Abstract

Introduction. National Kidney Foundation Kidney Disease Outcomes Quality Initiative (NFK-KDOQI) recommends thrice weekly hemodialysis (HD) However, in Indonesia, most patients are given HD twice a week. Therefore it is necessary to investigate the clinical features and quality of life in End Stage Renal Disease (ESRD) patients undergoing twice-weekly HD. This study aimed to identify the clinical features and the quality of life in ESRD patients undergoing hemodialysis twice a week. Methods. A cross-sectional study was conducted in ESRD patients undergoing twice-weekly HD and thrice-weekly HD at Cipto Mangunkusumo Hospital and some private hospitals in Jakarta. Blood samples were obtained for laboratory examination. Meanwhile, quality of life were assessed by using Kidney Disease Quality of Life (KDQOL-SF 36). Results. A total of 80 subjects were included in this study, most of them were male and over 50 years old. Some clinical features were significantly higher in twice-weekly HD patients compared to thrice-weekly HD patients. Those features were Interdialytic Weight Gain (IDWG) (mean 4.91% (SD 1.52) and 3.82% (SD 1.28); p= 0.002); albumin (mean 4.05 mg/dl (SD 0.26) and 3.86 mg/dl (SD 0.48); p= 0.027); transferrin saturation (median 25.5% (range 12.0 to 274.0) and 21.95% (5.8 to 84.2); p= 0.004); phosphate level (mean 5.82 mg/dl (SD 1.68) and 5.82 mg/dl (SD 1.68); p= 0.026. Meanwhile, Total Iron Binding Capacity (TIBC) was significantly higher in thrice-weekly HD patients (mean 235.20 μg/dl (SD 55.72) and 273.73 μg/dl (SD 58.29 SB); p= 0.004). Approximately, 68% of twice-weekly HD reached Kt/V >1.8 and 93.3% of thrice-weekly HD reached Kt/V >1.2. Quality of life between the two groups was not significant either on Physical Componet Score (PCS) (p= 0.227), Mental Component Score (MCS) (p= 0.247), and Kidney Disease Component Score (KDCS) (p= 0.889). Conclusions. There are statistically significant differences between clinical features of twice-weekly HD and thrice-weekly HD on examination of IDWG, albumin, transferrin saturation, and phosphate levels which are higher in twice-weekly HD. Whereas, TIBC is higher in thrice-weekly HD group. Meanwhile, the patients quality of life in both groups showed no statistically significant differences.

Abstract

Introduction. Level of agreement comparison between biopsy per colonoscopy examination is used to measure of the quality of a colonoscopy examination and needed to be identified. This study aims to identify level of agreement comparison between histopathologic examination of biopsy per colonoscopy and histopathologic examination per surgical. Methods. A cross sectional study was conducted by using medical records from patients with colon tumor who had undergone histopathologic examination with biopsy per colonoscopy and then confirmed by surgical procedure in PESC RSPUN dr. Cipto Mangunkusumo between 1st January, 2006 31st December, 2015. Level of agreement calculated using comparative agreement category test in order to obtain the value of kappa. Results. A total of 48 subjects were included in this study. Analysis result showed that level of real agreement, level of agreement by chance, and level of agreement not by chance was 79%, 77%, and 23%, respectively. The kappa value was 0,134. Conclusion. The level of agreement comparison between histopathologic examination of biopsy per colonoscopy compared with histopathologic examination per surgical at PESC RSCM between 2006-2015 period was not good.

Abstract

Introduction. Rheumatoid Arthritis is a chronic disease requiring a long-term medication affecting quality of life. Short Form 12 is a generic questionnaire to assess patients quality of life and has been validated in England. This study was designed to test reliability and validity of Indonesian version of SF-12 questionnaire. Methods. Sixty-five patients with clinically diagnosed RA using ACR/EULAR criterion were interviewed using Short Form 36 and Short Form 12 questionnaire. Validity was assessed with construct validity and external validity, while reliability tested with internal consistency and test-retest method. Results. Short-Form 12 (Indonesian Version) did not proved having a good validity, as it have a poor correlation between RE and MH domain in SF-36 and SF-12. Indonesian version of SF-12 shown a poor internal consistency (Cronbach Alpha: 0.561- 0.754) but a good test and retest reliability ( intraclass correlation coefficient: 0.844-0.980, p <0,05) Conclusions. Indonesian version SF-12 is not reliable nor valid to evaluate quality of life in RA patients.

