Abstract

Low birth weight (LBW, <2,500 g) remains a critical public health problem, contributing to neonatal mortality and long-term risk of non-communicable diseases, and national estimates in Indonesia indicate a prevalence ranging from approximately 6.2% to 11.7%. Parity and interpregnancy interval (IPI) are important reproductive determinants of perinatal outcomes, and their effects may be modified by socioeconomic status (SES). This study aimed to investigate the association between parity and IPI with LBW and to assess whether SES modifies these associations in a prospective cohort of pregnant women in Yogyakarta, Indonesia. A total of 120 women (poor households n=48; non-poor households n=72) were recruited purposively from community health centers and a maternity hospital/clinic. Inclusion criteria were gestational age 26–35 weeks, maternal age 20–35 years, parity 1–2, non-smoker, and provision of informed consent. Pre-pregnancy BMI was obtained through maternal recall of the last measured height and weight before conception. Data were analyzed using Pearson’s χ² tests for baseline comparisons and stratified logistic regression models to estimate adjusted odds ratios (aORs) with 95% confidence intervals. The prevalence of LBW was higher among women with non-optimal IPI (<24 or >60 months and primigravida) compared to those with optimal IPI (24–60 months). Multiparity showed a protective tendency against LBW, particularly among non-poor women. SES significantly modified the association between IPI and LBW (p for interaction <0.05). These findings suggest that multiparity may reduce LBW risk, whereas non-optimal IPI increases it, with SES acting as an effect modifier. Strengthening preconception counseling on birth spacing and targeted support for low-SES families are warranted

Abstract

Ensuring optimal nutritional intake during pregnancy is fundamental to supporting healthy fetal growth and development. Consequently, enhancing the nutritional literacy of pregnant women is a critical public health objective to prevent adverse outcomes such as stunting and maternal anemia. This study aimed to evaluate the efficacy of a WhatsApp chatbot-based educational intervention in improving pregnant women's nutritional literacy. A quasi-experimental study with a pre-post test control-group design was conducted at the Cot Seumeureng Community Health Center in West Aceh. Using purposive sampling, 84 pregnant women were enrolled and divided into an intervention group (n=42) and a control group (n=42). The intervention group received structured nutrition education delivered via a WhatsApp chatbot over a four-week period, whereas the control group received standard antenatal services. Data were collected using a modified and validated Nutrition Literacy Assessment Instrument for Pregnant Women (NLAI-P) and analyzed using Paired and Independent Sample T-Tests. The findings indicated a substantial increase in the mean nutritional literacy scores of the intervention group, rising from 12.05 to 19.19. In contrast, the control group’s scores remained relatively stagnant, moving from 13.05 to 13.33. The Independent Sample T-Test confirmed a statistically significant difference in post-test outcomes between the two groups (t=−7.626;p=0.000), demonstrating the superior impact of the digital intervention. A WhatsApp chatbot-based educational intervention is a highly effective, accessible, and innovative modality for significantly enhancing maternal nutritional literacy.

Abstract

Measles and rubella (MR) remain significant public health concerns in Indonesia, with outbreaks linked to low immunization coverage. A notable measles outbreak in October 2023 underscored the urgency of achieving herd immunity through high vaccination rates. The Ministry of Health targets 100% coverage for supplemental MR immunization; however, in 2022 Central Sulawesi reached only 30.4%, and in 2024 Palu City recorded 57.8%, both far below the target. This study aimed to evaluate the implementation of the measles/MR supplemental immunization program in 14 CHCs under the Palu City Health Office. A descriptive, quantitative, evaluative approach was employed to identify the program's strengths and weaknesses, which were assessed across input, process, and output dimensions using questionnaires. Regarding input, 100% of the CHCs had immunization officers with relevant educational qualifications, and 93% had received immunization-specific training. The process showed high compliance with SOP documentation: 100% of CHCs recorded vaccine temperature, inflow/outflow, and calculated remaining stock at every issuance, while 93% maintained individual stock cards, documented VVM status, and recorded equipment such as needles, syringes, and cold chain devices by type and quantity. However, only 86% recorded immunization logistics in general logbooks. The output showed that 86% of CHCs failed to meet the 100% immunization coverage target. Overall, these findings highlight gaps in training, service delivery, and community awareness that must be addressed to improve coverage and prevent future outbreaks. The MR supplemental immunization program in Palu City should be strengthened by ensuring annual training for immunization officers, enforcing complete SOP documentation, including vaccine stock and logistics recording, and implementing targeted outreach to improve coverage in CHCs that failed to meet the 100% target.

