Jurnal Kesehatan dr. Soebandi (JKDS) is a national journal covering scientific studies in various health fields (Nursing, Midwifery, and Pharmacy). Jurnal Kesehatan dr. Soebandi is a scientific journal as a forum for academics and practitioners to present quantitative and qualitative scientific works. JKDS published by LPPM Universitas dr. Soebandi Jember. This journal was first published in October 2012 with two times publications in one year (April and October). Editors receive empirical research articles and studies in Nursing, Midwifery, and Pharmacy. It is hoped that the consistency and quality of scientific work can improve the quality of education in the future. The purpose of Jurnal Kesehatan dr. Soebandi: Supporting the Development and Improvement of Health Degrees in theory and practice in Indonesia with integrated and critical research results. Abstract

Background :  Diabetes is a chronic disease often accompanied by life-threatening complications, such as diabetic foot ulcers and lower limb amputations. Low self-efficacy in foot care is a major factor contributing to the development of diabetic foot complications. This study aimed to analyze the correlation between foot care self-efficacy and ulcer risk in patients with type 2 diabetes mellitus.  Methods :  This study employed an analytical observational design with a cross-sectional approach. A total of 112 respondents were recruited using a consecutive sampling technique. Data were collected using the Foot Care Confidence Scale questionnaire and the Inlow’s 60-Second Diabetic Foot Screen Tool observation sheet. Data were analyzed using the Spearman test. Results:   The results showed that the mean score of foot care self-efficacy was 37.96 (SD = 8.24), while the mean ulcer risk score was 6.32 (SD = 2.88). There was a strong negative correlation between foot care self-efficacy and ulcer risk (p < 0.001; r= −0.773). Conclusions: Higher foot care self-efficacy was associated with a lower risk of ulcers in patients with type 2 diabetes mellitus. Assessment of ulcer risk and interventions to improve foot care self-efficacy are essential to reduce the prevalence of ulcer risk in patients with type 2 diabetes mellitus.

Abstract

Background: Poor rest quality could be a frequent comorbidity among people with hypertension, contributing to negative cardiovascular conditions and decreased quality of life. This research assesses the significance of the HiAl (Hypertension Alert) intercession in improving rest quality among hypertensive patients in Jatisari, Jember, east Java.  Methods: This true experimental study selected one hundred registrants diagnosed with hypertension (50 registrants in the intervention team and the rest in the absence of the intervention team), selected through a straightforward random sampling technique. Participants were randomly assigned to either the intervention team or the rest in the absence of the intervention team receiving usual care from their attending physicians. The HiAl treatment included autonomous motivation, relatedness to health services, and perception of self-competence. Sleep quality was measured at baseline and the start of the research before treatment for both teams using the Pittsburgh Sleep Quality Index (PSQI) one month following the intervention. Results:   People who took part in the treatment experienced a statistically significant improvement in PSQI scores compared to the absence of an intervention team (24% before treatment and 76% after treatment people who took part in the intervention showing increasing in sleep quality, p = 0,002 using Wilcoxon signed-rank test). Conclusions: The findings indicate that the HiAl intervention is an effective approach for enhancing sleep quality in individuals with hypertension. HiAl may serve as a valuable tool for hypertension management and cardiovascular health promotion. Further research is warranted to assess its long-term impact and cost-effectiveness.

Abstract

Background: The elderly population is projected to continue increasing, reaching an estimated 2 billion by 2050. A 2023 meta-analysis study reported a prevalence of depression among the elderly at 35.1%. Effective preventive measures, routine screening, and timely interventions are critically needed. This study aimed to identify the relationship between the level of religiosity and family support with the level of depression among elderly individuals in Cilember Village. Methods :  A quantitative approach with a correlational design was employed. The total sampling technique was used, involving 37 respondents. The instruments utilized included the Spiritual Well-Being Scale (SWBS) to assess religiosity, a family support questionnaire, and the Geriatric Depression Scale (GDS-15) to evaluate depression levels. Results:  The findings revealed that the majority of elderly participants had a low level of religiosity (48.6%) and moderate family support (54.1%). Additionally, 37.8% of the elderly experienced severe depression. Data analysis using the Chi-Square test showed a significant relationship between religiosity and depression level (p = 0.047), as well as between family support and depression level (p = 0.048). Conclusions:  There is a significant association between religiosity and family support with depression levels among the elderly, with both factors serving as protective elements for the mental health of older adults.

