Abstract

Introduction: Fatigue that felt by diabetes melitus patient will be a barrier when doing self-management. An ineffectiveness of self-management will affect many complications, either macrovascular and microvascular. Objective: The goal of this research is determine the relation of fatigue and self-management of diabetes melitus type 2. Method: The design of this research is using observative correlative with cross – sectional way. There are 102 research samples obtained from the calculation G*Power. The methode of taking the data doing with way to gather use with Multidemensional Fatigue Inventory (MFI20) questionnaire and Diabetes Self-Management (DSMQ) that has tested validity and realibility for analyze the fatigue and self-management. The data from analyzed using spearman rank (α = 0,05). Results: Research’s result average of respondent showed that general fatigue (15,66), physical fatigue (13,8), reduced activity (12,44), reduced motivation (11,01), mental fatigue (11,37). Patient self-management showed if from 102 respondent in majority has good self-management 86 (84.3%) dan 15 (14,7%) the rest of respondent has enough – good. The second analyzed test showed that no relation that significant between fatigue and self-management. Abstract

ABSTRAKPasien dengan diabetes melitus (DM) tipe 2 akan menggambarkan penyakit yang dialami sesuai dengan apa yang ada dalam pikirannya untuk memahami masalah terkait. Persepsi sakit dapat ditingkatkan melalui pendidikan kesehatan oleh perawat tentang bagaimana mengelola penyakit dan manajemen perawatan diri. Penelitian ini bertujuan untuk menganalisa hubungan antara peran perawat sebagai edukator dengan persepsi sakit pada pasien DM tipe 2. Jenis penelitian ini adalah observasional analitik dengan pendekatan cross sectional. Teknik sampling penelitian ini adalah consecutive sampling dengan sampel sebanyak 112 responden. Pengumpulan data menggunakan kuesioner peran perawat sebagai edukator dan kuesioner Brief Illness Perception Questionnaire (BIPQ). Analisis data dilakukan dengan menggunakan uji korelasi Spearman-rank dengan tingkat signifikansi 0,05. Hasil penelitian menunjukkan terdapat hubungan yang signifikan antara peran perawat sebagai edukator dengan persepsi sakit (p value: 0,001; r: -0,433). Tingkat korelasi sedang dan bersifat negatif yang berarti semakin tinggi nilai peran perawat sebagai edukator maka semakin rendah nilai persepsi sakit. Semakin baik peran perawat sebagai edukator, semakin positif persepsi pasien tentang penyakit, yang berarti semakin sedikit ancaman penyakit yang dirasakan oleh pasien. Studi ini menunjukkan bahwa perawat harus dapat meningkatkan penyediaan pendidikan kesehatan untuk pasien dengan DM tipe 2 sehingga pasien dapat memiliki persepsi positif terhadap penyakit.   ABSTRACT Patients with type 2 diabetes mellitus will describe their illness based on what is in their minds to understand related problems. Illness perception can be improved through health education by nurses about how to manage the illness and self-care management. This study aimed to analyze correlation between the role of nurse as educator and illness perception in patients with type 2 diabetes mellitus. This research applied an observational analytic design with cross sectional approach. A total of 112 respondents were obtained by using consecutive sampling technique. Data collection was conducted by administering questionnaires of Role of Nurse as Educator and Brief Illness Perception Questionnaire (BIPQ). Data analysis was performed with Spearman-rank correlation test with significance level of 0,05. The result showed a significant correlation between the role of nurse as educator and illness perception (p value: 0,001; r: -0,433). The correlation was moderate and negative which means the higher the value of the role of nurse as educator, the lower the value of illness perception. The better the role of the nurse as educator, the more positive the patient’s perception about the illness, which means the less threatening the illness is perceived by the patients. This study indicates that nurses should be able to improve the provision of health education to patients with type 2 diabetes mellitus so that patients can have positive perceptions of the illness.

Abstract

ABSTRACT Introduction: Insulin therapy is one of the treatments in type 2 diabetes mellitus. Patients undergoing insulin therapy often experience side effects such as hypoglycemia. As a result, the patient feels a trauma that makes the patient change the insulin dose, ignores insulin injection time and lack of confidence when giving insulin injections independently. This condition makes managing blood sugar levels more difficult to achieve. This study aimed to analyze the correlation between self-compassion and adherence to insulin therapy in patients with type 2 diabetes mellitus at RSD dr. Soebandi Jember. Method: This research applied a descriptive correlational design with a cross-sectional approach. A total of 84 respondents obtained using consecutive sampling. Data collection conducted administering a questionnaire of the Self Compassion Scale (SCS) and the Morisky Medication Adherence Scale (MMAS-8). Data analyzed using the spearman correlation test with a significance level of 0.05. Result and analysis: The result showed a significant positive correlation between self-compassion and adherence to insulin therapy (p value=0.001; r=0.694). Discussion: Patients in managing diabetes need adherence to treatment. The existence of adherence in patients begins with the patient's self-acceptance of the illness. Self-compassion in patients is present when patients can accept themselves and love themselves without looking at the problem they face. The higher self-compassion makes a better adherence to insulin therapy. This study indicates the importance of self-compassion in patients with type 2 diabetes mellitus to improve insulin therapy compliance to control the patient's blood sugar levels. Abstract

