This journal covered basic nursing sciences, pediatric nursing, medical surgical nursing, maternity nursing, emergency nursing, mental health nursing, community nursing, family nursing, gerontic nursing, and nursing, and health policy. This journal is delimited into topics related to original research, reviews, case studies, and letter to the editor, which contribute to the development of the nursing scientific field. We aim to promote the development of nursing research into clinical evidence in practice. Abstract

Shadow teacher in their profession are always dealing with special needs children who have intellectual impairment, behavioral and emotional disorders and learning disorders. This is an exhausting job because shadow teachers should be able to ensure that the child is always adaptive during the lesson. Shadow teacher also must know how to adapt to the child’s character, and this task also spends a lot of time. However, the existence of children in the class is not supported by good learning guidelines. The purpose of this study was to explore the experience of shadow teachers with the child with special needed. This qualitative research used Interpretative phenomenology research design and data analysis used interpretative phenomenology analysis approach. Data collection process used in-depth interviews and observations, and this research involved six participants. This research produced 9 themes: (1) believe that this work is a blessing from Allah, (2) feel that there is a wisdom from helping each others (3) exhausting for mind and emotions, (4) feel not confident at the beginning of his profession, (5) havea feeling that this job is a meaningful job, (6) believe that the children can be disciplined with strict rules, (7) believe that visualization is the way to make children’s focus increased, (8) feel that togetherness must be build together with the children, and (9) feel that government and society’s concern to these children with special needs is still lacking. Guiding and leading children with special needs is hard to do, but if this job is done with a wholehearted feeling it can make an incredible effort and can make the children with special needs can live peacefully in the middle of society.Shadow teacher in their profession are always dealing with special needs children who have intellectualimpairment, behavioral and emotional disorders and learning disorders. Abstract

Indonesia doesn’t have a national standard for triage system which means implementation in each hospital can be different. Nurses have an important role in the application of triage. The Australasian Triage Scale method (ATS) is one of the triage system in the world used in some countries including Indonesia but it is very difficult to implementation in overcrowded emergency departement. Nurses ratio and unstandard triage room make the ATS standart times is difficult to achieve. The aim of this study was to analyze the various factors affecting Australasian Triage Scale (ATS) in emergency departement NgudiWaluyo Wlingi Hospital. We used quantitative research with cross sectional approach and the respondent are 28 nurses in Emergency Departement Ngudi Waluyo Wlingi Hospital. Univariate analysis is used toknow the description of each variable. Fisher test to determine the relationship between variables. Logistic regression analysis to determine the most dominant factor affecting the application of Australasian Triage Scale. Fisher test result show p-Value as follows: performance factor (leadership), p =0.028 < α = 0,05, client factor (waiting time) p = 0.011 < α = 0,05, equipment factor (triage documentation) p = 0,036 < α = 0,05, staff requirements factor (education) ) p = 0.013 <α = 0,05, staff requirements factor (training) p = 0.036 <α = 0,05 The result of logistic regression shows staff requirements factor (education) p = 0.019 < α = 0.05 with OR 0.059. In conclusion there is a significant affecting between performance leadership factor, waiting time factor, triage documentation factor,education factor, training factor on the application of ATS and education is a dominant factor to predict the accuracy of the application of ATS.

Abstract

Living with a chronic illness surely can affect clients and their families. Family capacity to adapt successfully can be quite different depend on characteristic of chronic illness and family perception about the illness. Consideration of the family’s view of illness support health care professionals in finding the best resources to assist family with chronic illness. The aim of this study was to describe impact of chronic illness to clients and their family. This research use a qualitative design with descriptive phenomenological approach. Using purposive-sampling, eight families living with chronic illness were participated in this study. Colaizzi’s method which involving participants to validate results was used in the research. The theme of this study are changes in the patient's health status, family system changes and family difficulties in maintenance tasks. Conclusion of this research is that family members are the biggest support to motivate clients and also challenged as they strive to assist member diagnosed with chronic condition to stay healthy. Involving clients and families in the decision making support the efforts of families in their self-management of chronic illness. Thus nurses could provide family counseling to strengthen family coping and capacity by providing family with adequate information about illness.

