Abstract

Sexsomnia is one type of parasomnia that can negatively impact the quality of life for individuals affected and potentially lead to legal complications. The growing prevalence of sexsomnia demands attention due to the widespread misinformation and the misconception that it is not a medical issue. This review will delve deeper into sexsomnia, including its diagnosis and management. Parasomnia refers to undesired physical events or experiences that occur during various stages of sleep. Sexsomnia, a form of parasomnia associated with non-rapid eye movement (NREM) sleep, is characterized by unconscious, abnormal sexual behavior. Sexsomnia can manifest during any sleep phase but is most frequently observed during NREM sleep. Clinical manifestations of sexsomnia include vocalizations, fondling, kissing, masturbation, and even engaging in sexual intercourse without recollection upon waking. A comprehensive evaluation, including a thorough medical history and additional investigations such as polysomnography with electroencephalography, aids in establishing a diagnosis of sexsomnia. Presently, there is no definitive treatment for sexsomnia. Management approaches involve non-pharmacological and pharmacological. Sexsomnia, if not recognized as early as possible, can have detrimental effects on both the affected individual and the surrounding environment. Lack of awareness about sexsomnia and hesitation to seek treatment contribute to the lack of information about its prevalence. In-depth knowledge is crucial for accurate diagnosis and determining the appropriate management strategies for this condition. Further research is needed to determine sexsomnia and its management.

Abstract

Infertility is a failure to get pregnant after one year of sexual intercourse without using contraception. The causes of infertility, especially in men, are very complex, including the aging process, hormonal disorders, lifestyle, environment, metabolic disease, and genetic problems. One of the most difficult causes in male infertility  is genetic problems. In this case, a 35 year old man was reported with primary infertility and often experienced premature ejaculation and even anejaculation. Previously, the patient had repeatedly consulted about his condition at other fertility service centers. The conclusion obtained was that the patient had azoospermia with bilateral varicoceles and had been given hormone therapy but had not yet found a final diagnosis so the patient was still confused about what had happened to him. At the first visit the patient underwent a sperm analysis and the results were azoospermia. Then, based on the results of the history and physical examination, which indicated hypogonadism, a Y chromosome microdeletion examination was carried out, and a deletion was found in the AZFc region, which is a marker of infertility that causes spermatogenic failure. The examination was continued with karyotyping, the result was 47.XXY, consistent with the condition of Klinefelter syndrome. Conclusion: Carry out a karyotyping examination if you find signs and symptoms that suggest primary hypogonadism in male infertility. This can be done to streamline the diagnostic approach time in patients with primary hypogonadism, especially those with infertility.

Abstract

Puberty is a transitional period experienced by humans. This period is closely associated with physical, psychological, and emotional changes. Adolescents' responses to puberty can vary. We conducted a qualitative study to determine the definition of puberty and opinions and feelings about facing or experiencing puberty. Our research was conducted in one junior high school by conducting interviews in two focus group discussions. We found two major themes in this study: first is the definition of puberty, and second is the feeling about puberty. Both themes consisted of several sub-themes. The respondents stated that puberty is a maturity period characterized by significant physical, psychological, and emotional changes. The respondents also expressed that they were surprised by the changes they experienced. In addition, for them, puberty is the beginning of new obligations in their social environment. Attraction to the opposite sex makes them more careful and they must be able to control themselves. The need to share friends and role models is one of their recognitions in puberty. Other than that sex education indeed for better information.  This study can be used as a reference for understanding the needs of adolescents regarding puberty so that they can better adapt and maintain sexual and reproductive health for good health and well being.

Abstract

Late Onset Hypogonadism (LOH), commonly known as andropause, poses significant hormonal challenges for elderly men, impacting their overall quality of life. With testosterone levels naturally declining from the age of 30 and accelerating beyond 40, this hormonal imbalance can manifest in symptoms that degrade the well-being of men. This community service initiative aimed to improve understanding and early detection of LOH among the Indonesian male population. A web-based application, "Skrining Andropause" was developed, allowing individuals to self-assess their LOH risk through the validated Androgen Deficiency in the Aging Male (ADAM) questionnaire. The application included educational videos to enhance public awareness of andropause. Pre- and post-tests were conducted as success indicators, involving 40 male participants aged 40 and above in Malang Regency. Statistical analyses were performed to assess the impact on knowledge related to andropause across different educational backgrounds and age groups. Significant improvement in public understanding of andropause was observed (p=0.000). While educational backgrounds were not significantly associated with initial knowledge (p=0.100), nor with the increase in pre-to-post-test scores (p=0.100). When associating age groups with initial knowledge (p=0.308) and the increase in pre-to-post-test scores (p=0.308), results were not statistically significant. The observed substantial improvement in andropause understanding highlights the efficacy of our educational approach. Respondents, irrespective of educational background, shared a baseline understanding, with those with basic education showing the most significant improvement. Notably, the youngest age group displayed the highest baseline awareness. However, the oldest age group exhibited the least responsiveness to our educational media.

