Abstract

Background: The transformation of maternal and neonatal care encourages midwifery education to develop a curriculum that focuses not only on content but also on achieving graduate competencies that meet global standards and the needs of midwifery practice. Curriculum evaluation is necessary to assess the alignment between learning design, implementation, and the education quality assurance system. Purpose: To systematically review the curriculum model, implementation results, and quality indicators in the evaluation of midwifery education. Method: A systematic literature review was employed, referring to the PRISMA 2020 guidelines. Article searches were conducted through PubMed, Scopus, ScienceDirect, Google Scholar, and Wiley Online Library. Of the 1,390 articles found, 15 studies met the inclusion criteria and were analyzed using narrative synthesis. Methodological quality was independently assessed by two reviewers using study-specific appraisal criteria Results: Competency-based curricula were widely implemented through the integration of simulation, midwifery continuity of care, student-centered learning, and International Confederation of Midwives (ICM) standards. Implementation of these curricula contributed to improved clinical skills, Objective Structured Clinical Examination (OSCE) scores, critical thinking, self-confidence, and student readiness for practice. Curriculum quality is influenced by alignment with standards, faculty competency, clinical facilities, supervision, and institutional quality assurance. Conclusion: Competency-based curriculum evaluation plays an important role in improving the quality of midwifery education and graduate readiness.

Abstract

Background: Miscarriage (recorded as “abortion” in hospital medical records) is a major health issue among pregnant women and can contribute to maternal morbidity and mortality. Hemorrhage resulting from miscarriage complications is a primary factor in maternal deaths. Purpose: To analyze the determinants of miscarriage among pregnant women at Haji Hospital, Medan City. Method: A quantitative approach with an observational, cross-sectional design was employed. The study population consisted of all pregnant women registered in the medical records at Haji Hospital, Medan City, in 2026. A sample of 64 pregnant women was selected based on inclusion and exclusion criteria, and data were analyzed using the Chi-Square test. Results: The results showed a p-value of 0.004 (p < 0.05); thus, the alternative hypothesis (Ha) was accepted and the null hypothesis (Ho) was rejected. This indicates a significant relationship of age on the occurrence of miscarriage among pregnant women at Haji Hospital, Medan City, North Sumatra. Conclusion: There is a significant relationship regarding the determinants of miscarriage among pregnant women at Haji Hospital, Medan City, North Sumatra Province.

Abstract

Background: Hypertension is a non-communicable disease that requires long-term management. Effective hypertension control involves not only pharmacological treatment but also self-care management practices, including medication adherence, physical activity, dietary regulation, blood pressure monitoring, and stress management. Family support is considered an important factor that may influence patients’ ability to perform self-care behaviors. Purpose: To determine the relationship between family support and self-care management among patients with hypertension. Method: A quantitative correlational design with a cross-sectional approach. The study population consisted of patients with hypertension at the Baros Public Health Center, Sukabumi. A total sampling technique was applied, involving 86 respondents. Data were collected using questionnaires measuring family support and self-care management. Data analysis was performed using the Spearman’s rho correlation test. Results: The majority of respondents received good family support (n=69; 80.2%) and demonstrated good self-care management (n=64; 74.4%). The Spearman’s rho correlation test showed a statistically significant relationship between family support and self-care management among patients with hypertension (p < 0.001; r = 0.878). Conclusion: Family support is significantly associated with self-care management among patients with hypertension. Strengthening family involvement through health education and hypertension management programs may support patients in improving self-care practices and maintaining better disease control.

