Abstract

The General Hospital of Dr. Ferdinand Lumbantobing Sibolga as a class B hospital has been accredited Plenary based on Hospital Accreditation Committee Letter Number: KARS-SERT / 569 / I / 2017 dated January 5, 2017. Changes in the paradigm of pharmaceutical services from previously oriented to medicine to patient-oriented demand pharmacists can provide quality pharmaceutical services, interact directly with patients, and are responsible for the drugs given by referring to Permenkes 72, 2016 regarding pharmaceutical service standards even though in practice they have not met the established standards. Performance can be interpreted as the output of the work done and also the process towards the expected results of a job. Many factors underlying pharmaceutical performance include individual variables (ability, education), organizational variables (service procedures), psychological variables (motivation). The problem of implementing a patient safety system is the occurrence of patient safety incidents (IKP) due to lack of optimal pharmaceutical services related to the limitations of human resources (HR) and the consistent implementation of operational standards.

Abstract

One of the efforts in controlling Dengue Hemorrhagic Fever (DHF) is to break the chain of transmission by controlling the vector through the eradication of Dengue Hemorrhagic Fever Mosquito Nest, which is an activity to eradicate eggs, larvae and cocoons of Aedes aegypti infectious dengue disease. This study aims to explain how the Relationship between Predisposing, Enabling and Reinforcing Factors with the Prevention of Dengue Hemorrhagic Fever (DHF) in the Working Area of ​​ Public Health Center of Londut, Labuhan Batu Utara Regency, with the type of survey research with cross-sectional design. The population of this study is the whole Head of the Family as many as 18,898 families. The sampling technique uses random sampling with 100 respondents. Data is collected by questionnaire. Data analysis was performed univariate, bivariate with Chi-square test at 95% confidence level, α = 5% and multivariate with multiple logistic regression. The analysis shows that there is a relationship between education (p = 0.032 <p = 0.05), occupation (p = 0.005 <p = 0.05), knowledge (p = 0.006 <p = 0.05), availability of facilities / infrastructure (p = 0.010 <p = 0.05), the role of health workers (p = 0.002 <p = 0.05) with the prevention of Dengue Hemorrhagic Fever (DHF). The most related variable is knowledge OR = 2,968. The management of Public Health Centre of Londut is expected to improve services in the Dengue Hemorrhagic Fever program to prevent an increase in dengue cases in the working area of ​​Public Health Centre of Londut.

Abstract

Monitoring is activity to carefully observe a situation or condition, including certain behaviors or activities, with the aim that all input data or information is obtained from the results of observations. Evaluation is act of making a decision to assess object, situation, event or activity that is being observed. The background of the research at Sultan Sulaiman Hospital, Kabupaten Serdang Bedagai. The absence of good monitoring and evaluation process on the availability of drugs, plus non-compliance of specialists in prescribing drugs according to the formulary contents. Research objective was to determine the efforts, benefits, constraints and follow-up in processing human resources, funds, facilities and methods after monitoring and evaluating drug availability and drug use at Sultan Sulaiman Hospital, Kabupaten Serdang Bedagai. The method this research is qualitative. Effort made that each sector makes a unit requirement plan (RKBU) so that head of the planning and development sector can summarize all the RKBU and make it an OPD work plan. The way is done to make use of by carrying out the correct planning process and using management principles. Then in terms of funds, the means and methods must be carried out effectively and efficiently and efficiently. The obstacles faced are limited district government budget and the low commitment of the leadership in building health facilities and infrastructure. The follow up to determine the working hours of specialist doctors with Regulation number 53 concerning civil servant discipline and make rules on compliance using hospital formularies in fulfillment of medicines for patients.

Abstract

The World Health Organization (WHO) reports that 15% of waste generated by health services is infectious waste, 1% chemical waste, 3% pharmaceutical waste, 1% radioactive waste. The purpose of this study was to determine the relationship between knowledge, attitudes, training, and specifications of health workers on medical and non-medical waste management in public health centers. This research was conducted using a cross-sectional method. The sample was 96 health workers at Batu Gana Health Center. The sampling technique was carried out by consecutive sampling. Data were analyzed using chi-square test for bivariate test and logistic regression for multivariate test at significant level α = 0.05). The results showed that there was a relationship of knowledge (p <0.05), RP 3.60; 95% CI (1.28-10.09), health personnel specifications (p <0.05), RP 7.54; 95% CI (4.79-19.72) with medical and non-medical waste management while attitude (p> 0.05), length of work (p> 0.05), and training in the last 1 year (p> 0, 05) no significant relationship was found. It can be concluded that there is a relationship between knowledge and specifications of health workers with medical and non-medical waste management with the most dominant relationship between health personnel specifications. It is recommended that the head of the Batu Gana Puskesmas be able to provide training to medical personnel at least once a year to improve medical and non-medical waste management.

