It is concerned with quality of this hospital. To answer this problem, research was done using Quality Function Deployment (QFD). Quality Function Deployment is a tool which design some needs include customers represented as a voice of customer and including some competitions and also groups some activities that usually called affinity graphic ang getting a benchmarking for it is competition. From the result analysis can be showed that main attribute for patience is a accuracy. And from House Of Quality can be found that getting a periodic meeting to evaluate this hospital and also increase a service can be made 20 concept

Abstract

Occupational health and safety is very important point for a company. CV. XYZ is a company that produces paving and have problem about the occupational accident. There are 4 production procceses that can be couse occupational accidents, which is taking raw materials procces, stirring batter, forming and drying. This case must be analyzed using HEART and JSA method as evaluating effort for occupational accident. The results of HEART method analysing imply that HEP value 0,8632is the most dangerous occupational, that is forpicking gravel from cement mixer machine. After known the value of HEP afterward analyzed using JSA method. The goal is as improvement to occupational accident in the production process in order can be reduced. The result of JSA method analysing are improvement suggestion such us levying mask, hat, design of additional components for machine, additional equipment for pallet and hand pallet, and assigning standart operational procedure.

Abstract

X Airlines is experiencing quality improvement issue related to complaints addressed to their service. Complaints are increasing in holiday and feast day, especially on January, August, September, and October. Improvement is needed to produce better service and better sevice is one of the key aspect of customer satisfaction. Customer satisfaction of X Airlines was measured based on several attributes. The service quality attributes were also grouped into must be, one dimensional, attractive, indifferent, questionable, and reverse category. The quality improvement was designed by using Quality Function Deployment (QFD) method and Kano Model. The result showed that customer’s satisfaction in all service quality attributes were still not being met the requrement. The key attribute indentified on satisfaction measurement and the Kano Category were the on time of departure corresponding to the schedule (one dimensional category), the efficient language storage facilities (one dimensional category), the tasty and good quality of the food (one dimensional category), and skilled staffs in serving the customer avoid tardiness, giving compensation when tardiness happened, conducting airlines staff service excellence program, conducting cooking class program, and providing high technology and up to date entertainment facilities on the plane.

Abstract

The competitive working neighborhood, company needs a plan to create the future of organization through change which was done since now. Along this time, PT Mayatama Manunggal Sentosa focused only in financial while measured the performance. But in reality, the company often got complains from customers so that there needed certain method to conclude the problem. In order to conclude the problem, there used Balanced Scorecard Method since the method translate organization’s vision, mision, and strategy in balanced way through four perspective. They were financial perspective, customer perspective, internal business process also learning and developing process. From the work performance measurement system of PT Mayatama Manunggal Santosa, there existed strategic destination, Key Performance Indicator and map strategy. Then there existed a weighed strategy through Analytical Hierrarchy Process (AHP) while the next step was relation system between perspective and strategic destination destined to map strategy. The result of work performance in third period 2007 by using OMAX method and Traffic Light System there was got 13 KPI in green indicator, 8 KPI with yellow colour and only 1 KPI in red colour, it was tempered glass production amount, so that management needs to do an exact repair to increase the performance in the next period.

Abstract

Production scheduling is an important thing done in the company so that consumer demand is met on time. PT Mitra Mulia Makmur implements the scheduling principle of First Come First Serve in producing plastic container products. The scheduling applied is not yet effective and efficient because there are some late consumer demands. The delay in consumer demand is due to longer processing time than the specified time limit. In the study using Integer Linear Programming (ILP) method to minimize total tardiness (total tardiness). ILP was chosen as a method of solving delay problems. ILP can help the company’s decision-making process to achieve a purpose function by considering the constraints or constraints in the company. Linear Integer Programming method with the help of LINGO 16.0 software can produce improvements.

Abstract

Company can be competitive in industry with giving a product which pursuit with the specification and have no defect, in order to produce product which have good and uniform quality existed variance had to be minimized. Six Sigma was a total approach to solve problems and increase process through DMAIC phase (Define, Measure, Analyze, Improve, Control). Measure phase with Pareto diagram of defect cone polyester 30, it was found that percentage of cone defect polyester 30 were: lapping, swelled, stitch, pattern, wrinkles, and ribbon and in the measure phase, it was known that sigma value equal to 3,05. Analyze phase used cause and effect diagram to analyze causes of a problem in detail. Improve phase used potential failure mode effect and analysis (PFMEA). In control phase statistical process control (SPC) was used, attribut data np control chart. The result of research were reducing DPMO to 29,87% and increasing value of sigma to 3.8 after the application of Six Sigma.

Abstract

This research was expected to apply QFD in the development process of product which refers to consumer request, so that the result of scheme process really answers market requirement. The research methods were conducting product survey in field, making questionaire of consumer request, spreading questioner, checking returned questioner data, and the processing questioner data by QFD. The matrix of concept assessment shows that concept A (funnel one semi automatic) gets value 0.238 and 16510%; concept B (funnel two semi automatic) gets value 0.211 and 16630%; concept C (funnel two semi automatic/manual) gets value 0.203 and 18470%; so that amongst the third concept, the one which is appropriate to be proceeded to the production department is only concept A.

Abstract

This research has goal to compare there alternative layout performances CRAFT,MULTIPLE, and BLOCPLAN. Using the problem of layout which include forward flow material and fordward backward flow material can be found result : Multiple Algoritm has total distance and material cost minimally but the problems that have lower departement,CRAFT algoritm can be used better than MULTIPLE algoritm because almost gives the same result with easier input process.

Abstract

Considering that raw material cost could reach 70% of product cost, supplier selection process was become important consideration to improve the efficiency of procurement department. The supplier selection process was multi-criteria problem involving qualitative and quantitative factors. UD Virgo Bersinar was one of fruit chips manufacturer located in Malang. The company often experienced shortage of jackfruits which interrupt production process. Thus, it was necessary to choose its supplier by considering company’s criteria and order allocation of selected supplier. This research would integrate supplier weight from Analytic Hierarchy Process and Preemptive Goal Programming model to determine priority of supplier and order quantity to the supplier. The model was formulated to achieve two objective functions, namely maximizing total value of purchasing and minimizing total cost of purchasing, by considering capacity of supplier and fulfillment of jackfruit. By running goal programming model, it was concluded that supplier from Semarang would supply 7.5 ton of jackfruit in November 2013-Januari 2014 and deficiency of needs would be supplied by supplier from Tajinan.