Abstract

Introduction. Health-related quality of life among HIV patients can be used as one of several indicators of successful therapy. WHOQOL-HIV BREF questionnaire is one of the instruments used to assess the patients’ quality of life that has been used in many countries, but never been tested for its validity and reliability in Indonesia. This study aims to determine the validity and reliability of the questionnaire WHOQOL-HIV BREF in bahasa Indonesia as a tool for measuring the quality of life of HIV patients. Methods. A cross-sectional study was conducted in HIV Integrated Service Unit Cipto Mangunkusumo General Hospital (RSCM) Jakarta in November 2016 with consecutive sampling method. The study was conducted in two phases: first phase for the language and cultural adaptation process and second phase to test the validity and reliability of the questionnaire. Results. Fifty six respondents filled the questionnaire, 69.6% of them were male. Using a multi-trait scaling analysis, correlation coefficient value had a high correlation with the total score domain. This meant that it had a good validation. Correlation between questionnaire domain WHOQOL-HIV BREF and SF-36 questionnaire domain obtained 6 significant domains that shown strong correlation coefficient (r = 0.60 to 0.79; p <0.005). Reliability of the questionnaire was assessed by intraclass correlation coefficient of each domain, which was range from 0.401 to 0.484 with Cronbach Alpha 0.513 to 0.798 . Conclusions. The questionnaire WHOQOL-HIV BREF in the Indonesian language is valid and reliable.

Abstract

Introduction. According to WHO, hypertension is associated with 7.5 million deaths worldwide or 12.8% of all deaths. Meanwhile, based on Riskesdas 2013 in Indonesia, hypertension is still a major health problem with a prevalence of 26.5%. Noncompliance is a major cause of treatment failure of hypertension and risk factors for cardiovascular comorbidity. No previous research has been found that examines patient control compliance in the suburban community. This study aims to analyze the prevalence of patients who did not control after hypertension treatment in primary referral hospitals Methods. This study was a retrospective study, by tracking medical records in patients treated between October and December 2015. Results. A total of 80 hypertensive patients participated in the study (55 were females). The mean age was 57.5 ± 11 years, and 22,5% had diabetes mellitus. Mean systolic pressure was 161± 19 mmHg, diastolic pressure was 96 ± 10 mmHg. The majority of the patients had 2nd degree hypertension according to ESC 2013. Sixty percent of patient was given monotherapy and the most frequently prescribed drugs were calcium channel blockers (CCB) (70.0%). The prevalence of loss-to-follow up patient was 63,8% (51/80). Respondents with mono-therapy, without comorbidities, and admission from emergency department were more often loss-to-follow up than those with combination therapy (OR 10.3; 95%CI 3.5 – 30.1), with comorbidities (OR 4,3; 95%CI 1.6 – 11.4), and admission from outpatient clinic (OR 14.6; 95%CI 4.8 – 44.6) although the comorbidities variable was not significant in multivariate analysis. Conclusion. The prevalence of noncompliance of control is still high. Further research is needed to determine other etiological factors.

Abstract

Introduction. There are few studies regarding peripheral artery disease (PAD by intermittent claudication) on adults in Indonesia, including its risk factors. This study aims to determine relationship and comparison of PAD accompanied by intermittent intermittent claudication risk factors in adult, especially in rural areas of Sikka regency, Flores, measured by pulse palpation. Methods. A cross sectional study in 3 primary healthcare working areas in Sikka regency and consecutive sampling had been done. Interview of risk factors was conducted and blood pressure was taken from all four extremities in supine position. Ankle brachial index (ABI) value was defined as systolic blood pressure of foot arteries (dorsalis pedis and posterior tibia) divided by highest systolic blood pressure among brachial artery. PAD was then defined by presence of both intermittent claudication and ABI <0.9. Random blood glucose, total cholesterol, and uric acid had been measured using EasytouchTM while urine examination was done using urine dipsticks. Results. A total of 333 subjects had been participated in which woman (64.9%), farmers (71.2%), working, less-than-5-yearhypertension history (30.3%) and mean age 57.47 years old were our majority subjects’ characteristics. There were 135 subjects had intermittent claudication and 39 (28.89%) of them had PAD. Several independent significant (p<0.05) risk factors include sex (man vs. female; p=0.001; OR 0.167 [0.056–0.497]) and proteinuria (p=.000; OR 6.612 [2.725–16.045]). Conclusion. Proteinuria and sex are independent risk factors and significantly related on PAD accompanied by intermittent claudication in adult of rural areas in Sikka regency.

Abstract

Acute neuromuscular disorder is a commonly found medical condition, which may prove to be fatal in certain conditions. Hypokalemia is one possible cause of neuromuscular disorder. In patient with hypokalemia accompanied with hypertension, primary hyperaldosteronism might be the primary cause. Primary hyperaldosteronism (Conn syndrome) is clinically known as a triad, which is consists of hypertension, hypokalemia, and metabolic alkalosis. In this article, the case of a patient with neuromuscular disorder is presented, and after evaluation, it was found that the patient was suffered from the Conn syndrome

Abstract

Celiac disease is a reversible, proximal immune enteropathy resulting from the interaction of dietary gluten with the intestinal immune system. Celiac disease can produce varied manifestations and may develop at any age. New cases have increased substantially, due to increase awareness and better diagnostic tools. Standard diagnosis of celiac disease is based on positive serologic tests and histopathologic examination of duodenal biopsies. Treatment of celiac disease with free gluten diet will improve symptoms, quality of life, reduce risk of malignancy and complication. Nowadays, treatment strategy of celiac disease with non-dietary therapies is still underdevelopment.
