Abstract

Introduction: The incidence of natural disasters worldwide is increasing and unpredictable. As one of the high-risk disaster countries, good knowledge and disaster preparedness in Indonesia residence are needed to reduce the risk of a disaster. As agents of change, students should have competent skills in disaster situations. This study aims to determine the relationship between disaster knowledge and disaster preparedness attitudes among medical students at Universitas Negeri Gorontalo (UNG).Method: This research is a descriptive study with a cross-sectional design and uses a total sampling technique on active UNG medical students during the research period. The research was conducted at the UNG Faculty of Medicine from September to November 2022. The research instrument used a questionnaire that had been used in previous studies. The independent variable is disaster knowledge, while the dependent variable is disaster preparedness attitudes. Bivariate analysis was carried out using the chi-square test to determine the relationship between the two variables.Results: Of the 208 respondents, most have a high level of disaster knowledge of 81.7%. The description of disaster preparedness attitudes shows that most respondents have a good attitude towards disaster preparedness, namely 52.4%. There is no significant relationship between disaster knowledge and preparedness attitudes (p-value 0.160).Conclusion: There is no relationship between disaster knowledge and disaster preparedness attitudes among UNG medical students. It is hoped that the institution will continue to develop disaster education so that students can have better disaster knowledge and be able to deal with disaster situations. Key words: Attitude, disaster, knowledge, preparedness

Abstract

Introduction: Impaired lung function is a functional disorder of the lungs that causes a slowing of airflow during respiration. Bentor drivers are a profession with a risk of decreasing lung function due to work activities that are always on the road and exposed to motor vehicle pollution. Exposure to cigarette smoke in active and passive smokers is also significant in causing several other respiratory diseases, such as chronic obstructive pulmonary disease (COPD). The purpose of this study was to describe the risk factors for impaired lung function in Bentor drivers in Gorontalo City in 2023.Method: The research was conducted in June 2023 on eligible Bentor drivers accidentally found in Gorontalo City. The research instrument used a questionnaire, stress perception scale (SPS), and spirometry.Results: The results obtained are the distribution of risk factors for impaired lung function in Bentor drivers in Gorontalo City in 2023, based on length of work found more work more than five years (63.33%), more work more than 8 hours a day (75%), had smoking habits with a smoking duration of more than ten years (58.3%) and smoked more than ten cigarettes a day (62.5%) and had a severe perception of stress (66.6%).Conclusion: Bentor drivers are one of the contributing factors in impaired lung function. There is a relationship between the risk factors for smoking, length of work, duration of exposure to pollution, and perception of stress with lung function in Bentor drivers in Gorontalo City. Key words: Bentor drivers, lung function, smoking, stress perception

Abstract

Testicular torsion is a surgical emergency. To protect the affected gonad, early diagnosis and surgical therapy are essential. The pediatrician's difficulty in communicating his complaints is one of the potential patient-specific causes for a delayed diagnosis related to any of these symptoms. We describe a case of unilateral testicular torsion in a 13-year-old child who had complained of swelling but hadn't experienced intense scrotal discomfort in six days. His right testicle was completely shattered. When the right testicle was discovered to have a 360-degree torsion and to be detorted but not viable in the operating room, it had an orchidectomy, and the left testis underwent an orchidopexy. Three internet databases—PubMed, Google Scholar, and Science Direct—were used to search the literature. An English journal that discusses pediatric testicular torsion has inclusion criteria. There is insufficient research on testicular torsion that manifests with painless symptoms. Treatment could be postponed if this phenomenon isn't considered appropriately.Keywords : Delayed management, painless, pediatrics, testicular torsion

Abstract

Introduction: In diabetic conditions, damage of β cells and changes the structure of pancreatic islets was exhibited. Metformin can improve this condition. The nanoparticle form of metformin can improve bioavailability and accelerate cell regeneration, and pancreatic islets can be repaired. The aim of this study to know the effect of nanoparticles metformin on fasting blood glucose levels and pancreatic islet morphometry in diabetic rat models.Method: An experimental research with posttest-only controlled group design was conducted on 16 white male Wistar rats. The streptozotocin (STZ) 40 mg/kgBB were injected i.p. Rats were divided into four groups: K1: normal control; K2: negative control (diabetes model); K3: diabetes model treated with metformin 100mg/kgBB; K4: diabetes model treated with nanoparticle metformin 100mg/kgBB. The body weight and fasting blood glucose levels were measured periodically. The histology of pancreatic islets was performed with hematoxylin-eosin staining and quantified using ImageJ software. The data were analyzed with GraphPad Prism 8.0.0 using nonparametric Kruskal-Wallis test.Results: Metformin therapy decreased the fasting blood glucose levels in K3 starting on day 21 and K4 starting on day 7, but there was no statistical difference (p=0.0597). Pancreatic islet morphometry showed the pancreatic islet area was found to be statistically different (p=0.026), and the perimeter was not statistically different (p=0.115).Conclusion: Metformin nanoparticle form decreased the fasting blood glucose levels and effectively improved the area and perimeter of pancreatic islets of the diabetic rats model, but the perimeter of the pancreatic islets is not statistically significant.Keywords: Diabetes, metformin, nanoparticles, pancreatic islets, morphometry.

