Abstract

Congestive Heart Failure (CHF) or heart failure is a condition in which the heart cannot pump blood effectively to meet the body's needs. This study aims to provide an overview of the implementation of Nursing Care for patients with a diagnosis of Congestive Heart Failure (CHF) in the lotus room of Waled Hospital, Cirebon Regency. The method used is a case study approach. The subjects in this case study were one patient with a medical diagnosis of Congestive Heart Failure (CHF) in the inpatient room. The results of the study showed that the management of nursing care in patients with medical diagnosis of Congestive Heart Failure (CHF) in meeting basic needs with Congestive Heart Failure (CHF) nursing problems carried out foot oedema intervention for approximately 10-15 minutes. Swelling in the lower extremity of the patient is reduced or slightly deflated. Based on the results of nursing care carried out on ny.w based on the interventions carried out so that it can be concluded that nebulizer removal can help increase oxygen saturation and foot oedem techniques can reduce swelling in the patient's feet

Abstract

The basic need for oxygenation is one of the most vital basic needs in human life. This is related to the cellular metabolic process, where this process requires oxygen in the body. The impact that occurs if the lack of oxygen in the body is not immediately resolved can cause a lack of oxygen in the body's cells. Body cells that lack oxygen will have difficulty concentrating because metabolism is disrupted due to a lack of oxygen supply in the blood. This case study aims to explain nursing care for patients with a diagnosis of Bronchopneumonia medicine using oxygenation therapy interventions, nebulizer inhalation therapy, and semi-fowler position or head up 30o. This research uses a qualitative approach with case studies as the main method, using observation sheets, interviews, and documentation studies. The subject of this research was one patient, An. K patient with bronchopneumonia at Sumber Kasih Hospital, Cirebon City. The results of the case study showed clinical success that the nursing problem of ineffective airway clearance and ineffective breathing patterns was resolved by increasing airway clearance, improving breathing patterns, and increasing oxygen saturation.

Abstract

Non-hemorrhagic strokes usually occur due to blockage of brain blood vessels caused by thrombosis, embolism and global hypoperfusion which results in decreased blood supply to brain tissue and leads to ischemia. Therefore, it is necessary to handle it using procedures correctly, namely implementing nursing care including: assessment, nursing diagnosis, planning, implementation and evaluation. The aim of this research is to provide appropriate application of nursing care to clients with non-hemorrhagic stroke in the Stroke Unit. This type of research uses a descriptive method with a case study approach in the form of assessment, data analysis, formulating nursing diagnoses, planning, implementation and evaluation in handling found nursing problems. After providing nursing care to the client Mr. B, the results showed that the nursing problem was partially resolved. The client said his blood pressure had started to decrease, he could move his left leg a little.

Abstract

One of the nursing problems commonly experienced by mothers after post partum is ineffective breastfeeding. Ineffective breastfeeding is a condition where the mother and baby experience dissatisfaction or difficulty in the breastfeeding process. One way to solution this issue is breastcare. This research aims to provide nursing care with a more comprehensive approach covering the entire body system to improve the client's health status. This type of research uses a descriptive method with a case study approach in the form of assessment, data analysis, formulating nursing diagnoses, planning, implementation and evaluation. After 3 days of management of post partum mothers using data collection techniques using interviews, physical examination, observation, and documentation studies, it was found that nursing problems in spontaneous post partum clients, namely acute pain and ineffective breastfeeding, had been resolved. After providing nursing care to the client, complaints of pain decreased and breast milk production increased.

Abstract

The 2018 Lower Health Study (Rikesdas) stated that the total number of SC births in Indonesia was 17.6% of 78,736 births. Post-SC mothers do not start breastfeeding on the first day after giving birth because it makes them feel great pain and makes breastfeeding difficult. Another factor that inhibits the formation of breast milk is also influenced by the consumption of post-operative pain relievers. Objective: This research is to determine the description of nursing care for post-operative caesarean section patients with indications of premature rupture of membranes (KPD) in the Mawar ward at Waled Hospital. Method: This type of research is a descriptive method with a case study design. Results: the results of the evaluation at the end of treatment in the diagnosis of acute pain and risk of infection resolved the problem but in the diagnosis of ineffective breastfeeding the problem was partially resolved. Conclusion: The author carried out a comprehensive assessment that was suitable for the conditions of post-operative cesarean section nursing care. The intervention was carried out 2x24 hours with the cooperation of the nurse. The author has taken action in accordance with the nursing plan that has been made.
