Abstract

Introduction and Objective Myopia is a refractive error that causes images of distant objects to be focused in front of the retina by the non-accommodating eye. Myopia is one of the most common visual disturbances in the world. It is believed that the use of spectacle can reduce the progressivity rate of myopia. Spectacle can help light fall on the retina and prevent axial elongation of the eye. This theory needs to be investigated further so that it can be used as a reference. This study aimed to know the correlation between compliance of spectacle wear and myopia progressivity rate in medical students of Diponegoro University (UNDIP). Methods This research was an observational analytical study with cross-sectional design on 54 subjects ranging from 18 to 22 years old. Data are primary data obtained from questionnaires and objective eye examinations using an autorefractometer. The research was conducted from October to November 2021. Data were analyzed using Spearman Rank test which p<0,05 was considered significant. Results The mean of compliance of spectacle wear and myopia progressivity rate in medical students of UNDIP are (1,57±0,108) and (0,259±0,515) dioptres respectively. The correlation between compliance of spectacle wear and myopia progressivity rate are p=0,082 and r=0,239. Conclusion There are no correlation between compliance of spectacle wear and myopia progressivity rate in medical students of UNDIP.

Abstract

Introduction: Cataract is an eye disease with clouding of the lens causing visual interference. Blindness due to cataracts is still the leading cause of blindness in the world. Cataract screening using the questionnaire integrated interview method can be an easy instrument for non-medical cadres to use. Cataract Symptoms Scale (CSS) is a complaint-based questionnaire in cataract patients which is used to screen for visual disturbances due to cataracts. Objective: To find out whether CSS has good validity as a screening tool for visual impairment due to cataracts by non-medical cadres. Methods: This study used a diagnostic test to assess the accuracy of CSS. All respondents who met the inclusion and exclusion criteria were included as research subjects. Results: 151 respondents included in the vision screening activity of the Community Ophthalmology Unit of Cicendo Eye Hospital. Respondents with cataracts in this study amounted to 59 (39.07%) and non-cataracts amounted to 92 (40.93%). The most complaints of respondents were blurry with 145 complaints and the least complaints were changes in the color of objects, namely 36 complaints. The CSS questionnaire sensitivity value of 61.01% indicates a weak value statistically. The specificity value of 78.26% indicates a moderate specificity value statistically. The positive predictive value of 64.28% indicates a statistically weak NDP value. The negative predictive value of 75.78% indicates a statistically moderate value. The value of accuracy is 71.52% statistically moderate. Conclusions: The Cataract Symptoms Scale has poor validity to be used by non-medical cadres as a screening tool for patients with visual impairment due to cataracts. Further research is needed by adjusting the components of the questionnaire and adding variables such as visual acuity and cataract laterality.

Abstract

Introduction & ObjectivesOne of the elements in Vision 2020: the right to sight is the fulfillment of resources to reduceavoidable blindness. According to the Indonesian’s prevention of blindness roadmap about sources,we need to collect the data about health facilities serving cataract surgery. The aim of this study isto provide the data about health facilities serving cataract surgery in South Sulawesi Province. MethodsA descriptive study using a questionnaire which is distributed to hospitals/clinics in South SulawesiProvince. In addition, secondary data was also used from previous research conducted by one of theresidents of Ophthalmology at Hasanuddin University. All the data were collected from Januari toJune 2022. ResultsThe distribution of health facilities in all districts/cities in South Sulawesi has at least one healthfacility serving cataract surgery, with the most distribution in Makassar. Twelve of 62 hospitals andfive of 27 eye clinics have not provided cataract surgery services..There are 6 districts (25%) thathave not fullfil the minimum number standard of ophthalmologist according to WHO. Alldistrict/cities in the Province have all the instrument required for cataract surgery. ConclusionEvery district/city in South Sulawesi has at least one health facility serving cataract surgery.Information on the distribution of health facilities serving cataract surgery needs to be routinelyconducted twice a year according to Indonesian’s prevention of blindness roadmap.

