Abstract

The decrease in erectile function performance is the inability to sustain an erection long enough during intercourse (NIH Consensus Conference, 1993). The etiology of decrease in erectile function performance is multifactorial; organic factors, and psychogenic factors involved in the ability to get and maintain an erection (Agarwal, 2006). The decrease in erectile function performance in this study was measured using IIEF-5 questionnaire. This study is a cross-sectional observative research that use the results of the questionnaire answers of the respondents. Obtained a population of 161 people who were then screened to 109 people who fit the criteria of this research by using insidential sampling method. The results of this study showed a decrease in erectile function performance of 56.88% of the total respondents. Correlation test results showed a significant association of each variable age, duration of smoking, number of cigarettes per day, diabetes, dyslipidemia, and hypertension with a decrease in erectile function performance. Then for Body Mass Index (BMI) in this study did not show any significant relationship with decreased in erectile function performance. This is because the respondents in this study consist only of normal wight (18.5 to 24.9) and over weight (25 to 29.9). Advice given to avoid decrease in erectile function performance is to reduce cigarette consumption, and improve or treat an unhealthy condition such as BMI above normal, Diabetes, Dyslipidemia and hypertension. This study only retrieve data from the questionnaire so that the data are less and incomplete clear risk factors.

Abstract

Examination of semen / ejaculates / sperm is the first screening examination that a subfertile / infertile couple needs to undertake in an attempt to find out the cause of infertility that may come from his side. Besides spermanalysis, it is also necessary to do physical examination, especially about internal and external genital tool because as it is known abnormal result of sperm analysis can be caused by anatomical abnormalities and or testis function. To determine the relationship of testicular volume to semen analysis on male patients in Andrology clinic RSUD Dr. Soetomo Surabaya. The research is a kind of observational analytic research to find out the correlation of testicular volume to semen analysis on male patients in Andrology clinic RSUD Dr. Soetomo Surabaya by using secondary data. Based on the results of statistical tests of 64 samples by using chi square test can be seen that the value of significance or p-value 0.005 <0.05. So it can be concluded that there is a relationship between testicular volume with morphology, motility and sperm concentration. Data were analyzed using chi square test to determine the relationship between testicular volume and semen analysis. Testicular volume is one of the risk factors of decreased semen analysis results

Abstract

Infertility is a condition after a year of regular sexual intercourse, without the use of contraceptives, and ends with the absence of pregnancy. Intrauterine Insemination (IUI) is a procedure that is widely used in fertility clinics and is usually the first treatment option in patients who are experiencing infertility disorders, diagnosis of infertility due to cervical factors, diagnosis of factor infertility in men, and diagnosis of unexplained infertility. the cause. IUI is also a simple, inexpensive, and quite effective infertility treatment. One of the factors that can influence the success of IUI is leukospermia. This study is an analytical study using a cross-sectional design and using medical records of patients diagnosed with infertility, aged from 25 to 45 years, and undergoing IUI procedures from January to June 2022 at the RSIA Puri Bunda Denpasar. The sampling method in this study used a purposive sampling method which was selected based on inclusion and exclusion criteria. In this study, 53 samples were obtained. Of the 53 samples that have been collected, 13 people experienced leukospermia (24.5%), 40 people were non-leukospermic (75.5%). The success of IUI in this study was 17 people (32.1%) consisting of 3 leukospermic (23.1%) and 14 non-leukospermic (35.0%). The results of the Fisher's Exact test obtained, namely the p value = 0.511, it can be concluded that leukospermia with the success of IUI does not have a significant relationship. Calculation of odds ratio (OR) obtained a figure of 1.759 (CI95% 0,423-7,610).

Abstract

Background: Every human being, especially a married couple, wants children. Obstacles to obtaining children are called infertility. One of the Assisted Reproductive Technology (ART) procedures used for treating infertility is intrauterine insemination (IUI), a non-invasive procedure with affordable costs. The success of IUI is influenced by many factors, one of which is sperm agglutination. Objective: This study determined the relationship between sperm agglutination and the success rate of the IUI procedure. Methods: The study is a descriptive-analytic study with a cross-sectional approach. The sample collection technique uses non-probability. The data used were medical records of male patients diagnosed with infertility, aged 25-45 years, and undergoing IUI procedures at the Rumah Sakit Ibu dan Anak Puri Bunda Denpasar from January to July 2022. Results: Of the 91 samples obtained, 9 people experienced sperm agglutination (9,9%), and 82 people did not experience sperm agglutination (90,1%). The success rate of the IUI procedure was 29 people (31,9%), 1 person (11.1%) experiencing sperm agglutination, and 28 people (34,1%) not experiencing sperm agglutination. Fisher's Exact test carried out has obtained a result (p=0.263). And the value of the odds ratio (OR) obtained=0.241) Conclusion: This study concluded that there is no significant relationship between sperm agglutination in infertile men and the success rate of the IUI procedure. The value of OR showed sperm agglutination is not included in the factors that affect the success rate of IUI. Keywords: Sperm Agglutination, Intrauterine Insemination, Male Infertility