Abstract

Background: Triage is a crucial competency for nurses in the field of emergency nursing, as it determines the level of urgency for each patient. The concept of emergency care focuses on the triage system, which classifies patients based on the severity of their condition. Although triage methods vary across countries, the fundamental principle remains the same: prioritizing patients according to the severity of their condition and the resources available. Two triage methods that can be applied based on patients’ clinical signs and are in line with nursing competencies are the Emergency Severity Index (ESI) and the Early Warning Score (EWS). Nursing students, as novice healthcare providers preparing for clinical practice, must be equipped with a strong understanding of triage. It is therefore essential to introduce various triage methods to students, particularly through the application of these methods in clinical case simulations, which they are likely to encounter during clinical practice. This underpins the urgency of research comparing triage time between the ESI and EWS methods. The objective of this study was to analyze nursing students’ ability to apply triage using ESI and EWS, and to compare triage duration between these two methods in simulated clinical cases (vignette-based scenarios). Methods: The study employed an experimental design with a one-group pretest–posttest approach, where clinical case vignettes were presented to be assessed using both ESI and EWS methods. Results:  The respondents consisted of 200 undergraduate nursing students drawn from five nursing education institutions in Malang City and Regency. Mann–Whitney test results indicated a significance value of 0.000, demonstrating a significant difference in triage duration between the ESI and EWS methods. Conclusions:  The findings revealed that the ESI method was faster than the EWS method. All respondents were undergraduate nursing students with limited clinical experience. Future studies are recommended to examine the implementation of ESI and EWS methods in emergency departments or critical care units in hospital settings

Abstract

Background:  The Fear of Missing Out (FOMO) phenomenon is increasing among Generation Z due to high social media exposure. FOMO is a psychological condition characterized by feelings of anxiety about being left behind from the experiences of others, so that individuals are driven to stay connected to social media. The impacts of FOMO include increased anxiety, sleep disturbances, decreased self-esteem, and social isolation. Efforts can be made to prevent FOMO behavior in Generation Z by implementing promotive and preventive measures. This study proposes the psychoeducational film series "Nakula: Kesatria Tanpa Takut Ketinggalan" as an audiovisual media that has great potential in promoting mental health, especially for Generation Z who are more interested in visual-based content. The aim is to analyze differences in mental health in Generation Z adolescents at SMP N 10 Malang after watching the psychoeducational film series "Nakula: Kesatria Tanpa Takut Ketinggalan". Methods: This quantitative research uses a Quasi-experimental design, with a purposive sampling technique. The population in this study were 314 8th grade students at SMP N 10 Malang, the sample in this study was 176 respondents and divided into 2 groups, namely the intervention group of 88 people and the control group of 88 people. The inclusion criteria in this study were 8th grade students who attended SMP N 10 Malang, willing to participate in the study until the end. The questionnaire used was the Fear of Missing Out scale and the Mental Health Support Scale for Adolescents (MHSSA) questionnaire. Analysis with the Wilcoxon Test using SPSS. Results:     The results of the study showed that there were significant differences in the intervention group and the control group with a p-value = 0.000. It is hoped that further research will make better innovations. Conclusions:  Diharapkan pada penelitainselanjutnya untuk membuat inovasi yang lebih baik.

Abstract

Background:   Microplastics causing tissue damage and increasing the number of lymphocyte cells and neutrophils (segment and band). Telang flower infusion contains flavonoids as antioxidants that can as immunomodulatory. The purpose of this study was to determine the effect of telang flower infusion on the number of lymphocyte cells, band and segment neutrophils. Methods:   Methode begins with the manufacture of telang flower infusion, the manufacture of microplastics (heating bottled drinking water at a temperature of 65oC), the analysis of the size of microplastics using electron microscopes, the analysis of microplastic types using the FTIR method, the grouping of negative control, positive control, and treatment with a dose of 4 ml/250g BW, exposure to microplastics, blood smear, giemsa staining for lymphocyte and neutrophil cell analysis, and statistical test using One-Way ANOVA (p<0.05) with Tuckey HSD (p<0.05). Results:  The results showed that the average number of negative control band neutrophil cells (3.50+4.50), positive control (3.17+3.81), and treatment (5.33+3.61) and the One-Way ANOVA analysis showed values (p=0.81>p=0.05), the average segment neutrophil cells of the negative control (23.67+6.80), the positive control (21.33+5.82), the treatment (24.00+8.19),  and the One-Way ANOVA analysis showed values (p=0.776>p=0.05), negative control lymphocyte cells (68.50+14.84), positive control (72.17+4.99), and treatment (64.00+12.10). The One-Way ANOVA analysis showed a value of (p=0.481>p=0.05). Conclusions:  It can be concluded that telang flower infusion has a tendency to act as an immunomodulator at the biological level characterized by a decrease in lymphocyte cells, an increase in band neutrophil cells and segment nuetrophic cells.

