Abstract

Breastfeeding provides both short-term and long-term benefits for both the child and the mother, including protecting the child against a variety of acute and chronic disorders. However, the high incidence of CS and the low number of breastfeeding mothers with cesarean delivery have a lot of negative effects on the breastfeeding process. Data obtained from Dairi district in 2021 showed that the coverage of breastfeeding was only 40%, while in Sidikalang Hospital, post-sc mothers gave breast milk. only 35%. The purpose of this study was to determine the effect of factors that influence the release of breast milk for post SC mothers at Sidikalang Hospital, Dairi Regency in 2021. This type of research was mixed method with sequential explanatory design. The population is 120 respondents and purposive sampling is 54 respondents. 3 key informants representing Post SC mothers , 8 supporting informants. Data analysis used univariate, bivariate and multivariate analysis as well as qualitative data analysis including reduction, display and conclusion drawing/verification. The results showed that there was an effect of breastfeeding frequency (sig 0.000), breast care (sig 0.002), mother's diet (sig 0.000) breastfeeding technique. (sig 0.000) the SPEOS method (sig 0.000 ) , while there was no effect of hospitalization (sig 0.210), birth weight (sig 0.201), gestational age (sig 0.201), the most influential variables were maternal food with a value of B = 4.084 and OR = 59.93 for the expenditure of breast milk for post SC mothers at Sidikalang Hospital, Dairi Regency in 2021.

Abstract

Very few studies have been conducted in the ENT profession that did not include the use of an ENT expert. For this cross-sectional study, we sought to determine the level of activity in the pediatric ear, nose, and throat department. This was a prospective, cross-sectional research including ENT functional unit patients who had been observed for a period of time. A patient's age ranged from 7 months to 180 months during a consultation. A sex ratio of 1.2 (61/51) was recorded. With 33.5 percent of the patients, pharyngolaryngeal disease was followed by otological, rhinological, and other pathologies with 30 percent and 26.88 percent, respectively. Pediatric ENT has a significant role in the field of ENT. As a result of our research, we were able to demonstrate the wide range of services available in pediatric ENT pathology in remote areas. It is clear that these findings support the idea of distributing specialist services more widely. As a result of these findings, it is recommended that ENT services be decentralized in order to better serve this vital segment of the population.

Abstract

Haemodialysis patients are more likely to suffer from hyperkalaemia than the general population. It puts them at danger of a fatal accident. In haemodialysis, it is not part of the standard examinations. The purpose of our study was to find out how common it was among individuals on long-term dialysis. Our research was cross-sectional in nature. Participants had to have been on haemodialysis for at least two months and be willing to participate in the research. Before the start of their first and second weekly dialysis sessions, we did a clinical examination, blood potassium, sodium, and ionized calcium tests, and an electrocardiogram in each of them. 88 patients were chosen for the study. Before the first dialysis session began, 38.7 percent of patients had hyperkalaemia, and 17 percent had it before the second. In three cases, the blood potassium level was determined to be at least 7.5 mEq / l, indicating that the patient was suffering from a condition known as paresis. Hyperkalaemia is associated with a tall T wave on the ECG in two circumstances. Although hyperkalaemia is widespread in our dialysis patients, its clinical and electrocardiographic symptoms are less so.

Abstract

Infection Prevention and Control (PPI) is an effort to prevent and minimize the occurrence of infections in patients, staff, visitors and the community around health care facilities. Nosocomial infection control is an activity of planning, implementing, monitoring, and fostering with the aim of reducing the incidence of nosocomial infections. The purpose of this study was to determine the description of infection prevention and control in the internal room of the Prof. Dr. Aloei Saboe Gorontalo City. This study used a qualitative descriptive research method with a sample of 15 respondents obtained using random sampling technique. The results showed that the infection prevention and control program in the internal room had not been fully implemented, seen from 15 participants there were 3 people who did not practice handwashing properly where the 3 respondents did not apply the 6 steps of hand washing according to the SOP, there was 1 person who did not using PPE in the procedure, overall able to apply coughing and sneezing etiquette, there was 1 person who did not apply safe injection practices in the procedure.overall able to apply Decontamination of equipment after completion of actions.

