Abstract

Objective: To evaluate the effect of facilitated tucking on physiological and behavioural responses to painamong infants between experimental and control group before and after the IM vaccinationMethod: A quantitative approach with true experimental design and consecutive sampling technique wasused in this study. Researcher selected 100 infants up to the age of 6 month receiving IM vaccination inthe immunization clinic, AIMS, Kochi, and they are randomly assigned to control and experimental group.Demographic and clinical data was collected using structured interview schedule. Heart rate and SPO2was monitored 2 minutes prior to the vaccination and facilitated tucking was given to the experimentalgroup before and during the immunization and routine position was given to the control group, after thatbehavioural response was assessed by using neonatal infant pain scale and physiological response such asheart rate and SPO2 was assessed after 5 minutes.Result: The result showed that all the subjects in the control group had severe pain 100 %, whereas in theexperimental group 50 % of the subjects had moderate pain and 50% of them had mild pain. Also there wasa significant difference in the mean score of the physiological and behavioural response between the controland experimental group after the interventionConclusion: The study revealed that the Facilitated Tucking improves the emotional security and reducesthe pain perception. The nurse or care givers can effectively implement facilitative tucking for reducing painduring vaccination in any setting as it is a very simple technique

Abstract

Purpose: State anxiety is an unpleasant psychological state experienced by cricketers due to perceivedperformance stress. It is often an overlooked criterion for return to sports after an injury. There is a need toevaluate presence of anxiety as it leads to physical symptoms and hence injury. Mental training is a veryimportant tool for handling anxiety but there is lack of training which further predisposes players to reinjury.This study aims to evaluate presence of anxiety in cricketers who have suffered sport-related injuryand have returned to play. It also explores the method used by these cricketers to handle anxiety.Method: 60 injured cricketers were selected based on inclusion and exclusion criteria. A baselinequestionnaire was prepared to collect basic demographic data and Sports Anxiety Questionnaire (SCAT)which is a self-administered scale was included to determine presence of anxiety. Anxiety scores werecalculated to determine the players level of anxiety.Results: It was found that 65% players experience high anxiety, 32% had average anxiety and 3% had lowanxiety levels. 45% of players reported re-injury.Conclusion: Undiagnosed anxiety was found to exist in injured cricketers which may predispose themto re-injury. All the players were aware of mental training techniques but reported lack of guidance andprofessional help.

Abstract

Vasculitis where underlying cause is not known is often related as Kawasaki Disease. It’s important toknow about such auto immune conditions as it directly affects the vessels of the heart and is responsible fordeath at an early age. The peculiarity of the disease is its characteristic oral manifestations which are oftenmisdiagnosed as an oral disorder. Dental examination has a vital role in improving the diagnosis of autoimmune conditions as auto immune conditions primarily manifest oral signs in their early stage. So, earlydiagnosis can drastically improve the treatment outcome of the disease and quality of life.

Abstract

The analysis of the inspection of the employees of Uzagrotehmash JSC was performed. A total of 1495 peopleaged 25-75 years have been examined (average age 46.3±0.3 years). The frequency of hypertension was 598people. High blood pressure was recorded more often in women (25.9%) than in men (16%), hypertensionis much more common at the age of 44-60 years and older, and hypertension has been found to be commonin overweight people (31, 6%), a large proportion was found in the structure of hypertensive patients withhypodynamia (50.2%). Among the protesters, 26.9% of people know about high blood pressure, but do nottreat it, and in 20.0%, high blood pressure was detected for the first time. The role of serum lipoproteins inthe hypertension development was confirmed, which allows patients to be assigned to a high-risk group forthe formation of cardiovascular diseases in subsequent life periods.

