Abstract

Vascular pathologies are considered as severe complications of diabetes mellitus, which occur veryfrequently in patients. Namely, diabetic retinopathy, being one clinical form of diabetic angiopathy, maylead to distressing socioeconomic burden like decreasing visual acuity and blindness. In the literature, manytreatment method have been described and used in many centres.We present a case of 60-year-old diabetes sufferer, who underwent combined treatment strategy includingpanretinal laser coagulation, enzyme therapy, vitreoretinal surgery and intravitreal administration of aninhibitor of angiogenesis, which was highly effective in terms of restoring visual acuity with minimalperiprocedural complications.

Abstract

In recent decades, in many countries of the world there has been an increase in the incidence of colorectalcancer (CRC). The annual occurrence of 50 new cases of colorectal cancer per 100,000 population determinesa 5% population risk of developing the disease throughout life 1. An alarming fact is that for the 100 newlydiagnosed patients with CRC, more than 70 deaths occur, of which, in the 1st year from the moment ofdiagnosis, about 40%. This circumstance is due to the fact that when patients first go to the doctor, advancedforms of CRC (stage III-IV) are diagnosed in 62.4% of patients. In 25-30% of patients, distant metastasesare detected at the time of surgery. In recent years, there has been a tendency to expand indications fororgan-preserving operations in the CRC.At the same time, it is necessary to formulate clear criteria fortheir implementation on the basis of a comparative analysis of long-term treatment results, the frequencyof local regional relapses, the development and implementation of techniques using staplers, as well asthe justification of indications for the use of combined and complex treatment programs that increase theradicalism and ablasticity of surgical interventions.

Abstract

Under conditions of unilateral occlusion of the ureter, the development of microhemodynamic disorders ofthe cortical substance of the kidneys was studied both on the damage side and on the contralateral (opposite)side. It has been established that disorders on the side of occlusion depend on the duration of obstructionof the lumen of the ureter, which determines the development of tissue hypoxia and violates trophic renalparenchyma. Dynamically functional disorders of the microvasculature occur at the early (on the 1st, 3rd,and 5th day), and irreversible changes in the renal parenchyma at the later (on the 14th and 30th day) stagesof the experiment. The contralateral one responds to turning off the function of one kidney by adaptiverestructuring of the microhemocirculatory system, which is manifested by dilatation of microvessels and anincrease in the linear velocity of blood flow in them, which ensures its hyperfunction.

Abstract

In recent decades, there has been a significant increase in the number of patients with thyroid disorders[1,4].The widespread introduction of highly informative diagnostics method (ultrasound, fine-needle aspirationbiopsy, tests on hormones and levels of antibody) and screening studies into medical practice led to anincrease in detection rate of thyroid disorders[4,5,8]. Among patients with thyroid pathology, the thyrotoxicosissyndrome significantly complicates the course of thyroid disorders, which makes it difficult to choose arational treatment strategy, affects the long-term result of treatment, and significantly worsens the diseaseprognosis[8,10].

Abstract

In modern cardio surgery, it has become possible to predict not only the outcome of surgical interferencein order to save life, but also its effectiveness in terms of the quality of life improvement. It is proved thatcoronaro correcting technologies can not only soften the clinical picture of chronic coronary insufficiency,but also increase the contractility of the ischemic myocardium, as well as significantly improve the patient’squality of life, if the myocardial ischemia has the character of hibernation, and not stanning. The ability todetermine which variant of the ischemic syndrome occurs in the patient before surgery helps to predict theinterference effectiveness. The method used for this purpose (radioisotope scintigraphy, positron emissiontomography, MRI, pharmacological stress echotests) are technically or economically inaccessible, difficultto apply in the perioperative period, and sometimes dangerous.The purpose of the research: to offer an informative, accessible and safe method of preoperative identificationof the hibernating myocardium among patients with IHD for successful prediction of the interferenceeffectiveness.Materials and Method: 83 patients with acute and chronic forms of IHD were under study. In the first group(49 people), there was a coronary stenting (CS). In the second group (34 patients), there was a heart bypasssurgery (HB). There was the comparative assessment analysis in regard to variant of ischemic syndromeand the effectiveness of both operations by means of precardial ECG mapping and stress echotest withdipyridamole.Results: In case of IHD acute forms (acute coronary syndrome, acute myocardial infarction) CS and HBoperations had significant positive dynamics in regard to indicators of coronary blood flow within thecourse of ischemia and myocardial contractility in accordance with precordial mapping (area reductionof ischemia - AST and the decrease of the ischemia degree - ?ST) and stress echotest with dipyridamole(increased contractility of EF indicator and a reduction of ESV, increase in the number of eukinetic segmentsof the myocardium and decrease of hypokinetic segments). We observed the same dynamics, but much lesspronounced, in accordance with ECG mapping and stress echotest data after both operations within thegroup with chronic forms of ischemia.Conclusion: The precardial ECG mapping is informative and available for use in regional cardiosurgicalclinics as a method of the hibernating myocardium identification, which is a more perspective variant ofischemia in case of IHD in terms of left ventricular contractility increase.

