Abstract

Mohs micrographic surgery is a specialized form of skin cancer surgery that has the highest cure rates for several cutaneous malignancies. Certain skin cancers can have small extensions or ‘‘roots’’ that may be missed if an excised tumor is serially cross-sectioned in a ‘‘bread-loaf’’ fashion, commonly performed on excision specimens. The method of Mohs micrographic surgery is unique in that the dermatologist (Mohs surgeon) acts as both surgeon and pathologist, from the preoperative considerations until the reconstruction. Since Dr. Mohs’s initial work in the 1930s, the practice of Mohs micrographic surgery has become increasingly widespread among the dermatologic surgery community worldwide and is considered the treatment of choice for many common and uncommon cutaneous neoplasms. Mohs micrographic surgery spares the maximal amount of normal tissue and is a safe procedure with very few complications, most of them managed by Mohs surgeons in their offices. Mohs micrographic surgery is the standard of care for high risks basal cell carcinomas and cutaneous squamous cell carcinoma and is commonly and increasingly used for melanoma and other rare tumors with superior cure rates. This review better familiarizes the dermatologists with the technique, explains the difference between Mohs micrographic surgery and wide local excision, and discusses its main indications.

Abstract

Alopecia areata (AA) is a condition characterized by sudden local hair loss without signs of skin inflammation or scarring. Data regarding the incidence of alopecia areata in Indonesia is still scanty, a retrospective study at Cipto Mangunkusumo Hospital Jakarta showed that the diagnosis of alopecia areata was the most common type of baldness in skin and genital polyclinics from 2009-2011 (39.7%). This case report was obtained from primary data through history taking, physical examination and supporting examination. The patient came with complaints of baldness on the scalp that occasionally itch. Another complaint is the growth of grey hair in areas that are bald and break easily. Experienced baldness appeared for the first time 3 months ago. On examination of the dermatological status, efflorescence of bald patches was clearly demarcated, on almost the entire scalp with the largest lesion on the right temporal and on the edge of the baldness growing white hairs that were easily broken. The location of the capitis region, the size varies (lenticular, numular, plaque), round / oval shape with regional distribution. On investigation, scalp scrapings with 10% KOH did not reveal hyphae and spores, anti-HIV and non-reactive VDRL, routine blood examinations revealed leukocytosis. Patients were treated with intralesional corticosteroids, topical corticosteroids and minoxidil according to the alopecia areata treatment algorithm.

Abstract

Gastrointestinal tract is a condition in which bleeding occurs in the oesophagus, stomach, duodenum, jejunum, ileum, and rectum. This is caused by many things, such as in the upper gastrointestinal tract due to gastric ulcers, various esophageal ruptures, Mallory Weiss syndrome, esophagitis, and tumors. While the lower gastrointestinal tract can be caused by Crohn's disease, ulcerative colitis, diverticulitis, intestinal polyps, anal fissures, hemorrhoids and tumors. The description of age characteristics at the time of gastrointestinal bleeding at Cut Meutia General Hospital, North Aceh Regency in 2019- 2021 was mostly at the age of 20 - 34 years, which found 17 people (28.7%). Description of the characteristics by gender in the female gender recorded 42 people (65.5%). The number of patients who underwent endoscopy at Cut Meutia General Hospital, North Aceh Regency in 2019-2021 presented 73 people of whom 59 people underwent colonoscopy (77.5%) and 14 people who underwent gastroscopy (22.5%).

Abstract

A 17-month-old female patient was brought by her family to the emergency room at the Lhoksukon Health Center with complaints of weakness for the last 2 days. Weakness was felt to be burdensome since the patient had a fever on the 5th day. Fever is felt up and down throughout the day. The fever is accompanied by chills, sweating, and restlessness. On the first day of fever, the patient was given fever medicine by the patient's mother at home, but the fever did not subside. The patient also experiences nausea and vomiting. Nausea appears sometimes when eating. Vomiting is in the form of food that is eaten but the vomit volume is not much. The patient's appetite also decreases. Primary data were obtained through alloanamnesa, physical examination, supporting examinations, as well as making home visits, filling out family folders, and filling out patient files. The assessment was carried out based on the initial holistic diagnosis, process and end of the visit both quantitatively and qualitatively. Interventions carried out include education about the causes of typhoid fever to their families, education about hygiene and handling of the disease and explaining complications that may arise from the patient's illness so that they can make efforts to prevent and treat it if it recurs.

Abstract

Hypertension or high blood pressure is a chronic condition characterized by increased blood pressure on the walls of the arteries. This situation causes the heart to work harder to circulate blood throughout the body through blood vessels, even causing degenerative diseases and even death. The purpose of hypertension exercise is to increase the knowledge of residents in Gembor village RW 007 RT 008 Persada Total Housing related to hypertension exercise to reduce blood pressure. The method used is by socializing and providing counseling about the steps in hypertension exercise. Health counseling was held on Saturday 1 July 2023. The population in this counseling were residents of RW 007 Persada Total Housing. After carrying out health education about hypertension, it was found that 55.2% of the residents understood, understood and were able to give an example of how to exercise hypertension to reduce blood pressure.