Abstract

Introduction: Hernias happen when an organ or tissue bulges through a weak  point in the abdominal wall. Inguinal hernias are common and may need surgery.  The present study report the overview of the risk to affect the inguinal hernia at  Aloei Saboe Hospital.Method: The research focused on patients with inguinal hernias who underwent  examinations at Aloei Saboe Hospital, in Gorontalo Province from January to  December 2022. The study used the total sampling technique, with a sample size  of 42 individuals. It was conducted in September and October 2023 at the same  hospital.Results: The data indicates that a significant portion of patients in the range of age  56-65 years (28.6%), with a majority being male (85.7%). Additionally, the  majority of patients who were not employed (19.0%) were also found to have this  condition, as well as a vast majority of those with a primary diagnosis of inguinal  hernia (88.1%).Conclusion: Most patients with inguinal hernia at Aloei Saboe Hospital are males  in the late elderly age group who are no longer working. The main diagnosis for  most patients is inguinal hernia. The study's findings suggest that the hospital can  enhance its system, particularly in archiving medical records or electronic medical  forms, to serve its patients better. Key words: Gorontalo, hernias, inguinal hernia

Abstract

Introduction: Beta-beta leaves (Lunasia amara Blanco) have anticancer and antibacterial activity, but the secondary metabolites involved are still not clearly known. The aim of this research is to screen secondary metabolites from beta-beta leaves that have potential anticancer and antibacterial properties using the Passonline server and determine potential compound target proteins using the Superpred online server.Method: Extraction of beta-beta leaves was carried out using the maceration method using ethyl acetate and identification of secondary metabolites using Liquid Chromatography High-Resolution Mass Spectrometry (LC HRMS). Prediction of the anticancer, antibacterial and toxicity potential of secondary metabolites of beta-beta leaves using Prediction of Activity Spectra for Substances (PASS) Online. Analysis of similarity to drugs using Lipinski rule of five and prediction of target proteins using the online superpred server.Results: There are 10 secondary metabolites each which have anticancer and antibacterial bioactivity. The compounds methyl cinnamate and 3,5-pyridinedicarboxylic acid are similar to drugs based on Lipisnki rule of five analysis and are non-toxic based on online PASS analysis. There are 3 target proteins for methyl cinnamate which are involved in anticancer and antibacterial activity, namely NFκβ P105, ADAM10 and Catephsin D.Conclusion: Ethyl acetate extract from beta-beta leaves contains secondary metabolites as anticancer and antibacterial based on Passonline server analysis. Methyl cinnamate is a potential secondary metabolite candidate as an anticancer and antibacterial with 3 target proteins, namely NFκβ P105, ADAM10 and Catephsin D.   Key words: Anticancer, antibacterial, beta-beta, Lunasia amara, passonline, prediction

Abstract

Congenital nephrotic syndrome (CNS) is a rare disorder characterized by massive proteinuria, hypoalbuminemia, hyperlipidemia, and edema that occurs in the first three months of life. The incidence of CNS is about 1.5% of all cases of nephrotic syndrome in children, while the incidence in children is about 2 per 100,000. The pathogenesis of CNS is thought to be due to genetic defects in the components of the glomerular filtration barrier, particularly nephrin and podocin, which cause most similar cases. A perinatal infection may also cause CNS. The most common type of primary CNS is the Finnish type, inherited in an autosomal recessive manner. We report a case of a six-month-old male patient who presented with swelling of the face, abdomen, and genitals since the age of 3 months, which had worsened a week before admission to the hospital. There was no history of nausea, vomiting, fever, or seizures, and the child could drink milk. Physical examination revealed ascites with positive shifting dullness and pretibial, dorsum pedis, and scrotum edema. The liver and spleen were difficult to assess. Laboratory examination revealed leukocytosis (27.8x103/µL), anemia (7.7 g/dL), thrombocytosis (537x103/μL), hypoalbuminemia (1.5 g/dL), dyslipidemia (total cholesterol 374 mg/dL, HDL 7 mg/dL, LDL 69 mg/dL, triglycerides 1196 mg/dL), proteinuria (+3/300), and hematuria (+3/200). A history of urological ultrasound showed right and left nephropathy with mild hydronephrosis and ascites. To sum up, we reported male patient 6 months with congenital nephrotic syndrome. Chromosomal analysis is  recommended in  this case.

