Abstract

Background: In recent years, the use of tobacco in Indonesia increasingly varied in the forms and methods of consumption, one of which is shisha. Smoking shisha is increasingly popular due to a misperception that smoking shisha is harmless and lack of knowledge about the effects of shisha smoking in oral health.Literature analysis: “PubMed” used as a search tool to identify all empirical studies related to the effects of shisha smoking on health, especially in oral cavity.Discussion: Shisha smoke contained various toxic substances such as Nicotine, Tobacco Specifc Nitrosamines, Polycyclic Aromatic Hydrocarbons, Volatile Organic Compounds, Carbon Monoxide, Tar, and high-temperature metal heating causing shisha smoke contained toxic heavy metals such as arsenic, nickel, cobalt, chromium, lead, and cadmium. The content of these toxic substances showed that smoked shisha is associated with dependence, acute and long-term negative health effects similar to cigarette smoking. Toxic substances may cause various infections of microorganism such as Candida sp, Herpes Simplex Virus (HSV-1), Epstein Barr Virus, Mycobacterium tuberculosis, Human Immunodefciency Virus; Oral mucosal changes such as Hairy Tongue, Smoker’s Melanosis, Nicotine Stomatitis, Frictional Keratosis, Fissured Tongues, gingival or periodontal inflammation, and leukodema; and lead to malignant lesions such as Keratosis, Leukoplakia, Erythroplakia, Oral Submucous Fibrosis and Lichenoid Lesions.Conclusion: Smoking shisha gives bad impact for human health especiallyoral health. Shisha smoking can lead to the development of various infectious diseases and potentially lead to malignancy in the oral mucosa. These foundings breaks the belief that shisha smoking is safe for health.

Abstract

Background: Estrogen plays a key role in human physiological processes. Polymorphisms of estrogen receptors have been implicated in the development of numerous diseases. The aim of this study was to evaluate the frequency of ERα gene Pvull and Xbal polymorphisms and assessing their association with CD4+ T-cell counts in HIV-infected children on highly active antiretroviral therapy.Methods: CD4+ T cell counts were determined using the FACS count system. ERα PvuII and XbaI polymorphisms were analyzed by PCR-RFLP.Results: This study enrolled 34 HIV-infected children on HAART. The frequencies of the PvuII and XbaI gene polymorphisms were PP 41,2%, Pp 26,5%, pp 32,4% and XX 35,3%, Xx 17,6%, xx 47,1% respectively. CD4+ T-cell counts were significantly associated with XbaI polymorphisms (p<0.05), but not PvuII polymorphisms (p>0.01).Discussion: Host genetic factor polymorphism is an important determinant of HIV  disease progression and treatment response. The ERα Pvull and Xbal polymorphisms can increase risk for the development of HIV-related complication,including oral diseases.Conclusion: The ERα gene XbaI polymorphism was significantly associated with CD4+ T-cell counts. It may explain the role of estrogen in the regulation of HIV replication. Studying human genetic variation in HIV-infected individuals is important to guide a new therapeutic approach.

Abstract

Background: Cleft palate leads to oroantral communication and also malformations of the palate and agenese of teeth adjacent to the defect. Abnormal defect closure was done by replacing the hard, soft tissues and missing teeth using an intraoral maxillofacial prosthesis called obturator. Purpose: Assess the use of prosthetic rehabilitation using frame obturator combination with acrylic resin on patient’s palatum defect to restore esthetic, speech, swallowing, mastication functions.Case: 23 years old male patient with chief complaint palatum defect that cause nasal voice. Intraoral examination revealed a large palate defect in the right of palatum durum segment, also defect on left labial gingiva, and missing teeth in 13, 15, 16, 21, 22, 23, 24, 25, 26, 35, 36, 45, 46.Case Management: Obturator was made to cover defect of palate and replace missing teeth. Impression was taken with hydrocoloid irreversible and gauze that cover the defect to avoid alginate entering nasal cavity. Insertion of obturator showed cleft palate was covered well by frame combination with acrylic resin base. The retention, stabilization and occlusion were good, nasal voice was reduced, defect on left labial gingiva were covered by labial wing.Discussion: Patient was satisfed because nasal voice was reduced, estethic,mastication and swallowing function were restored, also. Patient can clean and use obturator easily.Conclusion: Frame obturator in patient’s palate defect can reduce nasal voice, restore esthetic, swallowing and mastication functions.

