Abstract

Introduction : Chronic low back pain is defined as pain in the lower back that lasts more than 12 weeks. The average prevalence of LBP sufferers is 39% in adults. In Lower Cross Syndrome there is tension in the thoraco-lumbar extensor muscles that cross the m. iliopsoas and m. rectus femoris. As well as weakness in the abdominal muscles that cross the m. gluteus maximus and medius. Schober test is used to determine ROM in the lumbar. Aquatic exercise can be used for the purpose of facilitating Range of Motion (ROM) exercises, facilitating weight-bearing exercises, minimizing the risk of injury or re-injury during rehabilitation, and promoting relaxation. Case Presentation : A patient named Mr. AN is 36 years old and works in the TNI. The patient complained of pain in the lower back and limitation of bending motion. Diagnosis of Chronic Low Back Pain (non-specific). Schober test measurements obtained results of 2 cm. Management and Outcomes : The physiotherapy process was carried out on patients in the hospital with 5 meetings. The aquatic exercise movements used include Stretching, Strengthening, Stabilization, Pelvic Tilt. Measurement of lumbar ROM using the Schober test. Discussion : After the patient received 4 sessions of physiotherapy, evaluation of the lumbar ROM measurement was carried out using the Schober test. The results of the measurement of lumbar ROM with the Schober test showed a significant improvement, from 2cm to 3.1cm. Where with these results it can be concluded that there is an additional lumbar ROM. Conclusion : The physiotherapy program which was carried out four times in cases of Chronic Low Back Pain using aquatic exercises which included stretching, strengthening, stabilization, and pelvic tilt was proven to be able to increase the Range of Motion (ROM) in the lumbar.

Abstract

Introduction: Stress is a phenomenon that occurs quite often in everyday life that we cannot avoid and will be experienced by each person. Stress is a condition that can be caused by several things such as uncontrolled physical demands, the environment, and social situations. Premenstrual syndrome (PMS) is a common problem that is often encountered by women which includes a collection of emotional and physical symptoms that occur in women in the 2-14 days before the onset of menstruation, or in the luteal phase of the menstrual cycle and then subsides soon after the onset of menstruation. College students are one of the groups that have a high potential for experiencing premenstrual syndrome due to increased levels of stress during academic education Objective: This study aims to determine the relationship between stress levels and premenstrual syndrome symptoms in final year nursing students. Methods: This type of research is observational with a cross-sectional study approach. The sampling technique used was purposive sampling, with the number of respondents as many as 115 final year D3 nursing students at Kusuma Husada University Surakarta in January. The instrument used to measure the level of stress is the Kessler Psychological Distress Scale (K10) and to measure the level of premenstrual syndrome using the Shortened Premenstrual Assessment Form (SPAF). Results: The results of the study using the Sperman rho' test, it was found that the p-value was 0.000 < 0.05, which means that there is a significant relationship between stress levels and premenstrual syndrome, and a correlation of r = 0.846 which means the relationship is strongly correlated with the direction of the correlation. positive Conclusion: The heavier the stress level, the more severe the premenstrual syndrome symptoms experienced.