Abstract

Compliance occurs when individual do the demand which is given because of she/he wants to receive the better reaction from the other individuals or groups. The scope of National Health Insurance Participants such a date December 31st 2016 that include as self-supporting categorical in all over Indonesia which is reached 5.060.927 peoples. In September 2017, BPJS which as the National Health Insurance Implementer states that it has deficit up to 9 billion. The goal of this research analyzed the factors that influence the compliance of BPJS self-supporting participants when they paid the month premium in Namu Ukur PUSKESMAS Sei Bingai Sub district Langkat Regency in 2018. The research type was analytic survey whereas by using cross-sectional approach. The sample amounted 185 based on probability sampling. The data collection is done by interview; the data analysis was bivariate that is done by using chi-square test with credibility level 95% (α=0,05). The research result showed that reciprocity variable include good category and the activeness for pay the premium (80,1%), Validation Social include good category and the activeness for pay the premium (86,6%), Commitment / Consistency (80,8%), Friendship/liking (91,6%), Authority (81,1%). The most related variable is reciprocity which got the value exp (B) 270,190 that means it has the opportunity for having the compliance to pay the premium as many as 270,190. It is suggested for Langkat Regency BPJS so that constant to evaluate the implementation of National Health Insurance and make the coordination with health facility that have cooperative with BPJS in order to keep the compliance quality so that National Health Insurance Participants feel positive reciprocal from their activeness pay the premium.

Abstract

The existence of private health facilities is needed in assisting health services. Health facilities are required to be able to provide quality health services with affordable medical expenses so as to increase the number of patient visits. The purpose of this study is to determine the effort increase of patient visits number that observed from service quality in Clinic of Pratama Pelita Kasih Medan in 2017. This study use cross sectional analytic. The population of this study is all patients who had record and get treatment in Clinic of Pratama Pelita Kasih, Medan, as many as 229 patients and use accidental sampling technique. Data collecting use questionnaire. Data is analyzed by used chi square test and data processing program. The result of research shows that there is significant correlation (the value of p 0,041) between health service quality and patient satisfaction, there is no significant correlation (the value of p=0.653) between health service quality and number of respondent (patient). There was a significant relationship (the value of p=0.021) between satisfaction with the number of respondents (patient). It is suggested that the performance of health services can be improved in order to be able to compete with the clinic of Pratama through the addition of medical personnel (general practitioner, dentist, pediatrician, nerve and geneacologyst), competent paramedical personnel (nurse, midwife, chemical analyst), diagnostic equipments (such as ultrasonograph) and change previous types of medication to more affordable medicines to reduce expensive medical treatment.

Abstract

Regristration and reporting system of Public Health Center is vital instrument in health system. The information about the pain, the use of health service at Public Health Center, death and many others health information is beneficial to have decision and policy maker at Regency stage, city and Sub-regency. This research aims to analyze the influence quality of registration and reporting toward administrative system at Pubilic Health Center of Sibolangit in 2017. This research is explanatory research with cross sectional as discover, analyze and explain how the influences quality or registration and reporting toward administrative system at Pubilic Health Center of Sibolangit in 2017. The sample of this research is all populations (random sampling) who are the staff who work in Public Health Center of Sibolangit as many as 50 people. The result of input factors found that budgeting in registration and reporting is good (82%), capacity resource support (86%), mainstay variable in handling good registration and reporting (74%), good respond (76%). The factor of process, data respondents collecting who said good (68%), good sending process of respondents data (52%), bad sending process (48%). From data analysis found that respondents who said good (65%). Output factors, good planning variable (36%), with good evaluation (58%), punctuality to handle and make good report (60%), safety of saving data file (52%). The bivariat result show that there is significant influences between collecting data, sending data, analysis data, planning, punctuality, completencess with implematation of administration system at Public Health Center of Sibolangit. There is no significant influences between budgeting, capacity resources, mainstay, respond capacity, and evaluation with administrative system at Pubilc Health Center of Sibolangit. The result of multivariate analysis found that dominant factors is collecting data with value 1,637. For health department needs the efforts comprehensive ordering on organizing of Registration and reporting system of Public Health Center to progress and application of health information system at Public Health Center.

