Dr. Saiful Anwar Malang is a state hospital has done it is job and function, but in 3rd class of pavilion room, the number of patient decrease dramatically. It is concerned with quality of this hospital. To answer this problem, research was done using Quality Function Deployment (QFD). Quality Function Deployment is a tool which design some needs include customers represented as a voice of customer and including some competitions and also groups some activities that usually called affinity graphic ang getting a benchmarking for it is competition. From the result analysis can be showed that main attribute for patience is a accuracy. And from House Of Quality can be found that getting a periodic meeting to evaluate this hospital and also increase a service can be made 20 concept

Abstract

UD. BJ Mendit on Mendit Barat, Kec. Pakis is one of centrale of home industry in Malang with it's roof production process. From the observation that the researcher do into UD. BJ Mendit, finded that the production process had been doing often with technical and non technical problems that make some failed roof product with the criterion are keropos, break, barst, gopel, sandbank. Way of this roof production planning repair is important to do so that can make this production unit produce an unfailed product and reduce loss. Purposes of this research are identifying types of roof failed product, analysing main cause of roof failed product with FMEA, and giving recommended action for Mendit's roof production process repair. This research doing the same steps as same as Six Sigma cycle: DMAIC, but not doing the last step, Control. On Define step, are doing product identifying and make the Six Sigma team. On Measure step, are determining priority failed product identifying into CTQ (Critical To Quality) and measurement of present performance with counting the DPMO (Defect Per Million Opportunities) dan Sigma level. On Analyze step, is detecting main atributs that influencing the failed product on CTQ. On Improve step, is doing process improvement with developing alternative to reduce failed product atributs. From the steps had done, finded that the biggest failed criterion is keropos. Keropos is caused by lack of skill and correctness of worker on processing the raw material, can also caused by process execution which not fully corret so that produce critical process to be repaired first to increase the quality of Mendit's roof.

Abstract

Companies who produce several of cigarettes, one of the kinds is BM Universal 12. To produce product with best quality and due to give satisfaction toward its consumer, there are several problem faced by the cigarette production process which is there are still defect product in its production process. From this condition, they need one method to deal with the matter. Using six sigma methods, they can identify defect product and by doing that they could improve the quality of the product. In this experiment it's using integration method by combining six sigma method and ISO 9001:2000. Six sigma methods is expected to be able minimizing the defect in PT "X" by creating production process more effectively, better performance of the company and still fulfilling the need and the satisfaction of the consumer while also making efforts to improve and inclined its quality. Meanwhile ISO 9001:2000, is an international standard that has orientation toward the qualifying products. Based on its internal data and observation commence inside the company, there would be only three main clauses from ISO 9001:2000 requirements that is going to be made as measuring points of product quality improvement, which is clause 5 (management responsibilities), 7 (product realization) and 8 (measurement, analysis and improvement). While Six Sigma phase comprises of Define, Measure, Analyze and Improve. In Define phase there would be defining the product, the making of Six Sigma team and the making of production flow process. Measure has 3 kind defects that usually have bigger percentage than other defects which is unbalance weight defect (including empty shell) and korep defect. The percentage that goes with unbalance weight defect (including empty shell) is 41.9% and korep defect with 21.03% from all three defect we gained DPMO value reaching 7776 and sigma level reaching 3,9. By viewing the FMEA and RPN, the improvement alternatives that suppose to be done by the company is creating job instruction, implementing job instruction in rolling process then the company should be able to save Rp3,942,975.00 for one month.

