Abstract

Introduction: Elderly individuals are vulnerable to depression and cognitive decline, which can impair memory, orientation, and daily functioning, reducing quality of life. Family and community support help mitigate these effects.Objectives: This study examined the relationship between depression and cognitive decline in elderly patients at Hospital X.Method: A cross-sectional study included 55 elderly respondents selected purposively from 121 patients. Depression was measured using the Geriatric Depression Scale Short Version (GDS-SV), and cognitive function using the Mini-Mental State Examination (MMSE). Bivariate analysis with Chi-Square tested the association between depression and cognitive function.Results: Most elderly with mild to moderate depression had normal cognitive function, while those with severe depression predominantly exhibited cognitive impairment. The relationship between depression level and cognitive function was statistically significant (p = 0.004).Conclusion: Depression levels are significantly associated with cognitive decline in the elderly. Early detection of depression and cognitive stimulation are essential to prevent further cognitive deterioration. Interventions should include family- and community-based psychosocial support, cognitive stimulation, counseling, and emotional guidance. Family presence, such as living with children or relatives, serves as a protective factor for emotional stability and cognitive function. Healthcare professionals should integrate depression screening, family education, and cognitive interventions into elderly care plans to improve quality of life and independence.

Abstract

Introduction: Hypertension is a common chronic condition among older adults that can negatively affect their quality of life. In addition to physical factors, psychosocial and spiritual aspects play an important role in helping older adults adapt to their condition. A well-developed spiritual state is associated with better coping mechanisms, emotional stability, and a more positive outlook on life.Objective: This study aimed to determine the relationship between spiritual condition and quality of life among elderly patients with hypertension.Methods: This study used a quantitative analytical observational design with a cross-sectional approach. The population consisted of elderly patients with hypertension, and respondents were selected using a purposive sampling technique based on predetermined inclusion criteria. Data were collected using standardized instruments to measure spiritual well-being and quality of life. Data analysis was conducted using univariate and bivariate methods, and the relationship between variables was tested using the Chi-square test with a predetermined level of significance.Results: The findings indicated that most elderly respondents with better spiritual conditions tended to have better quality of life. Statistical analysis showed a significant relationship between spiritual condition and quality of life, indicating that higher levels of spirituality are associated with improved perceptions of well-being among elderly patients with hypertension.Conclusion: There is a significant relationship between spiritual condition and quality of life among elderly patients with hypertension. These findings emphasize the importance of integrating spiritual assessment and interventions into nursing care as part of a holistic approach to improve the overall well-being of elderly patients.

Abstract

Introduction: The postpartum period is a critical phase for mothers in providing optimal nutrition through breastfeeding. However, ineffective breastfeeding remains a common problem, characterized by inadequate milk production, poor infant attachment, maternal anxiety, fatigue, and difficulties in positioning the infant during feeding. These conditions may interfere with successful breastfeeding and reduce maternal confidence. Lactation massage is a nonpharmacological intervention that may stimulate oxytocin secretion, enhance milk flow, and improve breastfeeding effectiveness.Objective: This study aimed to describe the implementation of nursing care using lactation massage therapy in a postpartum patient experiencing ineffective breastfeeding.Method: This study employed a case study approach involving one postpartum patient diagnosed with ineffective breastfeeding at RSUD dr. R. Soedarsono Pasuruan. Data were collected through interviews, observation, physical examination, and documentation. Nursing interventions included breastfeeding education and lactation massage therapy applied to the neck, shoulders, scapula, back, and breast areas for 15–20 minutes daily over three consecutive days. Evaluation focused on breastfeeding effectiveness indicators, including milk flow, infant attachment, maternal positioning during breastfeeding, infant urination frequency, sucking strength, maternal comfort, and confidence.Result: Following three days of intervention, improvements were observed in several indicators of breastfeeding effectiveness. Breast milk flow became smoother, infant attachment and positioning improved, sucking strength increased, and infant urination frequency rose from five to nine times per day. In addition, maternal anxiety and fatigue decreased, while comfort and confidence in breastfeeding improved. By the third day, the nursing problem of ineffective breastfeeding was considered resolved.Conclusion: Lactation massage combined with breastfeeding education was associated with improved breastfeeding effectiveness in this postpartum patient. This intervention may be considered a supportive nonpharmacological nursing strategy to promote successful breastfeeding and maternal well-being during the postpartum period.