Abstract

Introduction & ObjectivesIntroduction: Prevalence of clinically significant astigmatism of more than 1D can be found in 20 to 50% population those undergo cataract surgery. During phacoemulsification surgery, astigmatism could be corrected by toric intraocular lens (IOL) or incisional technique such as limbal relaxing incision (LRI). LRI are safe and inexpensive procedures thus results in satisfying outcomes with surgeon’s precise phacoemulsification incision and accurate LRI arc position, which most appropriate treatment choice for surgeons in rural area that has problematic access towards IOL supply. Objectives: To compare refractive outcomes of LRI technique with incision depth of 500um and 600um using diamond knife corresponding with phacoemulsification cataract removal. Methods : Prospective cumulative interventional case study included 30 eyes of consecutive cataract corneal astigmatic patients power >1.0D those undergoing LRI and phacoemulsification. Length, numbers, and arc position of LRI were calculated on LRI Calc application to obtain best results in minimal astigmatism residual. Uncorrected visual acuity, intraocular pressure, and keratometric cylinder were also analysed before surgery, Day-1, Day-7, and 1-Month post-operation. Results : Day-1 follow-up keratometry shown that corneal astigmatism was still fluctuated, day-7 follow-up was even better with significant improvement in visual acuity. 1-Month post-operation has reached target correction. Moreover, visual acuity and residual astigmatism was better in 600um incision depth. Conclusion : When toric IOLs are not available or contraindicated, LRI could be a good option in correcting astigmatism with better refractive outcomes trend in 600um incision depth. LRI results better be evaluated in day-7 and 1-Month post-LRI, which may due to more stable corneal surface.

Abstract

Introduction: People with type 2 diabetes have an increase risk of cardiovascular disease (CVD). Framingham risk score is used to determine 10 years the risk of CVD in asymptomatic patients for primary prevention. Up to now, there have been no similar studies regarding the risk of CVD in diabetic retinopathy (DR) patients in Indonesia. Therefore, we would like to investigate the corelation between DR and CVD risk through the population using Framingham risk score. Methods: This research is a Population-Based Cross-sectional Study. The data was taken from the villages in Malang Regency. Participant with diabetes aged > 40 was recruited. The available data was then processed according to the variables and assessed using Framingham Risk Score. The scores were classified as high, intermediate, and low risk. Results: From 953 samples there were 155 respondents with Diabetes Mellitus (DM), 23 of them experience DR. There were more female participants than men in DM group without DR (73.9%) and DR group (73.9%). A significant correlation between diabetic retinopathy and CVD risk was found (r=0.407, p=0.001). The risk of CVD in 10 years is low in diabetic patients without retinopathy ( p=0.001), but in patients with diabetic retinopathy, the risk of CVD in 10 years is intermediate-high (p=0.001). Conclusion: There is corelation between Diabetic Retinopathy and risk for cardiovascular disease in intermediate-high category using Framingham risk score.

Abstract

Introduction & ObjectivesGlobal initiative, also known as “VISION 2020: the Right to Sight”, suggested that determining thenational target of human resources in eye health substantially reduces avoidable blindness andvisual impairment. Shortage of human resources is currently recognized as one of the mainproblems in achieving progress in eye health services to attain the global initiative. The purpose ofthis study is to analyze the situation of eye services in South Sulawesi. MethodsA descriptive study with a qualitative and quantitative approach was performed through aquestionnaire to ophthalmologists in South Sulawesi Districts and also secondary data fromophthalmologists, ophthalmic nurses, the Indonesian Ophthalmologists Association committee,provincial and district health office staffs Hospital in 2022. ResultsThe total population of South Sulawesi Province in 2022 was 9.07 million people. Human resourcedata showed 116 ophthalmologists (ratio 1:250,000 population), 108 trained ophthalmic nurses(ratio of 10 nurses per 1 million population), 1536 trained instructors, 10.918 posyandu cadres, 5trained eye cadres, and 71 refractionists/optometrists (ratio is 20 persons per 1 million people).There were 7 of 24 regencies/cities that have complete mandatory diagnostic examinationequipment. ConclusionThe ophthalmologist ratio in South Sulawesi is normal, however, their distribution is uneven,necessitating a rise in the number of nurses and optometrists with a balanced distribution.

Abstract

Introduction : The zonular defect leads to ectopia lentis, which is a hallmark of Marfan ocular abnormalities. Bilateral dislocation of lens in marfan’s syndrome occurs frequently in same location. In this case the lens of RE dislocated in the anterior chamber and the lens of LE was found in vitreous cavity. Iris fixation IOL and Yamane’s scleral fixation technique are prompt treatment for crystalline lens displacement. Case Illustration : A 14 year-old male patient came with chief complaint a painless blurred vision on LE since 2 weeks before visit Ophthalmologist. The BCVA in both eyes was 6/30 RE and 1/60 LE. There was no history of ocular injury nor development disorder. Lens dislocation in anterior chamber was found in RE and aphakic lens was found in LE during slit lamp examination. The IOP ODS were 9.7 mmHg and 3.0 mmHg. Further examination from funduscopy showed the lens of LE was seen in vitreous cavity. Discussion : Bilateral dislocation of lens in opposite position are quite rare. To avoid many of the potential complications, Iris Fixation IOL placement is preferred choice to improve visual impairment on the right eye. Yamane’s Scleral Fixation technique has been performed on left eye however challenging and time consuming it is, to achieve excellent improvement with minimal or no intra- and postoperative complications. Conclusion : This case shows that manifest of crystalline lens in marfan syndrome could happen differently to each eye. Different surgical approach especially IOL placement technique is crucial and must be considered carefully depends on the condition during surgery.

