Abstract

Background: Ayurveda advocates fumigation as a method of sterilization and therapeutic tool for differentinfective diseases and mental disorders. However, it is not popular and proven scientifically hence there isgreat scope and prospects for research. For patient safety, fumigation is a regular protocol.Aim and Objectives: It is the need of the hour to search a safe, cheap, effective and easily available drugsfor fumigation hence Maheshwar Dhoop-MD was chosen and compared its efficacy with control- Bacillocid.Material and Method: Study was performed in Shalakya (eye-ENT), Panchkarma procedure rooms andminor operation theatre by pre-post swab and Agar plate exposure on 2nd (MD) and 4th week (Bacillocid)monthly once each for total three months. The microbial load was estimated for different micro-organismslike bacteria, fungus etc and subjected to statistical analysis.Observation and Results: The study revealed that the effect of Dhoopana was significant in comparisonwith Bacillocidto control both during study period and follow up period up to 6 days against bacterias andfungus. Conclusion: It was found that MD is safe, cheap and equally efficient as Bacillocid (p<0.0001) withone-week residual effect.

Abstract

Background: In this modern era life is full of glamour and faced many health related problem. In modernscience there are huge numbers of drugs said for treatment of diseases but these drugs get resistance to thatparticular disease. Traditionally medicinal plants have been used for many years as topical and internaladministration in the treatment of fungal and bacterial diseases. In Rastarangini Gunjadya Taila mentionedin Kushta and Kandu. In Ayurveda all skin diseases are involved in Kushta Roga. Skin diseases mainly causedby microorganism as per modern science such as Staphylococcus aureus, streptococci and Candidiaalbicanetc. Hence to provide scientific data and statistical validation the study on pharmaceutical analysis and invitro antimicrobial activity of Gunjadya Taila prepared by Murchchhita and Amurchchhita Tila Taila isundertaken.Objective: To study antimicrobial activity of Gunjadya Taila prepared by using Murchchhita andAmurchchhita Tila Taila.Material and Method: Gunjadya Taila will be prepared in Rasashastra department by two method onewith Murchchhita Tila Taila and other with by using Amurchchhita Tila Taila. The prepared drug GunjadyaTaila will be evaluated for organoleptic characters and physicochemical parameters. Gunjadya Taila willbe assessed for antimicrobial effect on gram positive bacteria,gram negative bacteria and fungal group.Inhibition of microbial growth will be investigated by diffusion method.Observation and Results: The data will be assessed for its antimicrobial and antifungal effects by usingANOVA test & unpaired ‘t’ test.Conclusion: This research work on Gungadyataila will show all analytical parameters within range andsignificant effects on gram positive and gram negative bacteria.

Abstract

Summary: Pemphigus folliculitis or Pemphigus foliaceous is one of the autoimmune disorders in whichsuperficial blisters are induced by IgG (mainly IgG4) auto-antibodies which are directed against desmogleini.e. molecule of a cell adhesion, which occurs in the granular layer of the epidermis in Pemphigus folliculitis,this special response as a result of pathogenic & non-pathogenic antibodies produces specific blisters in thislayer of skin. Though it found to be rare in the world’s population, it doesn’t respond satisfactorily withcurrent established treatment. Considering its pathophysiology, lifestyle modification plays an essential rolein its prevention & management. Therefore, this review study is planned with the aim & objective to reviewthe lifestyle modification w.r.s. pathya & Apathya (Ahara & Vihar) for Pemphigus folliculitis with Ayurvedicperspective. Pemphigus folliculitiscan be correlated with Visphotaka (type of Kshudra Kustha Roga) in theAyurvedic perspective. The concerned material related to topic has been reviewed from various modern aswell as Ayurvedic literature,including Samhita and numerous review articles in different journals. Ayurvedai.e. ancient system of medicine described the psychosomatic platform for the healthy lifestyle including thepharmacological and non-pharmacological measures, even for the prevention as well as management forKustha i.e. dermatological disorders,which are always found successful in checking over the pathogenesis ofthis disease. More clinical research studies are expected to prove their efficacy on scientific clinical ground.