Abstract

Introduction: Cerebrovascular disease is the second leading cause of death and the third most common cause of disability in the world. In developed countries, one in four men over the age of 85, and one in every five women over the age of 85 have a stroke. Research in Iran shows that 139 out of 100,000 people each year suffer a stroke. Damage to the cerebral can be classified into Ischemic Stroke (IS), which consists of 87% of cases and is caused by thrombotic occlusion of arteries and veins; or Intracerebral Hemorrhagic Strokes (ICHS), which comprise 13% of cases and are caused by rupture of blood vessels by hypertension or aneurysms, trauma, and translucent brain injury. Ischemic stroke is the death of brain tissue due to impaired blood flow to the brain, caused by clogged cerebral or cervikal arteries. Case Presentation: 72 year-old female office came to the hospital with complaints of difficulty moving the left side of her limbs, difficulty speaking, and the condition was so weak that it was difficult to respond. She is being hospitalized about 7 days and her condition still weak and hard to response. Management and Outcome: This therapy is carried out 3 times with a duration of exercise of 20 minutes. Muscle strength examination in acute stroke cases is measured using the Oxford Scale and level of neurological deficits measured using National Institute of Health Stroke Scale (NIHSS). Discussion: Muscle strength is strongly related to the neuromuscular system which is how much the nervous system can activate muscles to contract, so the more muscle fibers are activated, the greater the strength that the muscles produce. Thus, the more muscle fibers are activated, the greater the force produced by the muscles. In addition, the benefits of early passive exercise in stroke patients can also increase or maintain flexibility and muscle stiffness, maintain heart and respiratory function, prevent stiffness in joints, stimulate blood circulation, and prevent deformities, stiffness and contracture. Conclusion: physiotherapy program in acute stroke cases who are still hospitalized three times using Early Passive Exercise can increased muscle strength, and for neurological deficits still in the very heavy category. This therapy is advised to continue to be done in order to achieve the goal of patient recovery.

Abstract

Introduction: Osteoarthritis (OA) is known as degenerative joint disease where there is joint abnormality causing inflammation involving cartilage and surrounding tissue which result in morbidity and functional limitations, especially for older adults. Method: The aim of the study was to determine the benefits of performing Transcutaneous Electrical Nerve Stimulation (TENS), Patellar Mobilization, and Quadriceps Setting in Bilateral Knee Osteoarthritis Grade 2 on Elderly. The assessment used was Numerical Rating Scale Pain Assessment (NRS), Manual Muscle Testing (MMT), and Western Ontario and McMaster Universities Osteoarthritis (WOMAC). Discussion: The giving of Patellar Mobilization is able to increase function in older adults and also decrease knee joint stiffness and pain, meanwhile the giving of Quadriceps Setting is also able to increase Quadriceps muscle strength, and the application of Transcutaneous Electrical Nerve Stimulation (TENS) is able to decrease pain caused by OA and increase physical performance in patient with Knee OA. Conclusion: After doing physiotherapy as many as 4 times using Transcutaneous Electrical Nerve Stimulation (TENS), Patellar Mobilization, and Quadriceps Setting, it is gained the decrease of pressure and movement pain, the increase of right and left leg muscle strength, and also the increase of WOMAC functional Test result after being given Transcutaneous Electrical Nerve Stimulation (TENS), Patellar Mobilization, and Quadriceps Setting.

Abstract

Introduction : Hernia Nucleus Pulposus (HNP) is a condition that occurs due to biochemical changes in the spinal disc tissue. This change will cause rupture of the annulus fibrosus which makes the nucleus pulposus bulging (protrudes) towards the spinal canal which will put pressure on the nerve roots, causing symptoms in the form of neurological pain. Case Presentation : Mrs. E is a 37-year-old housewife with low back pain due to disc herniation with a history of post chemotherapy for breast cancer. Pain is felt when the patient is undergoing treatment, pain is like being stabbed and radiates from the lower back to both legs and especially the right leg, numbness and weakness in the right leg Management and Outcome : Treatments provided such us infrered, TENS, stretching, and core stability exercise performed 2 times a week for 4 weeks. The measuring instrument used is NRS, MMT, and Oswestry Disability Index. Discussion : One type of exercise that can be done for patients with lumbar HNP is core stability exercises in the form of abdominal hollowing exercises, prone lying, prone on elbow, prone press-up, prone press up with overpressure which can be selected by taking into account the ability of the patient. Core stability exercise is an exercise that functions as a synergistic activation that contracts the trunk and pelvic stabilizer muscles, to improve the control of the neuromuscular system and the strength of the trunk muscles and to maintain spinal stability which functions to maintain body posture and optimize the patient to control the trunk position. Core stability performed by the patient is able to reduce pressure on the nucleus pulposus during the spine extension position, so that nerve clamping can be reduced and can reduce pain symptoms, Conclusion : It can be concluded that in this case, the provision of physiotherapy programs in the form of infrared, TENS, stretching hamstrings, and core exercises performed 2 times a week for 4 weeks was able to reduce pain and improve functional ability in patients.