Abstract

Introduction and Objective: The social service of diabetic retinopathy (DR) and proptosis screening by going directly to the community is one of the efforts to reduce blindness. The aim of this study is reporting the result of DR and proptosis screening toward priests in Intaran Village, Sanur Kauh, Bali. Method: This is an observational study with cross-sectional approach conducted in April 2022.We performed DR screening using portable funduscopy and proptosis screening using Herthel exophthalmometry. Collected data were analyzed descriptively. Results: In the diabetic retinopathy screening, a total of 8 participants had their eyes examined. Based on the results of the examination, 3 people and 5 eyes were found to have diabetic retinopathy. Of the 3 participants who had RD, two participants had bilateral RD, while 1 had unilateral RD. Of the 5 eyes that had RD, four eyes had a mild degree, while 1 eye had a moderate degree. None of the eyes had severe degrees. Participants with RD were given referrals for further treatment. The average Hertel exophthalmometry examination result in the right eye was 14.30 ± 2.14 mm while in the left eye was 14.46 ± 2.11 mm. This data can be used as a reference for normal values in the population. Conclusion: DR screening is important because it can detect the disease before the appearance of complaints. DR screening by going directly to the community can also reach people who have difficulty in accessing health care facilities.

Abstract

Introduction & ObjectivesPterygium is associated with excessive exposure to sunlight and UV rays in the eyes. This causesisland countries in Asia which are in the "pterygium belt", including Indonesia, to have a highprevalence of pterygium. Other risk factors, such as gender, age, and medical conditions, such asblood pressure and diabetes, have also been linked to this chronic eye disease. However, of themany studies that have examined pterygium and its risk factors, very few have linked these riskfactors to the severity or grade of the pterygium. This study aimed to study the correlation betweenpterygium severity and its risk factors, within the people of Bawean Island, Indonesia. MethodsThis research was an analytic cross-sectional, with the research sample selected by total samplingmethod based on the results of a diagnosis performed by ophthalmologists. The research variablewere visual acuity (good-blindness), the severity / grade of pterygium (I-IV), gender, age,occupational group, pterygium location, blood pressure, and fasting blood sugar. ResultsFrom 62 patients with pterygium, 75,8% categorized as grade IV, and 41,93% has a moderate toblindness visual acuity. More than half of the patients are old female above 50 years old with 40,32%has an occupational risk of sunlight exposure. ConclusionThe results of this study indicate that although all the risk factors studied did not have a significantyon the visual acuity, age (p=0,13) and occupational groups (p=0,20) are determinant factors on thevisual acuity of pterygium patients on Bawean Island, Indonesia.

Abstract

Introduction : An electric shock is experienced when an electric current passes through a part of the body from an entry point to an exit point. Cataract is frequently described as a complication of electrical injury and it is usually bilateral. We report a rare case of high voltage electric injury in young male patient resulting in monocular cataract. Case Illustration : A 23 years old man was presented with a diminution of vision one year after sustaining an overhead high voltage power transmission wire that stung his head. The visual acuity of the right eye was hand movement dan 1.0 in the left eye. On slit-lamp examination, opacities were presented in the lens of the right eye and a clear lens in the left eye. Being diagnosed as having monocular cataract, the patient was then performed phacoemulsification on his right eye (April 12th 2023). The pupil shrank until 5 mm during the surgery but foldable IOL was successfully inserted in the capsule. The right eye vision was improved to 0.5 one day after surgery. Discussion : The consequences of high-voltage electric burns are complex. Electrical shock can cause protein coagulation and cataract formation. Lens manifestations are more likely when the transmission of current involves the patient’s head. Proper management of electrical cataracts will result in good visual rehabilitation if the eye does not suffer additional damage as in this case. Conclusion : Secondary cataract formation due to electric injury may occur, therefore routine examinations with the ophthalmologist are needed after the electrical trauma. The incidence of monocular cataracts after the electric injury is rare.