Abstract

The damage features of the cerebral hemodynamics disorders and decreased responsiveness of brainvessels among the patients with HIV are the pathogenetic basis for the formation of HIV encephalopathydevelopment. We observed patterns of changes in cerebral blood flow and immunological reactivity amongpatients at preclinical stages of HIV. It indicates that the vascular system of the brain is highly sensitive tothe action of treponemous toxin and is responsive to damages at the early stages of the disease.

The article discusses the issues of small anomalies of heart development in young athletes. The question ofthe admission of young athletes with small anomalies of heart to sports and their impact on the state of thecardiovascular system during exercise is still an open question. To avoid subjectivity and the subsequent overdiagnosis of small anomalies of heart, their objectification is necessary, through a quantitative assessment.

Abstract

Using the express method of a thick smear and morphometry, the state of erythrocytes obtained from theacne and finger region has been studied. Using laser Doppler flowmetry, the state of microcirculation of theskin of the finger and the acne location area has been studied.It has been found out that comprehensive treatment with the use of FarGALS promotes the reduction inblood of pathological forms of erythrocytes and the restoration of microcirculation parameters.

Abstract

Background: Elderly people have a variety of medical problems and functional limitations. Most older peopleexperience multiple chronic illnesses as they age, which is one of the reasons for reducing of their physicalactivity. Undoubtedly, one of the ways to prevent the occurrence and exacerbation of chronicdiseasesis tomaintain a healthy lifestyle, which increases the period of active life and helps support physical activity.Method: We used a random selection method to obtain reliable and representative data: cluster and stratifiedselection. In Tashkent city, we selected 2 clusters out of 11 clusters, where the proportion of elderly andsenile people was the highest and 674 elderly were included into the research; in the Tashkent region 3clusters were selected, and 473 elderly were included to our sample. Our sample was divided into 3 socialcategories: the first category included lonely elderly people living alone in their apartments in the amountof 310 people, the second one included lonely elderly people living in nursing homes for the elderly, in theamount of 240, and the 3rd category included elderly people living with their families, in the amount of597 people. In order to assess the respondents’ lifestyle, we studied their daily routine, dietary regimen andnature, leisure, bad habits and physical activity.Results: Married respondents were 2.1 times more among elderly living with their families, and 5.7 timesmore amongelderly living alone in their apartments compared to elderly living in nursing homes. Lonelyelderly of “middle-old” age are in more difficult state due to significant proportion of this age-category sufferwith various disabilities. It is find out that most respondents of the research do not pay proper attention toa balanced diet. Every second elderly person in our study suffered with obesity and had an increased bodymass index (BMI: 25.1<). More than half of the respondents (54.6 + 3.2%) from nursing homes smokedtobacco products 3 times more than those living with their families. Alcohol consumption among olderpeople living in nursing homes was 6 times higher compared to older people living with their families. Withan increase of the age, the number of elderly engaged in any kind of physical exercise decreased sharply.Conclusions: The study and analysis of the socio-demographic characteristics and lifestyle of elderly inUzbekistan revealed that divorce and loss of spouse are negative factors, whereas the presence of a familyand the residence of elderlywith relatives have a positive impact on their lifestyle and, accordingly, on theirhealth status.
