Abstract

In order for hospitals to detect and intervene early, pediatric patient care must be of high quality. When making clinical decisions, assessing the risk of clinical deterioration, and providing appropriate emergency and critical care, the Pediatric Early Warning Score (PEWS) can be an excellent tool for nurses. This study aims to determine how Harapan Anda Hospital in Tegal City applies patient safety compliance and adherence to PEWS implementation. A cross-sectional approach was used in this quantitative study. A complete sampling approach was used to generate a sample size of 88 nurses from various healthcare facilities, including pediatric inpatient wards, PICU, NICU, and the emergency department. Patient safety surveys and PEWS compliance surveys were used to collect data. Spearman's test was used to analyze the data. The results showed that the majority of individuals (45 out of 63) were categorized as compliant in the implementation of the Pediatric Early Warning Score (PEWS). Meanwhile, the majority of nurses, 41 out of 106, or 46.6%, were compliant in their efforts to ensure patient safety. There was a significant relationship between the compliance of PEWS implementation and patient safety at RSU Islam Harapan Anda Tegal City with a p-value of 0.000. The results of this study are expected to be applied by healthcare workers to implement PEWS according to procedure, in order to ensure pediatric patient safety quickly and accurately.

Abstract

A 60-year-old male patient presented with the chief complaint of numbness in both legs, which had been experienced for the past two years and had worsened over the last three months. This complaint was accompanied by persistent fatigue, even without engaging in strenuous physical activity. In addition, the patient reported frequent urination, especially at night (nocturia), and excessive thirst that had occurred for the past five years. The patient denied symptoms such as fever or disturbances in bowel movements. Regarding his medical history, the patient regularly consumed Glimepiride as part of his treatment for diabetes mellitus. Primary data were obtained through auto-anamnesis and physical examination during a home visit. During the visit, the family physician also completed the family folder and patient records to comprehensively document the patient’s health condition. Case assessment was carried out based on an initial holistic diagnosis, followed by continuous monitoring and final evaluation using both quantitative and qualitative approaches. The interventions provided included health education for the patient and his family regarding the causes and pathophysiology of diabetes mellitus. The physician also emphasized lifestyle modifications, such as adopting a balanced diet, engaging in light but regular physical activity, and maintaining adherence to prescribed medication. Furthermore, the patient was educated on the long-term management of diabetes mellitus and the importance of regular medical check-ups. The family physician also explained the potential complications that could arise, such as neuropathy, nephropathy, and retinopathy, so that the patient would be more compliant with treatment and preventive efforts. This comprehensive approach was expected to improve the patient’s quality of life while preventing further disease progression.

Abstract

Vitamin A Deficiency (VAD) and iron deficiency anemia (IDA) are global nutritional issues affecting children, particularly in developing countries. The interaction between these two conditions can lead to a more complex condition known as Vitamin A Deficiency Anemia (VADA). This study aims to review the molecular relationship between Vitamin A deficiency and iron metabolism leading to VADA, as well as its impact on children's health. The methodology used in this article is a literature review of various clinical studies, epidemiological research, and relevant molecular experiments. The results show that VADA is primarily a functional iron deficiency, where iron absorption is impaired despite overall body iron levels not being critically low. This mechanism involves the regulation of hepcidin, redistribution of iron, and its effects on erythropoiesis. Additionally, therapy with Vitamin A supplementation has been shown to increase erythropoietin and hemoglobin production in children with dual deficiencies. The implications of these findings highlight the need for more integrated therapeutic strategies that consider both Vitamin A and iron status simultaneously. Further research is needed to understand the molecular mechanisms more deeply involved in this interaction and to develop more effective therapies for children with VADA.

Abstract

Nurses' caring behavior is a fundamental element in quality healthcare services. However, pressures such as high workload and burnout can hinder its implementation. This study aims to analyze the relationship between workload and burnout on the caring behavior of nurses in the Inpatient Ward of “X” Hospital. The research method used is quantitative with a cross-sectional design, involving 96 practicing nurses through a total sampling technique. Data were collected using the Subjective Workload Assessment Technique (SWAT) questionnaire, the Maslach Burnout Inventory (MBI), and a caring behavior questionnaire based on Jean Watson's theory. The results showed that the workload (p<0.001) and burnout (p<0.001) variables have a significant negative relationship with caring behavior. Logistic regression analysis identified workload as the more dominant factor influencing caring behavior. Simultaneously, these two independent variables could explain 57.9% of the variance in nurses' caring behavior. This study concludes that an increase in workload and burnout levels significantly decreases nurses' caring behavior, making workload management a priority intervention to improve the quality of nursing care.

