Abstract

Bronchopneumonia is a type of pneumonia which affects one or several lung lobes which are marked by the presence of infiltrates caused by bacteria, viruses, fungi, and odd object. Children with bronchopneumonia are usually characterized by symptoms of fever, tachypnea, productive cough, decreased appetite, decreased breath sounds, nasal lobe breath, chest wall retraction and lethargy. The impact that will occur on children with bronchopneumonia is OMA, atelectasis, pleural effusion, emphysema and meningitis. The purpose of writing is that students are expected to be able to provide nursing care to children with bronchopneumonia. Writing method is descriptive or description of a case. The results of scientific papers obtained by An. F with bronchopneumonia with signs and symptoms of fever, productive cough and sound ronchi in the right lung. Nursing problems at An.F are: ineffective airway clearance, risk of nutritional deficits, risk of hypovolemia, knowledge deficit and risk of infection. After three days of implementation, it was found that two problems were resolved and three that had not been resolved were ineffective airway clearance, risk of hypovolemia and risk of infection.

Abstract

Imminent abortion is spotting bleeding that shows a threat to the survival of a pregnancy (Yeyeh, 2010). The purpose of writing this journal is to get real experience in providing nursing care to mothers with imminent abortion. The method of this journal is descriptive narrative with data collection techniques through case studies by providing nursing care. The rate of spontaneous abortion is around 11 million out of 208 million pregnancies in the world, that is almost 90% occur unsafem thus contributing 11%-13% of maternal deaths (Ministry of Health, 2015). The results of this research is : patient who have imminent abortion get a weakness, patient complained have a bleeding such as menstruation, patients said dizzy, patient complained of pain in the lower abdomen, patients said pain like being stabbed, the patient said the scale of pain 6, pain felt settled and lost.

Abstract

The family is two or more people, individuals who are interdependent with each other to a variety of support, including emotional and economic support the family is the smallest unit of society and as a recipient of nursing care, for that family health care is needed to help improve public health. Currently, public health problems can be caused by habits such as junk food, spicy and sour foods, gas-containing foods, and irregular eating. This can cause diseases, one of which is Gastritis. To be able to overcome problems in families with gastritis, nurses can provide comprehensive nursing care using a nursing process where nurses have an important role in the aspects of promotive, preventive, curative, and rehabilitative.

Abstract

Violent behavior is a form of behavior that aims to hurt someone physically and psychologically. This will have a major impact if not addressed. For example people with violent behavior can cause a high risk of injuring themselves, others and the environment. Based on the impact of violent behavior above the role of nurses is needed in carrying out nursing care, namely nurses play a role in the promotive, preventive, curative, and rehabilitative aspects. Nursing care is done by interviews, both to clients, nurses and families, nursing actions are carried out directly on the client with the assistance of nurses. The client experiences violent behavior because the client feels hurt because he is always ridiculed by his neighbors and brother. Nursing diagnoses that can be found on the client are low self-esteem, sensory disturbance perceptions: hallucinations, risk of violent behavior, ineffective therapeutic regimens. Planning has been carried out in accordance with hospital standards. Client nursing evaluations seen in clients are able to control violent behavior by deep breathing and hit the pillow / mattress, taking medication regularly, expressing, asking, and refusing verbally as well as spiritually, namely dhikr and prayer. Another ability that the client achieves is that the client is able to control the hallucinations that arise, and the client is able to increase his self-esteem.

Abstract

Diabetes mellitus (DM) is a group of metabolic disorders characterized by increased blood glucose levels (hyperglycemia) due to damage to insulin secretion, insulin action or both. Diabetes mellitus can cause complications in hypoglycemia and diabetic ketoacidosis, while chronic complications can include coronary heart disease, cerebrovascular disease, peripheral vascular disease, diabetic retinopathy, diabetic neuropathy and diabetic nephropathy. The client is named Mr. R, male, aged 25 years with a medical diagnosis of type 2 diabetes mellitus. After the assessment, a nursing diagnosis was obtained, namely ineffective airway clearance associated with retained secretion, the risk of fluid volume deficiency associated with excess gastric loss: diarrhea, risk of changes in nutrition less than the body's needs associated with nausea, insufficiency of insulin, acute pain associated with hyperperistaltic and high risk of spread of infection to sepsis associated with high glucose levels, effects of invasive procedures: infusion installation, pre-existing respiratory infections. The results achieved after performing nursing care are three nursing diagnoses that have been overcome ie ineffective airway clearance associated with retained secretions, the risk of lack of fluid volume associated with excess gastric loss: diarrhea and acute pain associated with hyperperistaltic. In addition, there is 1 (one) nursing diagnosis that is partially resolved, namely the risk of changes in nutrition less than the body's needs related to nausea, insufficiency of insulin and there is 1 (one) nursing diagnosis that has not been resolved, namely the high risk of spread of sepsis associated with high glucose levels, effects invasive procedure: infusion installation, respiratory infections that existed before.