ABSTRAKStroke dapat menyebabkan penurunan kesadaran. Pada kasus stroke dengan penurunan kesadaran dapat mengakibatkan pasien mengalami kematian, defisit neurologi, semakin lamanya waktu perawatan, dan akan meningkatkan biyaya perawatan. Kasus stroke dengan penurunan kesadaran banyak dijumpai di RSD dr. Soebandi Jember. Namun upaya perawat dalam meningkatkan kesadaran pasien berfokus pada terapi farmakologi sehingga memerlukan terapi non farmakologi seperti terapi Familiar Auditory Sensory Training (FAST) untuk membantu proses pemulihan kesadaran. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh FAST pada tingkat kesadaran pasien stroke. Penelitian didesain dengan quasi experimental menggunakan rancangan Non equivalent control group melibatkan 29 pasien stroke yang mengalami penurunan kesadaran. 29 pasien dibagi menjadi 15 kelompok intervensi yang diberi terapi FAST selama tiga hari. FAST diberikan 3 kali sehari. 14 pasien berikutnya dalam kelompok kontrol hanya dilakukan pemeriksaan GCS. Tingkat kesadaran pasien diukur menggunakan instrumen Glasgow Coma Scale (GCS) yang terdiri dari 3 komponen yakni respon mata, verbal, dan motorik. Data dianalisis dalam SPSS menggunakan uji Mann-Whitney dan uji Wilcoxon. Hasil analisis uji baik pada kelompok intervensi maupun kontrol diperoleh nilai p 0,010 (nilai p α). Hasil uji yang signifikan membuktikan bahwa ada pengaruh FAST dalam meningkatkan kesadaran pasien stroke. FAST dapat dijadikan terapi nonfarmakologi untuk membantu proses pemulihan kesadaran pada pasien stroke.ABSTRACTStroke can cause a decrease in consciousness. In the case of stroke with decreased consciousness can result in the patient experiencing death, neurological deficits, the longer treatment time, and will increase the cost of treatment. Cases of stroke with decreased consciousness are often found in RSD dr. Soebandi Jember. However, nurses' efforts in increasing patient awareness focus on pharmacological therapy so that it requires non-pharmacological therapy such as Familiar Auditory Sensory Training (FAST) therapy to help the process of recovering consciousness. The purpose of this study was to determine the effect of FAST on the level of awareness of stroke patients. This study involved 29 stroke patients who experienced a decrease in consciousness. 29 patients were divided into 15 intervention groups who were given FAST therapy for three days. FAST is given 3 times a day. The next 14 patients in the control group only performed GCS examination. The level of patient awareness was measured using the Glasgow Coma Scale (GCS) instrument consisting of 3 components namely eye, verbal, and motor response. Data were analyzed in SPSS using the Mann-Whitney test and Wilcoxon test. The results of the test analysis in the intervention and control groups obtained p-value 0.010 (p-value α). Significant test results prove that there is an influence of FAST in increasing stroke patient awareness. This study shows that nurses should be able to increase the application of nonpharmacological therapies such as FAST to help the process of recovering consciousness in stroke patients.

Abstract

Metabolic disorders in blood sugar levels are caused by damage to insulin secretion so that it affects energy production. Fatigue is one of the complaints and symptoms of Diabetes Mellitus (DM) patients. This study aims to analyze the relationship between energy management and fatigue in patients with diabetes mellitus. This study used a correlational design with a cross sectional approach. Calculation of the sample in this study using the Slovin formula, which obtained 34 respondents with diabetes mellitus at the Kejayan Health Center, Pasuruan Regency using purposive sampling technique. Data collection in this study used an energy management questionnaire and Multidimensional Fatigue Invantory (MFI20). The results show that in energy management, energy management in patients with diabetes mellitus is obtained with a good category of 55,9%. On fatigue, the median result is 60.50 with a min-max value of 34-73. The correlation test showed that there was a relationship between energy management and fatigue (p = 0,033, r = -0,366, = 0,05). There is a negative correlation which shows that the higher the energy management, the lower the fatigue felt. Fatigue felt by diabetic patients does not go away easily so that energy management must be carried out properly. It is expected that diabetes mellitus patients maintain good energy management. It is a concern for nurses in improving energy management of diabetes mellitus patients to reduce the fatigue they feel.

