Abstract

THE EFFECT OF IRON PILL SUPPLEMENTATION ADDED WITH VITAMIN E (ALPHA-TOCOPHEROL) TO HEMOGLOBIN AND IRON STATUS OF CHILDBEARING AGE WOMEN SUSPECTED SUFFERING FROM CARRIER THALASSEMIA.Background: Anemia control programme among pregnant women had been conducting for about three decades, but prevalence of anemia still considered high. It was reported by Nutrition and Food Research Centre (2000) that 37.4% of childbearing women indicated fragility of red blood cell membrane wich assumed related to anemia. One of the vitamins which have positive effect to the wall of red blood cells is vitamin E through its rolle as antioxidant. It was recommended to conduct a study to determine the effect of supplementation of iron pill, "Ferro sulfate" (60 mg elemental iron with 25 mg folic acid) added with vitamin E (20 mg a-tocopherol) to hemoglobin level and iron status of childbearing age of women.Methods: The study design was "Intervention with quacy experiment trial".  The study sites covered 10 villages in Cianjur district of West Java. A total of 210 childbearing age women (19-45 years old) with indication of fragility of red blood cell membrane which was determined by procedure of NESTROF (Naked Eye Single Test Tube Red Cell Osmotic Fragility), divided into two groups (105 women/group). Group I received one iron pill daily and vitamin E twice a week for 10 weeks. While group II received Iron pill only, without vitamin E. Data collection was conducted before and after intervention.Results: After 10 weeks intervention, the average of Hb increament of group I (0.4 g/dl) was slightly higher than that of group II (0.3 g/dl). There was improvement of serum ferritin of both groups with increament 4.3 mg/dl for group I and 7.2 mg/dl for group II. Vitamin E status of both groups still considered low: 3.1 mg/L for group I and 3.1 mg/L for group II. Normal value of serum vitamin E is 5.0 mg/L.Conclusions: Vitamin E (20 mg of a-tocopherol) which was given twice a week bisides daily supplementation of iron pill (60 mg elemental iron of sulfas ferrosus + 25 mg folic acid) for 10 weeks increased the effectiveness of Hb synthesis 1.5 times than that of supplementation of iron only without vitamin E.Keywords: iron supplementation, Hb level, iron status, vitamin E status, NESTROF, fragility of cells membrane

Abstract

EFFECT OF MATERNAL NUTRITION STATUS DURING PREGNANCY AND LACTATION TO SUCCESSFULLY BREASTFEEDING.Background: Infant should be only got breast milk in first six-months, but most of mothers do not successfully breastfed their infant because infant consumed low energy from breast milk. Maternal nutrition status during pregnancy and lactation may contribute to successful breastfeeding. During lactation, there is mobilization of fat to produce breast milk.Objective: The objective of this study is to evaluate contribution of maternal nutrition status before pregnancy, during pregnancy and lactation to successful breastfeedingMethods: In the first four-month of life, if infant consumed energy from breast milk ≤ 65% of RDA categorized as not successful breastfeeding, and if infant consumed energy from breast milk > 65% of RDA categorized as successful breastfeeding.Results: The result shows that maternal nutrition status during pregnancies doesn't contribute to successful breastfeeding. Maternal nutrition status during lactation significantly contributes to successful breastfeeding after controlling other factors (mother factors, infant factors, food pattern and social economics factors). During lactation, the risk of thin mothers (IMT < 18.5) to unsuccessful breastfeeding is 2.26 to 2.56 times compared to normal mothers (IMT ≥ 18.5-25.0) (RR = 2.26 to RR = 2.56).Conclusion: Mother should consume more energy and protein to achieve successful breastfeedingKeywords: successfully breastfeeding