Abstract

Introduction : Rupture is the tearing of tissue caused by trauma. The anterior cruciate ligament (ACL) is one of the main ligaments of the knee which functions to prevent the tibia from shifting anteriorly from the femur and to control rotational movement of the knee. case presentation: A 29-year-old male patient with as a sports teacher came to physiotherapy complaining of pain and stiffness in the knee joint at Wongsonegoro Hospital, Semarang, Central Java. The patient did a surgery of anterior cruciate ligament on October 2020. The management of exercises set for phase 2 rehabilitation. Management and outcomes :The exercises rehabilitation program included static bicycles, forward lunges on bosu, side lunges on bosu, step ups on bosu, squads with theraband, forward step down, lateral step down with resistance bands for 1 month. Several physical examinations were carried out to examine pain of knee using VAS (Visual Analog Scale), muscle strength examination using Manual Muscle Testing (MMT) on the hamstring and quadriceps muscles, a goneometer used to measure joint range of motion for knee flexion and extension movements, and functional examination measured by Knee Injury and Osteoarthritis Outcome Score (KOOS). Discussion: exercises rehabilitation for phase 2 post surgery o anterior cruciate ligament (ACL) served to restore balance, increase muscle strength by stabilizing and strengthening during exercise for activation of neumuscular control. Conclusion : A month of exercises programs for three times a week were successfully decreasing pain, increasing muscle strength, range of joint motion and functional activity as well.

Abstract

Introduction: The slump test is a neurodynamic test to evaluate the mechanosensitivity of neuromeningeal structures within the vertebral canal. The straight leg raising (SLR) is a provocation test that evidences radicular irritation in the lumbosacral region by lower limb flexion and can be due to multiple causes. Active stretching effectively reduces pain and improves function in adults with LBP (first degree) in sub-acute and chronic phases. Methods: This study was a case report that observed twelve patients with chronic LBP divided into two groups. Group 1 received slum and SLR stretching, while in the group 2 did not treated with stretching, however only got the SWD and TENS. The pain intensity and functional ability were evaluated after treatment by VAS and Oswestry scale respectively. Objective: To determine the effectiveness of slump and SLR stretching for reducing pain and increasing ADL in patients with chronic myogenic LBP. Results: Fifty patients with a diagnosis of chronic myogenic LBP, 12 patients met the inclusion criteria devided into 2 groups. 11 of 12 patients reported decreased pain and increased functional activity. There was a decrease in pain 9 points and an increase in the functional activity of 12.3% in the group 1. While in the group 2, there was a decrease in pain as 3 points and an increase functional activity 2.33% as well. Conclusion: Slump and SLR stretching can reduce pain and increase functional activity in chronic myogenic LBP patients, although the intervention was only given twice and few samples. Further research needs to be done by increasing the number of samples and training doses.

Abstract

Introduction: Falls` is the third leading cause of disability among the elderly and a public health problem with major social impacts worldwide in countries with significantly aging populations. More than 30% of people over the age of 65 fall each year and half of the cases fall again. Dynamic balance and leg muscle strength have a close relationship with the risk of falling. Reduced hip muscle activity also affects the body's ability to maintain balance which has negative consequences on functional performance. Case Presentation: Mr. IW is 74 years old with complaints of pain from the lower back sometimes radiating to the left side of the leg since 1 year ago. The left lower leg feels heavy and the pain increases when doing strenuous activities and walking long distances. Complaints reduced with rest. The patient's history of comorbidities has heart disease and gout. Management and Outcome: Patients received the Otago Exercise Program and Bridging Exercise in 1 month with a prescribed dose. Based on the results of the BBS examination, 30 second Chair Stand, TUG Test, and the Indonesian version of the ODI modification, there was an increase in balance and muscle strength of the lower extremities as well as an increase in functional activity seen in patients Discussion: History of low back pain that can causes changes in balance and postural control so that it has a higher postural sway and greater displacement of the center of pressure. For the elderly to be able to enjoy a healthy life and be free from worries about accidents that result in bruises such as falls, they need good muscle strength and balance skills. Lower leg muscle weakness is one of a predictor of falls. Another study found that regular exercise, lumbar muscle strengthening exercises and balance exercises were effective in preventing injury in the elderly and improving the function of the sensory-motor system, which is necessary to keep the body safe. The specially designed OEP consists of strengthening and balance exercises targeting and improving dynamic balance outcomes in the elderly. Core strengthening exercise in the form of Bridging Exercise can restore the body's postural control function and increase range of motion and is considered an appropriate exercise for the elderly. Conclusion: Physiotherapy programs for the elderly at risk of falling using the Otago Exercise Program and Bridging Exercise can improve balance and muscle strength in the lower extremities as well as increase the ability to functional activity in elderly individuals.

