Abstract

Introduction: Congestive Cardiac failure (CCF) is an anomalous clinical condition involving insufficientheart pumping and filling Cardiac failure causes the heart to be unable to provide enough blood to meet thetissue’s oxygen needs. Heart disease is the most common. Why older people are admitted to hospital Thisputs a huge economics stress on the health care system.The dynamic, progressive nature of heart failure also leads to poor results, with hospital readmissions beingthe costliest About half of those patient die within 5 years after diagnosis.3 Case presentation: A malepatient of 62 years from Arvi Naka was admitted to Medicine Intensive Care Unit (MICU), AVBRH on 11thJanuary 2020 with a known case of Ischemic Cardiomyopathy which was diagnosed itself at AVBRH on22nd October 2018 and a known case of diabetes and hypertension for 10 years. My patient was brought toAVBRH Emergency Unit on 11th January 2020 with a chief complaint of breathlessness for 2 days, sweatingover both feet for 5 days and generalize weakness for 2 weeks. He was having difficulty in breathing forabout 2 days which eventually become severe on 11th January 2020 evening and was brought immediatelyto AVBRH and got admitted on the same day. The patient was delirious and vomit two times on admission.

Abstract

Introduction: Ischemic Heart disease is one of the leading causes of mortality in the world. Advancementin the diagnosis and treatment of this disease available, but still they were considering as major healthproblems in the world.Case Presentation: The purpose of this report only to reduce the major risk factor that is obesity andprovide preventive base Health care treatment which has the greatest effect in the shortest period. Themain symptoms of this disease are chest pain (Angina) and pain mostly occur on the middle or left side ofchest and pain sharp and radiate towards the neck, arm or back, other symptoms like shortness of breath.Obesity is the one of risk factors in heart diseases. The main diagnose of ischemic heart disease is includesthe complete medical history,electrocardiogram, bloodanalysis, chest x-ray, echocardiography, coronarycomputed tomography and coronary angiography. Therapeutic interventions or treatment for ischemic heartdisease includes that pharmacological therapy, lifestyle modifications and regular exercises are the mostcost-effective interventions. Cardiac rehabilitation is for all patients with coronary artery disease is required.Outcome: The effective interventional strategies to help the patient to change the lifestyle and it helps toprevent risk factors such as obesity and also helps to the patient in identifying the symptoms.Conclusions: Considering the patient’s recovery process regarding physical strength it seems that thecombination of pharmacological treatment and lifestyle changes as a control on obesity,regular exercisesand cardiac rehabilitation helps to treat coronary heart diseases.

Abstract

Background: Systemic Lupus Erythematous (SLE) is a multi-system chronic but often episodic,autoimmune condition defined by widespread inflammation of connective tissues and immune complexmediated vasculitis as well as the involvement of Antinuclear Antibodies.Case Presentation: The 11 years old female patient who was apparently alright 2 months back admittedin “Acharya Vinoba Bhave Rural Hospital Sawangi (M), Wardha,” Maharashtra on date 22/02/2020 withthe chief complaint of high-grade fever on and off since 2 months and facial puffiness since 6 days, ascites,constipation and pain in joints. The patient started experiencing fever 02 months back which was highgrade and she took treatment on an OPD basis but there was no relief. After that she was admitted at GMCChandrapur for the complaint of cough, cold, fever and oral ulcer and symptomatic treatment was given andgot discharged after 12 days. Fever, facial puffiness, pain in knee joints. The patient had undergone variousinvestigations like complete blood count, urine analysis, lipid profile, anti-nuclear antibodies (ANA), ANTIDSDNA (Anti-Double Stranded DNA). The patient was treated with antibiotics, corticosteroid, antacid,DMARD (Disease-Modifying Anti-Rheumatic Drug), a loop diuretic, antiemetic and oral paste to treatmouth ulcers. Monitor all vital signs, checked and recorded intake and output, administered medication asprescribed.Conclusion: The patient was admitted to AVBRH with chief complaints of fever on and off, facial puffiness,ascites, constipation and pain in joints. Immediate treatment was started by health team members and nowthe patient’s condition is satisfactory.