Abstract

The biggest threat to newborns is a change in temperature in the uterus and room temperature that can cause hypothermia. Also, the bathing procedures have a risk of causing hypothermia especially in premature babies. The swaddle bath can prevent heat loss through the mechanism of convection, conduction, evaporation, and radiation. This study aims to determine the effectiveness of swaddle bath in maintaining body temperature stability. 50 premature infants who weigh> 1500 grams and body temperature> 350C-.5037.50C were involved in this study. They are divided into swaddle bath and conventional bath groups. The research method was quasi- experimental pretest-posttest control groups for 1 month at Al Ihsan Public Hospital Regional in West Java. Statistical tests use the Wilcoxon test to test pre and post-test data between each group and Mann Whitney test to test post-test data between two groups. The results of the analysis proved that swaddle bath can maintain body temperature with p value 0.018 <α 0.05. The conclusion of this research is the swaddle bath technique can maintain the stability of the body temperature of premature babies after bathing procedures. It is recommended that swaddle bath can be used as a permanent procedure for bathing premature babies in perinatology care rooms.

Abstract

CHF or Congestive Heart Failure is a clinical syndrome or a set of signs and symptoms are characterized by shortness of wicked breath and fatik ( at nest or during activity). This cause by the inability of the heart to pump blood throughout the body, due to structural and functional disordes of the heart. Moreover, signs and symptoms will be found like shortness of breath at rest or during activity, feeling tired, not powerful, water retention such as pulmonary congestion. Leg edema, and abnormalities of heart structure and function (Marulam M, 2015). According to LeMone (2012), the consequences of heart failure quickly affect the lack of blood supply, thus causing cell death due to lack of oxygen carried in the blood it self. The goals of this research is to get real experience in nursing care for the patient. The method in this research is descriptive method or description of a case. The nursing problems in the form of cleaning the airway is not effective, in effective airway clearence associated with increased sputum production, decreased cardiac output associated with changes in myocardial contractility, structural changes, excess fluid volume is associated with retention. Four nursing diagnoses have not been resolved because congestive heart disease is an end-stage heart disease that is irreversible where the heart will not be able to recover as before

Abstract

Low self-esteem is a feeling of worthlessness, insignificance and prolonged inferiority due to negative evaluations of oneself and self-ability. The loss of self-confidence, feeling like a failure because it is unable to achieve desires according to self-ideals. Someone who experiences low self-esteem will tend to be shy and aloof when in a public place, and prefers to withdraw from social life. Based on the impact of low self-esteem above the role of nurses is needed in carrying out nursing care, namely promotive, preventive, curative and rehabilitative. Nursing care is done by interviews, both to clients, nurses and families, nursing actions are carried out directly on the client with the assistance of nurses. The client experiences low self-esteem because the client is attacked by residents in the home environment of his ex-lover because client often come to the home of his ex-lover. Nursing diagnoses that can be found in clients are low self-esteem, social isolation, risk of sensory perception disorders: hallucinations and self-care deficits. Nursing planning is carried out in accordance with Hospital standards. Nursing evaluation can be seen in the client being able to identify the positive abilities possessed by the client, carrying out the abilities possessed, namely singing, sweeping, folding clothes and tidying up the bed. Another ability achieved by the client is able to get to know one person or more, controlling the hallucinations that arise and the client can maintain his cleanliness.

Abstract

Chronic Kidney Failure (CRF) is a progressive and irreversible renal function disorder in which the body fails to maintain metabolism, fluid and electrolyte balance, causing uremia. Hemodialysis is an attempt or an act of cleansing the blood of toxic substances that cannot be excreted by the kidneys from the body. The impact of chronic kidney failure if not treated immediately is hypertension, anemia, pulmonary edema, cardiac tamponade, proteinuria, hematuria, decrease in white blood cells, bleeding disorders, fetor uremic, renal osteodystrophy, psychotic symptoms of convulsions and coma, bruising, excoriation, irregular menstruation . The purpose of writing is expected that students can gain real experience in providing nursing care to clients with chronic kidney failure on HD. The research method is descriptive method and library study. The results of this scientific paper are students gain real experience about providing nursing care to clients with chronic kidney failure on HD. Nursing problems in the form of ineffective airway cleaning, ineffective breathing patterns, risk of damage to gas exchange, high risk of decreased cardiac output, excess fluid volume, risk of nutritional disorders less than body requirements, risk of infection.

Abstract

Dengue hemorrhagic fever (DHF) is an acute epidemic disease caused by a virus transmitted by the Aedes aegypti and Aedes albopictus. Dengue hemorrhagic fever (DHF) is infectious disease caused by dengue virus with some symptoms include fever, joint and muscle pain, rash, trombhocytopenia and bleeding diathesis. The purpose of writing is that students are expected to get real-life experiences to provide nursing care for children with DHF. The methods of writing are descriptive and literature. The result of the scientific paper is that students get an overview of nursing care in children with DHF. The nursing diagnosis in case of An.A: Risk of fluid volume deficit, Imbalance of nutrients less than body requirements, Risk of bleeding and anxiety.