Abstract

Sjogren's Syndrome is an autoimmune disorder characterized by the loss of salivary and lacrimal glands, resulting in dry mouth and eyes. Erectile dysfunction (ED) is a prevalent complaint among people with Sjogren's Disease, however the underlying mpoiechanism is not yet known. The purpose of this article is to examine the mechanism through which Sjogren's Disease causes ED and to identify potential therapies. Using MEDLINE, EMBASE, and the Cochrane Library databases, a systematic review was done. Papers investigating the link between Sjogren's Syndrome and ED published between 1990 and 2021 were included in the study. The review paper discusses a number of potential mechanisms for the association between Sjogren's Disease and ED. They include low levels of testosterone, endothelial dysfunction, autonomic neuropathy, and psychosocial issues. The article also discusses potential treatments for ED caused by Sjogren's Disease, including inhibitors of phosphodiesterase type 5, testosterone replacement therapy, and psychological counseling. The research implies a connection between Sjogren's Disease and ED, although the underlying mechanisms remain unknown. It is necessary to conduct additional studies to better comprehend the relationship and develop targeted remedies.

Abstract

To know the correlation between the level of knowledge about reproductive health, attitude about reproductive health and gender with the teenager sexual behavior at SMAN 4 Kendari. The research population is class X and XI SMAN 4 Kendari student which amount 827 people. The sample retrieval technique is proportional stratified random sampling thus gained as much as 270 respondents. Data collection technique are done with an analytic survey through a cross sectional approach. The data analysis technique’s using descriptive and inferential statistics. The results: Showed that the men as much as 117 respondents (43,33%), women 152 respondents (56,67%), high level of knowledge as much as 160 respondents (59,26%), level of intermediate knowledge 73 respondents (27,04%), low knowledge level 37 respondents (13,7%), a good attitude as much as 76 respondents (28,15%), a medium attitude 147 respondents (54,44%), a deficient attitude 47 respondents (17,41%), sexual behavior is risk as much as 244 respondents (90,37%), and sexual behavior is not risk 26 respondents (9,63%). The level of knowledge is unrelated to the teenager sexual behavior (p > 0,05), attitude is related to the teenager sexual behavior (p < 0,05), and gender is related to the teenager sexual behavior (p < 0,05).

Abstract

Background: Converting Ego-dystonic homosexual to heterosexual is still controversial. homosexuals are encouraged to be accepted and do not require conversion. Currently, many cases of homosexuals are required to accept their identity and do not require conversion. However, there are times when patients want to be heterosexual and experience sexual dysfunction because of this orientation. Then, how a medical professional deal with such a case especially in the era of ease of information and technology? Choose to demand that the patient accept their sexual orientation or help the patient to become heterosexual. Objective: Here we present the case of a man, 44 years old, with a psychogenic erection with ego-dystonic homosexual disorientation, along with a review of the literature related to the case. Method: A case report with literature review Case/Result: We report 44-year-old man who has been married for six years complains of difficulty and decreased erection for 5.5 years. This man had a history of sexual intercourse with men during college but decided to become a heterosexual man and married his current wife. For the first six months, the patient performed oral sex with his wife. Then afterwards, the patient has difficulty getting an erection and even fails. The patient feels guilty toward his wife until their relationship becomes a crisis. The patient wants to commit to being a heterosexual man. So, the patient came to the andrology clinic for therapy. We applied intersystemic assessment approach to the patient. There is progress in the patient’s condition until now. Conclusion: Many experts say that homosexuality should be accepted and should not be converted. But in the end, the choice is returned to the patient, whether to choose to convert or not. Physicians only need to support the patient's therapy and choices so that he does not experience any mental disorders that ultimately endanger his life. Keywords: , , , ,

