analysis supported by NVivo 14 software. The findings identified several major themes, including community and client readiness, service provider readiness, regulatory and legal conditions, technological limitations, service effectiveness and quality challenges, and capacity building and innovation. The study revealed that stigma, digital inequality, unclear regulations, and therapist burnout significantly affected the effectiveness of DMHS implementation. Nevertheless, stakeholders adopted adaptive strategies such as hybrid services, peer learning, structured evaluations, and digital innovation to maintain service quality. In conclusion, strengthening regulations, infrastructure, professional competence, and ethical standards is essential to ensure sustainable and effective digital mental health services in Indonesia.

Abstract

Magnetic Resonance Imaging (MRI) with Diffusion-Weighted Imaging (DWI) evaluates the microstructure of brain tissue through the Apparent Diffusion Coefficient (ADC) parameter. Although often used clinically, the accurate standardization of physiological values of ADCs is still a challenge due to the high regional variability. This study aims to analyze the difference in ADC values in various anatomical structures of the adult brain using the DWI Readout Segmentation of Long Variable Echo-trains (RESOLVE) sequence. This retrospective analytical observational study involved 12 normal subjects examined using 1.5 Tesla MRI. Measurements were made manually on 14 anatomical structures using a circular Region of Interest (ROI) (±20 mm²). Reliability is validated by the Intraclass Correlation Coefficient (ICC). Statistical analysis included Repeated Measures ANOVA, Spearman correlation (age), and Wilcoxon test (grey vs White matter). The results showed that the ROI had excellent reliability (ICC 0.845–0.998). There was a very significant difference in ADC values between anatomical structures (p < 0.001; Partial η² = 0.700), with an average range of 0.70–0.83 ×10⁻³ mm²/s. There was no significant correlation between age (22–68 years) and regional ADC fluctuations (p > 0.05). However, the ADC of Grey matter was found to be significantly higher than that of White matter (p = 0.002). In conclusion, the RESOLVE DWI sequence results in reliable ADC measurements. Variations in diffusion values are inherently dependent on anatomical location and tissue type, but are very stable with age.

Abstract

Anemia pada kehamilan masih menjadi masalah kesehatan maternal yang meningkatkan risiko komplikasi ibu dan janin, termasuk perdarahan, persalinan prematur, dan berat badan lahir rendah. Meskipun Tablet Tambah Darah (TTD) merupakan terapi standar, efek samping gastrointestinal seperti mual dan konstipasi serta kepatuhan yang belum optimal menunjukkan perlunya intervensi pangan pendamping yang dapat diterima ibu hamil. Susu kedelai dipilih karena menyediakan protein, asam folat, zat besi nabati, dan zat gizi lain yang mendukung pembentukan hemoglobin, mudah diperoleh, serta relatif terjangkau bagi masyarakat. Research gap penelitian ini adalah terbatasnya bukti pemanfaatan susu kedelai sebagai pendamping TTD pada ibu hamil trimester II di layanan primer. Kebaruan penelitian terletak pada evaluasi intervensi 200 ml susu kedelai dua kali sehari selama 14 hari pada konteks pelayanan antenatal Puskesmas Karangnunggal tahun 2026. Penelitian pra-eksperimental one group pretest-posttest melibatkan 30 ibu hamil trimester II dari populasi 48 orang yang dipilih secara purposive dengan kriteria inklusi yang telah ditetapkan. Kadar hemoglobin diukur menggunakan hemoglobin meter digital pada awal dan akhir intervensi. Data berdistribusi normal berdasarkan Shapiro-Wilk (pretest p=0,153; posttest p=0,063) dan dianalisis menggunakan paired t-test. Rerata hemoglobin meningkat dari 9,81 g/dL menjadi 10,27 g/dL. Analisis berpasangan menunjukkan perbedaan bermakna (t=4,044; df=29; p<0,001), interval kepercayaan 95% peningkatan 0,25–0,75 g/dL, dan ukuran efek Cohen's dz=0,74. Proporsi ibu tanpa anemia meningkat dari 30,0% menjadi 50,0%. Temuan menunjukkan manfaat statistik dan klinis tingkat sedang-besar. Susu kedelai dapat dipertimbangkan sebagai pendamping, bukan pengganti TTD, disertai pemantauan kadar hemoglobin dan edukasi gizi oleh tenaga kesehatan dalam pelayanan antenatal secara berkelanjutan.