Abstract

Postpartum gymnastics is exercise that mothers do after giving birth which aims to maintain and increase maternal circulation during the puerperium, as well as to help uterine involution. The purpose of this study was to determine whether there was an effect of postpartum exercise on the decrease in uterine fundal height. The type of research used in this study was quasi-experimental with a pre-test post-test control group design, to analyze the effect and differences in fundal height reduction between the intervention group and the control group. So it was determined the number of samples that met the criteria as many as 20 people. The results showed that based on the results of the mean difference test, there was a significant difference in the decrease in uterine fundal height between pre and post in the postpartum exercise group and not in the postpartum exercise group with p-value <0.05 so it can be concluded that there is an effect of postpartum exercise on the decrease in uterine fundal height. on post-partum mothers. There is an effect of postpartum exercise on the decrease in uterine fundal height in postpartum women in the Ulaweng Health Center Work Area

Abstract

Hemostatic parameters of pregnant women with hypertensive disorders in owerri were studied using standard methods. The parameters include PT, APTT, Fibrinogen, t-PA, D-dimer. A total of 200 pregnant women between ages of 18 and 45 years of age were recruited for the study. They consisted of 50 normotensive pregnant women as the control group and 150 hypertensive as the test subjects. The test group was further divided into 3 viz; 50 chronic hypertensive, 50 gestational and 50 preeclamptic pregnant women as group as groups 1, 2 and 3 respectively. The mean PT(secs.) values were 11.69±1.02, 11.98±0.94, 12.79±0.78 and 13.86±1.4 respectively, in control, groups 1, 2 and 3. The APTT(secs) mean values in control, groups 1, 2 and 3 were 28.59±2.33, 29.43±2.67, 30.20±2.73 and 31.52 respectively. The mean values obtained for D-dimer (ng/ml) were 191.72±41.92, 207.30±60.71, 249.52±62.08 and 268±59.51 in control, groups 1, 2 and 3 respectively. In control, fibrinogen (mg/ml) mean value obtained was 521.74±118.02, where as in the test groups, 532.97±111.40, 602.52±103.80 and 671.98±97.37 were respectively obtained in groups1, 2 and 3. The mean respective values of t-PA (ng/ml) in control, group 1, 2 and 3 were 2.43±0.55, 2.49±0.49, 2.84±0.54 and 2.90±0.57. All the haemostatic parameters: PT, APTT, Fibrinogen, D- dimer and t-PA values increased across the test groups when compared with the control groups and they were found to be statistically significant (P=0.0001). From this study, haemostatic parameters increased in the test subjects indicating inflammatory activities in hypertensive disorders of pregnancy.

Abstract

The Covid-19 pandemic has made many changes, especially in the health sector. During this Covid-19 pandemic, pregnant women are required to have their pregnancy checked by a midwife or doctor, at least 6 visits. The Covid-19 pandemic has led to a decrease in visits by pregnant women to facilities due to several factors. The purpose of the study was to analyze the factors that influence the compliance of pregnant women in conducting ANC visits during the Covid-19 pandemic. This type of research is a mixed method (mixed method). This research was conducted in the working area of ​​Batu-Batu Health Center Subulussalam City with a population of 36 people and all of them were used as samples. The research informants were 13 people. Quantitative data analysis was performed using univariate, bivariate using chi-square and multivariate using multiple logistic regression at 95% confidence level (=0.05). Qualitative data analysis was carried out by means of data reduction, data presentation, and drawing conclusions. The results showed that the factors that influenced the compliance of pregnant women in carrying out ANC visits during the Covid-19 pandemic in the Batu-Batu Health Center work area were attitude (p = 0.024), anxiety about infection (p = 0.020), information (p = 0.042), and communication/transportation infrastructure (p=0.045). Variables that have no effect are knowledge (p=0.197), and husband/family support (p=0.564). The most dominant factor influencing the compliance of pregnant women in conducting ANC visits during the Covid-19 pandemic in this study was the variable of anxiety about contracting Covid-19. Qualitatively, some pregnant women are worried about contracting Covid-19 because pregnant women are also vulnerable to contracting Covid-19 which poses a risk to pregnant women and the fetus they contain. What is unique is that pregnant women do not feel anxious because they do not have information about Covid-19 so that they continue to carry out pregnancy checks according to schedule. The leaders of the Batu-Batu Health Center are advised to socialize ANC visits at least 6 times with door to door counseling or create a WA or Facebook group so that correct information about ANC and Covid-19 is easier to reach and mothers do not experience excessive anxiety