Abstract

Breakfast is an essential component of a healthy diet for school-aged children, yet it is often neglected. This study aimed to analyze the association between breakfast intake, blood glucose levels, and short-term memory among elementary school students in Nagan Raya Regency. This study employed a cross-sectional design involving 150 students from grades IV, V, and VI across three primary schools in Nagan Raya District with a purposive sampling technique.  Short-term memory was assessed at 08:30 a.m. using a picture recognition memory test, and blood glucose levels were measured at 09:00 a.m. using the finger-prick method. Breakfast intake was measured using the recall method.  Data on student characteristics (gender, daily allowance, and mother’s education) and nutritional status were sourced from the 2023 GENIUS program (Nutrition education and snacks program for school children). The statistical tests used were the Mann–Whitney U test and the Spearman rank correlation test. The results showed that breakfast energy intake was positively and significantly associated with blood glucose levels, and blood glucose levels were positively and significantly associated with short-term memory. Consuming a complete breakfast plays an important role in maintaining stable blood glucose levels and supporting of short-term memory among primary school students.

Abstract

Dysmenorrhea is a prevalent gynecological condition that adversely affects women’s daily functioning and quality of life. This cross-sectional study examined associations between lifestyle and biological factors and dysmenorrhea among 310 women of reproductive age recruited from community settings in Padang, West Sumatra, Indonesia. Data on menstrual patterns, history of candidiasis, alcohol consumption, medication use, sleep quality, and body mass index (BMI) were collected using structured questionnaires and anthropometric measurements. In bivariate analysis, candidiasis and alcohol consumption were associated with dysmenorrhea; however, these associations were attenuated and did not remain statistically significant in the adjusted multivariate model. Multivariate logistic regression identified two independent associations, poor sleep quality was associated with increased odds of dysmenorrhea (OR = 6.12, 95% CI: 2.07–18.08, p = 0.001), while medication use was associated with decreased odds of dysmenorrhea (OR = 0.27, 95% CI: 0.08–0.99, p = 0.048). These findings indicate that, after adjustment for other factors, sleep quality and use of medication were the primary correlates of dysmenorrhea in this sample. Interventions that promote sleep hygiene and ensure appropriate symptom management may help reduce the burden of dysmenorrhea.

Abstract

Anemia remains a major public health problem among adolescent girls in low-income countries, leading to long-term consequences such as decreased academic performance and increased risks during future pregnancies. Although iron supplementation programs have been widely implemented in schools, adherence among adolescent girls remains low. This study aimed to identify factors associated with compliance with iron supplementation among teenage girls using the Health Belief Model (HBM) framework. A cross-sectional study was conducted from February to June 2024 in Bantul Regency, Yogyakarta Special Region, Indonesia. Data were collected from 230 adolescent girls using a structured questionnaire. Bivariate analysis was performed using the Chi-square test, followed by multivariate logistic regression to identify independent predictors of compliance. The 230 participants, only 49.5% adhered to iron supplementation. Most respondents reported low perceived benefits (62.6%) and low perceived health threats (66.7%). Bivariate analysis showed significant associations between perceived benefits, perceived health threats, and perceived barriers with compliance (p<0.001). However, multivariate analysis identified perceived health threats (p=0.004; AOR = 3.52; 95% CI: 1.48–8.38) and perceived barriers (p<0.001; AOR = 7.11; 95% CI: 3.26–15.51) as independent predictors of compliance. Compliance with iron supplementation among adolescent girls is significantly influenced by perceived health threats related to anemia and perceived barriers to tablet consumption. Interventions should focus on increasing risk awareness and reducing perceived barriers through targeted education, supportive school environments, and multisectoral engagement to enhance anemia prevention efforts.

Abstract

Out-of-hospital cardiac arrest (OHCA) is most common in densely populated. Adolescent students are expected to contribute to the rescue as part of the social fabric. This study aimed to examine changes in the proactive behavior of school adolescents in basic life support education. The research used a pre-experimental design with one group pretest-posttest on 102 respondents at SMK Multimedia Mandiri Jakarta using descriptive and Wilcoxon tests.  The results showed that the respondents were predominantly late-phase adolescents (59.8%), male (79.8%), not family members working in health services (95.1%), and not familiar with basic life support (79.4%); (2) The cooperative behavior of adolescents in basic life support education show an increase in cognitive aspects, with an average pre-test score of 52 (bad) to 83 (excelent) in the average post-test score; (3) The results of proactive behavior show a significant clout on the readiness of adolescents in providing basic life support in the community environment living in densely populated settlements (p<0.001). The proactive behavior of teenage students as part of the community in basic life support education is very important. In addition, this study has implications for the independence and readiness of school adolescents to assist victims of out-of-hospital cardiac arrest (OHCA), especially in densely populated areas.