Abstract

Erectile dysfunction (ED) is a common condition that has a substantial impact on quality of life. It is described as the continuous inability to develop or maintain sufficient stiffness of the penis to allow satisfactory sexual performance. Early detection and treatment of ED and comorbidities associated with ED, such as hypertension, diabetes mellitus, and hyperlipidemia, can significantly improve the quality of life for men and their partners. About 35.6% of Indonesian adults have ED. The bulk of ED treatments now on the market are pharmacological and surgical procedures. Even though there has been substantial progress, it seems that the aforementioned treatment has a high dropout rate due to cultural restrictions and taboos. These patients would then look for herbal nutritional supplements like ginseng as an alternative. This systematic review evaluates the current evidence for the effectiveness of red ginseng in managing erectile dysfunction. We performed systematic review conducted from MEDLINE, ScienceDirect, ProQuest. The authors screened the articles based on inclusion criteria : (1) Mild-to-moderate erectile dysfunction; (2) Ginseng as interventional therapy; (3) Human studies that have IIEF score as an outcome; (4) Written in English. Four randomized control trials (RCT) using keywords “((Erectile Dysfunction) AND (Korean Ginseng) AND (Ginseng))” were included. Three randomized control trials (RCT) study involving 454 male patients with mild to moderate erectile dysfunction, aged from 20 to 70 years old. Our findings support that the mean in International Index of Erectile Function scores were significantly higher in patients treated with Korean ginseng than in those who received placebo. Korean ginseng could be an effective alternative treatment for male erectile dysfunction if more research with larger-scale clinical trials and higher standards is done in the future on the safety and effectiveness of ginseng.

Abstract

Electronic cigarettes that are currently popular are used as nicotine replacement therapy. There are several substances contained in electronic cigarette liquid that have carcinogenic effects. These substances can also increase free radicals which in turn can cause oxidative stress. Nicotine, propylene glycol, and glycerin can cause disturbances in male reproductive function. Infertility can occur due to lifestyle factors, one of which is smoking. Nicotine found in cigarettes can reduce sperm quality, including concentration, mobility, viability, and sperm morphology. The purpose of this study was to determine the effect of electronic cigarettes on sperm quality. The research method uses a literature review system. The twelve studies that are in accordance with the topic of discussion are experimental studies on mice and rats, two studies on human, and one study on both human and rats. Five journals showed a significant decrease in sperm count and testosterone serum level, three other journals showed a significant decrease in sperm morphology and viability, six journals showed a decrease in sperm motility, seven journals showed a significant impairment of testis histological structure, four journals showed a decrease of steroidogenesis enzymes, and one journal showed a significant increase of DNA fragmentation due to the use of electronic cigarettes. Electronic cigarettes had an effect on sperm quality.

Abstract

Based on epidemiological data, the incidence of ED and BPH increases with the same risk factors, such as age, metabolic disease, and others. Aim: This study aimed to determine the prevalence of ED and its relationship to age and the presence of hypertension (HT) and type-2 diabetes mellitus (T2DM) comorbid in BPH patients. This is a cross-sectional study conducted at RSUD Dr. Saiful Anwar and Hermina Hospital Tangku-banprahu Malang utilizes two questionnaires, including the translated version of  The Interna-tional Index of Erectile Function (IIEF-5) and Erectile Hardness Score (EHS) in Indonesian. There were 44 research subjects with only 21 respondents to the IIEF-5 question-naire, while the EHS questionnaire was given to all subjects. The prevalence of ED was found to reach 100.0% at the age of 70-79 years through the IIEF-5 questionnaire and aged ³ 80 years through the EHS questionnaire. The prevalence of ED in BPH patients with HT was found to be higher than non-HT patients through both questionnaires. The prevalence of ED in BPH patients with T2DM was found to be higher through the IIEF-5 questionnaire and slight-ly lower through the EHS questionnaire than non-T2DM patients. Age had a significant rela-tionship with ED through the EHS questionnaire (p = 0.006), while no significant relationship was found through the IIEF-5 questionnaire (p = 1.000). HT and T2DM did not have a signif-icant relationship with ED (IIEF-5: p = 0.229 and 0.526; EHS: p = 0.518 and 1.000). The prevalence of ED in BPH patients increases with age and the presence of comorbid HT and T2DM. Even so, there was no significant relationship between HT and T2DM with ED in BPH patients. A significant relationship between age and ED was only found in the EHS questionnaire but not in the IIEF-5 questionnaire.