Abstract

Intoduction: Osteoarthritis is a degenerative disease characterized by the deterioration of joint cartilage which causes bones to rub against each other and triggers pain, stiffness and movement disorders. Osteoarthritis comes from the Greek word osteo which means bone. arthro which means joint, itis which means inflammation. Case Presentation : A 87 years old patient with Knee osteoarthritis come to PFDA Pekalongan Clinic. The patient had pain in both knees since 1 year ago. The patient feels pain in the knee when sitting to standing position, the patient complains of aches and pains in both knees when walking long distances. Pain is felt during activity. The intensity of pain increases at night. The patient has been to the doctor and has been injected 4 times. Management and Outcome : IR, TENS and strengthening exercises using theraband with hip extension standing position with elastic band and knee extension sitting position with elastic band was performed for 30 minutes per session, 3 times a week for 4 weeks. Before the intervention, the patient still felt pain and impaired functional activities with a NRS value of 6 and a WOMAC score of 28 . However, after a 4-week intervention, there was a decrease in pain (NRS) to 2 and functional disability increased with a WOMAC score of 23. Discussion : Exercise is carried out repeatedly according to a predetermined dose, This is where there is a decrease in subchondral inflammation resulting in decreased pain, and increased stability. To increase functional activity, theraband exercise provides direct stimulation to the skin receptors and which affects proprioception and maximizes sensation around the knee joint thereby increasing the performance of daily activities Conclusion : It can be concluded that in this case, the provision of a physiotherapy intervention program in the form of: IR, TENS and Theraband Exercises performed 3 times a week for 4 weeks was able to reduce pain, increase muscle strength and increase functional activity.

Abstract

Introduction: One of the ligaments in the knee is the anterior cruciate ligament where the function of the ligament itself is joint stabilization. 60% sports injuries occur in the knee, one of which is specifically a tear in the anterior cruciate ligament (ACL) ligament with the largest incidence rate of 16% during sports activities then the role of physiotherapy using close chain kinetic is expected to overcome pain, increase range of motion, reduce muscle atrophy and increase muscle strength. Case Presentation: A 23-year-old man last year had an accident while playing soccer and the patient felt the instability of his right knee when he was running, then after an MRI was done at the hospital 6 months ago the patient had ruptured anterior cruciate ligament ligament (ACL) so 3 months ago it was done post anterior cruciate ligament (ACL) reconstruction measures. Management and Outcomes: Giving therapy Exercises such as close chain exercise are effective for reducing pain, increasing muscle strength around the knee joint, reducing atrophy in the muscles around the knee joint as measured by the medline. Discussion: After doing physiotherapy rehabilitation for 6 times therapy, the pain reduction from the first therapy was worth 3 then the last therapy was 2 measured by NRS (numeric rating scale), then the range of motion increased by 5o from before exercise measured with a goneometer, for anthropometry it increased by 0.5 cm to 2 cm and muscle strength was stable at 5. Conclusion: After 2 weeks of therapy for 6 x sessions with intervention of close chain kinetic exercise therapy, the results were decreased pain, increased range of motion, increased muscle strength, reduced repeated falls, and reduced atrophy in the muscles that work on the knee joint.
