Abstract

Introduction: Visuospatial impairment, as a form of cognitive impairment, could serve as an early indication of dementia in the elderly. Sarcopenia, a progressive and generalized skeletal muscle disorder, can be associated with the risk of visuospatial impairment in the elderly. Aim: This study aims to get an overview of visuospatial impairment with its associated risk factors in the elderly at Pusaka Kebon Jeruk, West Jakarta. Methods: This study uses a cross-sectional involving 100 respondents aged 60 years and above. Visuospatial impairment is measured using constructional praxis. An elder is considered to has visuospatial impairment if scoring < 11. Sarcopenia is assessed according to European Working Group on Sarcopenia in Older People 2 (EWGSOP2). Frailty status is evaluated using the Fried frailty phenotype. Blood glucose and serum cholesterol levels are measured through laboratory tests. Results: This study included 100 respondents, with 50 respondents having visuospatial impairment. Bivariate analysis found a significant association between sarcopenia and visuospatial impairment (P: 0.016, OR: 2.667, 95% CI: 1.188 – 5.985). Multivariate analysis found a similar result, that is a significant association between sarcopenia and visuospatial impairment (P: 0.017, OR: 2.752, 95% CI: 1.203 – 6.296). Bivariate analysis was done by using χ2 test, while multivariate analysis was done by using binary logistic regression using backward analysis. Discussion: Sarcopenia is significantly associated with visuospatial impairment, increasing the risk of visuospatial impairment by 2.752-fold. Keywords: Elderly, Sarcopenia, Visuospatial impairment,

Abstract

Introduction : In line with the rapid growth of elderly population, the quality of life of the elderly needs to be considered. There are several factors that affect the quality of life of the elderly such as balance, depression, and olfactory function. Aim: This study aimed to analyze the association between depression and olfactory function with quality of life among elderly. Methods: This was a cross-sectional study of 100 subjects aged ≥ 60 years in Pusaka Kebon Jeruk, West Jakarta. The variables were assessed by using the World Health Organization Quality of Life–BREF (WHOQOL–BREF) questionnaire, Geriatric Depression Scale-15 (GDS-15), Berg Balance Scale (BBS), and Olfactory Test. Binary logistic regression with backward conditional analysis were used to analyze the association between quality of life and determinant factors. Results: Based on sociodemographic characteristics, the respondents were mostly aged 60-74 years (75 %), had more than 9 years of education (97 %), and mostly woman (71 %). The analysis showed that depression was significantly associated with the social relationship domain (OR= 3,237 ; 95% CI 1,035-10,119 ; p= 0,043) and the overall quality of life (OR= 6,231 ; 95% CI 1,134-34,246 ; p= 0,050). Likewise, olfactory function was also significantly associated with the social relationship domain (OR = 3,327; 95% CI 1,249-8,865 ; p= 0,016). While gender was found to be a protective factor for environment domain (OR= 0,143 ; 95% CI 0,033-0,619 : p= 0,009 Discussion: In this study, depression was significantly associated with social relationship domain and overall quality of life. Olfactory function was also significantly associated with social relationship domain. While gender was found to be a protective factor for environment domain Keywords: Quality of life, depression, olfactory function, balance disorder, elderly.

Abstract

Introduction: The pre-hospital period plays a critical role in determining the outcomes of patients with acute stroke. Various demographic factors can influence the time at which a patient arrives at the hospital.Aim: This study aims to examine the correlation between demographic factors and the duration of prehospital stay in acute stroke patients at Abdoel Wahab Sjahranie Samarinda Regional Public Hopsital (RSUD).Methods: The research design employed int this study was observational and analytical, utilizing a cross-sectional methodology. Data for the study were collected through interviews with the patients’ family members and by reviewing their medical records. The sample consisted of 80 participants who met the inclusion criteria, , including 61 patient with ischemic stroke and 19 patients with hemorrhagic stroke. Results: The findings of the study revealed that the average age of acute stroke patients was 63, with a majority of them being male. Most of the individuals who experienced acute strokes had completed high school education. In comparison to those who were unemployed, a higher percentage of acute stroke patients were employed and earned more than minimum wage. All 80 acute stroke patients had health insurance coverage. The majority of acute stroke patients did not own cars and resided within a 10-kilometer radius of the hospital. The study found no significant correlation between the duration of pre-hospital in acute stroke patients and factors such as age, gender, education level, occupation, income or car ownership (p = 0.246 ; 0.174 ; 0.515 ; 0.475 ; 0.769 ; 0.771).Discussion: It was observed that the distance of the patient’s residence from the hospital (>10 km) was associated with delayed hospital arrival in acute stroke patients, with a p-value of 0.008.Keywords: : Acute Stroke, Demographic, Pre-hospital