Abstract

This study's objective is to investigate the variables influencing the compliance of the National Health Insurance (JKN) Participant Segment of Non-Wage Recipients (PBPU) in Paying JKN Contributions in the BPJS Health Work Area, North Tapanuli Regency. This sort of research is quantitative and employs a cross-sectional study design. This study's population consists of all Non-Wage Recipient Workers (PBPU) in Siborong-borong District, North Tapanuli Regency, a total of 12,323 persons, with a sample size of 99 calculated using a purposive sampling technique. The study used univariate, bivariate, and multivariate techniques. The findings of the chi-square test indicated that education (p = 0.066> 0.05), occupation (p = 0.008 0.05), knowledge (p = 0.003 0.05), income (p = 0.000 0.05), and distance (p= 0.0010.05) were significantly correlated with each other. The logistic regression test reveals that income, with an Exp (B) value of 4,778, has the greatest impact on compliance with making JKN payments. This research concludes that employment, knowledge, income, and distance have a link with JKN contribution compliance, however education has no association with JKN contribution compliance. The community need socialization in order to raise public understanding and awareness of their monthly obligation to make JKN payments.

Abstract

The Datu Beru Takengon Hospital is the location of this research, and its purpose is to investigate the factors that influence patient satisfaction. This study is a quantitative research with a cross-sectional study design, which is a measurement of the dependent variable and the independent variable (age, treatment class, and length of treatment) with the dependent variable (physical appearance, handling, responsiveness, assurance, and empathy (SERVQUAL) and making use of the survey method. According to the findings of the research, the variable guarantee with Exp (B) 25,263 has the most significant link with patient satisfaction, making it the most influential factor that determines the level of satisfaction experienced by patients. Whereas physicians and nurses who have high levels of confidence and trust are 25 times more likely to have the same level of satisfaction as their patients. In this context, "assurance" refers to the knowledge, courtesy, and capacity of corporate personnel to develop client faith in the organization, including the knowledge, abilities, politeness, and trustworthiness of the staff. It is possible to draw the conclusion from this research that insurance plays a more significant role than the other variables in determining the degree to which patients are satisfied with the care they get in the inpatient unit. It has been indicated that there is a need for assistance from the administration of the Datu Beru Takengon Hospital in order to boost patient satisfaction. Additionally, the hospital should make an effort to build patient trust in the provision of better services.

Abstract

The purpose of the study was to analyzethe implementation of prevention and control of nosocomial infections in thecentral operating room of Panyabungan Hospital, Mandailing Natal Regency. The research design used qualitative research methods through in-depthinterviews accompanied by direct observation (observation). Hospital PPI(Infection Prevention and Control) Committee, IPCN Hospital, Central Operating Room Doctors and Central Operating Room Nurses amount 2 people. The results of the study showed that the number of personnel wassufficient, there was no special budget for PPI (Infection Prevention and Control),infrastructure was not adequate, the policy was complete, the implementation of hand hygiene was not in accordance with the 5 moments and 6 steps of washing hands, the use of PPE was carried out accordingly, Cough and sneeze etiquette has been done properly and safe injection practices have been done, implementation of prevention and control of nosocomial infections. The conclusion of this study is that the implementation of prevention and control of nosocomial infections was not all in accordance with the standards. It is suggested to IPCN and IPCLN in order to increase supervision of officers in doing prevention and control of nosocomial infections in the Central Surgery Room of Panyabungan Hospital, Mandailing Natal Regency in accordance with SOPs, especially in the implementation of hand hygiene, use of PPE, coughing and sneezing etiquette, and injecting practices.
