Abstract

Highlights: Angka kejadian kelainan sumbing bibir dan langit-langit di sepuluh wilayah Jember memiliki angka lebih dari 5 penderita. Profesi orangtua, penggunaan pestisida, dan pencemaran logam berat menjadi faktor resiko terjadinya bibir sumbing dan langit di Jember dimana geografisnya adalah perkebunan dan pertanian. Abstrak: Latar Belakang: Angka kejadian sumbing bibir dan langit-langit masih tinggi di Indonesia sedangkan proses penatalaksanaannya masih belum optimal.  Penelitian  ini  bertujuan  untuk  melakukan  analisis  kejadian sumbing bibir dan langit-langit berdasarkan tinjauan geografis sehingga dapat  menemukan  model  penanganan  yang  tepat  untuk  mengurangi angka kejadian sumbing bibir dan langit-langit. Metode: Penelitian  ini  merupakan  jenis  penelitian  deskriptif  dengan menggunakan  data  rekam  medis  di  salah  satu  rumah  sakit  di  provinsi Jawa  Timur  Indonesia.  Data  yang  digunakan  adalah  data  penderita sumbing  bibir  dan  langit-langit  antara  tahun  2017-2020.  Data  yang diperoleh kemudian dikelompokkan berdasarkan wilayah asal penderita yang  kemudian  dituangkan  dalam  sebuah  peta  untuk  memberikan gambaran  penyebaran  penderita  sumbing  bibir  dan  langit-langit. Sedangkan  karakteristik  penderita  seperti  angka  prevalensi,  jenis kelainan dan pekerjaan orang tua ditampilkan dalam bentuk tabel. Hasil: Penelitian menunjukkan  bahwa  kejadian  kelainan  sumbing  bibir dan  langit-langit  hampir  merata  di  seluruh  wilayah  Jember  bahkan terdapat  sepuluh  wilayah  yang  memiliki  angka  kejadian  lebih  dari  5 penderita.  Tingginya  angka  kejadian  ini  kemungkinan  berhubungan dengan  faktor-faktor  risiko seperti  pekerja  orang  tua  sebagai  petani, penggunaan  pestisida  dan  pencemaran  logam  berat  yang  banyak ditemukan di wilayah Jember yang secara geografis merupakan daerah perkebunan  dan  pertanian  sebagai  penyebab  kejadian  kelainan kongenital ini. Kesimpulan: Pemetaan penderita  secara  geografis  akan  memudahkan dalam menentukan prediksi angka kejadian dan faktor resiko terjadinya kelainan  sumbing  bibir  dan  langit-langit  serta  memudahkan  dalam membuat sistem penanganan penderita sesuai dengan kondisi wilayah masing-masing penderita.

Abstract

Highlights: Complex syndactyly in Apert syndrome, particularly when complicated with synonychia and synostosis, poses a significant surgical challenge. Successful surgical techniques for Apert syndrome syndactyly are crucial to achieving these goals. Abstract: Introduction: Complex syndactyly in Apert syndrome, especially complicated with synonychia and synostosis, is a surgical challenge. The incidence of Apert Syndrome is reported to be approximately 1 per 100,000 to 160,000 live births and its incidence in Indonesia is not yet known. It is practically symmetrical causing significant dysfunction and infection if not treated properly. The goals in the treatment are the separation of independent digits without disturbing function and growth, the creation of a lined commissure, the provision of skin cover for the denuded nail edge and exposed bone, and to creation of aesthetically pleasing individual fingertips with proper nails, nail folds, and adequate pulp fullness. This is due to the unreliability of the vascular branching pattern to the digits. Case Illustration: In this serial case, we reported 5 cases of Apert syndrome. We described the clinical findings, incision design, immediate and post-surgery follow-ups. Discussion: In this case, we combined dorsal   rectangular flap and palmar triangular flap to make a new web space, Buck Gramcko pulp flap for the nail fold reconstruction, and full thickness skin graft w was taken from the groin to cover the raw areas. Absorbable 5.0 simple interrupted sutures were used in each patient. The results were uneventful, with satisfying function and aesthetic appearance. Conclusion: Choosing the best surgical technique for releasing the web space in syndactyly depends on the patient's condition and the plastic surgeon's decision was needed. In 5 patients, up there was no meaningful complication attained from the postoperative result.