Abstract

Measurement of severity and mortality predictions in cases of head injury is an important step to support appropriate, effective and efficient clinical decision-making. Glasgow Coma Score, Trauma Score, and Revised Trauma Score are physiologycal scoring systems that can be used as predictors of mortality of head injury patients. The objective of the study was to know which scoring systems had the best accuracy as a predictor of mortality of head injured patients. The observational analytic study with this retrospective cohort design approach was carried out at dr. Iskak Tulungagung on Medical Record. Data taken from all patient medical records from January 2016 to July 2017 amounted to 150 medical records. Variables used were GCS, RTS, and TS scores when patients were admitted to hospital IGD as well as patient mortality data within 7 days of treatment. The data obtained is then processed with SPSS 20 using Mann Whitney Test, logistic regression and ROC curve comparison. Based on the results of multivariate logistic regression analysis, GCS accuracy results: sensitivity= 0.7586, specificity= 0.7934, PPV= 0.468, NPV= 0.932; RTS accuracy: sensitivity= 0.766, specificity= 0.8, PPV= 0.489, and NPV= 0.932; TS accuracy: sensitivity= 0.7, specificity= 0.783, PPV= 0.4468, NPV= 0.9126. The comparison graph analysis of ROC curve showed no significant difference of AUC between GCS, RTS, and TS because the difference was too low (<15%). There was no significant difference of accuracy between RTS, GCS, and TS use in predicting head injury mortality

Abstract

Disasters occur when vulnerable people are faced with threats that they are unable to cope with or have low capacity to overcome them. Preparedness is the most critical phase in the range of disaster management, the inadequacy of disaster preparedness planning has created the suffering of survivors and loss of life. The purpose of this study was to analyze factors related to family preparedness in Ternate city. This research use descriptive analytic design with Cross sectional study approach. Respondents in this study were 113 Head of Family (KK) by using systematic random sampling. Chi-square test shows the related relationship (p = 0,005), attitude (p = 0.000) and social capital (p = 0.000) to risky family preparedness in Ternate city. The result of logistic regression test shows that social capital has the strongest strength of relationship (p = 0,022, OR = 2,725) compared to attitude (p = 0,025, OR = 2,680) and knowledge (p = 0,041, OR = 2,520). His result shows that good knowledge, attitude and social capital will increase the family preparedness in the face of disaster impact in Ternate City.

Abstract

 Acute Coronary Syndrome (ACS) is a group of symptoms that are classify as unstable angina (ATS), ST-Segment Elevation Myocardial Infarction (STEMI) and non ST-Segment Elevation Myocardial Infarction (NSTEMI). Main objective to provide timely reperfusion therapy is essential to improve survival. Initial and rapid intervention after the occurrence of coronary artery disease is associated with better clinical outcomes, but it is hampered by the nature of the patient's seeking decisions Patients expect the type of heart attack they often see in medical movies and television: like chest pain that grips and causes it to fall to the floor. This study aims to analyze the chest discomfort experienced by patients associated with delay in reperfusion therapy of Acute Coronary Syndrome. The method used is observational analytic with cross sectional approach. Nonprobability sampling with time quota technique obtained sample of 60 respondents. The results obtained as many as 55% of patients come more than 120 minutes after the onset of discomfort in the chest. The patient's delay in coming to the associated hospital is an accompanying symptom factor at onset with p = 0.048, and OR = 4.596 (1.014-20.828). There were no significant differences in the scores of knowledge, behavior, and beliefs based on the ACS response index between patients who came <120 minutes and> 120 minutes. The conclusions of this study present the accompanying symptom presentations at onset with the time interval between the appearances of the SKA symptoms until arriving at the IGD door. Seeing the results of this study suggests the need for efforts from medical staff to optimize the interpretation of "chest pain characteristics" to patients and families or who are susceptible about specific and non-specific cardiac signs. Keywords: acute coronary syndrome, prehospital delay, onset to door, reperfusion

