Abstract

Purpose: this study was to analyze the implementation of early detection in non-communicable disease programs at the elderly posyandu at the Beras Basah Health Center, Langkat District, North Sumatra.Methode: uses qualitative with a descriptive approach. Data collection techniques are carried out by observation, in-depth interviews and documentation. This study used SWOT analysis. Results: The implementation of early detection activities has not been optimal, which can be seen from inconsistencies in program implementation in the field. Limited medical equipment is also a significant obstacle, reducing the ability of officers to carry out effective early detection. Lack of village government participation and minimal awareness and family support for early detection activities also hamper the implementation of this program. Apart from that, the inadequate knowledge of posyandu cadres is a challenge in optimizing program implementation. Opportunity analysis shows that there are several strategic steps that can be taken to increase the effectiveness of this program. Advocacy and education to the village government regarding the benefits of posyandu for the elderly can increase support and participation from the local government. Providing support and assistance to elderly families will increase their awareness and participation in early detection activities. Training for posyandu cadres is also an important opportunity to increase their knowledge and skills in implementing early detection programs. Conclussion: The program for non-communicable diseases in the elderly is still underdeveloped and has not achieved its goal of ensuring the health of the elderly.

Abstract

The JKN mobile application aims to reduce queues at BPJS Kesehatan Branch Offices by providing the best service to the community. This research aims to improve JKN Mobile information system services in order to provide better services to the community. The study was conducted using a crossectional approach and quantitative descriptive method at the Batang Kuis Health Center in February 2024, with a population and sample using JKN Mobile of 70 patients with a total sample technique. The research instrument was a questionnaire with 10 questions using a Likert scale of 1-4. Currently, as many as 48% of the total population of Deli Serdang have become JKN participants. However, there are still many people who have not used JKN Mobile, except for patients who need referrals to hospitals. A survey shows that most of the 70 respondents who use JKN Mobile need a further understanding of the usefulness of the application. Based on the findings that the JKN Mobile application does not help consult doctors because this is rarely used by the public which causes them not to understand its use. In addition, education is also needed, especially for female respondents who still do not understand the use of the JKN Mobile application. One of the problems that is often complained by the community is the difficulty in the login process which causes forgetting usernames, passwords, and errors.

Abstract

Hypertension is often said to be a silent killer, because it is a deadly disease without being accompanied by symptoms first as a warning for the victim. Hypertension with the incidence of obesity is still common. The prevalence of hypertension accompanied by obesity also continues to increase. According to some studies, obesity and hypertension have a meaningful relationship. One alternative treatment that can be an option to lower blood pressure is herbal therapy. Herbal medicine is classified as complementary medicine is a phenomenon that appears today among many other non-conventional treatment phenomena, such as treatment with herbs or herbal therapy, acupuncture, and cupping. The use of herbs is one of the alternative treatments chosen by the community other than conventional (medical) treatment. One of the natural products produced is propolis. Propolis is one of the natural products produced by bees and is widely used as a medicine or supplement, anti-inflammatory, disease treatment, accelerating wound healing, and others. The patient in this case is a woman with the age of 50 years. Patients come with complaints of dizziness, headache and low back pain accompanied by a history of hypertension, and obesity. The patient was diagnosed with Grade 2 Hypertension on Treatment, and Obesity was given therapy in the form of Amlodipine 10 mg 1x1, Paracetamol 500 mg 3x1 and Vitamin B Complex 2x1.Hypertension suffered by patients is hypertension obtained due to several predisposing factors in patients, namely diet, obesity, age, and gender.

Abstract

During perimenopause, there can be a decrease in estrogen production, which indicates the end of a woman’s reproductive years. The production of the hormone estrogen that continues to decrease will cause hypercholesterolemia. Decrease in LDL cholesterol levels can be increased by giving antioxidant properties, which are rich in vitamin C and contain flavanoids and phenolics that function as antioxidants. The method used in this study is Literature Review with Design Narrative Review. The result from the literature it was found the effect of giving ajwa dates (Phoenix Dactylifera L) before and after administration showed a change, namely a decrease in LDL levels after giving ajwa dates. This is because ajwa dates contain flavanoids, phenolics, and plant sterols that function as antioxidant that can prevent the formation of lipid peroxidation. The conclusion based on the results of the literature review, it can be concluded that giving ajwa dates to perimenopause women decreased LDL levels where ajwa dates have antioxidant functions to prevent lipid peroxidation to reduce LDL. The suggestion from the study is necessary to conduct further research on the effect of consumption of ajwa dates on LDL levels in perimenopause women using experimental methods of pretest and posttest design.

