Abstract

Background: Preeclampsia is a leading global cause of maternal and perinatal mortality, marked by hypertension and proteinuria after 20 weeks of gestation. High blood pressure triggers endothelial dysfunction and renal impairment, while changes in hemoglobin levels often indicate disease severity. Analyzing these clinical parameters is vital for assessing organ involvement in patients. Purpose: This study aimed to analyze the association between blood pressure, proteinuria level, and hemoglobin concentration among preeclamptic patients at St. Khadijah 1 Hospital Makassar. Methods: This analytical observational study employed a cross-sectional design using secondary data obtained from medical records of 51 preeclamptic patients during January–December 2024. Patients were classified into preeclampsia and severe preeclampsia based on blood pressure severity, with severe hypertension defined as systolic ≥160 mmHg and/or diastolic ≥110 mmHg. Data included blood pressure, dipstick proteinuria results, and hemoglobin concentration. The Chi-Square test was used to assess the relationship between variables, with a significance level of p<0.05. Results: Most patients experienced severe preeclampsia (54.9%), had proteinuria grade 2 or higher (56.9%), and had normal hemoglobin levels (80.4%). A significant association was found between blood pressure and proteinuria (p=0.003). However, no significant association was observed between blood pressure and hemoglobin concentration (p=0.740). Conclusion: Blood pressure is significantly associated with proteinuria in patients with preeclampsia, suggesting a potential role in renal endothelial dysfunction. In contrast, hemoglobin concentration does not exhibit a significant relationship with blood pressure. Regular monitoring of blood pressure and proteinuria remains critical for assessing disease progression and informing timely interventions in preeclampsia.

Abstract

Background: Pulmonary tuberculosis remains a major global health problem with high morbidity and mortality. Clinical symptoms, haemoglobin levels, and chest radiograph findings are important indicators of disease severity and physiological impact, making their relationship relevant for early assessment and clinical management. Purpose: This study aimed to analyse the relationship between clinical symptoms, haemoglobin levels, and chest radiograph findings among patients with pulmonary tuberculosis at Ibnu Sina YW-UMI Hospital Makassar in 2023–2024. Methods: This cross-sectional analytic observational study used total sampling and included 100 pulmonary tuberculosis patients who met the inclusion criteria. Clinical symptoms, haemoglobin levels, and chest radiographs were collected from medical records. Univariate and Chi-square bivariate analyses were performed, with p < 0.05 considered significant. Results: Most patients had decreased haemoglobin levels (81%) and far advanced radiographic lesions (81%). Common clinical symptoms were productive cough (86%), shortness of breath (75%), fever (77%), decreased appetite (59%), and weight loss (69%). Only shortness of breath (p = 0.000) and decreased appetite (p = 0.029) were significantly associated with haemoglobin levels. In contrast, all clinical symptoms showed significant associations with chest radiographic severity. Conclusion: Shortness of breath and decreased appetite were significantly associated with decreased haemoglobin levels in pulmonary tuberculosis patients, while other symptoms showed no significant association. All clinical symptoms were significantly associated with chest radiographic severity. Most patients also had anemia and far advanced radiographic lesions, indicating advanced disease.

Abstract

Background: Total cholesterol is an essential lipid component required for the formation of hormones, vitamin D, and bile acids. However, elevated cholesterol levels (>200 mg/dL) can lead to hypercholesterolemia and increase the risk of cardiovascular diseases. Besides pharmacological therapy such as simvastatin, which may cause side effects with long-term use, herbal alternatives are increasingly being explored. Moringa oleifera leaves contain phytochemicals with potential cholesterol-lowering effects. Objective: To determine the effect of Moringa oleifera leaf decoction on total cholesterol levels in male white rats (Rattus norvegicus). Methods: This study employed an experimental pretest–posttest control group design and was conducted from August to September 2025 at the Research Laboratory, Faculty of Medicine, University of Muslim Indonesia. Male white rats were divided into treatment groups receiving doses of 400 mg/kgBW and 800 mg/kgBW. Data were analyzed using the Paired Sample t-test and One-Way ANOVA. Results: All groups exhibited total cholesterol levels above normal prior to treatment. Administration of Moringa oleifera leaf decoction at doses of 400 mg/kgBW and 800 mg/kgBW significantly reduced total cholesterol levels (p = 0.000). The decoction significantly reduced cholesterol levels in obese rats, with efficacy comparable to simvastatin and no dose-dependent differences observed. Conclusion: Moringa oleifera leaf decoction has potential as an effective natural alternative for lowering total cholesterol levels in white rats.

