Abstract

Hyperglycemia is the main symptom in patients with diabetes mellitus. One of the risk factors that can increase blood sugar levels in diabetes mellitus (DM) patients is anxiety and stress, as a result of diabetes mellitus sufferers who experience anxiety, their physical condition does not improve, and their blood sugar levels are always high. Progressive muscle relaxation therapy (PMR) is a therapy that aims to focus the patient's attention so that they can differentiate the feelings that occur when the muscles are relaxed or relaxed from when they are contracting so that the patient can manage their body condition against their anxiety. This study aims to analyze the provision of progressive muscle relaxation therapy to control anxiety levels and blood sugar levels in patients with type 2 diabetes mellitus in the Gardena medical ward of Dr. Soebandi Hospital Jember. This research uses descriptive methods in the form of case studies on a patient with Type 2 DM. Therapy was given once a day for 20-30 minutes. Anxiety levels were measured using the DASS questionnaire, and blood sugar measurements were made using a blood sugar stick. After being given PMR therapy for 3 x 24 hours, the patients experienced a decrease in anxiety levels and blood sugar levels. Providing PMR therapy is effective in treating unstable blood glucose levels and anxiety for patient with Type 2 DM.

Abstract

Background: Multiple Fractures or broken bones are a condition where the continuity of the bones is broken in a complex manner, this is caused by trauma caused by frequent traffic accidents. Objective: To analyze the effect of a combination of non-pharmacological therapies Quantum Spiritual Touch Therapy" (QST) and Finger Holding in multiple fraktur patients with acute pain at RSD dr. Soebandi Jember. Method: This research is an observational study with a case study design. Primary data obtained through anamnesis and physical assessment. Assessment of the effect of QST therapy and finger holding was carried out a total period of 35 minutes, 20 minutes for QST therapy and 15 minutes for finger holding therapy, based on initial, process, and final data. Results: The results showed a reduction in pain levels with a Numerical Rating Scale (NRS) was carried out for 6 meetings with the implementation of therapy once a day. Pain levels decreased in the pre-implantation measurements with an average of 5,4 to 4,7 at the time of post-implementation. Discussion: The combination of pharmacological therapy from QST therapy and finger holding aims to help the effectiveness of analgesic drugs by improving the psychological structure with implementations that contain spiritual elements.

Abstract

In the aging process, various changes occur regarding biology, cognition, and psychosocial life. The elderly are easily affected by diseases such as non-communicable (degenerative) diseases such as hypertension. As a result of aging, it can reduce the elasticity of the blood vessel walls and cause stiffness in the blood vessels; as a result, blood pressure in the elderly tends to increase. This study aims to determine the impact of applying acupressure therapy to chronic pain problems in the elderly. The research method used in this study was a descriptive case study. The research was conducted for five days on one patient with hypertension and chronic pain at an Elderly Residential Home. Acupressure therapy was carried out once a day for 20 minutes. Evaluation of the implementation is given using the NRS pain scale. The result of Acupressure therapy showed that the level of chronic pain was decreased from the NRS scale 5 to the NRS scale 2, blood pressure 140/80 mmHg, pulse rate 85 x/minutes, respiration rate 18 times per minute, and SpO2 96. This study recommends that acupressure therapy can be applied to treat chronic pain in hypertension patients as a complementary therapy.

Abstract

Background: Nursing problems in brain injury patients include the risk of ineffective perfusion due to reduced oxygen flow to the brain, which, if not treated immediately, will increase intracranial pressure (ICP). One way to increase ICP is with 30-degree head-up position intervention. Objective: This study aimed to analyze the application of 30-degree Head Up Position on Cerebral Perfusion in Moderate Brain Injury Patients. Method: This research is an observational study with a case study design. Primary data was obtained through anamnesis and physical assessment with a sample of a patient being treated in the Hospital by assessing the effect of the 30-degree head-up position carried out for 30 minutes for 3 consecutive days. Results: In this study, the results showed an increase in cerebral perfusion as indicated by cerebral perfusion indicators, namely blood pressure, MAP, pulse, patient breathing improved, and the patient's level of consciousness increased until the final evaluation. Conclusion: From the results of the innovation analysis in this case, providing a 30-degree head-up position in patients with a risk of ineffective cerebral perfusion is one form of effectiveness in increasing the patient's cerebral perfusion so that it can prevent an increase in intracranial pressure.

Abstract

Background: Management of secretions is one of the main problems faced in the medical condition of respiratory disorders. The use of complementary therapy techniques to treat airway clearance such as the active cycle of breathing technique is still low when compared with other interventions in treating respiratory problems. Aims: The research aims to determine the effectiveness of providing active cycle of breathing technique therapy to overcome nursing problems ineffectiveness airway clearance in nursing care. Method: This type of research is a case study using nursing care methods. The research sample was patients treated in the Catleya room at Dr. Soebandi Jember Hospital. Patients are given nursing intervention in the form of therapy active cycle of breathing technique for 4 consecutive days 2 times a day for 15-20 minutes with 4 cycles. Results: providing active cycle of breathing technique therapy can reduce nursing problems ineffectiveness airway clearance. Providing active cycle of breathing technique therapy effectively shows improvements in conditions such as improved respiratory frequency, reduced shortness of breath, and decreased sputum production. Conclusion: providing active cycle of breathing technique therapy effectively solve nursing problems ineffectiveness airway clearance.