Abstract

Based on the 2016 Riskesdas, the prevalence of high cholesterol in Indonesia among women aged 35-59 years was 52.9%, and among those aged ≥60 years, it was 58.7%. According to provincial data, the highest percentage of patients with high cholesterol levels at Posbindu and FKTP in Indonesia was in West Papua Province, reaching up to 70%. The concentration of cholesterol levels in women's blood tends to increase with age, particularly for those over 40 years old, due to hormonal factors, especially the decrease in estrogen production and function. The decline in estrogen levels leads to an increase in lipids or total cholesterol, resulting in changes in body fat composition associated with hypercholesterolemia. This case report aims to investigate the impact of consuming Ajwa dates on the total cholesterol levels of perimenopausal women. Results: Ajwa dates contain higher polyphenol concentrations with an impressive nutritional profile that enhances blood lipoproteins. Additionally, dates can increase estrogen levels due to the estradiol and estrone present in their pollen. Several experimental studies have shown a significant difference in total cholesterol levels in subjects before and after consuming Ajwa dates. Conclusion: There is a notable difference in total cholesterol levels before and after the administration of Ajwa dates.

Abstract

Nurse performance can significantly impact the quality of healthcare services in hospitals, meaning that low nurse performance can affect patient trust, satisfaction, and ultimately, the quality of life and well-being of the community. One factor influencing nurse performance in hospitals is job satisfaction. Job satisfaction reflects employees' feelings towards their jobs. Thus, this study aims to describe the relationship between job satisfaction and nurse performance. The research uses a cross-sectional study design with a sample size of 77 inpatient nurses at Malahayati Islamic Hospital in Medan, employing total sampling techniques. The research instruments include the Job Satisfaction Survey developed by Spector (1994) and the Individual Work Performance Questionnaire (IWPQ) developed by Koopmans et al. (2014). Data analysis was conducted using univariate and bivariate analysis with the chi-square test. The results showed that the majority of nurses' job satisfaction fell into the optimal category (71.4%) and the majority of nurse performance was categorized as good (63.6%). There is a significant relationship between job satisfaction and the performance of inpatient nurses at Malahayati Islamic Hospital in Medan, with a p-value of 0.000 (P value <0.05).

Abstract

In children under five years of age, premature complications are now the second leading cause of death. Colostrum has important benefits in promoting the maturity of premature infants' organ functions and preventing infections. Preterm infants are recommended to be given colostrum orally and pharyngeally as early as possible after birth to provide immune protective effects for their bodies. However, the sucking and swallowing functions of preterm infants are often not optimal, partly due to comorbidities, which poses a significant challenge. As a result, the immune system of preterm infants is often low. This literature review aimed to evaluate the effect of oropharyngeal colostrum feeding through subgroup analysis based on complications. Descriptive research was conducted in April-June 2024 using the literature review method with PRISMA guidelines for RCTs. Literature sources were obtained from Pubmed (MEDLINE), Science Direct, Google Scholar, and Cochrane with a publication limit of the last ten years (2014-2023). Of the 1,657 results, 13 articles met the criteria. A total of 1,451 preterm infants were included in the 13 RCT studies. Results showed a lower incidence of late-onset sepsis and Necrotizing Enterocolitis (NEC) in the group that received oropharyngeal colostrum. The administration of oropharyngeal colostrum was shown to have a positive impact on the development and defense of the immune system of preterm infants, reduce health problems such as sepsis and NEC, accelerate the recovery of birth weight, and reduce the incidence of mortality.

Abstract

Malnutrition especially stunting, is a major public health issue in many parts of the world including the Ponelo Islands of North Gorontalo. Stunting among toddlers is associated with the dietary patterns adopted in the communities and the accessibility of government relations such as the PMT – Supplementary Feeding Program of ‘this area’. Fresh from interviews with health officials, health cadres and parents of stunted children in Ethiopia, the study identified poor dietary diversity and improper feeding schedules as common factors for stunting. Despite enriching enough nutritional support through PMT program, its effectiveness is limited by problems like the erratic implementation and the absence of wide ranging and permanent solutions for the malnutrition problems. The study underscores the necessity of a dual approach, initial nutritional interventions accompanied by a focus on issues affecting pre-school children’s access to nutritious foods, availability and procurement authorities vested in caregivers. By addressing these causes, it becomes possible to empower a community to ensure all children when given opportunity grow to their full potential in eliminating stunting.
