Abstract

Introduction: The National Health Insurance program (JKN), administered through BPJS Kesehatan, plays a crucial role in improving access to equitable and quality health services. However, the utilization of BPJS services remains uneven, particularly in rural areas such as Labuhan Sumbawa, where various structural and individual barriers still exist. Objectives: This study aims to identify the factors influencing the utilization of BPJS Kesehatan services and to analyze the relationship between these factors and service utilization among the community in Labuhan Sumbawa.Method: This study employed a quantitative approach with an analytical cross-sectional design. Data were collected from randomly selected respondents using a structured questionnaire that measured socio-demographic characteristics, financial status, knowledge, membership status, and perceived service complexity. Data analysis included univariate and bivariate tests using the Chi-square method to assess relationships between variables.Result: The findings indicate that financial status is the most dominant factor influencing the utilization of BPJS Kesehatan services. Other contributing factors include limited understanding of BPJS procedures, inactive membership status, and perceptions of complex administrative processes. Statistical analysis demonstrates a significant relationship between these factors and the utilization of BPJS services, indicating that both economic and non-economic aspects play an important role in shaping health service utilization behavior. Conclusion: The utilization of BPJS Kesehatan services is influenced by an interplay of financial capacity, knowledge, and perceived service accessibility. Strengthening health literacy, improving administrative simplicity, and ensuring economic affordability are essential strategies to enhance service utilization. These findings highlight the need for integrated policy interventions to optimize the effectiveness of the national health insurance program.

Abstract

Introduction: Diabetes mellitus (DM) is a chronic disease characterized by elevated blood glucose levels due to impaired insulin function, impaired insulin secretion, or a combination of both. This disease requires long-term management to prevent acute and chronic complications. Although DM cannot be cured, blood sugar levels can be controlled through the four main pillars of diabetes management: education, diet, physical activity, and pharmacological therapy. Effective self care education is considered an essential strategy to improve patients' ability to manage their condition and reduce the risk of diabetes-related complications. Objectives: This study aims to examine the effectiveness of Diabetes Self-Management Education (DSME) on medication adherence and blood sugar control in patients with Type 2 Diabetes Mellitus through a literature review.Method: This study used a literature review method by searching for scientific articles published between 2021 and 2026 through the Google Scholar database. The search was conducted using the PICO approach, namely P (Population): Type 2 Diabetes Mellitus patients; I (Intervention): self-care education or Diabetes Self-Management Education (DSME); C (Comparison): standard care or without self-care education; and O (Outcome): increased medication adherence in Type 2 Diabetes Mellitus patients. Articles were selected based on predetermined inclusion and exclusion criteria using the PRISMA flow.Result: Based on an analysis of six articles that met the inclusion criteria, self-care education was found to be effective in improving medication adherence, self-care ability, self-management, self-efficacy, quality of life, and blood glucose control among patients with Type 2 Diabetes Mellitus. Conclusion: Based on the review of the six selected articles, it can be concluded that Diabetes Self-Management Education (DSME) is an effective intervention to improve medication adherence, strengthen self-care behaviors, and support better blood glucose control in patients with Type 2 Diabetes Mellitus.

Abstract

Background: Hand hygiene is a fundamental component of infection prevention and control in healthcare settings. Proper hand hygiene practices play an essential role in reducing the transmission of microorganisms, preventing healthcare-associated infections, and improving patient safety. Nurses are among the healthcare professionals who interact most frequently with patients, making their compliance with hand hygiene procedures crucial in maintaining quality healthcare services. Despite the availability of guidelines and infection prevention programs, compliance with hand hygiene practices remains a challenge in many healthcare facilities. Various individual and environmental factors may influence nurses’ adherence to hand hygiene recommendations. Objective: To analyze the factors associated with nurses’ compliance in performing hand hygiene in the surgical ward.Methodology: This study employed a quantitative design with a cross-sectional approach. The study population consisted of nurses working in the surgical ward, and respondents were selected using a total sampling technique. Data were collected through structured questionnaires that assessed respondents’ characteristics, knowledge, availability of facilities, workload, and hand hygiene compliance. Data analysis was performed using the Chi-square test.Research Results: The findings showed that knowledge, availability of facilities, and workload were significantly associated with nurses’ compliance in performing hand hygiene. Nurses with better knowledge, adequate facilities, and manageable workloads tended to demonstrate higher levels of compliance. In contrast, length of service and gender were not significantly associated with hand hygiene compliance. Conclusion: Nurses’ compliance with hand hygiene is influenced by both individual and organizational factors. Improving knowledge through continuous education, ensuring the availability of adequate facilities, and managing workload effectively are important strategies to enhance hand hygiene compliance. Strengthening these aspects may contribute to better infection prevention practices and improved quality of patient care.

Abstract

Introduction: Hypertension is one of the major health problems among adults and can lead to various complications if not properly controlled. Physical activity and smoking habits are important factors that may influence blood pressure levels and the severity of hypertension. The prevalence of hypertension in Indonesia remains high, reaching approximately 36%, while the Indonesian Basic Health Research reported a prevalence of 34.1%. This condition indicates that hypertension continues to be a significant public health concern requiring effective prevention and control of its risk factors. Objectives: To determine the relationship between physical activity and smoking habits with the degree of hypertension among adults at Ciracas Community Health Center, East Jakarta.Method: This study employed a quantitative research design with an observational analytic approach using a cross-sectional method. A total of 88 adult patients with hypertension were selected through purposive sampling. Data collection was conducted using the latest blood pressure records classified according to the Joint National Committee 7 (JNC 7), the Global Physical Activity Questionnaire (GPAQ) to assess physical activity levels, and the Glover–Nilsson Smoking Behaviour Questionnaire to evaluate smoking habits. Data were analyzed using the Chi-Square test.Result: The results showed that most respondents had moderate physical activity levels (39.8%), did not have smoking habits (70.5%), and experienced stage 1 hypertension (61.4%). Statistical analysis indicated a significant relationship between physical activity and the degree of hypertension (p < 0.05). However, smoking habits were not significantly associated with the degree of hypertension (p > 0.05).Conclution: Physical activity is significantly associated with the degree of hypertension among adults, whereas smoking habits are not significantly related to hypertension severity. Regular physical activity may contribute to better blood pressure control and reduced hypertension severity. Therefore, healthcare professionals, particularly nurses, should strengthen health promotion and preventive programs by providing education and motivation to encourage patients to engage in regular physical activity.