Abstract

The article is devoted to the study of the professional activities characteristics of university teachers, thepossible risk factors that affect the state of teacher’s health in the course of his work are considered, solutionsfor protecting the health of the teaching staff and the increase in the labor efficiency of teachers of highereducational institutions are proposed.It was revealed that the actual time costs of teaching staff exceed the established standards of the teacher’sworking time; however, the teacher’s work does not have clear regulation (except for scheduled classes);all types of work performed take up 95% of the working time and are integral elements of the teacher’sactivities. A general assessment of working conditions by the tension level, the harmfulness and dangerdegree, performed in accordance with SanPiN 0141-03, considering the most significant factors (the laborprocess intensity), allows to classify the labor of university professors in our republic as class 3 (harmful),3 degrees of harmfulness

Abstract

The goal of research was to study the cytokine status in women with a combination of fibroids andadenomyosis to determine their role in the course of the disease. Method 165 women with uterine myomaand/or adenomyosis were examined. Patients were divided into 3 groups: 1-group - 75 women with acombined form of myoma and adenomyosis, 2-group - 47 women with diffuse adenomyosis, 3-group - 43women with uterine myoma. The study was conducted in 2017-2018, on the basis of the Tashkent MedicalAcademy in the department of gynecology of the obstetric-gynecological complex No. 9 of Tashkent city.The research method were a dynamic study of node blood flow by ultrasound with dopplerometry and animmunological blood test. For myoma 5 mg ulipristal acetate was prescribed for treatment for 3 months forat least 2 courses by 3 months with a 2 month break, and for adenomyosis - 2 mg dienogest for 6 monthscontinuously. For the combined form of myoma and adenomyosis, treatment was started with dienogest for6 months continuously, then UPA was prescribed for 3 months.Results: The determination of the level of pro-inflammatory cytokines in patients before treatment showedthe following results: the level of IL-1, IL-6 and TNF-? was higher in all three groups than in the controlgroup, and in the group with combined pathology IL-6 was 1,5 times higher than in groups with isolatedpathologies. After therapy, the decrease in the levels of IL-1, IL-6, and TNF-? was observed in all threegroups. Also the level of anti-inflammatory cytokines IL-10 and VEGF exceeded significantly the controlgroup, especially in the group with combined pathology, after the treatment, their level decreased significantly.Discussion: An increase in the level of cytokines in the blood is most pronounced in combined forms ofmyoma and adenomyosis compared with isolated forms of myoma and adenomyosis.

Abstract

Introduction: There are several forms of successful treatment and prevention for obstruction of thepyelourethral segment in pediatric patients. However, each method has its advantages and disadvantages.Namely, some those widespread surgical techniques frequently associate with different complications andsubsequent financial deprivations. Therefore, the optimal method of external trans-anastomotic drainageis still disputable. In this study, we studied the results of pyeloplasty in children using a transanastomoticcatheter.Method: In this observational study, which was conducted from 2013 to 2018, we evaluated the outcomesof pyeloplasty in 132 childrenunder 3 years old. For all patients a trans-anastomotic catheter for urinaryevacuation was used during the postoperative period. The observation period ranged from 6 months to 3years (an average of 22±6,8 months). Antegrade pyelography was performed in 119 patients on the 6th dayof postoperative period. The average hospital stay was 8.4 days (ranging from 6 to 16 days), most of thepatients were discharged from hospital 7 days after surgery.Results: 137 pyeloplasty was performed on 132 patients using a transanastomotic catheter. The observationlasted from 6 months to 3 years. Antegrade pyelography was performed in 119 patients on the 6th day aftersurgery. Of those 119 pyelograms taken, 108 (91 percent) showed adequate drainage after pyeloplasty. In 11patients, partial obstruction was revealed, while in 8 (73 percent) children the possibility of the anastomosisreturned to normal on its own about within 12 days. The transanastomotic catheter was removed on average7-8 days after surgery. The average hospital stay was 8.4 days.Conclusion: Urine evacuation with transanastomotic catheter after pyeloplasty in pediatric patients ishighly effective and safe. Other comprehensive studies may be required to prove the further upsides of thistechnique.