C/O pain in abdomen during menses since 1 year. No irregular menses.Past History: Client has no history of diabetes, TB, hypertension, Asthma, epilepsy, thyroid disease. Shehas no other history of hospitalization other than obstetrics. No history of blood transfusion.Clinical Finding: Pain in abdomen. Ovarian dysfunction causing the egg development to be absent ordiminished.Investigations: Serological Investigations, Sonography, Semen analysis in her Husband and Laparotomyin my client.Surgical Management: The laparotomy was done and the cyst was removed by surgical intervention.Medical Management: Patient was treated with antibiotics, antacids, anti- cholinergic, anti inflammatory,IV fluids and analgesics.Nursing Management: Nurse need to obtain history as prenatal, family and other relevant history. Nurse hasto perform primary physical examination and collect other relevant information regarding reports of patients.Give psychological support throughout the counselling. Collect other information about tests, reports anddocuments. Establish plan of care with family and co-ordinate care with other health care professionals.Maintain privacy and confidentiality of all cases. Ensure follow up and supportive services to individual andfamily during counselling.Conclusion: Infertility is an important social and medical problem that affects couples all over the world.Factors both female and male are equally responsible. Evaluation is essential for both partners. Treatmentdepends on the cause of infertility and can vary from ovulation to surgery to ART. My patient come to thehospital as nulligravida with primary infertility and anxious to conceive. As a treatment, cause of infertilitydetected and exploratory laparotomy is done as a treatment regimen

Abstract

Background: In most developed countries accidents at home are a significant public health problem sincethey are a common cause of injury and death. Also, because a large portion of these incidents includes theelderly, it is important to determine their social effects in this community. Most incidents will result in abruise or scratch, but elderly people falling will cause even more severe injuries like hip fractures, headtrauma or even death.Aim: The study aimed to explore the knowledge regarding domestic accidents among elderly people.Objective:1. To assess the knowledge regarding domestic accidents among elderly people.2. To evaluate the effectiveness of planned teaching regarding domestic accidents among elderly people.3. To correlate the findings with selected demographic variables.Method and Material: This was a Cross-sectional research design. The study was undertaken in selectedrural areas of Wardha district. Sample size was 100 and Convenient sampling technique was followed.Results: Majority of samples 44% of the age 60-64 years, 54% of them were male, 62% were married people,75% were educated up to primary education, 62% of them belong to the farmer, 82% of the people livingin a Pakka house, 90% people it is known their occurrence of domestic accident and 27% got informationfrom the newspaper.

Abstract

Background: Mental health can be disturbed among adolescents at any time. One in five adolescents areaffected by mental illness age between 14-24 years. 10-20% of adolescentsis affected by mental illness bythe age of twenty all over the World.Aims: The study aims to Screening of adolescents for schizophrenia and depression and to identify its riskfactors.Objectives: 1. To identify the risk factors of specific mental illness among adolescents. 2. To screen andidentify the adolescents for depression through BECK depression scale. 3. To screen and identify theadolescents for neurotic disorders through Brief psychiatric rating scale. 4. To associate the risk factors ofspecific mental illness with their demographic variables.Method: Research approach: Exploratory approach, Research design: Descriptive design.Sample: Adolescents aged between 10-20 years. Sample size: 300. Sampling technique: Non probabilityPurposive sampling Tools: BECK depression inventory to identify depression, Brief psychiatric rating scale:To identify for neurotic disorders. Setting of study: Selected Schools of Wardha district.Results: The main risk factors identified are: Most of the adolescents was having enmeshment relationshiptowards their parents and 2% were having disengagement relationship; 15.67% family members ofadolescents were diagnosed with psychiatric illnesses and on psychiatric treatment. In BECK depressioninventory, most of the adolescents were adopted everyday ups and down are considered normal whereas20% of them were having moderate depression.Conclusion: More interventions are required to deal with mental disorders among adolescents.