Abstract

Background: Pregnant women with poor nutritional status are at risk of experiencing low breast milk production, which in turn can affect the fulfillment of infant nutrition from an early age. Suboptimal breast milk production impacts the growth and development of infants and has the potential to increase the incidence of malnutrition. Based on preliminary data in the Siti Hajar Room of Sultan Hadlirin Islamic Hospital, Jepara, it was found that several postpartum mothers experienced delayed breast milk production with a history of malnutrition. This condition shows the importance of maternal nutritional status during pregnancy as a factor that directly influences lactation. Objective: This study aims to determine the relationship between the nutritional status of pregnant women and postpartum breast milk production in the Siti Hajar Room of Sultan Hadlirin Islamic Hospital, Jepara. Method: The study used a cross-sectional design with a total sampling technique. A total of 30 postpartum mothers were selected as respondents. The independent variable consisted of the nutritional status of pregnant women as measured by Body Mass Index (BMI), Upper Arm Circumference (LILA), and hemoglobin (Hb) levels. The dependent variable was postpartum breast milk production as observed clinically. Data analysis was performed using the Chi-Square test with a significance level of p < 0.05. Results: The results of the study showed a significant relationship between the nutritional status of pregnant women and breast milk production. Statistical tests found that BMI (p = 0.018), LiLA (p = 0.027), and Hb levels (p = 0.006) had a significant association with the adequacy of breast milk production. Conclusion: The nutritional status of pregnant women was proven to be closely related to postpartum breast milk production. Mothers with good nutritional status tend to produce sufficient breast milk production. This finding emphasizes the need for routine nutritional monitoring of pregnant women and nutritional education during pregnancy to support the success of exclusive breastfeeding.

Abstract

Bronchopneumonia is the most common form of pneumonia in children and remains a leading cause of death in those under five years of age. This condition is characterized by shortness of breath due to lung infection. One of the interventions used is nebulizer inhalation therapy, which helps relieve breathing difficulties and thin secretions. This study aims to evaluate the effectiveness of inhalation therapy in children experiencing ineffective airway clearance. The method applied is a case study based on evidence-based practice (EBP) involving one patient, with nebulizer interventions administered for three consecutive days, each lasting 10–15 minutes. The findings showed improvement, including reduced shortness of breath, decreased productive cough, and the disappearance of rhonchi. Therefore, nebulizer therapy is proven effective in managing respiratory problems in children with bronchopneumonia, and its implementation should be accompanied by education for patients and their families to support collaborative nursing care.

Abstract

Menstrual cycle abnormalities are one of the reproductive health problems that are commonly experienced by adolescent girls, especially in the early days after menarche. These disorders can affect quality of life, including discomfort during activities, risk of anemia, and potential fertility disorders. A preliminary study at SMA Negeri 4 Pandeglang showed that 7 out of 10 grade XI students experienced menstrual cycle abnormalities, while there was no special program related to reproductive health in schools. This study aims to analyze factors related to menstrual cycle abnormalities, namely stress levels, diet, and physical activity. The research design used was quantitative with a Cross Sectional approach, involving 70 respondents who were selected through the Purposive Sampling technique. Data collection was carried out through a structured questionnaire and analyzed using the Chi-Square statistical test. The results of the analysis showed that there was a significant relationship between stress (p=0.011), diet (p=0.021), and physical activity (p=0.015) with menstrual cycle abnormalities. These findings confirm the importance of educational interventions in the school setting to improve adolescents' understanding of reproductive health and encourage healthy lifestyles. This research is expected to be the basis for the development of more comprehensive promotive and preventive programs in supporting the welfare of adolescent girls.

Abstract

Inguinal hernia is one of the most common abdominal wall disorders, accounting for approximately 75% of all hernia cases, with 50% classified as lateral inguinal hernias, and it occurs more frequently in older adults. We report a 57-year-old male patient who was admitted to the hospital with a complaint of a lower abdominal mass. The mass initially appeared in the right inguinal region one year prior and subsequently developed in the left inguinal region two months later. The swelling progressively increased in size. Intermittent pain, predominantly on the left side, had been present for the past month without associated nausea, vomiting, or fever. Physical examination revealed a moderately ill general condition, stable vital signs, and a soft, tender mass in both inguinal regions. Based on the clinical history, physical examination, and supporting investigations, the patient was diagnosed with bilateral irreducible lateral inguinal hernia. The patient received intravenous Ringer’s lactate and intravenous ketorolac, followed by an elective herniorrhaphy using Lichtenstein Tension-Free Repair Technique under spinal anesthesia. The operation was completed successfully without intraoperative complications.

Abstract

Labor is a complex interaction between psychological and physiological drives in women. (90%) delivery is accompanied by pain. In the active phase there is an increase in the intensity and frequency of contractions, so that the peak pain response is in this phase. Pain and soreness during labor are basically caused by strong contractions during labor. Breathing relaxation techniques can control pain by minimizing sympathetic activity in the autonomic nervous system. This study aims to determine the effect of deep breathing relaxation techniques on reducing labor pain in the first active phase. The type of design in this study is pre-experimental with The number of samples was taken according to the Accidental Sampling technique, samples were taken when patients went to the Andra Agustiana PMB Clinic with a minimum of 30 respondents. The data collection technique used a questionnaire sheet and a Numeric Rating Scale (NRS) pain scale. Data analysis used the Wilcoxon test. The results showed that before being given the breathing relaxation technique, most respondents experienced severe pain or pain levels on a scale of 7-10, namely 25 respondents (71.4%) and after being given deep breathing relaxation technique treatment, most respondents experienced moderate pain (scale 4-6) as many as 28 respondents (80%). The results of data analysis obtained the Asymp.significancy value = 0.000 (ρ<0.05). The conclusion is that there is an effect of providing deep breathing relaxation techniques on reducing labor pain in the first active phase. It is hoped that deep breathing relaxation techniques can be applied in the management of labor care as an alternative to reduce labor pain for mothers giving birth in the first active phase
