The Journal of Palembang Nursing Studies (JPNS) is a scholarly platform that has been designed to accommodate scientific works for researchers, lecturers, and nursing students. JPNS platform was started in January 2022. JPNS was designed using worldwide references that focus on the field of nursing with a particular emphasis on achievements in the Asian area. Abstract

Background: Breast cancer stands as the most prevalent form of cancer worldwide. Metastasis to the bones represents the third most frequent occurrence in breast cancer, potentially leading to radiculopathy due to the presence of lesions. Consequently, in patients with breast cancer and myeloradiculopathy experiencing medical complications, it becomes imperative to conduct follow-up care. Such care aims to assess the treatment regimen’s effectiveness, offer motivation to patients, and impart education to both patients and their family caregivers.Objective: To assess the evolving levels of care dependency in complex breast cancer patients, employing a case report methodology.Case: This study provides an analysis of patient dependency levels seven days prior (a0) and during follow-up care, one month post laminectomy (a1), aiming to compare dependency changes in individuals diagnosed with breast cancer accompanied by myeloradiculopathy before and after laminectomy. The methodology adopted involves a case report focusing on a single patient diagnosed with breast cancer and myeloradiculopathy, assessing dependency levels using the barthel index and care dependency scale. Results revealed notable differences in the dependency levels observed one week before and one month after the laminectomy procedure. Conclusion: This report indicates that the patient’s dependency level aligns with limited mobility and an inability to maintain personal hygiene independently. The findings underscore the necessity for comprehensive nursing follow-up care.

Abstract

Palliative care is in demand as more people are diagnosed with life-threatening illnesses. Palliative care provides primary, end-of-life, and bereavement care, considering the diversity of patients’ cultural backgrounds. Various strategies have been developed and investigated to improve care access for the targeted populations. Additionally, palliative care is intended to help patients live well till death. While death is inevitable, it is nonetheless essential to life. However, some people still struggle to discuss their life-threatening conditions and accept receiving the appropriate palliative care. Notwithstanding the multitude of palliative care guidelines, the public’s limited understanding or misconceptions concerning palliative care pose a significant obstacle for healthcare workers in delivering effective palliative care services. Therefore, it is critical to continue advocating for palliative care services, as each and every person deserves to experience a quality of life until their final moments. While being diagnosed with a life-threatening illness may be an unpleasant experience, it is acceptable to discuss it to allow others, including healthcare workers, the chance to offer appropriate support.

Abstract

Background: Nausea and vomiting early in pregnancy are experienced by around 70-80% of pregnant women. If nausea and vomiting occur too often, it will become a pathological condition called hyperemesis gravidarum. Many factors trigger hyperemesis gravidarum, necessitating more in-depth research regarding the risk factors for hyperemesis gravidarum and their impact on the weight of pregnant women in the first trimester. Purpose: This study aimed to analyze the correlation between hyperemesis gravidarum risk factors and the body weight of first-trimester pregnant women in East Jakarta.  Methods: A quantitative study employing descriptive analysis and a cross-sectional case-control method was conducted, including 84 first-trimester respondents selected using targeted sampling techniques. Data collection was performed using questionnaires, and data analysis was conducted using Chi-square. Results: There is a significant relationship between the risk factor variables for severe morning sickness, namely age, occupation, history of vomiting, anemia, and the outcome, which is the measurement of pregnant women’s weight. Conclusions: There is a relationship between age, occupation, pregnancy, history of vomiting, anemia, and mild stress with the results of weight measurement of pregnant women in the first 3 months of pregnancy.

