Abstract

Pain is a significant primary complaint among breast cancer patients. The WHO Analgesic Ladder guidelines recommend the use of opioid analgesics as a primary management strategy for cancer pain. However, a periodic evaluation of its effectiveness in specific populations remains necessary. This study aimed to evaluate the effectiveness of opioid analgesic therapy in reducing pain intensity for breast cancer inpatients at Dharmais Cancer Hospital, Jakarta, in 2023. This research was an observational study with a retrospective design. The study sample comprised 58 medical records from breast cancer inpatients who had received opioid analgesic therapy, collected using a total sampling technique. Data analysis was performed using the non-parametric Wilcoxon signed-rank test to compare pain intensity before and after the administration of therapy. The results of the statistical analysis demonstrated a highly significant reduction in pain scores following the administration of opioid analgesic therapy (p < 0.001). The administration of opioid analgesics were significantly effective in the management of pain for breast cancer patients at Dharmais Cancer Hospital, Jakarta.

Abstract

Pneumonia remains a major health problem in Indonesia, with a national prevalence of 0.48% based on health worker diagnosis. Irrational antibiotic use may increase bacterial resistance, prolong hospitalization, and worsen clinical outcomes. This study aimed to evaluate the rationality of antibiotic use in adult community-acquired pneumonia (CAP) patients at RSUD Dr. Harjono Ponorogo, analyze its association with clinical outcomes, and identify factors contributing to irrational antibiotic use. This analytic observational study employed a retrospective cohort design involving adult CAP inpatients treated from January to December 2023. The rationality of antibiotic therapy was evaluated using the Gyssens method based on the American Thoracic Society/Infectious Diseases Society of America (ATS/IDSA) guidelines and the Indonesian National Guidelines for Medical Services. Data were analyzed descriptively and inferentially using the chi-square test. The results showed that 69% of patients received rational antibiotic therapy, while 31% received irrational therapy. Clinical outcomes were predominantly categorized as improved (74.7%) with an average length of stay of 3 to 5 days. Statistical analysis demonstrated a significant association between rational antibiotic use and favorable clinical outcomes (p=0.028; OR=3.58). Fishbone analysis identified several contributing factors to irrational antibiotic use, including patient-related factors, healthcare providers, drug availability, laboratory support, and managerial aspects. In conclusion, rational antibiotic use was significantly associated with better clinical outcomes in CAP patients. These findings highlight the importance of optimizing antibiotic stewardship programs to improve pneumonia management in regional hospitals.

Abstract

Pharyngitis, an upper respiratory tract infection commonly caused by Streptococcus pyogenes, remains a significant health concern. The growing problem of antibiotic resistance has driven researchers to explore alternative therapeutic options derived from natural sources. One promising candidate is the tempuyung leaf (Sonchus arvensis L.), which is rich in secondary metabolites known to possess antibacterial properties. This study investigated the antibacterial activity of tempuyung leaf ethanol extract against Streptococcus pyogenes and assessed its potential application in an edible film formulation. Extraction was performed using maceration with 96% ethanol, yielding 13.85% extract. Phytochemical screening confirmed the presence of flavonoids, polyphenols, tannins, quinones, and steroids. Antibacterial activity was evaluated through the disk diffusion method at extract concentrations ranging from 2% to 10%. The highest inhibition zone, measuring 65.53 mm, was recorded at the 10% concentration, indicating strong antibacterial potential. The extract was subsequently formulated into edible films at concentrations of 3%, 4%, and 5%, and subjected to physical quality evaluations covering organoleptic properties, pH, thickness, dissolution time, moisture content, and weight uniformity. Results showed that all formulations satisfied the standard requirements for most parameters, with the exception of weight uniformity, which did not yet meet established criteria. The edible film containing 5% extract demonstrated the greatest antibacterial activity, producing an inhibition zone of 27.12 mm, classified under the very strong category. These findings suggest that tempuyung leaf ethanol extract holds considerable promise as a natural antibacterial agent and may serve as an effective active ingredient in edible film preparations intended for managing pharyngitis caused by Streptococcus pyogenes.