Abstract

VITAMIN A DEFICIENCY AND ITS RELATED FACTORS IN INFANTS IN BOGOR DISTRICT.Background: Based on clinical indicators, vitamin A deficiency in Indonesia is not a public health problem because the prevalence of xerophthalmia has been decreased to 0.34%. But, this decrease has not been followed by a decrease of marginal deficiency of vitamin A in vulnerable groups, especially infants.Methods: The cross-sectional baseline study was conducted at Bogor District. The aim of the study was to collect information about the vitamin A status of the breastfed infant, and to look for a relationship between many factors related to it. The samples were 183 breastfed infants aged 2-0 months without chronic disease, congenital disease, severe PEM nor twins. The chi-square and the prevalence odds ratio (POR) at the 95% confidence interval were used to measure the association between variables. Multiple logistic regression analysis was used to measure the closest factors to infant's vitamin A status.Results: The study showed that 54.1% of breastfed infants were at risk of vitamin A deficiency. Based on bivariate analysis, there are two significant independent variables related to infants vitamin A status, which are maternal vitamin A status and infant infection status. Multiple logistic regression analysis showed that infection status is the closest factor to vitamin A deficiency in infants, followed by maternal vitamin A status and breast feeding frequency. There are no significant association between supplementary feeding, age, nutritional status and the infant's vitamin A status.Conclusions: Marginal vitamin A deficiency in infants aged 2-10 months is still a public health problem in the research area. The infection status is the closest factor to vitamin A deficiency in infants, followed by maternal vitamin A status and breast feeding frequency.Keywords: vitamin A deficiency, breast feeding, infection, complementary feeding, infant

Abstract

POSITIVE FACTORS TO INCREASE THE POTENTIAL OF CADRE'S INTEGRATED SERVICES POST IN ORDER TO ACHIEVE THE NUTRITION AWARENESS FAMILY.Background: One of indicators of the nutrition awareness family (KADARZI) is family member's attendance for health and growth monitoring to POSYANDU, especially children aged 1-5 years (8ALITA) and pregnant women. The successful of BALITA growth monitoring depends on the cadre ability to conduct the POSYANDU activities, among others.Objectives: The objective of this study is to describe the positive factors related to POSYANDU cadre ability to increase the family nutrition awareness regarding BALITA growth monitoring.Methods: Cross­ sectional survey was used in this study. The study was conducted in 16 POSYANDU of the Kampar and Pelalawan Districts, Riau Province, and covered 58 cadres as respondents. Interview with respondents was used to collect data using a questionnaire.Results: All cadres are women and most of them are 21-40 of age about 50 percent that having high school education, and almost all of respondents work as housekeeper. Motivation to be cadres is mainly for helping the community member or village development. About 53 percent of cadres had never received training, and the rest of cadres had received training only once although they have been a cadre for more than two years. Besides, most of the POSYANDU has no materials or booklets for community education. About 71 percent cadres have no booklet and about 52 percent have never given information to the community because they have no knowledge in nutrition counseling. Cadre knowledge in growth curve interpretation in the growth chart is very low; it was only nine percent of cadre that able to interpret growth curve correctly. Only two percent cadre knows that the weight change in monthly weighing should be told to mother in nutrition counseling. In order to increase nutrition awareness family, particularly in growth monitoring, the POSYANDU should be revitailzed by improving the cadre ability in growth graph interpretation as well as growth monitoring, nutrition counseling, and provide means of communication in POSYANDU activities.Keywords: POSYANDU cadre, nutrition awareness family, POSYANDU revitalization

Abstract

DEVELOPMENT OF NUTRITIONAL MESSAGES IN THE DIETARY GUIDELINES FOR INDONESIANS THAT MORE PRACTICAL TO BE USED BY FIELD NUTRITION STAFFS.Background: Nutritional messages in the Dietary Guidelines should be simple and relatively few in numbers to obtain maximum awareness and understanding among the users. Some observational studies indicated that nutritional messages of the Dietary Guidelines for Indonesians (PUGS) were considered complex so they were difficult for 'field' nutrition staffs to understand them.Objectives: The purpose of this study is to explore and modify the nutritional messages that are considered complex by 'field' nutrition staffs to become the understandable and practical nutritional messages.Methods: A capacity building method has been developed to explore and modify the nutritional messages that are considered complex. This process involved seven participants: four 'field' nutrition staffs (two PUSKESMAS nutrition staffs from Medan City and two from Deli Serdang District) and three nutrition staffs from the Health Office of Medan City and Deli Serdang District, North Sumatera Province. Nutritional messages were modified based on the inputs of capacity building forum.Results: The nutritional messages of number 2-3-4 in PUGS were difficult messages to be understood by nutrition staffs. The modified 2-3-4 messages by nutrition staffs were still somewhat difficult for them. The final result was the ten modified nutritional messages which were, according to nutrition staffs, more understandable and practical.Keywords: Nutritional messages, Dietary Guidelines for Indonesians (PUGS), nutrition staffs