Abstract

One of the construction material product that's have highly required in constructing a house is house roof that Is roof tile. On of the producen that's produce its in big scale is in Wendit, a country side of Mangliawan. This product as people known is calls as Wendit roof tilet.Wendit produce its roof tile in low quality and cost of its that people prefer choose another roof tile product that have more level either in cost also quality. If this condition is continue without a will to improve its quality, distribution and much more according to satisfy customer desire roof tile producen of Wendit in country side of Mangliawan will close down. A side impact of its degradation is will decreasing the earnings of country side society of Mangliawan and also they social economics in matter the loost of their only life-way to gain a money will decreasing a level of life in social economic and also rise-up the number of unemployment. To answer this problematic this research covered: identification requirement of customer and translating into characteristic of performance by using QFD, developing alternative of design and also Value product for the alternative with Value Engineering. Attribute wanted by customer to tile product for example model, strong, refinement, do not be bars , cool, tile size is not undersize, is easy to attached, price. Satisfaction of customer to the attributes when measured with scale 5 is have good with score above 3. By attention tile industry of Wendit exist in Worse luck, hence technical action and attribute which must be done conducted by tile industry of Wendit is as according to priority shall be as follows: repair of model according to desire of market, adjustment of size measure, printer appliance or of desk have to is made from good materials, have to pay attention pressure at the time of printing, one who do printing process have to which is experienced to be, adapted for by size measure of pres concrete, tile materials have to really solid, process combustion with heat which enough.

Abstract

Elizabeth Hospital of Situbondo represent one of the some hospital exist in Sub-province of Situbondo. With condition of like this is existing hospital have to show maximal performance and can give best service to society so that can become hospital society choice. Beside give service of health, as a hospital unit also have compulsion to always ever take care of the continuity of its life, even the including all existing shares or unit to expand in an optimal fashion. With condition of above, hence company in this case RS. Elizabeth Situbondo can become company of which can exploit system measurement of performance by using method of balanced scorecard to increase company performance on an ongoing basis so that can give required service consumer swiftly is, effective, inwrought, and professional. At this research of concept which is used in designing system measurement of performance balanced scorecard. As for in perspective which used in this research cover five in perpective that is finance, cutomer or client, internal business process, study and growth, and benefit to society. From this in perpective fifth is later then developed to become performance indicators able to be measured to utilize to know level attainment of Hospital performance. Pursuant to determination of performance indicator is later then conducted measurement by using ill obtained datas from home every its indicator, this measurement use objective matrix (OMAX), where hospital periodical can do planning and control to attainment of performance. In measurement use OMAX, included also wight element of each measured indicator, where in determination of wight every the indicator use analytical hierarchy process ( AHP). In measurement of in perpective fifth conducted performance yield 23 indicator. Result of performance measurement in 2007, index attainment of performance equal to 6,616.

Abstract

Occupational Safety and Health (OSH) culture among students has not become a full awareness that is applied in everyday life in college. This study aims to identify, test, and analyze how much OSH culture influences implementing OSH awareness for students. The research was conducted on Industrial Engineering students at Universitas Muhammadiyah Surakarta (IE UMS) as many as 120 respondents from the class of 2016 to 2019 filled out a questionnaire about OSH culture. The method used in this study is the analysis approach of Structural Equation Modeling (SEM) variance-based or so-called Partial Least Square (PLS-SEM).   The variables in the OSH culture are four independent variables, namely: individual, organizational, occupational, and environmental with 18 sub-variables. Meanwhile, one dependent variable, awareness, consists of six sub-dependent variables. It was found that environmental, organizational, and occupational factors on awareness level significantly affect implementing OSH culture, and individual factors have no significant effect on the awareness level of implementing OSH culture. Thus, an improvement strategy is obtained by increasing environmental, organizational, and occupational preferences to maximize the implementation of OSH culture.

Abstract

This paper addresses the periodic heterogeneous vehicle routing problem (PHVRP), an extension of the classical vehicle routing problems (VRP). This problem is known to be confined to various real-world instances where each customer's demand should be served within a specific time horizon and a maximum demand quantity that can be delivered at each visit. The heterogeneous capacitated vehicles are available to perform the services for each customer. This paper aims to minimize the total traveling time of routes for all vehicles over the time horizon so that the customers' demands can be delivered. Thus, a novel coding scheme is also proposed to directly convert a random sequence of integers into a feasible solution, which is then embedded into algorithms. Furthermore, this paper also compares the performance of the Genetic Algorithm (GA) with the particle swarm optimization algorithm (PSO). The numerical results of the experiments show that the proposed GA is superior to PSO. However, the computation time of PSO is faster than GA.