Abstract

Acute Coronary Syndromes is the leading cause of short-term and long –term mortality. An aggressive treatment approach has the potential to change the prognosis of patients with ACS, although its depends on risk factors. There are several risk scores  such as Thrombholysis in Myocardial Infarction (TIMI), Global Registry of Acute Coronary Events (GRACE), and Killip. The aim of this study is to analyse the differences of correlation of TIMI, GRACE, and Killip risk scores as predictor Length Of Stay patients with Non St-Elevation Myocard Infarction ACS. The method of this study utilized an analytic observational design with the cohort retrospective. The research was conducted at RSUD Dr.Iskak Tulungagung used 98 samples of medical records patient with NSTEMI ACS in the past 8 months (January-August 2017). The data collection techniques was completing risk score of TIMI, GRACE, and Killip and Length of Stay patient in ICCU ward. The results and analyse of Spearman Rank showed that TIMI has p = 0,000 r = 0.466,  GRACE p = 0,000 r = 0.598, and Killip p = 0,000 r = 0.441 (p < 0,05), so that it can be said that there was  significant differences between the risk score of TIMI, GRACE and Killip as predictor prognosis patient with NSTEMI ACS. It can be concluded that the TIMI, GRACE and Killip risk scores have correlation as predictor prognosis patient with NSTEMI ACS, which is GRACE risk score has superior to TIMI and Killip.

Abstract

Hospitalization is an issue that is not expected by everyone, especially children, because they have to undergo some procedure of action and examination. This problem causes anxiety and stress for parents, one of which is marked by muscle tension. This problem can be overcome by nursing actions using Progressive Muscle Relaxation Therapy and Supportive Therapy. The study aims to distinguish the influence of Progressive Muscle Relaxation Therapy and Supportive Therapy in reducing the anxiety parents with children undergoing hospitalization. The research applied quantitative approach with Quasi experimental pre post test, the number of respondents 25 pairs of parent each group, using purposive sampling technique. The research location at RSUD.Dr.R.Soedjono Selong, East Lombok regency on August to October 2017. Based on the result of the study showed decrease of anxiety score the Progressive Muscle Relaxation Therapy was lower than Supportive Therapy, which was 6.30 < 10.44, and t value was 0.000. The used of Progressive Muscle Relaxation Therapy was significant which p value was 0.000. The conclusion of the study results There was a difference in effect between Progressive Muscle Relaxation Therapy and Supportive Therapy in lowering parental anxiety levels, with the results of Progressive Muscle Relaxation Therapy is better to lower parental anxiety than Suportive Therapy. Further research needs to be developed modification of Progressive Muscle Relaxation Therapy versus Supportive Therapy in other groups.

Abstract

Stroke is a condition which occurs when the brain's blood vessels fail to supply oxygen to the brain cells which caused the brain not functioning properly. As such, stroke patients cannot fully support themselves. However, with constant and proper treatment, it can ease the burden on the patients, minimizing their disability, and reducing their dependence towards others while undergoing an activity. During the recovery phase, family plays an important role, they served as a source of support even during the healing and rehabilitation phase, as such, they are expected to be involved in the process of treatment since the very beginning. The purpose of this study is to identify the correlation of family support with stroke patients’ independence in the medical rehabilitation center of Dr. Iskak hospital Tulungagung. In order to focus on the stroke patients, this research uses a purposive sampling method which follows certain predetermined criteria. The research data were collected by using structured interview techniques by utilizing Barthel Index’s Instrument and family support instrument. Based on the results of the study, out of 57 respondents, the average score for family support was 87,84 while the stroke patients’ independence receives an average score of 81,75. Statistical analysis showed significant value = 0.00 whereas its sig value <α = 0.05 These numbers show a strong connection between family support with independence, which means that there is a correlation between family support and stroke patients independence. In conclusion, by giving a better family support the stroke patients can gradually become more independent. Essentially, in order for the stroke patients to gain better condition, it is advised that the paramedics should improve their role in providing education about the importance of family support towards stroke patients’ family members as the family plays an important role during the recovery process.

Abstract

Proper, readable, and complete documentation will have an impact on the quality of care and defense of potential malpractice suits, may be used as legal evidence in the event of a claim, but much of the nursing documentation is found to be incompatible or does not contain the necessary information in the case of the judiciary, lack of records or documentation in the patient’s medical record often hampers the exercise of the protection of nursing professional rights, both legally and administratively. The purpose of this research is to know the relation of workload factor based on workload assessment with the completeness of nursing care documentation by the nurses in the room IGD RSU. Anutapura Palu and RSU. Undata Palu. This research use correlation analytic design with cross sectional approach with total sampling to 65 samples period from 25 March 2017 until 25 April 2017. in IGD RSU room. Anutapura Palu and RSU. Undata Palu. From result of bivariate analysis known that there is correlation between work load factor with completeness of nursing care documentation with p value = 0,022. The result of regression test showed that the work load factor has regression coefficient value is -15.648 (negative value), which means that the higher workload, the percentage of completeness of the documentation of nursing care will be lower.
