Abstract

Background: Basal cell carcinoma (BCC) is the most common skin malignancy found in nonmelanoma skin cancer. There are multiple factors contribute to risks for BCC but the underlying mechanism is genetic alteration. The Sonic hedgehog (SHH) pathway is implicated in the etiology of the most common BCC. We reported BCC nodular type in the tip of nasal region with increase SHH. Case Illustration: 50 years old man, presented with hyperpigmented tumor on the nasal tip which easily bleed since 6 months ago. Dermatological examination: the tip of nasal region: a solitaire irregular immobile hyperpigmented tumor, with the size of 2x1x0,3 cm, pearly border with rubbery consistency. Dermoscopy examination found multiple blue-gray globule and telangiectasia feature. Histopathology examination found nodule and micronodule with palisade feature in the periphery and “slit like retraction” in central. Immunohistochemistry examination found more than 50% cell show SHH expression. Wide surgical excision was done with tumor margin 3 mm. Discussion: The hedgehog (Hh) pathway known have important role in embryonic development, that responsible for patterning development organ and tisuue. SHH is one protein ligand that most study and well known in Hh pathways. Conclusion: Activating SHH pathway can resulting in unrestricted SMO activity that causes uncontrollable cell proliferation and eventually carcinogenesis. Increased expression of SHH can be used as a marker in aggresivity of BCC.

Abstract

Background: Recent studies have suggested the relationship between diet and acne, and zinc plays a role through its anti-inflammatory mechanism. This study aimed to determine the association between zinc intake and serum zinc levels with acne severity. Methods: A comparative cross-sectional study was conducted to assess the difference in zinc intake and serum zinc levels with acne severity based on Indonesian Acne Expert Meeting (IAEM) criteria. Sixty patients with acne in Dr. Cipto Mangunkusumo General Hospital were selected. The effect of zinc intake on acne severity was analyzed via unpaired T test, whereas the effect of serum zinc levels on acne severity was examined with Mann–Whitney test. Data were further evaluated using SPSS 20.0. Results: Zinc intake in all the subjects was below the Indonesian recommended daily allowance, and the mean of zinc intake and serum zinc levels in the mild acne group was higher than that of the moderate–severe acne group. The zinc intake significantly differed between the mild and moderate–severe acne groups (p < 0.01). Likewise, serum zinc levels significantly varied between mild and moderate–severe acne group (p = 0.048). Conclusion: Although this study showed a significant difference between zinc intake and serum zinc levels with the severity of acne vulgaris, further follow-up studies should be performed to assess zinc for acne vulgaris therapy.

Abstract

Background: The use of prophylactic antibiotics in dermatologic surgery, particularly in Mohs micrographic surgery (MMS), remains a gray area. Despite the low rate of surgical site infection (SSI), SSI can result in some significant morbidities. Thus, numerous dermatologists keep administering prophylactic antibiotics in MMS. Method: A search was conducted to identify clinically significant topics followed by literature review through the main available medical literature database. The three selected studies that met the inclusion and exclusion criteria were then critically assessed for their validity, importance, and applicability using the guidance from Centre for Evidence-Based Medicine for Critical Appraisal of Prognostic Studies. Results: Three articles showed a low SSI rate with a narrow confidence interval (CI): 0.91% (95% CI: 0.38%– 1.45%), 0.7% (95% CI: 0.21%–1.19%), and 0.72% (95%CI: 0.5%–0.9%). Conclusion: Based on the available studies, we conclude that the absence of prophylactic antibiotics is not related to the SSI incidence.

Abstract

Introduction: Despite its rarity, nail involvement in Langerhans cell histiocytosis (LCH) may show various clinical presentations. This study aims to show the roles of nail involvement in LCH patients as the diagnostic and prognostic clues.Case illustrations: We presented four cases of multisystem LCH in children which were already confirmed by skin biopsy with various nail abnormalities. We were able to perform nail biopsy in two patients and confirmed the nail involvement. Histopathological examination showed the infiltration of Langerhans cells characterized by indented/reniform nuclei and CD1a expressions. All patients had high-risk organ involvements. Discussion: Langerhans cells may infiltrate the nail bed, proximal nail fold, and nail matrix. Further infiltration may destruct the nail plate. Hypothetically, we suggest that the nail bed as the initial infiltration site of Langerhans cells. The different sites of involvement lead to different clinical presentation. Nail abnormalities may predict a poorer prognosis, as they mostly occur in patients with multisystem disease. Conclusions: Nails should be routinely inspected in the suspicion of LCH. The presence of nail abnormalities in LCH patients may predict a poorer prognosis.

Abstract

Background: Narrowband ultraviolet B (NB-UVB) become the mainstay of widespread vitiligo therapy due to its great efficacy and safety compared with other modalities. The use of NB-UVB for vitiligo has been widely studied in adult patients, while studies on geriatric patients specifically are still very limited. This report aims to show the immediate response after the initial treatment, side effects of therapy, and other problems that were encountered during therapy in two geriatric patients with vitiligo who have been followed up for more than 12 months. Case Illustration: We reported two geriatric patients with vitiligo treated with NB-UVB phototherapy combined with topical treatment for more than 12 months. The first patient was 68-year-old woman, with Fitzpatrick skin type IV, who had vitiligo on her face, both lower arms and hands, and both legs and feet. The second patient was a 61-year-old man with Fitzpatrick skin type IV who had generalized vitiligo. The first repigmentation occurred on the 5th session in the first patient and on the 6th session in the second patient, using a cumulative dose of 1223 mJ/cm2 and 1541 mJ/cm2, respectively. Discussion: After NB-UVB phototherapy, there was a great improvement on the face in both patients, followed by the trunk and legs. However, the duration of treatment needed to achieve homogeneous repigmentation is longer in geriatric patients than in the general population. Conclusion: NB-UVB was well tolerated and showed good clinical response to treat vitiligo in the elderly. However, little is known about the safety and efficacy of phototherapy in elderly patients with vitiligo.