Abstract

Background : In Indonesia, the escalating prevalence of STDs and HIV/AIDS poses a significant health challenge. The transmission of HIV occurs through various modes, including unsafe sexual behaviors and mother-to-baby transmission. Insufficient knowledge regarding STDs and sexual education contributes to the rising of HIV/AIDS cases. Objective : To investigate the relationship between knowledge of HIV/AIDS and sexual activity behavior among couples aged 20 to 40 in Indonesia. Methods : A cross-sectional study involving 384 Indonesian couples was conducted using a causal associative technique. Purposive sampling was employed to select participants aged 20 to 40, familiar with STDs, engaged in various relationship statuses (dating, married, or in a relationship without formal status), and willing to complete the questionnaire. The variables used to quantify respondents' knowledge levels included understanding of HIV/AIDS, HIV/AIDS transmission, and HIV/AIDS prevention and treatment. The variable measuring couples' sexual activity pertained to their involvement in HIV/AIDS preventive activities. Results : Analysis of the data revealed that 282 respondents (73.4%) demonstrated good knowledge, 73 respondents (19%) exhibited moderate knowledge, and 29 respondents (7.6%) displayed poor knowledge regarding HIV/AIDS. Among the participants, 202 respondents (52.6%) expressed support for safe sex, indicating engagement in HIV/AIDS preventive activities, while 182 others (47.4%) did not. T-test results for the variables of HIV/AIDS understanding, transmission, prevention, and treatment yielded values of 0.259 (>0.05), 0.259 (>0.05), 0.264 (>0.05), and 0.522 (>0.05) for the F-test. Conclusion : The study concludes that couples' knowledge of HIV/AIDS does not exert a significant influence on their sexual activity behavior.

Abstract

Background: Andropause is a clinical and biochemical syndrome associated with increasing age characterized by a decreased testosterone level. Anxiety disorders are mental disorders that differ from normal feelings of nervousness or anxiousness and involve excessive fear or anxiety. Police officers has a high risk of experiencing andropause and anxiety disorders because of the andropause. Objective: This research aims to determine the correlation between andropause and anxiety disorders in Bali police officers. Methods: This research was carried out with a cross-sectional analytic design. Andropause was measured using an ADAM questionnaire. Anxiety disorder was measured using a BAI questionnaire. The sample for this research was male police officers aged between 35-58 years who worked at the Ditreskrimsus Polda Bali. The sample was determined using a purposive sampling technique. The sample for this research consisted of 50 people. Results: The results showed that 60% of samples were positive for andropause and 62% of samples were positive for anxiety disorders. There are 52% sample that tested positive for both andropause and anxiety. There is a significant correlation between andropause and anxiety disorders with p value <0.001. Calculated Odds Ratio (OR 95% CI) between andropause and anxiety disorders was 19.5 (4.527-83.992). Police officers that experiencing andropause have a 19.5 times higher risk of experiencing anxiety disorders compared to police officers that not experiencing andropause. Conclusion: There is a significant correlation between andropause and the occurrence of anxiety in police officers in Bali.

Abstract

Background: Every married couple hopes to have children, but not all can fulfil that desire. A married couple finds themselves unable to achieve pregnancy after 12 months of regular unprotected sexual intercourse, which can be called infertility. Infertility can also occur in men. In dealing with infertility, intrauterine insemination (IUI) is a cost-effective and non-invasive Assisted Reproductive Technology (ART) procedure. The condition of male infertility can be detected through semen analysis. One of the parameters in semen analysis is the total motile sperm count (TMSC). Objective: This research seeks to establish the relationship between post-preparation TMSC and the success rate of the IUI procedure. Methods: This research adopts a descriptive-analytical design employing a cross-sectional methodology. Non-probability sampling was utilized as the sampling technique, and the data analyzed were secondary data from the medical records of male patients aged 25 to 45 years old diagnosed with infertility. The subjects had undergone IUI at Rumah Sakit Kasih Ibu Denpasar from January 1 to December 31, 2022. Results: From the 35 obtained samples, 12 individuals (34.3%) had a post-preparation TMSC of ≥10 í— 106, while 23 individuals (65.7%) had a post-preparation TMSC of <10 í— 106. The success rate of the Intrauterine Insemination (IUI) procedure was 17.1%, with 4 couples (33.3%) having a post-preparation TMSC of ≥10 í— 106 and 2 couples (8.7%) with a post-preparation TMSC of <10 í— 106. The Fisher's Exact test carried out has obtained a result (p=0.151), while the value of the odds ratio (OR) obtained (OR=5.250) Conclusion: Based on statistical analysis, it can be concluded that there is no significant association between post-preparation TMSC in infertile men and the success rate of IUI procedures (p > 0.05). The odds ratio (OR) obtained is 5.250 (95% CI 0.799-34.496).