Abstract

Introduction: Fractures of the proximal humerus constitute about 4% of all fractures and 26% of humerusfractures. Treatment of fracture proximal humerus has always been controversial. Many studies indicate thatthe majority of favourable results of these fractures are achieved by conservative method. Whereas someother studies suggest that surgical management is better.Aim: The aim of this study was to compare functional outcome between Open Reduction and PlateOsteosynthesis and Closed Reduction with wire Fixation of Fracture of proximal humerus.Method and Materials: 30 patients with fracture of proximal humerus were first categorized according toNeer’s classification and were divided alternatively into Proximal Humerus Interlocking System (PHILOS)group which is named Group A and wiring group which was called Group B according to randomizedcontrol study design. These patients were then followed up for 12 months. The results were compared usingConstant-Murley score.Results: In our study, the favourable outcome was seen in 12 patients out of 15 in group A (80%) whereasin group B the favourable outcome was seen in 9 out of 15 patients (60%) which is statistically insignificant.At the final follow up after 1 year, the final mean Constant and Murley’s score was 78.4 for group A while itwas 70.13 for group B which is statistically significant showing better results with Philos.Complications: Complications seen in group A were 3 shoulder stiffness, 1 varusmalunion and 1 subacromialimpingement, 1 superficial infection while in group B, 5 shoulder stiffness, 2 varusmalunion, 3 superficialinfection, 2 wire migration. There were no incidences of non-union & osteonecrosis of the head of proximalhumerus.Conclusion: Based on the analysis of the results, we recommend that ORIF with PHILOS is desirablefor optimal outcome in patients of fracture proximal humerus. However for low demand patient, closedreduction and wire fixation is also an option.

Abstract

Introduction: Alcoholism is the most serious type of substance addiction which includes drinking habitsand unable to control them. It is also called as Alcohol Dependence Syndrome or Alcohol use disorder.Aim: To determine the efficacy of role play on ill effect of alcoholism among college going student.Materials and Method: Purposive sampling was used to collect data from college going students by usingstructured knowledge questionnaire regarding ill effects of alcoholism with their selected demographicvariables.Results: The mean pre-test knowledge score was 12.27 and mean post-test knowledge score was 25.19.There was a statistically significant improvement in the level of knowledge regarding ill-effects of alcoholismamong college going students. The tabulated value for n=100-1 i.e. 99 degrees of freedom was 1.98. Thecalculated ‘t’ value i.e. 58.96 are much higher than the tabulated value at 5% level of significance foroverall knowledge score of college going students which is statistically acceptable level of significance andnoassociation was found between post-test knowledge score with their selected demographic variables.Conclusion: The demand of increase knowledge regarding ill effects of alcoholism is important amongcollege going students because they are more prone to consuming alcohol due to recreation, experience andspecially peer pressure

Abstract

Background: Obesity comes with a huge number of co-morbidities such as type 2 diabetes mellitus (T2DM)and cardiovascular diseases. The most usually used factor for segregating different degree of adiposity isBody Mass Index (BMI) and Waist Circumference (WC).Despite being a convenient and commonly usedmethod it doesn’t show the occurrence of essential obesity-related co-morbidity & deplete quality of life.Hence, Edmonton’s Obesity Staging System has been proposed which incorporates such co-morbidities andfunctional status of individual. Serum levels of high-sensitivity CRP (hs-CRP) can be calculated using highlysensitive assays and has ability to detect augmented inflammation in vasculature. Thus, chronic systemicinflammation linked to the metabolic syndromes. The Study aims to correlate such factors.Aims: To estimate hsCRP levels in obese population.Material and Method: After thorough anthropometric and biochemical evaluation obese individuals willbe classified accordingly into Edmonton’s obesity staging system and their hs-CRP will be evaluated andthen correlation between two will be done.Result: A linear correlation between hs-CRP and Edmonton’s Obesity Staging System is expected.Conclusion: With the help of correlation between hs-CRP and EOSS co-morbidities of obese individualscan be tracked efficiently