Abstract

Background: Sepsis shock is a critical stage of sepsis characterized by impaired organ function and life-threatening consequences. Optimal management of sepsis shock necessitates a collaborative approach involving an interprofessional team inclusive of Intensive Care Unit (ICU) nurses. Prompt and accurate recognition of its clinical presentation is paramount for critical care nurses to ensure timely and effective intervention. Objective: This case report aims to elucidate the nursing care analysis of sepsis shock patients through the application of the one-hour sepsis bundle. Case: This case study revolves around a 69-year-old female admitted to the ICU due to sepsis shock secondary to pleural effusion and lung tumor. The patient exhibited hallmark indicators of sepsis shock, including hypotension necessitating vasoconstrictive support, fever peaking at 38.8°C, leukocytosis at 36.91 x 10^3/?L, elevated lactate levels at 2.7 mmol/L, and inadequate peripheral perfusion. Nursing diagnosis predominantly focused on assessing the risk of shock in the presence of sepsis and hypotension. The patient underwent immediate implementation of the one-hour sepsis bundle, commencing from the emergency room through ICU admission. Nursing interventions encompassed sepsis screening, continuous hemodynamic monitoring, fluid balance assessment, vigilance in maintaining airway patency, and meticulous infection control measures.  Conclusion: Subsequent to fluid resuscitation and initiation of inotropic and vasopressor support, notable improvements in hemodynamics were observed. However, persistent elevation in lactate levels underscored metabolic dysfunction, necessitating sustained vasopressor support beyond the seventh day of treatment due to refractory shock and multi-organ failure. The adoption of the one-hour sepsis bundle emerges as a recommended guideline for nursing management in sepsis shock patients.

Abstract

Background: Discharge planning is crucial in hospitals to ensure continuous patient care, thereby preventing re-admissions or complications. However, its implementation still requires improvement due to various obstacles and variances in nurses’ perceptions. Objective: This research aims to explore nurses’ perceptions regarding the implementation of discharge planning. Methods: This study is a descriptive qualitative research involving 14 participants selected through purposive random sampling. Data was collected from April to June 2023 through in-depth interviews, with notes taken and recordings made for transcription. Data analysis was conducted using the Colaizzi method. Results: Participants exhibited a good understanding of discharge planning, yet disparities emerged in their perceptions regarding ten key themes: knowledge, goals, and benefits; organizational aspects in implementation management; activity mechanisms; methods and media employed; implementation steps/flow; implementation content; particularly notable was the theme of implementation timing; evaluation processes in implementation; availability of specialized formats and documentation procedures/format completion; and encountered implementation categories and room-related obstacles. These themes emerged as significant areas needing attention based on the research findings. Conclusion: Nurses’ perceptions regarding the implementation of discharge planning have not been optimized due to constraints imposed by hospital management policies and room management, differences in nurses’ perceptions and attitudes towards implementation, and factors related to patients and their families.

Abstract

Background: Differences in interpreting the term ‘transcultural’ in nursing practice need to be addressed. Inappropriate terminology in the nursing practice environment may impact the quality of health care. Objective: The aim of this research is to identify the meaning of transcultural in concepts and theories and its relationship to the practice of the nursing profession. Methods: This paper was designed for Walker and Avant concept analysis. Reputable articles from indexing databases were used for review. “Transcultural” AND “Transcultural Nursing Practice” were the two key concepts used in the literature search. The inclusion criteria for articles to be retrieved were determined by the authors with high rigor. Then, an organizing framework was formulated using Walker and Avant’s conceptual analysis method.  Results: Antecedents, attributes, and consequences are inferred from the keywords or sentences in each article. The antecedents in this study include immigrants, acculturation, immigration, emigration, cultural diversity, cultural competence, interpersonal caring, transcultural nursing, and cross-cultural comparisons. The attributes in this study encompass cultural exchange, overcoming obstacles, research development, changes to standards and operating procedures. The consequences of the transcultural concept for nursing practice include its impact on organizations, patients/clients, and nurses. Conclusions: We suggest that healthcare professional thoroughly comprehend transcultural concepts in nursing practice in order to effectively apply transcultural approaches in nursing practice. This is one way to improve the quality of healthcare services.

Abstract

Background: The low awareness among nurses regarding the provision of spiritual services in healthcare settings results in unmet spiritual needs among patients, leading hospitals to be perceived solely as centers focusing on physical aspects. This stems from nurses’ low competence in providing spiritual care services. Objective: This study aims to analyze the correlation between nurses’ characteristics and their knowledge of spiritual care in a hospital setting. Methods: A retrospective cross-sectional study was conducted, with respondents recruited using a purposive sampling technique. Inclusion criteria included nurses working in hospitals who directly care for patients. Data were analyzed using univariate analysis (frequency and percentage) and bivariate analysis using Fisher’s exact test.  Result: A total of 117 nurses participated in this study, of whom 59.8% were female, 63.25% were aged <31 years old, 77.8% were permanent workers, 57.2% had work experience < 5 years, 91.5% were Muslim, 55.6% attended workshops/seminars, and 76.9% had moderate knowledge. Workshops/seminars had a positive correlation with knowledge and were statistically significant. Conclusion: Spiritual workshops/seminars have a positive correlation and are statistically significant in enhancing nurses’ knowledge of spiritual care in a hospital setting. Therefore, there is a need to improve nurses’ spiritual care knowledge through continuing education, such as training, seminars, or workshops.