Abstract

Background: The number of elderly population growing fastly in Indonesia along with the increase of life expectancy. Oral health of elderly people greatly affect the quality of life. Oral health in the elderly is reflected in the number of missing teeth (tooth loss), dental caries and periodontal disease. Objective: To know the relationship of Elderly oral health behavior with DMFT indexMethods: Cross-sectional survey with WHO’s method was done by multistage random sampling at 124 age 45-64 years old in Kronjo sub-district, Tangerang.The data was collected by DMFT examination and the standard questionnaire of WHO survey. Calibration test for four examiners with Kappa value was 0.75Results: The mean score of DMFT was 15.5 is included in the very high category. There was statistically significant association between elderly selfassessmentand toothpaste use with DMFT index (p=0,016; p= 0,009). There was no significant association between sweet consuming and dental pain with mean DMFT index (p = 0.216; p = 0.324)Conclusion: Oral Health Behavior of the Elderly in Kronjo sub-district is stillpoor with a very high score of DMFT that will require immediate attention fromthe government.

Abstract

Background: Mesial tipping of permanent molar could be corrected by many ways. One of the simplest way was by using T-loop spring. Objective: To report the result of using straight wire appliance system Roth with ¬T-loop spring in order correction of mesial tipping of second lower permanent molar on both side.Case Management: A 35 years old woman came with chief complain wanted to fx the lower left and right molar which was tilted and felt uncomfortable during mastication. OPG examination showed a mesial tipping of 37 and 47 tooth with 300 and 550 angulations. The measurement of angulations was using modifcation of the midline reference plane as used by Elsey and Rock. The straight wire appliance system Roth with .022” slot was used for the treatment. T-loop spring with .018x.022” rectangular stainless steel wire was used to upright of both second permanent molars. Observation of 4 months treatment using T-loop spring showed an improvement in angulations of both second molars; 550 on 37 and 630 on 47.Conclusion: Mesial tipping correction of second permanent molar can beaccomplished using T-loop spring with stainless steel rectangular .018x.022” wire and consistently yields satisfactory result.

Abstract

Background: Streptococcus sanguinis is known as an early agent of formation of bacterial plaque. White tea is one of plant with various biological activities that are beneficial to human health.Objective: to examine the antibacterial activity of white tea cathechin extract against bacteria S. sanguinis.Methods: test material was white tea cathechin extract. In vitro tests were conducted to obtain minimal inhibitory concentration (MIC) and minimal bactericidal concentration (MBC).Result: The MIC of White tea cathechin extract was 500ppm (0,5 mg/ml) and MBC was 2000ppm (2mg/ml).Conclusion: White tea cathechin extract has antibacterial properties. White tea cathechin extract can be used as herbal moutwash to control dental plaque.

Abstract

Background: In the case of mandibular resection that leaves a large mandibular bone defect, a mandibular reconstruction is required to restore functional and postoperative aesthetic factors. Mandibular reconstruction plate installation on mandibular resection is very important but difcult to achieve maximum results. The formation of reconstruction plates can be preoperative and intraoperative. Preoperative stereolithography (medical rapid prototyping/MRP) can be used to form rapid and accurate plates (precontours). Objective: Report the use of MRP as an alternative tool to precontour mandibular reconstruction plate.Case management: Reported reconstruction of mandibular defects in one case of hemimandibulectomy and two cases of segmental resection with avascular iliac bone grafts using MRP through a computer-aided design and computer aided manufacture procedure (CAD/CAM) to establish preoperative reconstruction plates (precontour) so the surgery doesn’t take long time to reconstruct, by forming a mandibular models under normal circumstances.Result: The formation of a reconstruction plates (precontour) using MRP (medical rapid prototyping) with CAD / CAM procedure gives results in accordance with the original form.Conclusions: The making of MRP through CAD / CAM procedure gives maximal result resembling the form of mandibular model. The MRP models get an accurate picture of the patient’s jaw as a pre contour tools for reconstruction plate. The formation of the reconstruction plates (precontour) with these MRP model is obtained from CAD / CAM and provides a normal jaw image, so that preoperative precontouring will give good and maximum result on the reconstruction plates made under the MRP models.

