Abstract

The aim of this qualitative study is to explore the experience of Healthcare Providers (HCPs) in Enteral Nutrition (EN) practice in hospital settings, the challenges they encounter, and the possible ways to improve the current practice. The study involved conducting in-depth interviews with expert panels, including clinicians, dietitians, nurses, and academic professionals who are involved in clinical or teaching practices related to EN. The recording of the interview was transcribed verbatim after each session. The coding process was done using the computer-assisted qualitative data analysis software Atlas-Ti. The thematic analysis procedure by Braun and Clarke is used to analyze the transcription. At the end of the study, eleven interviews were conducted, transcribed, and analyzed. Six themes addressing the challenges in EN were identified. The themes are standardization of EN practice, enteral feeding provision, knowledge of nurses, attitude of HCPs, skills of nurses, and resources in the healthcare facility. In addition to that, four themes addressing the current needs of EN practice were also documented. The themes identified are EN guidelines and protocols, improving Knowledge, Attitude, and Practice (KAP) on EN, continuous education on EN, and improving healthcare facilities. The insights shared by the participants can be thoroughly examined to develop strategies for enhancing existing EN processes.

Abstract

The aim of this case report is to explore dietitian’s clinical decision-making in providing nutritional care. Ms. C, an 84-year-old Chinese woman with underlying atrial fibrillation, hypertension and diabetes mellitus was admitted into ward due to worsening progression of heart failure complicated with Upper Gastrointestinal Bleeding (UGIB), Community Acquired Pneumonia (CAP) and hypervolemic hyponatremia. Ms. C’s BMI was normal for elderly (27 kg/m2) with estimated weight and height of 70 kg and 160 cm respectively. Patient was hyperglycemic, hypertensive, and breathing under ventilatory support. Prior to admission, she complained of lethargy, difficulty of breathing and stomach pain. Patient was on fluid restriction of 500 ml/day in view of body fluid retention. Patient was on enteral feeding 3 hourly 6 times per day tolerating 100 ml of diabetic formula providing energy of 456 kcal/day and protein of 20 g/day. Inadequate enteral infusion related to physiological changes (fluid retention) requiring restricted fluid intake as evidenced by estimated energy intake of 456 kcal/day and protein intake of 20 g/day less than energy requirement of 1,400 kcal/day and protein requirement of 84 g/day. The goal was to provide adequate energy and protein concomitantly adhering to the fluid restriction and achieving good glycemic control. Modular protein was added to the diabetic formula. Ms. C was able to tolerate 100ml enteral feeding throughout hospital stay. Managing patient with multiple organ complications prove to be challenging. High-density formula is often used for fluid restriction patient however inappropriate for diabetic patient. Product unavailability in the hospital also limiting patient care. Clinical reasoning and clinical judgment were necessary to ensure prioritization of patient care.

Abstract

This cross-sectional study aimed to determine the association between food consumption quality, as determined by Minimum Dietary Diversity (MDD), and children’s nutritional status using anthropometric measurements. A total of 287 children (150 boys, 147 girls) aged 6 to 24 months from Kuala Nerus and Kuala Terengganu participated in this study through purposive sampling. Socio-demographics and food consumption of children were collected through interviews with the mother. Anthropometric comprised weight, height, while nutritional status was determined based on WHO Child Growth Standards. MDD was assessed using WHO Infant and Young Child Feeding (IYCF) Indicators Questionnaire. Children achieving the MDD were rated on four or more dietary variety scores, while those receiving less than four were categorized as "not achieving" the MDD. Most of the children (98.6%) were Malay, with 69.3% aged between 12 to 24 months and 30.7% aged between 6 to 11 months. The prevalence of stunting, wasting, and underweight was 25.8%. 6.6% and 14.6%, respectively. The proportion of children achieving MDD was 90.2%. No significant association existed between MDD and stunting, wasting, or being underweight. However, Pearson’s Chi-Square test affirms that consumption of dairy products was significantly associated with stunting among children (p=0.001). In conclusion, dairy products significantly prevent stunting in children, and their continuation should be emphasized for the first two years of life after exclusive breastfeeding in health promotion programs.