Abstract

Degeneration of organs in aging causes joint inflammation in the elderly. The decrease in the hormone estrogen after women enter menopause makes women more prone to experiencing joint pain than men. The perception of joint pain in the elderly can get worse without any exercise. Alternatives that can be practiced include gymnastics and music. This study aims to determine the effect of a combination of gamelan music and elderly gymnastics on joint pain in elderly women in the working area of the Yosomulyo Banyuwangi Public Health Center in 2022. This research method is pre-experimental with One Group Pre-test Post-test Design. The sample size is 30 elderly women using a purposive sampling technique. The independent variables are gamelan music and elderly gymnastics which are given to respondents 8 times in 1 month. The dependent variable is joint pain as measured using the NRS observation sheet. Data analysis used the Wilcoxon test with SPSS 22. The results of the research on respondents before the intervention were that almost half of the respondents, 14 elderly (46.7%) experienced severe pain. After administering the intervention, half of the respondents, 15 elderly (50%) experienced mild pain. Wilcoxon test results showed Asymp. Sig = 0.000. This means that there is an effect of a combination of gamelan music and elderly gymnastics on joint pain in elderly women in the work area of the Yosomulyo Banyuwangi Health Center in 2022. With the results of this study, it is hoped that health workers can provide support to the elderly to increase their motivation to do gymnastics accompanied by gamelan music accompanied by their families or existing health cadres

Abstract

One of the causes of nurse job satisfaction is influenced by leadership style factors. Nurses who are satisfied with their jobs tend to provide better and more effective care to patients. This has a positive impact on patient satisfaction and trust in the hospital. Quantitative research with the type of associative analysis research using Cross Sectional. Sampling using the Total Sampling approach totaling 36 nurses in the acacia and sandalwood rooms. Data analysis using the chi square test. 1)The optimal autocratic leadership style is 38.9%; 2)optimal democratic 72.2%; 3) optimal free control of 33.3%; 4) Nurse job satisfaction has a satisfied category of 58.3%; 5) There is a significant relationship between the autocratic leadership style and the job satisfaction of nurses with a p-value = 0.000; 6) democratic leadership style with a p-value = 0.000; 7) free control leadership style with a p-value = 0.040. There is a significant relationship between autocratic, democratic, and free control leadership styles on the job satisfaction of nurses at Dr. Hospital. Jambi City Bratanata. It is recommended for the head of the room to have clear and continuous communication with the nurse in order to build a strong relationship between the leader and the nurse. Give nurses the opportunity to make decisions and take responsibility for their work.

Abstract

Appendicitis is an inflammation that occurs in appendix vermicularis, a tubular organ located at the base of the large intestine located in the lower right abdomen, which acute inflammation of the appendix. Appendicitis is the most common case of acute abdominal surgery. Appendicitis can hit all age groups, although it is not common in children before school age. can causes dangerous complications if surgery is not performed immediately. The patient's history of the course of the disease and physical examination are the most important things in diagnosing appendicitis. All cases of appendicitis require the removal of the inflamed appendix, both with laparotomy and with laparoscopy. If treatment is not taken, the mortality rate will be high, mainly caused by peritonitis and shock. Here we present a case of acute perforated appendicitis in a 29-year-old male patient.

Abstract

Diabetes mellitus is a state of chronic hyperglycemia accompanied by metabolic disorders due to hormonal disturbances, which causes various chronic complications in the eyes, kidneys, nerves and blood vessels accompanied by lesions on the basement membrane in electron microscopy examination. Gangrene is a process or condition characterized by the presence of dead tissue or necrosis, but microbiologically it is a process of necrosis caused by infection. Wounds can be interpreted as disruption or damage to the integrity and function of tissues in the body. Someone who suffers from an injury will feel imperfection which in turn tends to experience physical and emotional disorders. This means, someone who has an injury will experience health problems that can have an impact on the quality of his life.

Abstract

Colds are considered a "common" disease in Indonesia, despite the fact that they can affect various aspects of life and can be fatal. The cure for colds generally uses traditional medicine where scrapings are usually chosen as the first cure, besides herbal medicine, especially those made from ginger extract so that air can get out of the body. This study aims to describe the level of knowledge about traditional medicine for scraping and the consumption of red ginger extract against colds in the people of Kota Agung Bengkulu Village in South Bengkulu in 2021. This study used a descriptive approach with a research sample of 86 people. Data collection techniques using closed questionnaires and data analysis techniques using descriptive analysis and simple statistics. The results showed that the characteristics of the people of Kota Agung Village, Seginim District, South Bengkulu Regency, Bengkulu Province who utilized traditional medicine for scrapings and consumed red ginger extract as a treatment for colds were dominated by female sex as much as 59%, 66% aged ? 45 years, 67% high school education. and have other jobs as much as 53%. The level of knowledge about kerokan traditional medicine and the consumption of red ginger extract against colds in the people of Kota Agung Village, Seginim District, South Bengkulu Regency, Bengkulu Province has a high level of knowledge, namely 87% for the level of knowledge of kerokan and consumption of red ginger extract, and 93% for the level of knowledge of kerokan and red ginger extract consumption. cold knowledge.