Abstract

Background: Patients with chronic kidney disease (CKD) undergoing hemodialysis often experience anxiety due to long-term therapy, activity restrictions, and concerns about complications. The fun dialysis program is a nonpharmacological intervention that combines humor therapy, interactive games, and rewards to create a more enjoyable hemodialysis environment. Purpose: To analyze the effect of the fun dialysis program on anxiety levels among CKD patients undergoing hemodialysis. Method: A quasi-experimental study with a pretest-posttest control group design was conducted on 66 hemodialysis patients, consisting of 33 in the intervention group and 33 in the control group. The intervention group participated in the fun dialysis program for 8 cycles (4 weeks), while the control group received routine care. Anxiety levels were measured using the State-Trait Anxiety Inventory (STAI) and analyzed using the Mann–Whitney test. Results: Severe anxiety in the intervention group decreased from 42.4% to 18.2%, while mild anxiety increased from 42.4% to 60.6%. In the control group, severe anxiety increased from 27.3% to 33.3%. There was a significant difference between the two groups (p = 0.018). Conclusion: The fun dialysis program is effective in reducing anxiety among chronic kidney disease patients undergoing hemodialysis and has the potential to be implemented as a safe, low-cost, nonpharmacological nursing intervention.

Abstract

Background: Maternal mortality resulting from obstetric complications, including hemorrhage due to placenta previa, remains a significant challenge in achieving the SDGs. Various maternal risk factors are believed to influence the occurrence of this placental implantation malposition. Purpose: To determine the relationship between maternal age, parity, history of cesarean delivery, history of curettage. Method: This cross-sectional study used secondary medical record data from 39 pregnant women at Medan Haji Hospital, North Sumatra, during 2021–2025. Total sampling was applied. Maternal age, parity, cesarean section history, and curettage history were analyzed for their association with placenta previa using the Chi-square or Fisher’s exact test at α = 0.05. Results: Four maternal variables demonstrated a statistically significant association with placenta previa incidence: Maternal Age: p = 0.003, Parity: p = 0.001, Prior Cesarean Section: p = 0.002, Prior Curettage: p = 0.003. Conclusion: Advanced maternal age, high parity, and a history of uterine procedures (cesarean section or curettage) are major risk factors for placenta previa, likely due to uterine structural changes affecting normal implantation. Healthcare facilities should implement targeted early screening and enhanced prenatal monitoring for patients presenting with these risk factors.

Abstract

Background: Acute Myeloid Leukemia (AML) in children may be accompanied by severe complications, including hyperleukocytosis, Tumor Lysis Syndrome (TLS), and Acute Kidney Injury (AKI), which increase the risk of impaired tissue perfusion, multiorgan dysfunction, and mortality. This combination of complications is rarely reported and requires intensive nursing monitoring to facilitate early detection of clinical deterioration. Purpose: This case report aims to describe the clinical course and nursing management using a Fluid Perfusion Monitoring Bundle and to formulate monitoring recommendations for similar pediatric onco-hematologic emergencies. Case Study: A 15-year-old boy was referred with hyperleukocytosis and suspected acute leukemia following progressive weakness, fever, decreased appetite, and weight loss. Laboratory findings revealed a leukocyte count of 118,000/µL, hemoglobin of 5.2 g/dL, platelet count of 87,000/µL, and 74% peripheral blasts, accompanied by hyperuricemia, electrolyte disturbances, and elevated urea and creatinine levels of 194 mg/dL and 4.84 mg/dL, respectively. Immunophenotyping confirmed AML. The patient was diagnosed with AML complicated by hyperleukocytosis, spontaneous TLS, stage 3 AKI, and suspected leukostasis, requiring intensive care in the Pediatric Intensive Care Unit (PICU). Discussion: Impaired peripheral tissue perfusion was identified as the priority nursing problem due to hyperleukocytosis and leukostasis, further aggravated by severe anemia, TLS, and stage 3 AKI. The Fluid Perfusion Monitoring Bundle, comprising systematic monitoring of hemodynamic status, peripheral perfusion, oxygenation, fluid balance, urine output, renal function, and electrolytes, facilitated early detection of clinical deterioration and supported timely nursing and collaborative interventions. Conclusion: The Fluid Perfusion Monitoring Bundle supports structured monitoring and early detection of changes in tissue perfusion, fluid balance, and renal function in children with AML, TLS, and stage 3 AKI, thereby contributing to optimized nursing care and the prevention of clinical deterioration.
