Abstract

Introduction: Seizures are a severe clinical condition with complications of brain abscess. This report will describe the rare occurrence of generalized seizures due to Mycobacterium tuberculosis (MTB) brain abscess.Case: Male, 57 years old, admitted to the emergency department with two episodes of generalized seizures one hour before admission. The patient had a history of pulmonary tuberculosis during four months of treatment and was treated with an anti-tuberculosis fixed drug combination (FDC) 3. The patient was in a state of delirium and had lower extremity weakness. The chest radiograph shows a multifocal patchy opacity in the right upper lobe with thickening in the right pericardium. Pre-Contrast MSCT-Scan of the brain showed multiple hypodense lesions in the left frontal lobe, right frontoparietal, left basal ganglia, and pons. Post-contrast shows multiple low-attenuation, oval-shaped lesions with peripheral enhancement (double rim sign) with vasogenic edema. Finally, the patient was diagnosed with a tuberculous brain abscess.Discussion: CNS tuberculosis is a rare cause of seizures and is often followed by a history of pulmonary tuberculosis. Tuberculosis appears on the Contrast MSCT Scan Brain as an avascular mass lesion of low density and sometimes more significant than expected around cerebral edema. End-stage tuberculosis is well encapsulated and has peripheral ring enhancement with vasogenic edema.Conclusion: Pre-Post Contrast Brain MSCT is the diagnostic imaging of choice for suspected tuberculous brain abscess.

Abstract

Introduction: Laryngeal rupture is a potentially life-threatening aerodigestive injury, especially in cases with slow treatment, which can occur spontaneously, iatrogenic or as a result of trauma.Case: We present a case of pneumomediastinum due to sharp trauma causing a laryngeal rupture in a patient with psychiatric disorders. Pneumomediastinum is one of the complications that occur.Discussion: Trauma history and clinical complaints are very helpful in establishing the diagnosis, and imaging has an important role in describing pneumomediastinum and other air leak syndromes so that early treatment can be done immediately.Conclusion: The approach to diagnosing pneumomediastinum with imaging in psychiatric patients has an important role because of the lack of information about complaints and mechanisms of pneumomediastinum.

Abstract

Introduction: Tuberculosis (TB) is an infectious infectious disease caused by Mycobacterium tuberculosis. This study aimed to determine the relationship between clinical manifestations and the results of chest radiographs in adult pulmonary TB patients at Aloei Saboe Hospital.Method: The research design used in this study was cross-sectional by examining the relationship between clinical manifestations and chest radiography results in adult pulmonary TB patients at Aloei Saboe Hospital from October to November 2022. The population of this study was adult pulmonary TB patients using accidental sampling as a sampling technique. The research instrument used was the medical record which contained the patient's anamnesis status and the results of the chest radiography examination. The relationship between variables was then analyzed using the Kolmogorov-Smirnov test.Results: Of the 44 samples, the most common clinical manifestations were dyspnea and chest pain in 12 patients (26.67%). The most chest radiographic results were fibrotic in 21 patients (47.7%). According to the American Thoracic Association (ATA) classification, extensive lesions were predominantly found in 21 patients (47.7%). There was an association between productive cough and chest radiography results (p=0.012). Meanwhile, chest radiographs were not associated with productive cough, hemoptoe, and dyspnea.Conclusion: There is a relationship between productive cough and chest radiography lesions. Further study may determine samples for research that are minimally biased so that the results obtained are more representative of the population.

Abstract

Background: Panretinal photocoagulation (PRP) is the standard gold therapy to prevent vision loss in severe non-proliferative diabetic retinopathy (NPDR) and proliferative diabetic retinopathy (PDR).Contents: Photocoagulation is a therapeutic technique using a solid light source to coagulate tissue. This light energy is absorbed by the retinal tissue and converted into thermal energy. This energy then causes the abnormal blood vessels in the retina to shrink and disappear. It takes approximately 6-8 weeks for complete shrinkage of blood vessels.Conclusion: Laser retinal photocoagulation is the standard therapy in diabetic retinopathy patients. By causing thermal lesions of the pathological retinal vessels, the expected outcome can be achieved in 2-3 months of therapy.