Abstract

Background: During the Covid-19 pandemic, the management of drug inventory and sypply faces a great challenges. A common problem in the management of supplies at the pharmacy installation is the accumulation of drugs that are rarely used, which causes drugs to become damaged or expired, as well as the shortage of drugs that should be available. RSU Kaliwates Jember has implemented a strategy in 2023 to optimize the drug inventory in the Pharmacy department by applying a Just-In-Time strategy in drug purchasing. This study aims to analyze the impact of the strategy in three sector: drug procurement cost, total inventory cost, and inventory day. Methods:   The analysis applied the Quacy-experimental one group pretest-posttest design. The main goal is to analyze causal relationships by comparing the results of the two tests in order to determine the impact of the treatment. This study is a one-group pretest-posttest research design by comparing the value before and after the implementation of just-In-Time strategy, value at 2022 and 2023. The data was obtained from system management at RSU Kaliwates Jember (SIMRS) for procurement cost, inventory values, an inventory days. Results:   The analysis showed that there is a significant efficiency in drug procurement cost with a p-value of 0.041. For the total inventory value, the strategy could significantly reduce the value wiyh p-value of 0.006. In addition, the strategy could shorten significantly the inventory days with p-value 0.017. Conclusions:   This study conclude that the implementation of just-in-time strategy has an impact on improving the efficiency of drug inventory management especially in the reduction of the procurement cost, inventory value and inventory days. The impact of this strategy is worth to further analyze in the other sector such us in the drug wastage due to dead stock or expire drug and drug shortage that could affect the pharmacy care.

Abstract

Background : Traffic accidents along the North Coast Route (Pantura) in Tuban, East Java, remain a major public health concern, often leading to severe injuries and fatalities due to delays in early emergency response at the scene. Limited community capacity in providing first aid highlights the urgent need for effective training interventions. This study aimed to evaluate the effectiveness of simulation-based emergency response training in improving knowledge, attitudes, and practical skills among adolescents living in high-risk areas. Methods :  A quantitative quasi-experimental study with a one-group pretest–posttest design was conducted among 155 adolescents aged 15–18 years selected using cluster random sampling across five high-risk areas. The intervention consisted of four training sessions delivered within one week, combining lectures, demonstrations, and scenario-based simulations covering CPR, bleeding control, and victim evacuation. Data were collected using validated questionnaires and observation checklists before and after the intervention. Statistical analysis was performed using paired-sample t-test with a significance level of p < 0.05. Results:  Knowledge scores increased from 56.3 to 82.7, attitude scores from 60.1 to 84.2, and practical skills scores from 48.5 to 79.9, all showing statistically significant improvements (p < 0.001). Conclusions:  Simulation-based emergency training significantly improves community capacity in emergency response, particularly in practical skills. This approach is recommended as a scalable strategy to strengthen early emergency management in accident-prone areas and reduce preventable morbidity and mortality.

Abstract

Background: Breast milk production greatly affects the breastfeeding process because it is a physiological stage that involves complex interactions between stimulation of the breast, nerve reflexes that play a role in the release of prolactin and oxytocin. The prolactin hormone plays a role in the production of breast milk; on the other hand the oxytocin hormone plays a role in the release of breast milk. Lack of stimulation of these mechanisms causes disruption of breast milk production and release. Breast massage techniques such as Oketani Breast Massage, Wollwich Massage and Rolling Massage aim to increase blood flow to the breast tissue and stimulate the release of the oxytocin hormone which plays a role in the milk ejection reflex.  The study was to identify the average breast milk production before the administration of the Oketani Rolling Massage and Wollwich Rolling Massage methods and to analyze the differences in mothers who were lactating in the Sukorambi Community Health Center Working Area, Jember Regency. Methods :     This study used a quasi-experimental pretest-posttest design in one group on postpartum mothers in the Sukorambi Community Health Center working area. Twelve respondents were selected as samples using a purposive sampling procedure, and they were divided into two groups of six respondents each. The first group received a combination intervention of Oketani Breast Massage and Rolling Massage, while the second group received a combination of Wollwich Massage and Rolling Massage. Breast milk production was measured based on the indicator of infant weight gain before the intervention, then the intervention was given for seven days, and re-measured on the eighth day. Data were analyzed using bivariate analysis or the Wilcoxon test. Results:   showing a significance level of p-value of 0.002 (<0.05), which means there is a difference in breast milk production before and after the intervention.  Conclusions: the following study concludes that the combination method of Oketani Rolling Massage and Wollwich Rolling Massage is effective in increasing breast milk production, Suggestion: so it is recommended to be implemented by health workers, especially midwives.

Abstract

Background :  Akhlakul karimah (good morals) are all actions or behaviors that are good and commendable, indicating the perfection of a person's faith in God, whereas caring behavior is giving attention and concern. These values reflect the human side of a professional and ethical nurse in providing nursing care to clients. This study aims to determine the level of nursing students' knowledge about the values of akhlakul karimah contained in the concept of caring behavior, such as: honesty (shiddiq), justice (‘adlu), voluntariness (ihsan), wisdom (hikmah), confidence (tsiqoh), patience, humility (tawadhu), and all things that are good, where these traits can be reflected in the caring behavior of nurses. Methods :   The research method used a non-experimental design with a descriptive type, a cross-sectional approach, conducted over 3 months and involved 158 respondents. Results:   a total of 121 respondents (76%) had a high level of knowledge and 37 respondents (24%) had sufficient knowledge.  Conclusions:   In providing nursing care, the knowledge and application of akhlakul karimah contained in the concept of caring must be instilled early in students, so that when they graduate and work, they are able to reflect the character of a professional nurse. For educators, both in academics and in practice settings, they should always be figures who can provide good examples of how to apply the values of akhlakul karimah in caring behavior when working to serve clients.