Abstract

Introduction: Frailty is a multifactorial geriatric syndrome resulting from the loss of the body's ability to cope with stressors due to aging. According to a study conducted by Asenso et al., the global prevalence of frailty is 13.6%. Aim: This study is aimed to understand the relationship between physical fitness parameters and frailty syndrome in the elderly. Methods: This study uses a cross-sectional involving 98 respondents aged 60 years and above. ADL was measured using the Barthel Index. Balance was measured using the Berg Balance Scale. Physical activity was measured using the SQUASH questionnaire with cutoffs based on the GPAQ. Frailty status was assessed using the Fried frailty phenotype. Results: From 98 respondents, the majority were female (71.4%), aged 60-74 years (74.5%), with a higher education level (51%). A total of 18.4% of the elderly experienced frailty. Chi-square analysis showed that significant factors were age (p<0.001; OR: 7.408), ADL (p=0.019; OR: 3.944), Balance (p=0.042; OR: 7,800), and physical activity (p=0.014; OR: 3.714). Multiple logistic regression showed that age is the most influential factor in frailty (p=0.002; OR: 6,209) and followed by physical activity (p=0.030; OR: 3.834). Discussion: Age, ADL, balance, and physical activity were found to have a significant relationship with frailty. Among these factors, age was identified as the most influencing factor. Abstract

Introduction: Sepsis is a serious complication and major cause of mortality among intracerebral hemorrhage (ICH) patients in the intensive care unit (ICU). Serum lactate is commonly used as a prognostic biochemical marker for sepsis in critically ill patients. Aims: This study aimed to evaluate the relationship between serum lactate levels, sepsis incidence, and clinical outcomes in ICH patients in ICU of Dr. Kariadi Hospital. Methods: A retrospective observational cohort study was performed on 30 ICH patients hospitalized in the ICU during 2024. Data normality tested using Shapiro-Wilk; non-normal data were analysed by Mann-Whitney U, categorical data by Chi-square test. Results: Among 30 patients (12 males, 18 females), 20 had hyperlactatemia (>2 mmol/L). The hyperlactatemia group had higher sepsis incidence (90% vs. 50%, p=0.014), MODS (70% vs. 30%, p=0.037), high NLR (>8; 80% vs. 40%, p=0.028), longer ventilator (>8 days; 75% vs. 40%, p=0.007), and LOS (>8 days; 75% vs. 40%, p=0.007). Discussion: Routine serum lactate monitoring in ICH ICU patients is recommended for early detection and prognosis of sepsis. Larger prospective studies are needed to validate these findings. Keywords: ICU, intracerebral haemorrhage, lactate, prognosis, sepsis

Abstract

The history of neurointervention began with Prof. Egas Moniz, a Portuguese Nobel Prize-winning physician who discovered angiography in 1928. It later evolved into a minimally invasive procedure. This procedure is performed on patients with cerebral and spinal cord vascular disorders such as stroke, aneurysm, vascular malformation, and brain tumors. In Indonesia, neurointervention procedures are performed by neurologists, radiologists, and neurosurgeons who have completed additional fellowships in vascular neurointervention, neuroradiology, or vascular neurosurgery. The development of neurointervention in Indonesia is inseparable from the efforts of Dr. Fritz Sumantri Usman, who completed his Egas Moniz Neurointervention and Stroke Fellowship at Sir Ganga Ram Hospital in New Delhi, India, in 2008. The team then grew from one person to 10 in 2011, and by 2025, there were 152 neurointerventionists across Indonesia. Fellowship programs currently being pursued come not only from India, but also from South Korea, Vietnam, China, and Austria. Eight hospitals in Indonesia offer fellowship programs in vascular neurointervention. Various endovascular procedures performed by neurointerventionists, such as mechanical thrombectomy, have become the standard of care in acute stroke management. Other minimally invasive endovascular procedures offer advantages over surgery. Hopefully, Indonesian neurointervention will continue to grow and provide the best healthcare for the Indonesian people. As stated in the neurointervention march, "From the beginning, the goal is to climb and climb, it is not easy, unite your steps, with a strong determination for the sake of the motherland, build Neurointervention. Glory to Neurointervention."

Abstract

Introduction : Central post stroke pain (CPSP) is a chronic central neuropathic pain syndrome associated with somatosensory abnormalities caused by stroke lesion in central nervous system (CNS). Pain which occurs either with a stimulus or spontaneous, usually exist 1-6 months after ischemic or hemorrhagic stroke. CPSP can cause anxiety, depression, sleeping disorder that lead to decrease quality of life in patient due to chronic pain. Transcranial Magnetic Stimulation (TMS) is a non-pharmacological, as well a non-invasive intervention therapy used to relieve chronic pain and anxiety disorders by using a magnetic field converted by electricity through a coild placed on the skull bone, which useful for reorganizing intracranial structures and functions disturbed by stroke lesion. Case : A 49 year-old patient came to clinic with pain in the left arm and leg, suffered since 5 months before admission, occurred with or without stimulus, reduced with analgetic drug but it came again soon after drug consumption. She felt a continuous stabbing and burning sensation with neurological pain rating scale (NPRS) : 6-7. Stroke history 3 years ago and routinely control to neurologist for medication. She felt difficult to sleep and anxiety because this pain. Hamilton Anxiety Rating Scale (HARS) was measured before and after rTMS. Discussion : Improvement in HARS and NPRS score was noted after rTMS, thought to occur due to reorganization of neurotransmitters involved in chronic pain pathophysiology. Conclusion : rTMS helped to improve quality of life in patient with CPSP. Keyword : Central Post Stroke Pain, Repetitive Transcranial Magnetic Stimulation, Anxiety Disorder, Hamilton Anxiety Rating Scale, Neurological Pain Rating Scale.