Abstract

Background: Non-compliance with both medical and non-medical therapy regimens, such as dietary management, can lead to elevated blood glucose levels in patients with diabetes mellitus. Additionally, disturbances in circadian rhythms can impact glucose metabolism. The combination of these factors requires further investigation to understand their role in increasing glucose levels. Understanding how dietary non-adherence and circadian rhythm disturbances affect glucose metabolism may help improve glycemic control. Objective: This study aims to assess dietary non-adherence and circadian rhythm disturbances in a patient with type 2 diabetes mellitus and uncontrolled blood sugar. Case: A 65-year-old male patient was admitted to the inpatient unit from the emergency department. He had been diagnosed with type 2 diabetes mellitus since 2002. Blood sugar monitoring was performed every 12 hours using a sliding scale. Despite receiving a hospital-prescribed diet, the patient consumed additional foods such as bread, donuts, biscuits, and milk from outside sources. The patient also exhibited an irregular sleep pattern, sleeping only about 5 hours per night, going to bed after 9 p.m., waking up frequently during the night, and eating snacks (e.g., small bread) before sleep. On the first day, the morning glucose level (GDS) was 313 mg/dL and 140 mg/dL in the evening. On the second day, the morning GDS was 228 mg/dL, and 138 mg/dL in the evening. On the third day, the morning GDS was 145 mg/dL. The patient was discharged in the afternoon with an increased insulin dose from 6 to 8 units. Conclusion: Dietary non-adherence and circadian rhythm disturbances are potential risk factors for elevated blood glucose levels in patients with type 2 diabetes mellitus. Providing health education, continuous monitoring, and support to both the patient and family can encourage adherence to medical regimens and improve health outcomes.

Abstract

Background: Virtual exchange programs offer several benefits, including enhancing students’ knowledge, promoting technological literacy, and fostering cross-cultural exchange between countries. However, student participation in virtual exchanges remains relatively low.Objective: This study aimed to compare the motivation to participate in virtual exchange programs between nursing students with and without prior experience in online courses. Methods: A comparative study design with purposive sampling was used. Nursing students who had previously participated in online courses (n = 53) and those who had not (n = 47) completed a 29-item motivation questionnaire. Data were analyzed using the Mann–Whitney U test to assess differences in motivation between the two groups.Results: Students with prior online course experience demonstrated higher motivation to participate in virtual exchanges (84.8%) compared to those without such experience (70.4%). The Mann–Whitney U test revealed a significant difference in motivation between the two groups (p = 0.003).Conclusion: Online course participation appears to be a motivating factor for nursing students to engage in virtual exchange programs. Encouraging involvement in online learning may serve as an effective strategy to prepare nursing students for future participation in international collaborations and the global healthcare market.

Abstract

Background: Patients admitted to the Intensive Care Unit (ICU) are often in critical, life-threatening conditions, which are typically sudden and unplanned. This situation can lead to significant anxiety among their family members. Therapeutic communication is one strategy used by nurses to help reduce the anxiety levels of patients’ families.Objective: This study aimed to examine the relationship between nurses’ therapeutic communication skills and the anxiety levels of patients’ family members in the ICU.Methods: A cross-sectional study was conducted using purposive sampling to select 80 participants. Data were collected using the Hamilton Anxiety Rating Scale (HARS) and a therapeutic communication questionnaire. Descriptive analysis was performed, and the Spearman Rho test was used to assess correlations, with a significance level set at ? = 0.05. Results: The findings showed that 58 respondents (72.5%) perceived nurses’ therapeutic communication as being in the “good” category, while 52 respondents (65.0%) experienced mild levels of anxiety. Statistical analysis revealed a significant relationship between therapeutic communication and the anxiety levels of patients’ family members, with a p-value of 0.001 (p < 0.05) and a correlation coefficient of r = -0.351.Conclusion: The results indicate that better therapeutic communication by nurses is associated with lower anxiety levels among family members of ICU patients. Therefore, it is recommended that nurses enhance their therapeutic communication techniques to help reduce family members’ anxiety during ICU patient care.
