Abstract

The Plasmodium parasites are responsible for the severe public health malaria disease happening in Venezuela. In the year 2022, the estimated malaria cases in Venezuela were 178.000 with 190 deaths. An. aquasalis is the main malaria vector in the northern coastal part of Venezuela, distributed along the entire coast, from the Guajira Peninsula in the Zulia State to the Delta Amacuro State. This study aimed to investigate the genetic diversity and population structure of the vector An. aquasalis from coastal regions of Venezuela through single strand conformation polymorphism markers of the mitochondrial NADH dehydrogenase (ND5) gene. Adult mosquitoes were collected between November 2017 and December 2018. A 450-bp region of the ND5 gene was amplified by the polymerase chain reaction and tested for variation using SSCP among 358 An. aquasalis from seven localities near the coast of the States of Zulia, Falcón, Carabobo, Aragua, Miranda, Sucre, and Delta Amacuro of Venezuela separated by up to 1960 km. This study revealed that the populations of An. aquasalis in Venezuela are highly polymorphic, with high genetic diversity among 48 haplotypes detected throughout Venezuela and sorted into four clades. In addition, a significant An. aquasalis genetic population structure was found, apparently in response to geographical barriers and ecological differences.

Abstract

Dynamic balance refers to the ability to maintain postural stability during movement, which is essential for preventing falls and maintaining functional independence in elderly individuals. This study aimed to determine the effect of Square Stepping Exercise (SSE) on the dynamic balance of elderly participants. A quasi-experimental design with a one-group pretest-posttest approach was used. Ten elderly individuals were selected through purposive sampling based on specific inclusion and exclusion criteria. The Berg Balance Scale (BBS) was employed to measure dynamic balance before and after the intervention. The SSE intervention was conducted over four weeks, with three 45-minute sessions per week. Statistical analysis using paired t-tests revealed a significant improvement in dynamic balance after the intervention. The mean pretest score was 25.90 (SD = 2.76), while the mean posttest score was 28.30 (SD = 2.54), with a p-value of 0.000 (< 0.05). These results indicate that Square Stepping Exercise effectively enhances dynamic balance in elderly individuals, which may reduce the risk of falls and improve their quality of life. The findings highlight the potential of SSE as a practical, scalable intervention for fall prevention programs in elderly care settings. Abstrak: Keseimbangan dinamis mengacu pada kemampuan untuk mempertahankan stabilitas postural selama pergerakan, yang sangat penting untuk mencegah jatuh dan menjaga kemandirian fungsional pada individu lanjut usia. Penelitian ini bertujuan untuk mengetahui pengaruh Square Stepping Exercise (SSE) terhadap keseimbangan dinamis pada lansia. Desain penelitian yang digunakan adalah quasi-eksperimental dengan pendekatan pretest-posttest satu kelompok. Sepuluh lansia dipilih melalui teknik purposive sampling berdasarkan kriteria inklusi dan eksklusi tertentu. Alat ukur yang digunakan untuk menilai keseimbangan dinamis adalah Berg Balance Scale (BBS), yang diukur sebelum dan sesudah intervensi. Intervensi SSE dilakukan selama empat minggu, dengan tiga sesi per minggu, masing-masing berdurasi 45 menit. Analisis statistik menggunakan paired t-test menunjukkan peningkatan signifikan pada keseimbangan dinamis setelah intervensi. Rata-rata skor pretest adalah 25,90 (SD = 2,76), sedangkan rata-rata skor posttest adalah 28,30 (SD = 2,54), dengan nilai p sebesar 0,000 (< 0,05). Hasil ini menunjukkan bahwa Square Stepping Exercise secara efektif meningkatkan keseimbangan dinamis pada lansia, yang dapat mengurangi risiko jatuh dan meningkatkan kualitas hidup mereka. Temuan ini menegaskan potensi SSE sebagai intervensi praktis dan skala besar dalam program pencegahan jatuh di layanan perawatan lansia.

Abstract

Smoking impairs lung function as hemoglobin binds more easily to carbon dioxide than oxygen, weakening respiratory muscles and reducing thoracic expansion. This condition increases the risk of shortness of breath and respiratory complications. Pursed Lips Breathing (PLB) is a breathing technique that improves alveolar ventilation and strengthens respiratory muscles, ultimately enhancing thoracic expansion. This quasi-experimental study aimed to determine the effect of PLB on thoracic expansion in active smokers. The study was conducted from March 13 to March 26, 2023, involving 15 active male smokers aged 30–40 years in Salimpaung District, Tanah Datar Regency. Thoracic expansion was measured at three anatomical points: axilla, costa 4, and xyphoid process using the midline technique. The findings showed significant increases in thoracic expansion at all measurement points. The mean expansion at the axilla improved from 1.07 cm to 2.73 cm (p = 0.000), at costa 4 from 2.20 cm to 5.33 cm (p = 0.001), and at the xyphoid process from 4.73 cm to 7.27 cm (p = 0.001). These results confirm the positive effect of PLB on thoracic expansion in active smokers. In conclusion, PLB is an effective intervention for enhancing thoracic expansion and improving respiratory function in active smokers. It can be a practical and accessible method for respiratory rehabilitation, particularly for populations with limited access to medical facilities. The adoption of PLB may help reduce respiratory complications and improve overall lung capacity in smokers.