Abstract

Introduction: Physiology is a study of the normal functions of the human body. However, the traditional lecture-based approach to teaching this subject has been observed to cause disengagement and boredom among students. Universitas Negeri Gorontalo was exploring the use of PhysioEx®, an interactive e-learning laboratory to studying physiology. This study aims to identify effective teaching methods to enhance student learning outcomes in physiology courses.Method: A true experimental study was carried out at the Faculty of Medicine in Universitas Negeri Gorontalo in October 2023 with a pretest and posttest group. Each test comprised of nine questions and had a time limit of nine minutes. The study population consisted of 200 Public Health students and 69 medical students from the year 2023. A sample of 161 random individuals was selected for analysis using the Slovin formula, and the data was analyzed using the T-test analysis.Results: Based on the statistical analysis of the pretest and posttest scores, it was observed that both the lecture method and the PhysioEx® method resulted in a significant increase in scores (p0.05). However, no substantial difference was observed in the learning outcomes between the two methods (p 0.05).Conclusion: The comparative effectiveness of the lecture method and the PhysioEx® method in the instruction of physiology has been analyzed. The findings indicate that both methods are similarly effective when employed in educational contexts pertaining to physiology. The results of this analysis underscore the utility and potential of both pedagogical approaches. Keywords: Learning effectiveness, e-learning, physiology, physioEx®

Abstract

Primary effusion lymphoma (PEL) is a rare large B-cell non-Hodgkin lymphoma (NHL) that usually occurs in immunosuppressed individuals in advanced stages of the disease, presenting as body cavity effusions without detectable tumor mass. We report a 61-years old man presented to the emergency room complaining of effort-related dyspnoea, low-grade fever, night sweats, and a dry cough for several days. He was then put on empirical anti-tubercular treatment (ATT) suspecting Pulmonary Tuberculosis. General examination revealed a decrease oxygen saturation, tachypnea, a bulging tenderness lymph node on the neck. Chest computerized tomography (CT) scan indicated pericardial and pleural effusion with mediastinal dilatation suspected thymoma or lymphoma. A Cardiocentesis needle aspiration was performed; however, the histological pattern was not specific. The patient underwent a surgical biopsy of a left supra-clavicular lymph node. A Classical Hodgkin lymphoma nodular sclerosis type was found in the histopathology examination. Immunohistochemistry detected the tumour cellular markers (i.e., CD30). Chemotherapy treatment for 6 cycles resulted an excellent outcome. In conclusion, radiologists play a vital role in the diagnosis of PEL, detecting effusions in body cavities and excluding the existence of solitary masses or pleural nodules or masses, ultimately narrowing down the differential diagnoses.

Abstract

Introduction: Lower Urinary Tract Symptoms (LUTS) are common in older adults and significantly impact individuals, caregivers, and the broader healthcare system. As the elderly population with various comorbidities increases, the burden of LUTS will increase. The chronic disease management program (Program Pengelolaan Penyakit Kronis, PROLANIS) is a government program generally followed by the elderly. This study aims to describe the characteristics of LUTS in PROLANIS patients in Gorontalo Province.Methods: The analytical observational study design, using a cross-sectional approach, was conducted on PROLANIS patients living in rural and urban areas in Gorontalo Province is between July - October 2023. An incidental sampling technique was conducted on eligible patients and areas. LUTS symptoms were measured using the International Prostate Symptom Score (IPSS) questionnaire and categorized into mild, moderate, and severe. LUTS symptoms were considered significant if the IPSS score was ≥8.Results: Of the 52 participants, 38.50% experienced significant LUTS symptoms. The majority of symptoms felt were nocturia (score two, as many as 23.10%) and mild symptoms (53.80%). Geographical differences, gender, education level, and age were not associated with LUTS symptoms (p 0.10).Conclusion: The prevalence of LUTS in patients undergoing PROLANIS is relatively high. The majority of LUTS patients have mild symptoms, with nocturia being the most common complaint compared to other symptoms. Further studies with a larger sample size and involving multiple sites are needed to obtain a more valid description of the characteristics of LUTS epidemiology in the PROLANIS population.                                                                         Keywords: Aged; Disease Management; Lower Urinary Tract Symptoms; Nocturia