Abstract

Food waste remains a major challenge in school meal programs, as it not only causes economic losses but also leads to resource inefficiency, and reduced nutritional benefits. This study aims to analyze differences in food waste amounts across free nutritious meal programs at the elementary, middle, and high school levels, estimate nutrient loss, and identify related factors of food waste among students in East Aceh Regency. A cross-sectional design was employed involving 106 students purposively selected from three schools participating in the free nutritious meal programs. Data were collected using a weighing method over a 10-day menu cycle, anthropometric measurements, structured interviews on student characteristics, and food preferences. Data were analyzed using the Kruskal-Wallis test, Pearson and Spearman correlations, and linear regression. The results showed that the average food waste was 24,3% of the served portion, with the highest amount in middle school (29,8%) and the lowest in high school (16,5%). The largest proportion of waste came from staple foods, vegetables, and plant-based side dishes. Nutrient loss due to food waste was equivalent to 12 complete lunch portions for a child per year. Factors significantly associated with food waste were age, gender, BMI-for-age, pocket money, and menu variety (p<0,05). Older students, males, who perceived the menu as more varied, tended to waste less food. These findings highlight the importance of portion adjustment, appropriate serving times, diversified menus, and nutrition education to reduce food waste and optimize the benefits of the Free Nutritious Meal program

Abstract

Anemia is a serious global public health issue with the highest prevalence in adolescent girls. This study aimed to analyze the relationships among body image, eating behaviour, and the adequacy of energy and nutrient intake with anemia status in adolescent girls. This study used a cross-sectional design with 105 participants selected through purposive sampling. Data on subject characteristics were collected using a questionnaire, body image was assessed using the Body Shape Questionnaire (BSQ), eating behaviour using the Dutch Eating Behaviour Questionnaire (DEBQ), nutrient adequacy level using 2 × 24 H food recall, and hemoglobin levels were measured using HemoCue 301. Data were analysed using the Spearman Rank test. The results showed that 28.6% of the subjects had anemia. Based on bivariate analysis, there was no correlation between body image and anemia status (r=-0.108, p =0.271). There was also no correlation between emotional eating, external eating, and restrained eating (p>0.05) and anemia status. Similarly, energy (r= 0.133, p=0.177), vitamin B9 (r=0.169, p=0.085), and vitamin C (r=0.016, p=0.873) were not significantly correlation with anemia status. However, adequacy level of protein (r=0.288, p=0.003), iron (r= 0.194, p=0.048), and vitamin B12 (r= 0.216, p=0.027) were significantly positive correlation with anemia status. This study indicates that the majority of subjects had a negative body image, tended to display external eating behavior, and generally had insufficient nutrient intake. Therefore, nutrition education for adolescents is needed, with a focus on promoting healthy and high-quality eating habits.

Abstract

The effective management of postoperative pain and anxiety remains a challenge in patient care. Immersive virtual reality (IVR)-based distraction has been proposed as a non-pharmacological intervention to help reduce these symptoms. This study aimed to evaluate the effectiveness of immersive VR-based distraction in reducing postoperative pain and anxiety. This systematic review followed the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines by identifying studies that assessed immersive VR-based distraction in postoperative patients. Studies published from 2015 to 2025 were searched in ClinicalKey Nursing, EbscoHost, ProQuest, ScienceDirect, Scopus, and Taylor and Francis databases. Fourteen randomized controlled trials (RCTs) were included, covering various surgical contexts such as orthopedic, abdominal, cardiac, and oral surgery. Most studies showed that immersive VR-based distraction effectively reduced postoperative pain and anxiety, although some reported no significant effect. Variations in outcomes were influenced by patient characteristics, duration and frequency of VR use, and the type of VR content. Studies were excluded if they involved non-postoperative populations, pediatric samples, non-immersive VR, or non-interventional designs. Overall, immersive VR-based distraction shows promise in reducing postoperative pain and anxiety, although variability in effectiveness highlights the need for further research on technical parameters and clinical implementation