Abstract

Background: Malaria remains a significant global health problem with increasing cases and deaths every year. Travelers visiting endemic areas, especially those classified as Visiting Friends and Relatives (VFR), have a high risk of malaria infection due to a lack of awareness and compliance with chemoprophylaxis. The use of malaria chemoprophylaxis among international travelers in Bali is limited, so the low chemoprophylaxis use among travelers must be evaluated based on knowledge levels. Purposes: To determine the level of knowledge about the use of malaria chemoprophylaxis and analyze the distribution of its use among foreign and domestic tourists in South Bali. Methods: This cross-sectional study used a survey conducted at Mertasari Beach, Sanur Beach, Kuta Beach, and Jimbaran Beach. Consecutive sampling of 106 foreign and domestic tourists using a questionnaire validated by r count > r table by Pearson Correlation by SPSS version 27. Results: A total of 51% of respondents had a good level of knowledge, 41.5% were sufficient, and 10.4% were deficient. However, only 9.4% of travelers used malaria chemoprophylaxis, while 90.6% did not. Conclusion: Although travelers' knowledge of malaria chemoprophylaxis was high, their use of it was low, indicating a gap between knowledge and preventive practices. Therefore, more effective educational strategies and efforts are needed to increase compliance with malaria chemoprophylaxis among travelers.

Abstract

Background: Patient satisfaction is an indicator of healthcare service quality, particularly in unique facilities such as the Floating Hospital (RSA) Nusa Waluya II, which serves remote areas. Purposes: This study aims to analyze factors influencing outpatient satisfaction at RSA. Methods: This analytical observational study with a cross-sectional design involved 50 patients selected through consecutive sampling. Data were collected using a SERVQUAL-based questionnaire and analyzed using the Chi-square test and logistic regression with a significance level of α=0.05. Results: 74% of patients were satisfied with the services, while 26% expressed dissatisfaction. Bivariate analysis showed that age <50 years was significantly associated with dissatisfaction (OR=2.83; 95% CI=1.15-6.97; P-value=0.021). Variables such as low income (OR=2.64; P-value=0.080), education (OR=1.53; P-value=0.348), and insurance ownership (OR=2.01; P-value=0.182) were not statistically significant, although trends were observed. Conclusion: Younger age (<50 years) is the primary risk factor for patient dissatisfaction at RSA Nusa Waluya II. These findings highlight the need for service quality improvements focused on the productive age group, as well as further research with a larger sample to confirm the role of economic and insurance factors.

Abstract

Background: Indonesia's increasing population has prompted government efforts in family planning, with the recommended long-term contraceptive method being the intrauterine device (IUD), although its usage rate remains low. Objective: This study aimed to investigate the relationship between knowledge levels and attitudes of pregnant women toward IUD contraceptive planning. Methods: This study used an analytical, cross-sectional design. The sample consisted of 52 respondents: 26 who had used IUDs and 26 who had never used IUDs. Respondent data takers used consecutive sampling methods in accordance with the inclusion and exclusion criteria. The primary data was collected using a questionnaire. The variables used included age, education, occupation, number of children, knowledge, attitude, and IUD use. The data were analyzed using the chi-square test. Results: This study found that 57.7% of respondents were aged 21-30 years; 50.0% had a bachelor's level of education; 34.6% of respondents worked as private employees; 50.0% of respondents had one child; respondents' knowledge level was 73.1% good; and a positive attitude of 76.9%. This study also found a significant positive relationship between attitude and knowledge of IUD use, with a p<0.001 value. Conclusions: It can thus be concluded that the level of knowledge is related to the attitude of pregnant women toward the use of IUD contraception, which can be a basis for designing more effective interventions in improving public understanding and acceptance of this contraceptive method.

Abstract

Background: Pancreatic and duodenal homeobox 1 (PDX1) is a key transcription factor for pancreatic β-cell differentiation and insulin gene regulation. Mutations in PDX1, such as E178G, have been linked to monogenic diabetes, including MODY4. Understanding the structural and functional consequences of these mutations is critical for advancing diagnostic and therapeutic strategies. Methods: This study employed an in silico approach to assess the impact of the E178G missense mutation on PDX1. Bioinformatics tools were used to evaluate physicochemical properties, secondary and tertiary structure, residue-level interactions, and subcellular localization. Wild-type and mutant models were compared using ProtParam, PSIPRED, SWISS-MODEL, PyMOL, and DeepLoc. Results: The E178G mutation did not affect the overall structure or nuclear localization of PDX1. However, secondary structure prediction revealed a localized decrease in α-helix stability near the mutation site. Tertiary structure modeling and visualization indicated the loss of key polar interactions, which may weaken the DNA-binding capacity of the homeodomain. Discussion: Although nuclear localization remains intact, disruption of structural integrity within the homeodomain suggests impaired transcriptional activity. This supports the hypomorphic nature of the E178G variant, which may lead to partial β-cell dysfunction. The findings are consistent with previous reports linking similar PDX1 mutations to MODY4 phenotypes. Conclusion: The E178G mutation in PDX1 is predicted to cause subtle but functionally relevant structural changes, potentially impairing its role in insulin gene regulation. These results reinforce the value of in silico analysis in understanding the pathogenic mechanisms of genetic variants and guiding precision medicine in monogenic diabetes.