Abstract

Background: Menstrual cycle disorders are common problems among adolescent girls and are associated with reproductive health issues. Factors such as Body Mass Index (BMI) and stress are believed to influence menstrual irregularities. This study aims to examine the relationship between BMI, stress, and menstrual cycle irregularities as a preventive measure to maintain optimal menstrual health. Method: This observational analytical study utilized a non-probability sampling technique. Data analysis was conducted using Spearman’s Rho Correlation test and Cross Tabulation test at a significance level of 0.05. Results: The analysis showed a significant positive correlation between BMI and menstrual irregularities (p = 0.010, r = 0.258) and between stress and menstrual irregularities (p = 0.001, r = 0.480). Stress exhibited a stronger association with menstrual irregularities than BMI. Conclusion: This study highlights the relationship between BMI and stress with menstrual irregularities in female adolescents at SMA Hang Tuah 2 Sidoarjo. These findings emphasize the need for targeted interventions addressing nutritional and psychological support to improve menstrual health. Monitoring BMI and managing stress are essential strategies to enhance reproductive health in adolescents. Further research is recommended to explore effective interventions to address these factors and improve adolescent well-being.

Abstract

The worldwide occurrence of strokes reaches around 15 million cases per year while these conditions persist in more than 100 million patients at once. Worldwide stroke stands as the second main cause of mortalities since 88% of all stroke deaths occur within low- and middle-income countries (LMICs). Emergency Medical Services establish critical intervention paths because they detect strokes early while providing immediate on-scene treatment and quick response times that directly affect patient recovery. The effectiveness of proposed quality indicators to improve prehospital stroke treatment remains unknown independently from other indicators. The research tackles the developing EMS functions in stroke management through an analysis which shows how prehospital triage tools together with telehealth integration and mobile stroke services shorten the door-to-needle period and improve patient treatment results. The potential of point-of-care diagnostic tools for stroke subtype identification gets investigated to identify their contribution to optimizing prehospital decision-making processes. The text examines crucial issues within prehospital stroke care such as restricted EMS service accessibility and minimal training support and inadequate evidence-based protocol data. The reduction of stroke mortality and improvement of patient recovery requires future research to focus on EMS collaboration with multilevel stakeholders through technology innovation and policy equality to optimize stroke management in underserved healthcare environments.

Abstract

The doctor-patient relationship is a fundamental element of medical practice that plays a crucial role in determining patient satisfaction and health outcomes. In developing countries like Bangladesh, where the healthcare system faces various challenges, understanding the doctor-patient dynamic becomes even more critical. This study aimed to assess the relationship between doctors and patients from the patients' perspective in a selected hospital in Bangladesh. A descriptive cross-sectional survey was conducted over a one-year period with 422 participants aged 18 years and above. Data were collected through a self-administered questionnaire focusing on patients' perceptions of doctors' attentiveness, consultation time, clarity of medical explanations, and satisfaction with consultation fees. The findings revealed that 71.09% of patients perceived doctors as cordial, while 83.18% felt that doctors provided adequate consultation time. However, only 38.66% believed they received sufficient mental support from doctors, and a significant proportion of patients struggled to understand the prescribed medication and advice. Additionally, satisfaction with consultation fees was notably low, with only 23.70% of participants expressing positive feedback. These results highlight the need for improved communication and emotional support in doctor-patient interactions. Addressing these gaps could significantly enhance the overall quality of healthcare services in Bangladesh. The study provides valuable insights into the socio-economic factors influencing patient perceptions and emphasizes the importance of strengthening the doctor-patient relationship to improve healthcare outcomes. Future research should focus on broader samples across multiple institutions to obtain a more comprehensive understanding of this dynamic.

Abstract

Lannea coromandelica (Houtt.) Merr., commonly known as Tammate, is a traditional medicinal plant with potential as a source of bioactive compounds. This study aimed to identify the secondary metabolite content in ethanol extracts of Tammate leaves using four different extraction methods: infusion, maceration, reflux, and Soxhlet extraction. The research methodology involved extracting the plant material with each technique, followed by phytochemical screening through color reactions and Thin Layer Chromatography (TLC). The results showed that the reflux method yielded the highest extract percentage at 18.77%, followed by maceration at 13.08%, Soxhlet at 5.14%, and infusion at 3.94%. Phytochemical screening indicated that reflux and maceration were more effective in extracting alkaloids, flavonoids, phenolics, tannins, saponins, and steroids, as confirmed by both color reactions and TLC. In contrast, the infusion method showed the least effectiveness. These findings emphasize the importance of selecting appropriate extraction methods to optimize the isolation of bioactive compounds. The implications of this study support the development of efficient extraction techniques in the natural-based pharmaceutical industry and contribute to the utilization of local Indonesian medicinal plants. Further research is recommended to conduct quantitative analyses of the main compounds and to evaluate the pharmacological activities of the extracts.
