Abstract

The study relies on data received during treatment of 128 children and teenagers suffering idiopathicscoliosis. The subjects’ median age is 10,2±0,39 years old. In the treatment group (90 juvenile subjects),treatment of scoliosis has been carried out according to an optimized conservative integrated algorithm withreliance on bone mineral density indicators and the Cobb’s angle. The length of treatment averages 1,5±0,2years (1 to 3 years). The control group (30 subjects) has undergone traditional correction on an outpatient basis with the use ofstandard method: therapeutic exercise, massage, physiotherapy. The length of treatment averages 1.7±0,1years (1 to 3 years).Compared to traditional method, optimized conservative integrated treatment of idiopathic scoliosisdemonstrates a 29% higher occurrence of positive results. Efficacy of brace treatment of moderate andsevere progressive juvenile/adolescent scoliosis has been observed, as it has reduced spinal deformation andeased socio environmental adaptation.

Abstract

The article represents the results of the observation oft he comparative clinical examination of herpeticinfection coupled with HIV infection in 57 children aged 1 to 15 years. There have been 41 boys (71,93%)and 16 girls(28,07%). The comparison group included 23 children oft he same age infected with HIV withoutherpetic infection. The following laboratory research method have been used: a general analysis of blood, urine, feces, thestudy of the immune status, level of CD4 cells; Serological: by ELISA method antibodies to CMV and HSV(Herpes Simplex Virus) have been detected in blood. The results obtained indicate that coexisting diseaseswith the formation of complications and immunodeficiency development have been worsening the course ofherpetic infection in HIV infected children and leading to a severe relapsing course.

Abstract

Introduction: Much attention is paid to pharmacokinetic (FK) researches for studying the processes ofsupply, distribution, biotransformation and excretion of medications at the present time, as well as identifyinglinks between medical substances concentration and (or) its metabolites in biological fluids and tissues, andits pharmacological effect. In the treatment of rheumatoid arthritis (RA), non-steroidal anti-inflammatorydrugs (NSAID) are widely used for a symptom therapy, where Diclofenac Sodium (DS) is most commonlyprescribed. Duration of anti-inflammatory effect, effectiveness of NSAID in whole are directly dependent onthe level of effective concentration of medications and circulation duration in blood in free form. The Aim of the present research work is to study in a comparative aspect certain indicators of DiclofenacSodium pharmacokinetics in patients with rheumatoid arthritis with gastric microbiocenosis disorders(gastric dysbiosis) and without disorders (without dysbiosis).Materials and Method: 38 patients at the age from 18 to 60 years were examined, with I-II-III degree ofdisease activity. In addition to the general clinical examination, an immune-enzyme method and urease testwere carried out to determine Helicobacter Pylori and highly effective liquid chromatography to determineDiclofenac pharmacokinetics. Results: Conducted researches and analysis of their results indicate that in conditions of rheumatoid arthritis, particularly, in the presence of comorbid states, there is a decrease in the metabolism rate and an extension ofNSAID half-excretion period, which increases the risk of side effects, especially to the gastrointestinal tract, and significantly affects the disease course and the results of the treatment. Conclusion: In conditions of rheumatoid arthritis with comorbid states, there is more frequent occurrenceof the NSAID side effects. Research conducted and analysis of their results indicate that in conditionsof rheumatoid arthritis, particularly at comorbid states, there are reasonable changes in the NSAIDpharmacokinetics. The decreasing of metabolic rate and the extension of the NSAID half-excretion increasesthe risk of side effects, especially on the side of the GIT, which significantly affects the disease course andresults of the treatment. This circumstance dictates the need to take these results into account in the treatmentof the studied pathology and in the development of a personalized approach to the treatment of rheumatoidarthritis

Abstract

The causes of postoperative complications in the correction of anorectal malformation were studiedaccording to follow-up, general clinical and additional research method in 65 operated sick children. Inthe process of diagnosis and surgical correction, a number of diagnostics, tactical, and combined errorsand shortcomings were identified that caused postoperative complications, which required a large numberof repeated reconstructive operations. The authors recommend various options for tactical and operational- technical treatment approaches when solving the problems of repeated reconstructive operations. At thesame time, it is proposed to study the causes, analyze errors, complications and failures, indications, terms, accesses, the volume of the operation and evaluate their effectiveness. The authors, thanks to the optimizationof tactics, method of surgical and rehabilitation treatment, were able to improve the results of postoperativecomplications to 81.4%.In the course of the study of catamnestic and examination data 65 of children with postoperative non-NAS inthe process of diagnosis and surgical correction errors and deficiencies were revealed as: unreliable primarydiagnosis, insufficiency of preoperative preparation, inadequate indication and choice of the method ofoperation, inadequate postoperative rehabilitation. All these had diagnostic, tactical, technical and combinedcharacter, which led to the formation of the postoperative HCA, which required a large number of repeatedoperations. Therefore, the authors suggest that to solve the problems of repeated operations is not limitedto the improvement of surgical techniques, and to study the causes, analysis of errors, complications andfailures that lead to repeated interventions, indications, timing, access, scope of surgery and their evaluationof effectiveness, medical and social rehabilitation of patients. The authors, thanks to the optimisation of tactics, method of surgical and rehabilitation treatment, managedto correct the post-surgical NAC up to 80,4%.

Abstract

The article presents the literature data, results of diagnostics and treatment of 219 children with variousmorphologies of spinal herniation and neuroorthopedic disorders. It characterizes in detail motion disordersand deforming arthrosis of the lower extremities in 88.1% and 26% of patients. The authors state that thegenesis of disorders can include the manifestations of spinal cord fixation syndrome in patients with spinalhernias with combined congenital and acquired forms of latent spinal dysraphism. The article confirmsthe feasibility of early preventive and therapeutic measures corresponding to the nature of the combinedpathology, with the participation of specialists.

Abstract

This article presents the specific features of clinical and radiological abnormalities in the mixed dentition.Individuality of such features is determined by the parameters of the maxillofacial region and the maxillarydimensions, in the form of late milk dentition abrasion, shape of the submaxillary retromolar area and theeruption level of the lower first and second multi-cuspid teeth according to the dental panoramic radiogram. All above was used as a basis for the assessment algorithm development for children with the dentofacialanomalies. Based on the results obtained, the recommendations has been proposed that allow to performindividual treatment for the children with this pathology.

Abstract

Methemoglobin (MetHb) is a physiological blood component that performs protective functions andneutralizes the endogenous hydrocyanic acid, hydrogen sulfide and other poisons. MetHb is slowly formedin the blood, its surplus is constantly restored while the content in blood remains low.The spectrophotometric method has been used to measure the methemoglobin concentration in 72 traumaticsubdural and epidural hematomas of various prescriptions. Based on the data obtained, the limitation ofsubdural and epidural hematomas has been determined by the methemoglobin concentration. The results arerecommended for use in the forensic expert examinations.
