Abstract

Diabetes Mellitus (DM) in the elderly is a chronic health problem that often causes complications, one of which is impaired peripheral tissue perfusion that may lead to decreased mobility, risk of diabetic ulcers, and even amputation. Elderly individuals with comorbidities and genetic factors have a higher risk of peripheral circulation disorders. One of the non-pharmacological interventions that nurses can provide is diabetic foot exercise aimed at improving peripheral blood flow and neuromuscular function of the lower extremities. This scientific paper aims to analyze nursing care in depth in one elderly patient with Diabetes Mellitus, namely Mr. A, who had a history of chronic disease and family factors, with the nursing problem of ineffective peripheral tissue perfusion through the provision of foot exercises. The method used was a case study with a nursing care approach including assessment, diagnosis, intervention, implementation, and evaluation. The results showed improvement in Mr. A’s peripheral perfusion after diabetic foot exercises were performed for three sessions, with a duration of approximately 15–20 minutes in each session, indicated by reduced tingling complaints, improved acral condition, increased peripheral pulse strength, and increased Ankle Brachial Index (ABI) value. Diabetic foot exercise proved effective as an independent nursing intervention in improving peripheral tissue perfusion in elderly patients with Diabetes Mellitus.

Abstract

Diabetes mellitus is a non-communicable disease that continues to increase and requires long-term management through the implementation of four pillars of care: education, medical nutrition therapy, physical activity, and pharmacological therapy. The successful implementation of these pillars is influenced not only by clinical factors but also by patients’ perceptions of the disease and its management. This study aimed to determine the relationship between patients’ perceptions and the implementation of the four pillars in the management of diabetes mellitus at the Simpang IV Sipin Community Health Center. This was a quantitative study with a correlational analytic design using a cross-sectional approach. The population consisted of 250 patients, with 78 respondents selected through purposive sampling. Data were collected using questionnaires and analyzed using the Spearman Rank test. The results showed that most respondents had good perceptions (51.3%) and good implementation of the four pillars (70.5%). A positive and significant relationship was found between patients’ perceptions and the implementation of the four pillars (r = 0.607; p = 0.000). Healthcare providers are recommended to enhance perception-based education to support optimal diabetes management.

Abstract

Nutritional problems among children under five remain a major public health concern in Indonesia, including both undernutrition and the increasing prevalence of overweight and obesity in early childhood. Parenting practices influenced by local wisdom are still widely applied in rural communities and may affect children’s dietary patterns and nutritional outcomes. This study aimed to analyze the role of local wisdom-based parenting practices in supporting the nutritional status of children under five in rural communities. This study employed a quantitative approach with a cross-sectional design. The research was conducted in Sumbersekar Village, Dau District, Malang Regency, from November 2025 to February 2026. A total of 34 mothers with children aged 6-59 months were selected using stratified random sampling. Data were collected through structured questionnaires assessing local wisdom-based parenting practices and anthropometric measurements to determine children’s nutritional status. Data were analyzed using descriptive statistics, Chi-square tests, and multinomial logistic regression. The results showed that most respondents applied local wisdom-based parenting practices in the good category (55.9%). The majority of children had normal nutritional status (61.8%), while 14.7% were undernourished, 17.6% were overweight, and 5.9% were obese. Statistical analysis indicated a significant association between local wisdom-based parenting practices and children’s nutritional status (p = 0.032). In conclusion, local wisdom-based parenting practices play an important role in determining children’s nutritional status. Integrating cultural values with balanced nutrition education may improve healthy feeding practices in rural communities.

Abstract

Pediatric tuberculosis remains a major public health concern in Indonesia, particularly due to low case detection rates and diagnostic challenges in children. One key strategy to improve pediatric TB case detection is active case finding (ACF) involving community health workers (cadres). This study aimed to analyze the role of health cadres in ACF implementation on pediatric tuberculosis case detection in Banyuwangi Regency. This study employed a quantitative cross-sectional design with a sample of 60 health cadres selected through stratified random sampling. Data were collected using structured questionnaires to assess cadre roles and secondary data from TB registers to identify pediatric TB cases. Data were analyzed using the Chi-square test with a significance level of p < 0.05. The results showed that most cadres had a good role (41.7%), and pediatric TB cases were identified in 46.7% of their working areas. Statistical analysis revealed a significant association between the role of cadres in ACF and pediatric tuberculosis case detection (p = 0.003). Cadres with better roles demonstrated higher case detection proportions. The role of community health workers in ACF significantly influences pediatric tuberculosis case detection. Strengthening cadre capacity, particularly in early detection and household contact investigation, is essential to improve pediatric TB control efforts.

Abstract

Type 2 diabetes mellitus is a chronic metabolic disorder characterized by elevated blood glucose levels due to impaired insulin function or production, which may lead to complications if not properly managed. This paper aims to examine nursing care in two patients with type 2 diabetes mellitus using a case study method based on a systematic nursing process approach.Data were collected through observation, interviews, physical examinations, and documentation studies to formulate nursing diagnoses, including unstable blood glucose levels, ineffective airway clearance, ineffective peripheral perfusion, and hypervolemia. The implementation results showed significant clinical progress, characterized by the stabilization of the patients' general condition and a decrease in Random Blood Glucose (RBG) levels from an initial 298 mg/dL to 186 mg/dL, accompanied by a reduction in complaints of nausea and fatigue. Furthermore, regarding the issue of ineffective airway clearance, patients demonstrated a better ability to expectorate secretions, reduced coughing frequency, and a decrease in adventitious breath sounds such as crackles (rhonchi). The evaluation of nursing care for these two patients with Type 2 Diabetes Mellitus comorbidities indicated clinical improvement meeting the expected outcomes, evidenced by reduced symptoms, more stable vital signs, better-controlled blood glucose levels, and improved respiratory function and secretion clearance, proving that the nursing care provided was effective in enhancing the patients' health status.