Abstract

Lean Construction (LC) presented innovative practices to maximize the production value and minimize the production waste during construction. The problem that frequently occurs in the project is delaying the schedule, leading to time and cost overrun. This research aims to eliminate waste that causes delay and inefficiency. Several LC techniques were carried out to improve the efficiency of this project, namely: Value Stream Mapping (VSM) to discover wastes and determine Process Efficiency Cycle (PCE); Waste Assessment Model (WAM) to identify the inter-relationship among wastes; Fishbone (Ishikawa) diagram analysis to get the root cause of the waste.   Research results showed the three highest wastes: defect 14.06%, over production 21.44%, and the highest rank was unnecessary inventories 31.73%. After implementing LC techniques, the PCE increased from 72 % to 79%. The implication of this research is to provide recommendations to the project’s stakeholders in running the building construction to avoid delay and inefficiency.

Abstract

Currently, the issue of the fuel crisis has become a global concern. The distribution sector is one of the sectors that consume the most significant fuel. Therefore, an effective procedure for fuel energy efficiency is needed to resolve the routing problem. In addition, the vehicle load must be considered in delivery and pickup at each node. This research proposes the novel Hybrid Yellow Saddle Goatfish Algorithm (HYSGA) algorithm to solve the Fuel Consumption Vehicle Routing Problem Simultaneous Pickup and Delivery (FCVRPSPD) problem. The objective function to be achieved was to minimize fuel costs. This study conducted experiments with HYSGA parameters such as the number of Goatfish, iterations, and the number of goatfish clusters to optimize the FCVRPSPD problem. In addition, a sensitivity analysis was presented to examine the effect of the FCVRPSPD variable on fuel costs. This study also compared the proposed algorithm with several state-of-the-art procedures. The results showed that the parameters of the number of Goatfish and the HYSGA iteration affected fuel costs. Furthermore, based on experiments, the proposed algorithm provided a competitive fuel cost compared to other algorithms.

Abstract

PT. Industri Sandang Nusantara Pata! Secang Magelang is some factory that regarding in manufactur sector which thread producting. For this time the factory have a problem with stacking same kind of product and in the other way less  another product. This case cause that factory use product scheduling based on experience and reckoned of selling in last period.  Connected with this problem, the  case is how to make a good  schedule in order that  factory could be completing consumer demand and decided some main product schedule with simpleks methode.The articulate in factory for this time is reguler employee 65 persons.  If reguler employee is  not enaugh, then there are overtime job (maximal 2 hour/days), and if still  not enaugh too, then used subcontract. Based on capacity of production, the  number of  maximal employee  are 70 personsAccording to  teoritis,  with  using  of  forecasting  and  agregat  planning,  the  factory's problem can be done. This forecasting used to productin supossing in about to many periods.  With use simpleks methode can be planning the number of employee. In order that, the main production can be made.Analyst result get that  demand fluktuation for R 30/1,   PR 30/1  products use Moving Average Linear Trend (3) methode, R 24/ 1,  R I 11,  PR 40/l products using Moving Average Linear Trend (5) methode, PR 45/l  product using Linear Regression methode. Value analyst with based on forecasting result for future 12  periods,  supposed that factory will  pay for Rp.  460.340.000,• After applying Linear Programming giftotal cost Rp. 450.164.576,-.  Thrifting Rp. 10.175.424,- or 2,2%.   Sensitivity analyst made cost to be Rp. 448.651.040,-,   thrifting Rp.  11.688.960,-    or 2,5%  at the number of regular employees 66 persons.