Abstract

Introduction: Significant allergic reactions and anaphylactic type cytotoxicity are registered in thepathogenesis of cell damage in pregnant women with hypertensive conditions. Determining role ofindirect mast cell degranulation and allergic changes in neutrophils during pregnancy that complicated bypreeclampsia, allows to identify the allergic status of the body, which is very importance for improvingthe quality of early diagnosis, treatment and prevention. The purpose of the study was to identify theallergic reactivity of pregnant women suffering from hypertensive disorders and to develop practicalrecommendations to improve the quality of early diagnosis.Materials and Method: We observed 6 group of participants: pregnant women with clear allergy clinic,uncomplicated preeclampsia, pregnant women with light stage of preeclampsia, latent sensitization, pregnantwomen with preeclampsia combined with clear allergy clinic, conditionally healthy pregnant women – 1stcontrol group, practically healthy non-pregnant women – 2nd control group, practically healthy men – 3rdcontrol group. Using non-infection allergens, we called different types of allergic reactions and conductedclinical and allergic examinations, general test of bleed, urine, feces, indirect mast cell degranulation testand neutrophil alteration test.Results: Within all control groups, the rate of positive reaction against the allergic agents made up less than10 percent. In all pregnant women with clear clinical picture of allergies complicated or uncomplicatedpreeclampsia, as well as in pregnant women with preeclampsia with latent sensibilization, mast celldegranulation indexes were 4.5-8.6 times higher than that of control group. Among 90 examined pregnantwomen, positive reactions of indirect mast cell degranulation were observed in 204 cases.Conclusion: In cell damage pathogenesis during preeclampsia, anaphylactic and cytotoxic type allergicreactions play important role. The positive reactions of indirect mast cell degranulation in pregnant womenwith an explicit allergy complicated by preeclampsia is significantly higher than in pregnant women withpreeclampsia with hidden sensitization.

Abstract

Introduction: The number of HIV infected children exceeds 3 million worldwide. The most severe clinicalmanifestations and complications of the disease are HIV-associated neurocognitive disorder and HIVencephalopathy. These factors make the scientists search for method of optimizationin the diagnosis ofneuro-HIV and identifying various pathological processes that determine the clinical course.Materials and Method: 154 children were tested with HIV diagnosis. All patients were examined forneurological, psychological and cognitive disorders. Quality of life of sick children were examined by thequestionnaire of PedsQL. In order to compare the results, study included 20 healthy adolescents (controlgroup - CG), who also tested using the above method. Using the MS Excel software average valuessummarized and their standard deviations were calculated.Results: All patients were diagnosed with 3rd and 4th stages of HIV. HIV-associated neurocognitive disorder:grade 2 in 100 patients – 64.94% and grade 3 - in 54 patients - 35.06%. Epilepsy was diagnosed in 31 patients,peripheral polyneuropathy in 7, vacuolar myelopathy in 8, and consequences of acute cerebrovascularaccident in 9 with focal neurological deficit in 2. The Raven test: average test result was 18.68±4.51%.Aggressive behavior: negativism prevailed in 74 patients, and in 11 cases (7.14%) it was combined withphysical aggression, and in 12 cases (7.79%) - with resentment. In addition, indirect aggression was detectedin 18 patients (11.69%), resentment in 41 patients (26.62%), suspiciousness in 10 patients (6.49%), and 19patients (12.34%) - irritation. Quality of life questionnaire revealed difficulties associated with physicalactivity (18.30±4.51 with maximum score of 32 points), with emotional states (15.50±2.23 with maximumof 20), social activity (14.10±2.93 with maximum of 20) and life in school (19.29±1.23 with maximumof 20).Conclusion: HIV-associated neurocognitive disorder is manifested by a deterioration in intelligence, variousforms of aggressive behavior and is associated with a decrease in the quality of life in terms of physical,emotional, social activity and learning at school.