Abstract

Diabetes distress is an emotional problem that is directly related to the burden and worriesresulted from living with chronic disease. Stress has an impact on self-care behaviour whichaffects metabolic control. This study aimed to analyze the correlation between diabetesdistress and self-care behaviour in people with type 2 diabetes mellitus.This research appliedan observational analytic design with cross sectional approach. A total of 66 respondents wereenrolled in this study by using quota sampling technique. Data collection was conducted byadministering questionnaires of Diabetes Distress Scale (DDS) and Summary of Diabetes SelfCare Activities (SDSCA). Data were analyzed by using Spearman test with significance level of0.05. The result showed that the mean value of diabetes distress and self-care behaviour was2.16 and 3.97 respectively. The p value was 0.000 (p <0.05) with the correlation (r) of -0.629.There was significant correlation between diabetes distress and self-care behaviour. Thecorrelation was strong and negative which means the higher the level of diabetes distress thelower the performance of self-care behaviour. This study suggests the importance of assessingexperienced distress to optimize self-care behaviour in type 2 diabetes patients.Keywords: type 2 diabetes mellitus, diabetes distress, self-care behaviour

Abstract

Background: Phantom pain after amputation is ordinary. Phantom pain is challenging to treat, so it will last a long time which will cause depression and anxiety and reduce the quality of life. There are various treatment options: pharmacological (antidepressants, anesthetics) and non-pharmacological (acupuncture, hypnosis). However, there is still no proven effective therapy, so it is necessary to try a non-pharmacological therapy that is safe, cheap, and easy, namely mirror therapy. Purpose: This study aims to determine how the effect of mirror therapy on the phantom pain of post-amputation patients. Methods: Narrative literature review of a publication registered 2017-2021 on Pubmed, ProQuest, EBSCO, and Google Scholar. Search articles using the keywords" amputation," and " phantom pain," and "mirror therapy." This study uses inclusion criteria consisting of patients who experience phantom pain, research in the form of mirror therapy intervention, there is comparison, the study design uses a randomized controlled trial and case report, with results explaining the effect of mirror therapy in Indonesian or English, and indexed by SINTA or SCIMAGO. Results: 8 articles were included in the inclusion criteria. The majority of articles had a randomized controlled trial design of 7 articles and a case report of 1 article. Four articles discuss mirror therapy, and the other four articles discuss mirror therapy with other therapies. Respondents have an age range of 15-82 years, and most are male. Conclusion: Mirror therapy is recommended to be carried out routinely and according to procedures for post-amputation patients. It has been proven to reduce pain scales in patients before and after the intervention. Nurses can use mirror therapy in the client care process during the post-amputation rehabilitation process.

Abstract

Hypoglycemia is an acute complication of diabetes mellitus which frequently occur repeatedly identified by blood glucose level less than 70 mg/dl. Diabetic patients experience dilemmatic situation where they are required to obtain diabetes drug therapy to control blood sugar level remain normal, but facing another concerns about the side effects of treatment which can lead to complication of hypoglycemia. This condition will affect the psychological fearness of repeated attacks that result a traumatic feelings in diabetic patients. The study aimed to gain in depth understanding of diabetic patients experiences who had experienced of hypoglycemia episodes in Persadia Depok. This was a qualitative research which applied phenomenology method and involved six participants with diabetes mellitus experienced hypoglycemia episodes. Data were collected using in-depth interview and field notes. A Colaizzi's content analysis method was operated across data and revealed 6 themes: decline in physical function while in response to hypoglycemia, traumatic feelings when experiencing hypoglycemia, participants' understanding that caused of hypoglycemia, awareness of hypoglycemia prevention, internal beliefs became the main source of coping strategy to face hypoglycemia and nursing care needs. The results of this study suggested a need of improvement in nursing education for patients with hypoglycemia.

Abstract

Bacground: Chronic disease is the highest cause of death in the world and Indonesia which can affect patient’s quality of life. One of the key factors for a good patient’s quality of life is empowerment. Self-empowerment is important for patients in making decisions with health professionals regarding the patient's health. Purpose: The aim study was to explore self empowerment in patient with chronic disease. Methods: The methods was a narrative literature. Selection of articles using PRISMA flow diagram. The databases used Science Direct, Pubmed, and Google Scholar. Result: The result was 5 articles discuss empowerment and 6 articles discuss intervention related to empowerment. Empowerment develops while increasing self-efficacy and self-management. Empowerment can also improve the relationship between patients and health professionals. As for interventions related to empowerment, including health education, empowerment based on experiential learning, energy conservation, mobile health technology. Conclussion: Patients with chronic disease have a responsibility in managing their disease. Therefore, empowerment becomes important in patients with chronic disease. In addition, empowerment can also improve health outcomes. An effective intervention in increasing empowerment is to provide health education to patients and families about the illness.