Abstract

Introduction: Coronary artery anomalies presenting with ST-segment elevation myocardial infarction (STEMI) are uncommon and often are challenging to manage. We report a case of a successful percutaneous coronary intervention (PCI) of the right coronary artery (RCA) in a patient who presented with inferior STEMI and an anomaly of the left and right coronary arteries with a single coronary ostium in the right sinus of Valsalva.Case: Male, 71 years old, was admitted to the emergency department with 8 hours onset of chest pain. The patient was diagnosed with STEMI Inferior 8 hours of onset Killip I TIMI score 5. The patient was given a dual loading antiplatelet, high-dose nitrate and statin, then we planned him for primary PCI. Primary PCI was performed on the right transradial through the single osteal coronary artery from the right sinus Valsalva. We got the TIMI flow 3 without other complications during the action.Discussion: It is frequently difficult to identify the culprit lesion during primary PCI. The course of the anomalous vessel and the culprit lesion are clear often; the revascularization procedure hides several pitfalls.Conclusion: Coronary artery anomalies presenting with STEMI are uncommon but often challenging. Percutaneous coronary intervention is considered the first choice in patients with coronary anomaly presented with STEMI.

Abstract

Background: This article aims to (1) examine the current condition of education in the digitalization and disruption era and (2) examine medical education strategies in the digitalization and disruption era. Writing scientific papers is based on library research, whose object of study uses library data in the form of books as a data source.Content: Based on the literature review, it was found that: (1) global medical education currently uses the SPICES strategy (student-centered learning, problem-based, integrated, community-based, elective, and systematic), accompanied by nine educational trends; (2) SPICES is a medical education strategy that will produce graduate profiles that refer to World Health Organization (WHO) and World Organization of Family Doctor (WONCA) conference agreements. A doctor must have six-star doctor characteristics, namely: care provider, decision makers, communicators, community leaders, managers as well as creators and innovators; (2) the minimum standard of competence for doctors in Indonesia, constitute of seven areas of graduate competency namely; has noble professionalism, is always aware and develops himself, can communicate effectively, is reliable in managing information, always has a scientific basis in medical science, has clinical skills and can manage health problems well.Conclusion: A special strategy for medical education is required in the era of digitalization and disruption by applying a lifelong education approach. Indonesian medical education uses the SPICES strategy to create six-star doctors and master the seven defined competency areas.

Abstract

Introduction: Medical students need to learn histology to understand the relation between microscopic structure and function of cells and tissues. Pictures of manual preparation made by students can be used as notes and can describe students' understanding of the observed structures. Producing good pictures in a short time is a challenge for medical students. The use of digital illustrator technology is expected to be a solution to overcoming these problems. This study aims to measure the perception of acceptance and satisfaction rate of medical students in Gorontalo towards digital illustrators in apprehend histology practicum.Method: Cross-sectional descriptive survey with modified questionnaires from previous studies. The sampling technique used purposive sampling and involved 54 first-year students. The raw data were analyzed using Microsoft Excel. The mean value calculated for each item was compared with the theoretical mean value of 2.50 to determine whether the respondents agreed with the statement.Results: All items have an average rating exceeding the theoretical mean of 2.50. Also, the mean of 3.91 (SD = 0.58) for all items substantially exceeds the theoretical mean. Items with the highest perceived rate of perceived utility, ease of use, intention to use, and actual use of digital illustrator teaching materials in learning practical skills will significantly increase students' motivation, interest, and acquisition of practical skills.Conclusion: the use of digital illustrators is perceived positively by medical students in helping histology practicum learning, which is indicated by the high rate of acceptance and rate of satisfaction with its use.