Abstract

Vamana Karma (medicated emesis) is the act of eliminating the vitiated Kapha through the mouth. VamanaKarma is the internal purificatory practice described in Ayurveda which helps to clean the body of excessamount of the vitiated Kapha, Pitta Doshas consecutively. Tamak Shwas means a state in which a sense ofdarkness prevails due to movement of Prana Vayu in wrong direction. It is a disease in which the patientexperience severe symptoms of respiratory distress resemble like Bronchial asthma. Bronchial asthma isa syndrome of variable airflow obstruction. Vamana is the first line of treatment for Utkleshitavastha ofTamakshwasa. In Ayurveda, Shaman and Shodhana treatment is mentioned. We can’t deny the importanceof Vamana Karma in Tamakshwasa, because Kapha is the foremost Dosha that needs to be taken care of thanothers. In this article an attempt has been made to search the research works conducted on Vamana Karmarelated to Tamakshwasa and to establish its role in TamakshwasaData Source: The related data were collected from various research article from the years 2011 – 2019 andfrom the classical texts Charak samhita, Sushrut samhita and Astang hrudaya Observations: Total 16 variousreaserches found and overall outcome is vamanakarma is effective in tamakshwasa. Abstract

Vitiligo is a skin disease with unknown etiology and has unpredictability on the extent and rate of spread. Itis considered an autoimmune disorder that destroys melanocytes in the skin characterized by depigmentationor hypopigmented patches. Vitiligo patients feel distressed and suffer social stigma as it has a significantimpact on the quality of life of patients. Treatment available in contemporary medicine has its limitationsand side–effects. Hence, maintenance of health and prevention of disease is of utmost importance. Ayurvedahas incorporated this condition into broad terms of Shwitra, which is described under the heading of Kushta.Ayurveda states the causes of Shwitra may be Chhardivegadharan, Atisewan of Amla–Katu-Ushna–LavanAaharsewana, Virrudha Ahar and Atimatra Bhojan along with Navanna Sewana and Dadhi Sewana. Hence,a multidisciplinary approach is needed to treat such conditions with lifestyle medications and dietaryinterventions. Ayurveda has a holistic and scientific method in health management. It emphasizes muchmore on diet and regimen, along with medicines. Ayurveda deals with Pathya –Vyavastha in a very scientificway. Here an attempt has been made to compile the knowledge of ancient holistic science to throw lightupon the concept of Ayurveda regarding the modification of lifestyle and the preventive measures effectiveconcerning Vitiligo which will be a subject of benefit to humankind along with the awareness of the factthat the judicious use of food and healthy lifestyle interventions are the real medicines for health, so earlyresponses are necessary for better results.

Abstract

Background: The root canals of mandibular molar teeth show a wide variation in its system ranging fromsingle to multiple canals. Thorough understanding of the variations present in internal anatomy of teethis imperative for favorable outcome of the endodontic treatment. The insight of the tooth morphology,variations present related to its root structure, shape and information regarding root canal, with number oforifices and canal configuration, all this should be assessed prior to commence the endodontic treatmentCase Presentation: The present case report describes the variation in second mandibular molar, which isinfrequently mentioned in literature. Variation present may not always be in the form of extra canal, it canbe in the form of fewer or lesser canal than what is usually seen. On inspection of the pulpal floor withendodontic explorer, a single round orifice was seen at the centre of the floor of pulp chamber. Numerousangulated radiographs were obtained to assure the presence of independent single canal. Individual canalpreparation was carried out using ProtaperNiTi instruments (Maillefer, Dentsply) till F5. Using lateralcondensation technique along with sealapex sealer, obturation was carried out.Conclusion: Knowledge of the number of root canals present within the tooth is imperative for successfulroot canal treatment. Some teeth may show extra canals, while other may show fewer than normal numberof canals present. Mandibular second molar may present with single canal and this should be promptlyrecognized. Single canal identification helps to conserve the tooth structure by eliminating the need forremoval of excessive tooth structure in search of extra canal.

Abstract

Background: Calcification of root (calcific metamorphosis) is usually seen as defensive reaction of pulp totrauma. Intensity of trauma/hit, time span since trauma, pulp response to trauma are multiple factor decidingdegree of calcification. Calcification may be seen partially or entirely in root canal. Usually calcifiedtooth remain asymptomatic, but when tooth becomes symptomatic endodontic treatment need to be done.Treatment of calcified canal is challenge to dentist.Case report: A female patient reported to the department of conservative dentistry and endodontics at sharadpawar dental college with chief complain of pain in upper front region of jaw. On radiographic examinationcalcified root canal was associated with upper anterior teeth. Root canal treatment was planned for calcifiedtooth. Access opening was carried out using BR45. To achieve patency within canal file#8, 10 were used, butinitally patency could not be achieved due to calcified canal. With help of chelating agents 17% EDTA liquidand gel and small size K file #8,10 canal negotiation was carried out to achieve patency. Working lengh wasthen determined and canal was instrumented. Naocl, EDTA was used as intermittent irrigant. Final rinse wasdone using 2%CHX. Canal was obturated using cold lateral condensation technique followed by permanentrestoration. Entire procedure was carried out under magnification.Conclusion: Calcified canal treatment is bit tedious then routine endodontic procedure. But it can bemanaged if procedure is carried out under proper illumination, magnification, CBCT, understanding rootcanal anatomy, use of dye, modified access bur, files, chelating agent, irrigants and irrigation activationsystem.