Abstract

The era of National Health Security (NHS) enacted tiered referral system start in first level health services, then the hospital type D, type C, type B and type A. Community complaints and a significant decrease in the number of patients in type B hospital especially in outpatient services, the occurrence of patient accumulation and long queues in hospital type C, this indicates that the implementation of this tiered referral system policy program has not run well. The purpose of this study was to look at the impact of the implementation of this tiered referral program policy on outpatient care in poly Internal medicine at the Regional General hospital Deli Serdang Lubuk Pakam. This research is a qualitative research with a prosfective case study approach and uses a fixed case design. This study triangulation to obtain in-depth information on document from SIMRS data, in-depth interviews, Focus Group Discussion (FGD), direct observation in the field without intervention. This reseach was carried out at the Regional General Hospital Deli Serdang Lubuk Pakam , start from January until April 2019. The result of the study show that the implementation of the program has been carried out in accordance with the regulation of Director of Health Service No. 4 of 2014 regarding the referral system for health services carried out in stages. But there were several impacts from the implemention of the program, namely there was a decrease in the number of patient visits, patients cannot get the doctor’s service they want, patients service time is long because the patient must go through several referral procedur first. Recommendation for BPJS as The National Health Insurance Organizing Body to evaluate and improve existing regulations. If it is necessary to make new regulations so that people do not feel like they are being complicated to get services. Recommendation for the hospital to provide better facilities, both facilities and infrastructure, more competent medical personnel and paramedics.

Abstract

The hospital is a center for public health services in an effort to prevent, restore and cure diseases and improve health status. The increasing level of community life is the increasing also the demands of the community on quality of health. To find out factors that effect the low utilization of health services in the hospitalization of community in RSUD Tuan Rondahaim Pamatang Raya. The number of sampel was 97 responden who get 2.777 hospitalization in that hospital in 2017.The type of research used is analytic survey research with cross secsional design,namely research conducted at the same time but with different subject. The results showed that there was a significant influence between sex and service utilization, The level of education has a significant effect on service utilization. Income has a significant influence with the utilization of health services. Hospital facilities had a significant influence with the utilization of health services. The distance to the service location had a significant influence with the utilization of health services, trust with the utilization of health services. Types of illness with utilization of health services. Trust is the most dominant factor in the utilization of health services with a value of RP = 25,260 (95% CI: 4,022-158,645), it means that the tendency of respondents to believe in a good health service place is 25,260 times greater than those who do not believe in a health service in a hospital. There was no significant influence between age, occupation and service costs with the utilization of health services (p> 0.05).It is expected that hospitals will maintain and provide the best services, and continue to make improvements regarding the availability of more complete facilities, types or variations of health services.

Abstract

Health services received by BPJS participants are carried out through a tiered referral mechanism, which is a definition of a health service referral system that regulates the delegation of duties and responsibilities of health services both vertically and horizontally. The purpose of this study was "To analyze the factors that influence the referral request of BPJS patients at the Puskesmas. This type of research is an explanatory research cross-sectional design with a sample of 79 BPJS patients. The results showed that there was an effect of age (p = 0.01; p <0.05), the influence of BPJS information (p = 0.01; p <0.05) on referral requests for BPJS patients with successive RP 2.17 (95% CI = 1.04-6.60) and 3.70 (95% CI = 1.27-10.7. While education, income, perceptions of illness, perceptions of officers, perceptions of service there is no influence on requests for BPJS patient referrals ( P> 0.05) The BPJS information variable in this study was the most dominant variable affecting referral requests for BPJS patients (RP = 3.70) .The results of the study concluded that there was an influence of age and BPJS information on BPJS patient referral requests. BPJS Kesehatan to pay more attention to accurate and accurate information related to the development of the health care system, both with FKTP partners and JKN participants