Abstract

Background: Palpebral malignancy is still a significant health problem and consists of sebaceous gland carcinoma (SGC), basal cell carcinoma (BCC), squamous cell carcinoma (SCC), and malignant melanoma (MM). Although total resection using the Moh micrograph method is standard therapy for these cases, perioral chemotherapy may be an alternative therapy in patients where surgery cannot be performed.Content: Periocular chemotherapy in lid malignancy is recommended in cases that are inhibited or contraindicated for surgery, with obscure tumor margins, with markers of tumor invasion of vascular, lymphatic, nerve, or other orbital structures, signs of relapse after surgery, local pagetoid involvement, sizeable tumor size, and also presented an appearance of diffuse and multifocal tumor on histopathological examination. Choices of local chemotherapeutic agents based on the variety of palpebral malignancy, namely: (1) imiquimod for BCC; (2) mitomycin-C for SGC; (3) cisplatin, doxorubicin, bleomycin, peplomycin, methotrexate, and 5-fluorouracil for SCC; and (4) Imiquimod for MM. Topical chemotherapy is given to patients using an iodontotherapy method or ophthalmic cream.. More often than not, topical chemotherapy possessed a few side effects that are mild and tolerable and disappear on their own after treatment has finished.Conclusion: Local/periocular chemotherapy for palpebral malignancy is an alternative adjuvant therapy that can be considered, especially in cases where surgery is not possible. Key words: Malignancy, chemotherapy, palpebral, periocular

Abstract

Introduction: Longer work processes with non-ergonomic working postures by employees who use computers can certainly cause musculoskeletal tension. One of the musculoskeletal disorders (MSDs) associated with computer use is complaints of non-specific neck pain caused by a non-ergonomic posture of the neck muscles over a long period, which causes muscle tension. This study aims to determine the relationship between working posture and duration of work using a computer on non-specific neck pain complaints among Hasanuddin University Rectorate employees.Method: A cross-sectional research design was carried out on 121 administrative employees aged 22 years who met the inclusion and exclusion criteria. Assessment of working posture uses the Baseline Risk Identification of Ergonomic Factor (BRIEF) questionnaire, and for non-specific neck pain complaints using the Nordic Body Map (NBM) questionnaire. The Spearman's rho correlation test analyzes the relationship between variables.Results: 49 out of 121 respondents had a high-risk working posture for non-specific neck pain, and 59 out of 121 had a long working duration. Working posture is positively correlated with symptoms of non-specific neck pain (p = 0.001; r = 0.598). In contrast, work duration did not correlate with symptoms of non-specific neck pain (p = 0.063; r = 0.169).Conclusion: High-risk working postures can predict the incidence of non-specific neck pain in office administration employees. This can be a consideration for office leaders to pay attention to the working posture of their employees.Key words: Duration, non-specific neck pain, working posture

Abstract

Introduction: Eucheuma spinosum is one of Indonesia's potential marine resources that can be used as an antimicrobial. The red algae E. spinosum contains many secondary metabolites which can inhibit the growth of bacteria. This research aimed to determine the optimum inhibitory power of the red algae extract E. spinosum using n-hexane, ethyl acetate, and methanol solvents in inhibiting the growth of Pseudomonas aeruginosa bacteria.Method: The extraction process uses the maceration method with n-hexane, ethyl acetate, and methanol as solvents. Antibacterial testing was carried out using the paper disk diffusion method with soaking for 1 hour and incubating for 24 hours. The extract with the largest clear zone diameter was then tested further using Gas Chromatography-Mass Spectrometry (GCMS) to investigate the compound content in the sample.Results: The solvent that produced the largest inhibition zone diameter was ethyl acetate extract, namely 16.1 mm at a concentration of 4%. Analysis of the compounds contained in the ethyl acetate extract using GCMS showed the presence of hexadecanoic acid which is a terpenoid group.Conclusion: The optimum concentration of red algae E. spinosum extract to inhibit the growth of Pseudomonas aeruginosa was 16.1 mm in 4% ethyl acetate solvent. Key words: Antibacterial, Eucheuma spinosum, Pseudomonas aeruginosa