Abstract

Background : Perforation in normal pulp could be happened anytime. In sterile condition, direct pulp capping was the right therapy to maintain the vitality and pulp function. Direct pulp capping agent must contact with the pulp tissue. Generally, body had an immunologic respond with foreign body that contact with tissue. The respond could be inflammatory reaction. The used direct pulp capping agents were calcium hydroxide, and Mineral Trioxide Aggregate (MTA) which known as better agent but relatively expensive and difficult to obtain. So that, the alternative of MTA, Portland cement which was the same essential of MTA, was being developed. The aim of this study, to analized inflammatory reaction of the pulp tissue with direct pulpcapping agents calcium hydroxide, MTA, and Portland cement.Methods: Free caries M. Nemestrina’s posterior teeth were prepared to formpin point perforations on buccal surface of the crowns. The teeth were appliedwith one of the three capping agents. Teeth were filled and extracted. Theextracted teeth were proceed into histopatological preparation slides to evaluatethe inflammatic reaction.Results: The result showed no statistically significant differences of pulp inflammatory reaction between calcium hydroxide, MTA and Portland cement in7, 14, 42, and 90 days.Conclusion: The inflammatory reaction of pulp tissue of the three pulp cappingagent (calcium hydroxide, MTA, and PC) were insignificant different.

Abstract

Background: Improving the aesthetic needs of patients is followed by various improvement and development of aesthetic dentistry products. The aesthetic demand drives the patients to choose the type of restoration which has superiority on its aesthetic properties that is nanofller composite resin. This demand also causes the patients prefer to use whitening toothpaste which its abrasive materials allegedly cause surface roughness. Surface roughness of the restoration is one of the important factors that affect the durability of the restoration. High surface roughness causes the retention of plaque and dicoloration that will ultimately affect the aesthetic of the restoration itself. Objective: To determine the increase of surface roughness nanofller composite resin restorations after brushing with toothpaste whitening and non-whitening.Methods: 21 samples of nanofller composite resin, divided into 3 groups. Group 1 (control) brushed with distilled water, group 2 brushed with mixture of distilled water and whitening toothpaste, and group 3 brushed using mixture of distilled water and non-whitening toothpaste. Each group was brushed with a time of 30 seconds per day for two weeks (14 days). The measurements of surface roughness were taken before and after the samples were brushed.Results: There were an increase in the surface roughness of each groupafter brushing. The result showed that the highest value of increased surface roughness occured in group 2 (whitening toothpaste), while the lowest value occured in group 1 (control). There are also signifcant differences between the value of the increased surface roughness nanofller composite resin in each group.Conclusions: The increase of surface roughness of nanofller composite resin after brushing with a whitening toothpaste was higher than brushing with non whitening toothpaste

Abstract

Background: Oral mucosal lesions in children caused by nutritional problems, dental infection, poor oral hygiene and trauma. Oral health in children as the next generation of the nation should always be maintained. However there is still lack of data on the emergence of oral mucosal lesions in Indonesian children. Purpose. To describe an overview of the caries experience, Haemoglobin/Hematokrit (Hb Ht), weight (BB) and height (TB) in children aged 6-7 years in several elementary schools at Tanjungsari, Sumedang.Result: 44 of 137 students who had oral mucosal lesions, had completesupporting data. Total of 43 students had an average caries experience of 8.4 teeth/child (1-17 teeth/child), while 1 student was caries-free. The average of weight = 19.3 kg, height = 115.5 cm, and Hb/Ht = 12.5/36.5%. Oral lesions found were plaques on the tongue, ulcers, macules, nodules, pustules and desquamatives.Discussion: The condition of untreated dental caries can cause pain anddiscomfort, so the child will have difculty eating and interfere with the growth process. This can be seen through the child’s caries experience with data on weight and height. Tooth decay affects the nutrient intake and was estimated to lead to a decrease in Hb/Ht. Oral mucosal lesions due to trauma and poor oral hygiene were not expected to affect systemic conditions.Conclusion: The results of this study indicate that the caries experience was very high, the average value of weight and height were normal below the ideal, Hb/Ht levels within normal limits, while Coated tongue was the common diagnosis found from subjects