Abstract

Emesis gravidarum merupakan keluhan mual dan muntah yang umum dialami ibu hamil trimester pertama akibat perubahan hormonal, dengan prevalensi global berkisar 50–80% dan berpotensi mengganggu aktivitas sehari-hari, asupan nutrisi, serta kualitas hidup ibu. Salah satu intervensi non-farmakologis untuk mengurangi keluhan tersebut adalah akupresur pada titik Perikardium 6 (PC6). Penelitian ini bertujuan mengetahui efektivitas akupresur titik Perikardium 6 dalam menurunkan keparahan emesis gravidarum pada ibu hamil trimester I di Puskesmas Karangnunggal Kabupaten Tasikmalaya Tahun 2025. Penelitian menggunakan desain quasi experiment dengan Non-Equivalent Control Group Pretest-Posttest Design. Sampel berjumlah 30 ibu hamil trimester I yang mengalami emesis gravidarum, terdiri dari 15 responden kelompok intervensi dan 15 responden kelompok kontrol, dipilih secara purposive sampling. Keparahan emesis diukur menggunakan kuesioner PUQE-24, dan data dianalisis dengan uji Wilcoxon dan Mann-Whitney (α=0,05) setelah uji normalitas Shapiro-Wilk. Hasil menunjukkan pada kelompok intervensi keparahan menurun dari kategori sedang (73,3%) dan berat (20,0%) menjadi mayoritas ringan (80,0%) tanpa kategori berat, sedangkan pada kelompok kontrol tidak membaik bahkan kategori berat meningkat dari 6,7% menjadi 13,3%. Uji Wilcoxon dan Mann-Whitney sama-sama menghasilkan p=0,001 (p<0,05). Disimpulkan akupresur titik Perikardium 6 efektif menurunkan keparahan emesis gravidarum dan dapat diintegrasikan sebagai terapi non-farmakologis yang aman, murah, dan mudah diterapkan dalam pelayanan antenatal.

Abstract

Congenital Nasolacrimal Duct Obstruction (CNLDO) is one of the most common lacrimal disorders in children, affecting 6–20% of neonates and occurring at an incidence of 23.1 per 10,000 live births. This systematic review and meta-analysis compared the effectiveness of nasolacrimal duct probing and intubation in simple and complex CNLDO, evaluated the moderating effect of patient age on treatment outcomes, and synthesised complication profiles associated with both interventions. A systematic search of PubMed, EMBASE, Cochrane Library, and Web of Science identified relevant studies published between January 2002 and March 2026. Following PRISMA-compliant screening procedures, 21 studies involving 2,593 patients met the inclusion criteria, with excellent inter-rater agreement (kappa = 0.89). In simple CNLDO, treatment success did not differ significantly between probing and intubation (RR = 0.94; 95% CI: 0.88–1.01; I² = 38%), supporting probing as an appropriate first-line intervention. Conversely, intubation demonstrated significantly superior outcomes in complex CNLDO (RR = 0.57 favouring intubation; 95% CI: 0.49–0.66; I² = 52%). The pooled success rates for intubation and probing in complex cases were 82.4% and 45.8%, respectively. Age significantly moderated treatment effectiveness, with children older than 24 months experiencing markedly poorer outcomes following probing in complex CNLDO (interaction p = 0.003). These findings support subtype-based treatment strategies, recommending probing for simple CNLDO and early intubation for complex cases. Further high-quality randomised trials are required to strengthen the evidence base and standardise complexity definitions.

Abstract

Groundwater is a critical water source for rural communities, particularly in areas where centralized water-supply systems are limited or absent. Despite its essential role, groundwater quality in agricultural regions of Indonesia remains insufficiently monitored, creating a significant gap in public health surveillance and environmental management. Previous studies have assessed groundwater quality in urban or peri-urban contexts; however, integrated physicochemical, microbiological, and spatial assessments in rural agricultural settings of West Java Province remain scarce. This study aimed to assess the physicochemical and microbiological quality of groundwater in a rural agricultural area in Indramayu District, West Java Province, Indonesia, and to identify potential contamination risks associated with land use and local environmental conditions. A total of 30 household well samples were collected and analyzed for pH, temperature, total dissolved solids (TDS), electrical conductivity, dissolved oxygen, salinity, nitrate, phosphate, iron, manganese, and total coliform. Spatial distribution of contamination risk was assessed through ArcGIS-based distribution mapping. The results showed that most basic physicochemical parameters were generally within acceptable ranges. Nevertheless, several wells showed elevated TDS, nitrate, iron, and total coliform levels. Nitrate concentrations ranged from 2.40 to 62.50 mg/L, with sampling points S8, S9, S10, and S18 exceeding the WHO guideline value of 50 mg/L. Microbiological contamination was a major concern, as only 8 of 30 samples (26.67%) met the safety requirement of 0 MPN/100 mL for total coliform. Spatial analysis indicated that high-risk wells were concentrated near agricultural land, residential settlements, and drainage channels, reflecting the combined influence of fertilizer application, septic tank leakage, domestic wastewater, and surface runoff infiltration. These findings confirm that rural groundwater quality in Indramayu District is compromised by both agricultural and domestic pollution sources, with direct implications for waterborne disease risk and long-term public health. Integrated monitoring, improved well construction standards, adequate septic tank setback distances, sanitation system improvements, and community-level water treatment are urgently recommended.