Abstract

Background: Divided nevus, also known as “kissing nevus,” is a rare form of congenital melanocytic nevus that occurs on opposing margins of upper and lower eyelids. A paucity of literature on this rare anomaly exists, with most being case reports and series. Moreover, regression of this lesion was rarely reported. Case Illustration: We present a rare case of congenital divided nevus of the eyelid that regressed after eight years, confirmed with dermoscopy. A six-year-old boy presented to the Dharmais National Cancer Hospital with two pigmented macules on the upper and lower right eyelid since birth. A year ago, the lesions started gradually disappearing and were replaced by a hypopigmented area. We evaluated the clinical and dermoscopic findings for two consecutive years. The dermoscopy showed pseudopigment networks, surrounded by a hypopigmented area resembling a halo. The pigmented lesions cleared with no residual lesions. Discussion: The dermoscopic findings of the patient resemble a solar lentigo characterized by pseudopigment networks, a feature caused by the relatively flattened rete ridge on the face. The hypopigmented area reflects a regression process, like the halo nevus, and is accompanied by leukotrichia of the eyelashes, a feature usually found in patients with vitiligo. Regression of the divided nevus is related to an autoimmune process. A similar mechanism was also found in vitiligo. Since no atypical findings were present in this patient, we recommend only observation. Conclusion: Divided nevus is a rare case. We present a case of congenital divided nevus of the eyelid that regressed spontaneously. Clinical and dermoscopic findings of hypopigmented regression area were similar with vitiligo, which might share similar pathological mechanisms.

Abstract

Background: At present, there is no questionnaire in Bahasa Indonesia to assess the quality of life of female patients with melasma. The aim of this study is to adapt the existing English questionnaire, MELASQOL, into Bahasa Indonesia, and to assess its validity and reliability. Methods: This is a cross-sectional study consisting of two stages. The initial stage is cross-cultural and language adaptation. The final stage is the validity and reliability test. The original MELASQOL questionnaire is in English and is adapted into Bahasa Indonesia according to the cross-cultural and language adaptation guidelines. The subjects were recruited from two sites, The Cipto Mangunkusumo Hospital and a factory in Tangerang. A construct validity method was used for the validity analysis. Cronbach’s α was used for internal reliability analysis. Results: The initial stage involved 30 subjects, and 32 subjects were involved with the final stage. The validity of The MELASQOL Bahasa Indonesia was calculated by the item-total score. The correlation coefficient is 0.712–0.935. Its reliability was calculated by the Cronbach α score is 0.962. Conclusions: MELASQOL Bahasa Indonesia is a valid and reliable instrument for assessing the quality of life of female patients with melasma in Indonesia.

Abstract

Background: Acne vulgaris is an inflammatory condition of the pilosebaceous glands caused by various factors including excessive excretion of sebaceous glands. Excess sebum can be reduced using oil blotting paper (OBP), a thin paper with microspores that absorb the oil on the face. This study was conducted to determine the association between OBP application and sebum excretion according to the Oily Skin Self- Assessment Scale (OSSAS) questionnaire and acne severity according to the Global Acne Grading System (GAGS) questionnaire. Methods: This experimental study was conducted in 2017 among 60 students aged 17–22 years at the Faculty of Medicine, Atma Jaya Catholic University of Indonesia, recruited by purposive sampling. Data on sebum excretion and the degree of AV severity before and after intervention were analyzed using the chi- square test with a significant level of p < 0.05. Results: Before intervention, 11 (18.3%) students had dry skin, 23 (38.3%) had slightly dry skin, 17 (28.4%) had slightly oily skin, and 9 (15%) had oily skin. The prevalence of AV was 100%, with 48 (80%) students having mild severity and the remaining having moderate severity. There was no effect of using OBP on sebum excretion (p = 0.211); however, an improvement was noted on GAGS raw data score (p < 0.001). Conclusion: AV is an inflammatory disease of the pilosebaceous glands, and one of the etiologies is excessive sebum excretion, which can be reduced using OBP. This study demonstrated that OBP application did not reduce sebum excretion statistically but could improve the severity of AV.

Abstract

Background: The incidence of psychodermatologic diseases, a group of skin abnormalities associated with psychosomatic factor, are increasing recently. About 20-40% patients with skin symptoms have concurrent psychiatric problems which are often difficult to diagnose and treat. The new classification of primary psychiatric skin disorders are delusional disorders, obsessive-compulsive and related disorders, and factitious disorders. This classification could be used for effective treatment in each patient which involves multi-disciplinary approach, including dermatology, psychiatry and other discipline if necessary. Case Illustration: The first case was a 39-year-old unmarried female, with anemia and dermatitis artefacta occurred as wound due to blade cuts. Patient was with schizoaffective depressive type and was not under regular treatment. The second case was a 61-year-old male referred with unresolved prurigo nodularis for the past 30 years. After in-depth assessment, there were delusional parasitosis and neurotic excoriations disorders. The third patient was a 50-year-old female admitted with recurrent ulcer on her face. She was aware that the lesions were intentionally manipulated by her own fingers when she was depressed. Discussion: All patients were classified as primary psychiatric skin disorders with varied skin manifestations. Symptoms usually occurred when the patient was in the depression state or low compliance for the psychotropic drugs. Patients generally had poor insight and refused to be associated with psychiatric factors. Dermato-venereologists are expected to conduct early detection and treat this disease. Conclusion: It is important to approach psychocutaneous disease in multi-disciplinary manner, especially with the psychiatrist.