Abstract

The aim of this study is to assess the prevalence of physical activity among pregnant women during early pregnancy, which is during their first trimester, and to investigate the determinants of Gestational Diabetes Mellitus (GDM) related to sociodemographic and physical activity level. This cross-sectional study focused on pregnant women aged between 18–45 in their second or third trimester of pregnancy and visiting selected public healthcare facilities. Questionnaires including sociodemographic, anthropometric, medical history, and the Pregnancy Physical Activity Questionnaire (PPAQ) were distributed. About 123 mothers were enrolled in this study. About 86.2% of them are physically active during early pregnancy, whereas just 13.8% are physically inactive. There was no statistically significant difference between pregnant mothers with and without GDM in physical activity level. Women aged 30 and older are three times more likely to acquire GDM (AOR=3.363, 95% CI: 1.314‒8.609), while women with a pre-pregnancy BMI of overweight or obese are 2.622 times more likely to develop GDM (AOR=2.622, 95% CI: 1.170‒5.875). In conclusion, even though both pregnant women with and without GDM have a high level of physical activity, the length of time spent in sedentary activities is notable. However, no significant relationship was observed between physical activity and GDM development, as most individuals were found to be following the suggested physical activity duration, yet advanced age and obese pre-pregnancy BMI were found to be determinants of the GDM occurrence.

Abstract

This study aimed to investigate the association between Physical Activity Level (PAL) and body weight status, body composition (body fat percentage, muscle mass, total body water, visceral fat and bone mass) among adults in Terengganu. This study involved 150 participants age ranged from 19–60 years old (72 males and 78 females). PAL was determined using the International Physical Activity Questionnaire (IPAQ), while height, weight and body composition were measured using standard measurement tools. Cardiovascular health and fitness were assessed using blood pressure, heart rate and beep test. Multiple linear regression analysis was employed for data analysis. From the analysis, there was a positive association between PAL and body fat percentage independent of other factors (p=0.003), however, there was no other association with other factors. These findings contribute to our understanding of the influence between body weight, body composition, cardiovascular health and PAL in adults. Although the future study may require a bigger sample size, the finding of this study highlights the importance of physical activity to improve overall health.

Abstract

This study aimed to determine the validity of a newly developed questionnaire to assess dietitians' understanding of the factors influencing telenutrition adoption in Malaysia. As such, a questionnaire was developed to assess dietitians’ professional changes, acceptability, barriers, and facilitators to telenutrition implementation. It was constructed based on published questionnaires and an extensive literature review. The questionnaire comprising 52 items, underwent content and face validity assessments. Eight dietetics experts with academic and clinical backgrounds and more than five years of experience in the field evaluated the questionnaire items for relevance, clarity, simplicity, ambiguity, readability, and practicality. The experts quantitatively assessed the questionnaire using a Likert scale rating and additional comments. The Content Validity Index (CVI) was determined using item and scale indices (I-CVI/S-CVI/Ave). The findings show that the I-CVI score ranged between 0.78 and 1.00, and the S-CVI/Ave was >0.87. Thirteen items with an I-CVI score of <0.83 were deleted. Based on the feedback, revisions were made to the instructions and questions regarding clarity, redundancy, ambiguity, or item length. Then, face validation was qualitatively conducted by cognitive review (n=7). This utilized think-aloud and verbal-probing techniques on participants with experience providing telenutrition services for >2 years. The interview was transcribed verbatim and analyzed using ATLAS.ti software. Twelve items were amended due to ambiguity and unclear meanings. The final questionnaire with thirty-nine items is valid to assess the factors influencing telenutrition adoption among Malaysian dietitians.

Abstract

This study aimed to determine entrepreneurship intention, the factors that influence it, and the mediation effect of entrepreneurship education among undergraduate dietetics students in Malaysia. This cross-sectional study used a purposive sampling method and was conducted online among 221 Malaysian dietetics students from eight Higher Education Institutions (HEIs) in Malaysia. The validated questionnaire contained 30 items with 5 domains: attitude towards entrepreneurship, perceived behaviour control, entrepreneurship education, entrepreneurship intention, and self-efficacy. Descriptive analysis, Spearman's rho non-parametric test, and mediation analysis were performed to examine the relationships among the variables and the potential mediating effects. This study found positive and significant relationships between entrepreneurial attitude, perceived behaviour control, entrepreneurship education, self-efficacy, and entrepreneurship intention (p<0.001). Furthermore, entrepreneurial education partially mediates the link between entrepreneurial attitude, self-efficacy, and perceived behaviour control toward entrepreneurship intention. This study suggests the incorporation of an entrepreneurship education program into the curriculum and the encouragement of entrepreneurial activities among dietetics students in higher education institutions in Malaysia.