Abstract

Highlights: The orthognathic surgery combined with orthodontic intervention, leads to positive functional and aesthetic outcomes in individuals diagnosed with bilateral cleft lip and palate. It emphasizes the continued importance of adhering to the standard protocol, including alveolar bone grafting before permanent canine eruption, to achieve optimal outcomes. Abstract: Introduction: The incidence of cleft lip and palate is 8 in every 10,000 live births. A patient with this condition experiences a deficiency in maxillary growth. Maxillary hypoplasia leads to malocclusion and skeletal disharmony. Orthognathic surgery at skeletal maturity is the standard procedure at the end of the protocol to correct maxillary hypoplasia resulting in malocclusion not correctable with orthodontics alone. Case Illustration: We report the result of orthognathic surgery performed on a 23 year old male with complete bilateral cleft lip, palate, and alveolus. We proceeded with bimaxillary surgery despite the alveolar cleft. We also recorded a neglected alveolar cleft in which he should have had undergone alveolar bone graft prior to the current procedure. The pre-maxillary segment was stabilized with miniplate followed by Le Fort 1 advancement and mandibular setback guided by an occlusal wafer. Malar augmentation was done by onlay bone grafts. Mandibulo-maxillary fixation was maintained. Postoperatively, a good occlusion and better facial harmony were achieved. He was planned to undergo a septorhinoplasty in the near future. Discussion: Despite adequate treatments following the protocol recommended by many centres, some patients developed some degree of maxillary hypoplasia. A quarter of this population need osteotomies and Le Fort I maxillary osteotomy is the most common procedure to correct retrognathic maxilla. Conclusion: Orthognathic surgery combined with orthodontic treatment in a patient with bilateral cleft lip and palate provided good functional and aesthetic result. However, this procedure cannot replace the standard protocol of having an alveolar.

Abstract

Highlights: There were no significant differences in anthropometric measurements across various groups indicating consistent results in these facial parameters. Bilateral cheilorrhaphy using the Djohansjah technique at the Surabaya CLP Center achieved satisfactory symmetry of the lips, nose, and philtrum, with consistent anthropometric measurements in preoperative cleft conditions. Abstract: Introduction: Bilateral cheilorraphy has higher complexity compared to unilateral cheilorraphy. Anthropometric measurements provide nasolabial identification of the surgery result objectively. This study aims to assess the outcome of bilateral cheilorrhaphy with the Djohansjah technique at Surabaya CLP Center. Methods: 26 patients underwent bilateral cheilorrhaphy from January 1 to December 31, 2018, at the Surabaya CLP Center. Anthropometric measurements were performed on control photos one-year post-surgery with the GIMP application (GNU Image Manipulation Software) version 2.10.12 to measure the cupid's bow width ratio, hemilabial length ratio, nasal width ratio, and philtrum ridge length ratio. A statistical analysis was performed using Independent Sample t-Tes and Mann-Whitney Test. Results: An anthropometric size difference test based on preoperative condition was performed. No significant difference of the cupid's bow width ratio, hemilabial length ratio, nasal width ratio, and philtrum ridge length ratio between the group of identical cleft lip and unidentical cleft lip with p > 0.05 was found. Likewise, there was no difference between the identical cleft alveolar and unidentical cleft alveolar, as well as the group with or without cleft palate. Conclusions: Symmetry of lips, nose, and philtrum was achieved with bilateral cheilorrhaphy with the Djohansjah technique at the Surabaya CLP Center and there was no significant difference in cupid's bow width ratio, hemilabial length ratio, nasal width ratio, and philtrum ridge length ratio on preoperative cleft condition.

Abstract

Highlights: Kapsul ekstrak Channa striata meningkatkan kadar albumin dalam tubuh dan mempercepat penyembuhan luka pasien luka bakar. Efek positif kapsul ekstrak Channa striata adalah menurunkan kadar MDA serum dan meningkatkan balans nitrogen ke arah positif. Abstrak: Latar Belakang:  Luka  bakar  adalah  cedera  pada  kulit  atau  jaringan  organik  lainnya yang disebabkan oleh panas, radiasi, zat radioaktif,  listrik, gesekan atau kontak dengan bahan  kimia.  Bagian  tubuh  yang  mengalami  luka  bakar  akan vasodilatasi  akibat  adanya  stimulus  mediator  inflamasi  yang    dilepaskan    oleh    sel endotel,  platelet  dan  leukosit  yang  rusak,  mengakibatkan  peningkatan  tekanan hidrostatik  kapiler  yang  menyebabkan  meningkatnya  permeabilitas  membran  kapiler. Keadaan  ini  membuat  cairan  dan  elektrolit  di  intravaskuler  keluar  ke  ekstravaskuler. Albumin  juga  ikut  keluar  ke  ekstraseluler  pada  proses  ini,  sehingga  terjadi hipoalbuminemia. Diperlukan  adanya  solusi  untuk  peningkatan  kadar  albumin  selain    melalui transfusi,  dengan  syarat  alternatif  transfusi  albumin  ini  diharapkan  lebih  hemat  dan efisien  dibandingkan  dengan  albumin  transfusi  yang  terkenal  mahal.  Ekstrak  Channa striata  merupakan  sebuah  produk  baru  yang  diharapkan  dapat  menjadi  alternatif albumin transfusi ini. Tujuan penelitian adalah untuk mengetahui apakah pemberian kapsul ekstrak Channa striata dapat meningkatkan kadar albumin dalam darah pada kasus luka bakar. Metode: Metode  penelitian  ini  menggunakan  metode  systematical  review  dengan mengambil jurnal melalui Google Scholar dan ScienceDirect. Hasil:Dari 5 literatur yang dianalisa, semua menyatakan bahwa Channa striata mempercepat penyembuhan luka dan meningkatkan kadar albumin. Kesimpulan: Kesimpulan dari penelitian ini adalah kapsul ekstrak Channa striata dapat meningkatkan kadar albumin  dalam  tubuh  serta  mempercepat  penyembuhan luka pasien luka bakar. Kapsul ekstrak Channa striata  biasanya diberikan sebagai suplementasi bersama bahan lain untuk  memenuhi  kebutuhan  gizi pasien.  Penelitian ini  membuktikan  efek  kapsul  ekstrak  Channa  striata  terhadap kadar albumin menurunkan kadar MDA serum dan meningkatkan balans nitrogen ke arah positif.