Abstract

Cinnamomum burmannii (Nees & T. Nees) Blume is one of the most widely traded cinnamon species globally and an increasingly investigated botanical source for metabolic, antimicrobial, anti-inflammatory, dermatological, and gastroprotective applications. Cultivated predominantly in Indonesia and distributed across tropical Southeast Asia and southern China, C. burmannii is distinguished by its high coumarin content, chemically rich and variable phytochemistry—dominated by cinnamaldehyde, A-type proanthocyanidins, D-borneol-rich essential oil, and phenolic acids—and by the emergence of standardized bioactive fractions with clinical development. This review integrates the totality of published evidence across two parallel research streams: a broad international body of phytochemical and pharmacological work from Indonesian, Chinese, Belgian, Portuguese, and other research groups, and a focused translational program from Dexa Laboratories of Biomolecular Sciences (DLBS), comprising DLBS3233—a combined C. burmannii/Lagerstroemia speciosa fraction supported by multiple Phase II–III randomized controlled trials in prediabetes, type 2 diabetes mellitus, and polycystic ovary syndrome—and DLBS2411, a bark-derived proton pump downregulator and mucoprotector with clinical trials in GERD, peptic ulcer, and functional dyspepsia. The collective evidence demonstrates robust antidiabetic activity via α-glucosidase inhibition, insulin receptor phosphorylation, PPARγ/GLUT4 upregulation, and TRPA1/GLP-1 stimulation; antioxidant and antilipidemic effects across multiple animal and human models; broad-spectrum antimicrobial activity against foodborne pathogens, Staphylococcus aureus, Aspergillus flavus, and Malassezia furfur, with characterized molecular mechanisms; anti-inflammatory action through NF-κB/IKK suppression; wound-healing promotion via M1 macrophage inhibition; and gastroprotective dual-mechanism activity.

Abstract

Prolonged sitting in an unergonomic position has become a major concern in musculoskeletal health because it can alter physiological spinal alignment, particularly lumbar lordosis and cervical posture. Continuous static sitting may induce posterior pelvic tilt, reduce lumbar curvature, increase spinal loading, and contribute to low back pain and postural disorders. This study aimed to systematically review the effectiveness of pelvic cushions with different inclination angles in maintaining lumbar and cervical spinal alignment during sitting activities. A Systematic Literature Review (SLR) was conducted using the PRISMA framework. Relevant studies were identified from PubMed and PEDro databases through predefined keywords and Boolean operators. Articles published between 2015 and 2025 were selected based on PICOS criteria, resulting in 12 studies included for qualitative synthesis. The findings indicated that pelvic cushions and inclined seating surfaces generally improved pelvic positioning, maintained physiological lumbar lordosis, enhanced sitting comfort, and reduced biomechanical stress associated with prolonged sitting. Cushion angles ranging from approximately 10°–20° demonstrated optimal effects in supporting pelvic stability and improving load distribution. Furthermore, improvements in lumbar alignment were associated with indirect positive adaptations in cervical posture through the spinal kinetic chain mechanism. In conclusion, pelvic cushions with appropriate angle variations represent a promising ergonomic intervention for preventing postural abnormalities and musculoskeletal disorders caused by prolonged unergonomic sitting. Further research is recommended to evaluate long-term effects and direct cervical curve measurements.

Abstract

Patient satisfaction is a key indicator of healthcare service quality influenced by various factors, including therapeutic communication and service accuracy. This study analyzed the influence of therapeutic communication and the accuracy of beauty clinic outpatient services on patient satisfaction, with medical records as a moderating variable. Using a quantitative approach with a cross-sectional design, the study involved 160 patients from Muntira Skin Care as respondents. The results showed that therapeutic communication and beauty clinic outpatient service accuracy positively and significantly affected patient satisfaction. Furthermore, medical records were found to significantly moderate the relationship between both independent variables and patient satisfaction. These findings confirm that empathetic communication, service timeliness, and integrated medical records are key factors in improving patient satisfaction in beauty clinics. This research provides theoretical contributions by enriching healthcare management studies and developing patient satisfaction models with medical records as a moderating variable, as well as practical contributions as input for Muntira Skin Care in improving service quality through communication training, service flow optimization, and investment in electronic medical record systems. The managerial implications emphasize the importance of continuous communication training for staff, optimization of service flows to reduce waiting times, and investment in integrated electronic medical record systems to comprehensively improve patient satisfaction.