Abstract

Background : Andropause is a health problem in men caused by a decrease in free testosterone levels. Depression is a mental disorder that happens with a person that is related to mood.Objective : This research aims to determine the relationship between Andropause and Depression in police officers in Polda Bali in 2023. Methods : This research is a descriptive-analytical study with a cross-sectional study design. Andropause was assessed using the ADAM questionnaire, and depression was assessed using the PHQ- 9 questionnaire. The sample was selected using a purposive sampling technique. Results : There were 60 respondents in this study. A total of 31 people (51.7%) respondents were positive for Andropause, and 29 people (48.3%) were negative for Andropause. Meanwhile, 12 people (20.0%) of respondents experienced mild depression, and 48 people (80.0%) did not experience depression. Ten (10) respondents experienced Andropause with Depression. There is a significant relationship between Andropause and Depression (p=0.003). The odds ratio (OR 95% CI) obtained was 6.429 (1.270-32.541). Also there is a significant relationship between Stress and Depression with p<0,001 (0,000), with OR 21 (4,4-100,1) Conclusion : The majority of police officers in Polda Bali were found to experience Andropause and Depression, as many as 10 people (32.3%). As age increases, the incidence of Andropause increases, and in the respondent population, most of whom are in the mid-life crisis period, it increases the possibility of depression. Police officers in Polda Bali with Andropause have a 6.429 times greater risk of experiencing depression

Abstract

Background: Periodontitis (PD) remains a global public health problem. Evidence revealed that PD contributes to the development of systemic diseases, including effects on men's sexual health such as erectile dysfunction (ED). Objective: To review and explore the association between PD and ED. Methods: Following PRISMA guidelines, a systematic review of studies published in Scopus, PubMed, ScienceDirect, and Google Scholar was conducted. A comprehensive literature search until February 2024 by combining several keywords and applying eligibility criteria was performed. JBI critical appraisal checklist was employed for quality assessment. Results: A total of 15 studies involving 216,753 subjects were included. Of the 15 studies, 13 of them demonstrated a significant association between PD and ED, the other two did not. Only one study included examination of hormonal and biochemical levels. Also, one study revealed that periodontal treatment was significantly associated with improving ED. Summary: According to the findings, a significant association between PD and ED is evident. It is believed that the inflammatory processes associated with PD contributes to the emergence of ED, yet the exact underlying mechanism remains unclear. The scarcity of research and the complexity of the association between PD and ED suggest that future rigorous studies should be conducted. Additionally, dentists and general practitioners must be aware of the interconnection between these diseases, enabling them to provide comprehensive education to patients, particularly concerning the prevention and management of periodontal disease.

Abstract

Background : Male infertility affects millions globally, with 12-15% of Indonesian couples facing conception challenges, 50% of which are attributed to male factors. Despite extensive research, 70% of male infertility cases remain idiopathic. Diabetes, a prominent factor in male infertility, affects 9.3% of the global population, with projections estimating 700 million cases by 2045. Recent studies suggest the Mediterranean diet positively impacts semen parameters, particularly in diabetic individuals. This review aims to explore the Mediterranean diet's effects on fertility in male diabetic patients. Reviews : The literature review employed three databases, focusing on in-vitro, in-vivo, pre-clinical, and clinical studies from 2014-2024. Diabetes, a chronic disease characterized by abnormal blood sugar levels, affects insulin production or effectiveness, leading to complications if left uncontrolled. Erectile dysfunction (ED), prevalent in diabetic men, is attributed to nerve and blood vessel damage from high blood sugar levels. Treatments for ED include PDE5 inhibitors, but alternative plant-based therapies are being explored. Platelet-rich plasma (PRP) has shown promise in animal studies but lacks human trial data. The Mediterranean diet, rich in plant-based foods and healthy fats, may help prevent sexual function deterioration in diabetic individuals. Comprehensive nutrition education and government efforts are crucial in promoting healthy lifestyles and ensuring access to fresh foods. Summary : In conclusion, the Mediterranean diet shows promise as a dietary intervention for improving male fertility in diabetic populations. Further research is warranted to confirm these findings and to explore the mechanisms underlying the effects of the Mediterranean diet on male fertility.