Abstract

Highlights: Surgical hymenoplasty technique based on the type of the tear or cleft finding preoperative for satisfaction postoperative approach yields satisfactory results. Surgical hymenoplasty technique, tailored to the specific type of hymenal tear or cleft identified preoperatively. Abstract: Introduction: To describe our surgical hymenoplasty technique based on the type of the tear or cleft finding preoperative for satisfaction postoperative. Case Illustration: Hymenoplasty was performed on 4 patient on January 2017 until March 2017, we found three patients with U-type. We  performed hymenoplasty which is to create a new surface raw, on the right and left cleft to be stiched, we use a rapid absorbable suture material, with horizontal mattress technique. Discussion: No complications developed in the patients who had undergone hymenoplasty and all patients stated that the sexual intercourse they experienced was similar to the night of the initial        experience. Conclusion: Our new apporach for hymenoplasty is a technique that has good results, and this is a good approach in doing hymenoplasty. A comprehensive preoperatif consultation is essential to ensure patient and surgeon satisfaction postoperatively. Preoperative evaluation of the hymenal tissue is the basis on which a surgical technique is selected. The surgical technique is determining factor for a succesful hymenorrhaphy.

Abstract

Highlights: Silver sulfadiazine (SSD) in abrasion wound treatment giving the advantages in proliferation phase. SSD was found to be more effective in healing abrasions than gentamicin cream. Abstract: Introduction: Abrasion is injury to skin due to trauma that affects the surface of the epidermis or superficial epithelium, especially from friction to the depth of the dermal papillae. The basic principle of abrasion treatment is to maintain moist condition in wound surface.This study aims to compare the effectiveness of silver sulfadiazine (SSD) and gentamicin cream in the process of healing abrasions. Methods: The study used a post-test only control group design. A total of 30 samples of abrasion patients who came for treatment at the ED were sampled, and divided into 3 groups, namely wound care by providing SSD, gentamicin and tulle. Results: The wound dimensions are assessed from the length and width measured at the 1st day, the 5th day, and the 10th day. At the 10th day assessment, SSD was found to be more effective in healing abrasions compared to other groups. Conclusions: The use of SSD in abrasion wound treatment is believed to have advantages in proliferation phase where SSD is a sulfonamide class antibiotic that produces synergic effects along with silver which constantly absorbs exudates and bacteria, combines chloride tissue and several other proteins in the process of wound healing.