Abstract

Background: Respiratory diseases such as bronchopneumonia contribute to the highest cause of death in children aged less than two years. Airway clearance becomes a problem due to the accumulation of secretions, resulting in airway obstruction. Independent nursing interventions such as chest physiotherapy can be carried out to free the airway and reduce the burden of breathing. Objective: This study aimed to analyze the application of chest physiotherapy intervention in bronchopneumonia children with airway clearance problems. Method: The research design uses a case study with a nursing process approach. The sample for this study was one child patient aged nine months with bronchopneumonia who was taken using a convenience sampling technique. Data was collected using interviews, observation, and documentation studies using pediatric nursing care format instruments. Chest physiotherapy intervention was carried out three times with a duration of 10-15 minutes with a combination of postural drainage, which was then evaluated before and after the procedure. Results: Chest physiotherapy intervention in patients can improve coughing ability, increase sputum production and respiratory rate 40 times per minute, and decrease rhonchi and oxygen saturation by 96%. Conclusion: Chest physiotherapy can be an effective independent nursing intervention to overcome the problem of ineffective airway clearance nursing in children with bronchopneumonia.

Abstract

Background: The term speech delay is a disorder of speech delay which is classified as an expressive language disorder or with difficulty expressing, which occurs in early childhood. The lack of communication time between young children and their parents or peers can affect their language skills. Objective: To find out whether there is a relationship between mother's knowledge and attitudes regarding preventing speech delay in children aged 1-3 years. Method: This research uses descriptive analytical research with a cross-sectional research design. The population used was all mothers who had children aged 1-3 years with a total sample of 111 people, and the sampling technique was the Non- Probability Sampling technique. Results: The results of the Chi-square test statistic showed a p-value = 0.00. With a 95% confidence level ∝ = 0.05. This means that there is a relationship between maternal knowledge and the prevention of speech delay in children aged 1-3 years in the Pantai Labu Health Center, Deli Serdang Regency. The statistical test results of the Chi-square test showed a p-value = 0.00. With a 95% confidence level ∝ = 0.05. This means that there is a relationship between the mother's attitude and the prevention of speech delay in children aged 1-3 years in the Pantai Labu Health Center. Conclusion: It is hoped to provide support and information to mothers to find out their child's language development and detect early if there are problems with their language development (speech delay).

Abstract

Many diabetes mellitus sufferers still lack regular physical activity. Physical activity such as brisk walking can be a form of exercise that effectively controls blood sugar levels. This study aims to determine the effect of brisk walking on random blood sugar levels in Prolanis in the Kesamben Community Health Center working area, Jombang Regency. The research design uses a pre-experiment with a one-group pre-post-test design approach. The population of Prolanis members was 35 respondents, and the sample size was 20 respondents taken using non-probability purposive sampling. The independent variable is brisk walking, the dependent variable is random blood sugar levels, data collection uses the GlucoDR blood sugar checking tool, and the statistical test used is the Wilcoxon Test analysis. The research results showed that of the 20 respondents, the majority, namely 15 (75%), were in the high category before being given the fast walking treatment. On the other hand, most respondents were in the moderate category after fast walking exercise, namely 15 respondents (75%). The results of the Wilcoxon statistical test show p-value = 0.002 where p-value < α (0.05). It can be concluded that brisk walking affects random blood sugar levels. Brisk walking exercise affects changes in random blood sugar levels in Prolanis in the Kesamben Community Health Center working area, Jombang Regency. Brisk walking is recommended as a type II diabetes management therapy to control blood sugar levels because of the low side effects, and it has been proven to be effective.

Abstract

The incidence of stunting in toddlers is a global nutritional problem. Stunting causes delays in fine motor and gross motor development, increases morbidity and mortality rates, and increases the risk of metabolic disorders in adulthood. The aim of this study was to determine the relationship between body weight, exclusive breastfeeding, MP-ASI (weaning foods) and energy intake with stunting in toddlers aged 24-59 months. This research is descriptive analytical with a cross sectional design. The sample consisted of 92 participants selected using simple random sampling technique. Data collected with body weight measurements for birth weight, exclusive breastfeeding and MP-ASI questionnaires, energy intake calculate using daily energy requirements calculator, and stunting incidence questionnaire. Data analysis was carried out using the Chi-square test (P<0.05). The results of the study explain that there is a relationship between body weight (p=0.000), exclusive breastfeeding (p=0.003), MP-ASI (p=0.004) and energy intake (p=0.000) with the incidence of stunting in toddlers aged 24–59 months. The research conclusion is that there is a relationship between body weight, exclusive breastfeeding, complementary foods and energy intake with the incidence of stunting in toddlers aged 24-59 months.