Abstract

Background: Preservation of pulp vitality is of utmost importance during management of deep caries cases.There remains a possibility of pulp tissue to repair upon excavation of caries with no pulpal exposure.However, in case of exposure of pulp, such type of minimally invasive management becomes doubtful& impulsive. Indirect pulp capping(IPC) treatment plays an essential role in these cases to prevent pulpalexposure.Case Presentation: This case report describes the single visit indirect pulp therapy in mandibular molar.Patient reported with a chief complaint of food lodgement and mild intermittent pain. There was no historyof night pain and/or spontaneous pain. The clinical and radiological evaluation revealed no peri-radicularpathosis and no pulpal involvement. The tooth was examined for vitality using electric pulp test and revealedvital pulp. The treatment was done with the protocol of Single Visit Indirect Pulp therapy using light curecalcium hydroxide and cavity was disinfected with 2% chlorhexidine. Cavity was lined with Glass IonomerCement and final restoration was done using Composite resin restoration. The patient was followed up till6 months and showed satisfactory outcome eliminating the need for more invasive approach as root canaltherapy.Conclusion: Indirect pulp therapy is a safe, economical and proven treatment option in the teeth that havenot involved pulp and with sound peri-radicular tissues. The success of Indirect Pulp Capping relies onproper case selection and diagnosis, complete isolation, caries removal and disinfection with selection ofsuitable pulp capping material

Abstract

Background: Kidney involvement is an under-recognized complication of both Hodgkin and non-Hodgkinlymphoma (NHL).1,2 Many patient presents with acute kidney injury in newly diagnosed NHL, but patientbeing diagnosed with primary renal NHL is very rare. As it rarely occurs, it may not be considered initiallyduring patient presentation. In patient presenting with idiopathic kidney failure along with non-specific painin abdomen, primary NHL must be kept in mind. Below we present a patient who presented with pain inabdomen and was later diagnosed as a rare case of primary renal NHL.Case History: A 65 year old male presented with the chief complaints of pain in abdomen since 2 monthsand history of nocturia 3-4 times at night and dribbling of urine since 1 month. There were no significantfindings on general and systemic examination. CT scan revealed a solitary renal mass of 90 × 85 × 61 mmin upper pole of left kidney. Patient underwent Radical Nephrectomy and on histopathology specimen wasdiagnosed as - Infiltration of Non-Hodgkins lymphoma in kidney. Primary renal NHL.Conclusion: Primary involvement of kidney in NHL is extremely rare entity which should be always keptas differential diagnosis in case of radiological suspicion of primary renal mass. NHL is diagnosed onhistopathological grounds and can be first proven by performing renal biopsies in order to start chemotherapywhich is the main modality in treatment of PRL. Recent immunohistochemical tests allow correct diagnosisand also helps in classifying renal lymphomas.

Abstract

Background: GIST are the tumours originating in the gastrointestinal tract with a high malignant potentialand the chance to metastasize and recur. They are widely studied due to their distinctive biologic behavior,clinicopathological manifestations, molecular mechanisms and treatment modalities. They can affect anypart of gastrointestinal tract or extraintestinal sites. Most GISTs stain positive for KIT, CD117 andanoctamin1(DOG-1). Tumorresection along with Imatinib therapy is said to be mainstay of the treatment.Case Presentation: Here by presenting a case of a male aged 60 years operated for cholecystectomy andintraoperatively found to have a solitary nodule over the stomach, which was later found out to be GISTmetastasis to a lymph node on histopathology.Conclusion: GIST has a low incidence and rare tendency to metastasize to lymph node; therefore it isimportant to study clinical importance to lymph node in this malignancy. Lymph node dissection shouldtherefore be kept in mind during resection of GIST.