Abstract

Highlights: Economic factors and insufficient information about cleft palate treatment was the primary cause of delayed repairs,except for Parents' education levels. The essential responsibility of primary care physicians is to provide education on treatment stages to minimize delays. Abstract: Introduction:  Optimal time of Cleft palate repair is during the 10 to 12 month of  age.  In  this  time  produce  far  natural  results  in  terms  of  speech  because  it enabled  the  maturation  of  scar  tissue  postoperatively.  The  soft  palate  must function  properly before the  patient  starts learning  to  talk,  otherwise  speech disorders such as persistent rhinolalia aperta might arise. In pediatric patients, the role of parents is very important on adherence to therapy. Methods: This is a cross-sectional study. The first study group was parents of patients who had surgical repair before two years old and the second group was the parents  of  patients  who  had  repair  after  two  years  old.  We  compared  age, monthly income, education level, number of children, and residential distance from Surabaya of the two groups. Results:  The  data  of  this  study  were  obtained  from  the  medical  records  of patients with cleft lip surgery at CLP Center Surabaya in 2015th-2017th with total  of  358  patients,  172  were  female  and  186  were  male.  52  patients  with delayed cleft palate surgery. Patients' parents in both groups were mostly 31- 40 years old, were high school graduated, has one child, earned less than 1.5 million rupiah a month, and lived less than 100 kms from Surabaya. From the statistical  results,  parent's  income  has  the  strongest  correlation  with  the patient's age in cleft palate surgery (-2.7). A negative coefficient means that the less parent's income, the more patient likely had delayed cleft palate surgery. While other factors found weak and very weak correlations. Conclusions:The  results  form  patient's  parents'  interview,  concluded  that besides economic factors, the lack of information cleft palate treatment is the key  factors  that contributed to  the  delay  of  cleft  palate  repair.  The education level  does  not  affect  the  delay  in  cleft  palate  surgery,  because  even  in  high educated parents, sometimes they don't understand the stages of cleft lip and palate  treatment.  This  study  emphasized  the  necessity  to  educate  about  the stages of surgery by primary care physicians, to minimize delays.

Abstract

Highlights: The demographic data of patients with palate fractures is young adult men aged 19-30 years, the most common of palatal fracture type is parasagittal type, and causes of trauma being traffic accidents. The use of transmolar wiring and plating, occlusion was achieved well. Abstract: Introduction: Palatal fractures are often associated with  maxillofacial fractures and Le Fort fractures. The diagnosis and management of palatal fractures in the midface area is a challenge for a plastic surgeon in restoring function and aesthetics. With the results of this study, it is expected to be a database of maxillofacial fractures treated at SMF Reconstructive Plastic Surgery and Aesthetic Dr. Soetomo General Academic Hospital, Surabaya and gave the ability to make a fast and precise diagnosis for time and technical maxillofacial fractures. Methods: This study uses medical record data for all patients diagnosed with palatal fractures in Dr. Soetomo General Academic Hospital, Surabaya during January 2012 to December 2017. The variables studied were demographic data including sex, age, mechanism of occurrence of accidents, types of fractures, management, complications that occur and length of treatment. Discussion: There were 82 patients with palatal fractures, with traffic accidents being the most common cause of palate fracture (n=61) followed by workplace accidents and households in second place (12 and 9%). Most sufferers were men (68%), women (14%) with the highest age range of men aged 19-30 years who were followed by ages 31-45. The most were parasagittal fractures (56%), then Sagittal (15%), paraalveolar (9%), alveolar (1%), comminutive (1%). no fractures with anterior and posterolateral alveolar types, posterolateral type or transverse type  fractures. Hospitalization period with plating (12 days), transmolar wiring (10.6 days), and conservative (13.8 days). Conclusions: In this study assessed the experience in the reconstruction and aesthetic plastic surgery department of Dr. Soetomo General Academic Hospital regarding palatal fractures and accompanying demographic data. The type of fracture that occurs is also related to the management performed. Incomplete medical records caused problems in this study.

Abstract

Highlights: Severe Constricted Head in Crouzon Syndrome. Complex surgical interventions to address severe craniofacial abnormalities in Crouzon syndrome. Abstract: Introduction: Constricted head or cloverleaf deformity in Crouzon syndrome is a severe form of the syndrome involving trilobed coronal and lambdoid synostosis. Crouzon syndrome with acanthosis nigricans is distinct from the classic Crouzon syndrome, characterized by thick and dark skin in body folds. The major problems resulting from constricted head are related to craniostenosis, orbitostenosis developing from abnormalities of the skull base associated with progressive rise in intracranial and intraorbital pressures which could progress to hydrocephalus and cerebellar herniation. Case Illustration: Two cases with severe Crouzon syndrome were reported. The first was a five month old girl associated with acanthosis nigricans and the other was a sixteen month old boy, both whose phenotypic expressions were at the extreme severe end of the disease spectrum. On examination there  was serious corneal exposure, visual loss, severely narrow head, midface hypoplasia, and beaked nose. CT scan showed typical cloverleaf skull, expanded middle cranial fossa, foreshortened anterior and posterior fossae and honeycomb appearance in the occiput involving  pansutural  synostosis and extensive copper beaten deformity from the high intracranial pressure . Both were complicated with hydrocephalus requiring ventricul operitoneal shunt. Discussion: This required an arduous effort in releasing the brain from the multiply punctured calvaria, avoiding excessive dural tear and bleeding and ultimately preserving the brain. Excessive bleeding was also caused by the raised ICP. Both patients are planned to undergo occipital expansion three months later. Conclusion: Severely constricted head in Crouzon syndrome is an extreme manifestation and late stage of the syndrome which not only will result in irreversible complication but will require meticulous surgical technique.