Abstract

This case study aims to report on the nutrition management of low appetite in elderly due to age factor and underlying medical conditions. Mr. A, a 71-year-old Malay gentleman was referred to a dietitian at the medical ward with a diagnosis of hyperleukocytosis with Mean Corpuscular Hemoglobin Concentration (MCHC) anemia and hypercalcemia with underlying Diabetes Mellitus (DM), hypertension, and Chronic Kidney Disease (CKD) 3A. His weight and height were 59.5 kg and 159 cm, with a Body Mass Index (BMI) of 23.5 kg/m2. He claimed has lost 10 kg within 5 months. Biochemical data showed low albumin, hemoglobin, and magnesium. Intake was 69% adequacy for 24 hours of diet recall in the hospital. As for nutrition diagnosis, inadequate protein-energy intake related to decreased ability to consume sufficient energy due to lethargy as evidenced by loss of appetite and total energy intake (1,244 kcal) and protein (51.8 g) lower than recommendation (energy: 1,606 kcal, protein: 59.5‒71.4 g) were reported. The nutritional intervention aimed to encourage orally as tolerated, giving oral nutrition supplements, a palatable diet, and increasing the energy and protein intake by cutting the food into smaller pieces. As the progress of the intervention, patient’s calorie intake was increased up to 99% of the requirement. His weight on the second follow-up was 62.2 kg, increased by 2.7 kg in 10 days. However, the protein intake was still low, and the intervention was to add protein modular. As conclusion, malnutrition is common among the elderly due to decreased ability to consume sufficient energy and protein due to aging factors.

Abstract

This study aimed to evaluate the effectiveness of hypnotherapy in improving dietary intake and weight status of young adults with high Body Mass Index (BMI) in a public university in Terengganu, Malaysia. This pre-and post-intervention randomly assigned 107 participants with BMI≥25 kg/m2 to either the Intervention Group (IG=53) or the Control Group (CG=54), for 12 weeks. All participants received health education (diet + exercise + behavioral recommendations) and completed food records, while those in IG had additional three hypnotherapy sessions once a month. Descriptive statistics and repeated measure Analysis of Covariance (ANCOVA) were utilized (SPSS 23.0). A total of 104 participants completed the trial (female=82.2%; student=71.0%; BMI=31.3±5.0 kg/m2). Results demonstrated significant decrements in weight (Mean Difference (MD)=-1.4 kg; p=0.021), total energy (MD=-269 kcal; p=0.033), carbohydrate (MD=-1.0% EI; p=0.019) and total fats (MD=-1.8% EI; p=0.002) intakes in IG after controlling for age, gender and baseline variables on repeated measurements. Hypnotherapy had a positive impact on energy intake, indicating its effectiveness in tackling eating habits. These findings are promising and could serve as a basis for future studies in hypnotherapy for weight loss.

Abstract

The purpose of this study is to identify the relationship between eating behaviour and quality of life among youth in Malaysia. The systematic literature review and thematic content analysis initiated in Scopus, Web of Science, Science Direct, and PubMed published from 2018 to 2022 were reviewed. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) standards were followed for conducting the systematic review. A set of inclusion and exclusion criteria was used to identify relevant research, and data extraction and analysis were done to investigate the relationships. The study quality was assessed using the "Quality Assessment Tool for Quantitative Studies". The systematic result found 8 studies classified as eligible (cross-sectional) out of 111 articles reviewed which looked at the relationship between eating behaviour and quality of life, while also emphasizing the lack of research that the topic being studied. This study highlighted unhealthy eating behaviour and dietary patterns associated with lower quality of life. Furthermore, the importance of promoting public awareness about eating healthy and nutrition knowledge was highlighted in this study. Information obtained from this research will benefit the individuals, researchers, institutions, and community to advance knowledge and future practice.
