Abstract

Introduction: Burns serve as one of the most common devastating forms of trauma. Significant thermal injuries induce skin disruption and immunosupression that predispose patients to infectious complication. The importance of sampling, detection, and proper treatment for wound infections will significantly increase the likelihood of clinical improvement of the patients. Case: A 44-years-old male came to hospital due to electrical shock. There were serious burns to his face, chest, arms, and legs, some of which were second- and third-degree burns. The patient was conscious, without problems on airway and breathing. Leukocytosis with neutrophilia was presented from laboratory examination, with an increase in liver enzymes and serum creatinine. The gram-positive cocci with leucocytes were revealed from gram examination, and colonization of Staphylococcus aureus was found. Intravenous cefazolin was administered according to the sensitivity test. Discussion: Infection causes the increase in morbidity and mortality of burn patients. Clinical diagnosis of infection must be supported microbiologically by gram examination and culture. One of the most common infectious agents in severe burns is Staphylococcus aureus. The significance of S.aureus as the leading cause of infection must be supported by microbiological data. Dominant growth in culture media and induction of host inflammatory response were indicated as significance of infection. Conclusion: Clinical and microbiological diagnosis of burns infection play important role to prevent complications in severe degree of burns.

Abstract

Introduction: To this day, COVID-19 still cause high mortality and morbidity in most cases. The severity of COVID-19 can be detected by simple tests such as complete blood counts which available in most health facilities. Through early detection of the estimated severity of COVID-19, we may determine which patients need hospitalization or patients sufficient with self-isolation. Methods: We conduct an analytical, cross-sectional, retrospective study that analyzed the hematological parameters of 97 patients with positive rRT-PCR swabs in the period of March to August 2020. Hematologic parameters of COVID 19 patients with mild and moderate-severe symptoms were compared. The receiver operating characteristic (ROC) curve, area under the curve (AUC), and Youden's index were created to determine the optimal cut-off value in predicting the degree of severity of COVID-19 patients. Results: Statistically significant differences (p

Abstract

Background: Neonatal sepsis is increasingly recognized as an important global health problem that challenges neonatal survival. In 2018, sepsis caused approximately 15% of all neonatal deaths worldwide. Recent data regarding preterm birth and low Apgar score as risk factors for neonatal sepsis-related death have not been reported in Indonesia. Methods: This was a case-control study conducted in Dr.Soetomo General Hospital. A case group was obtained from medical records by a total sampling of all neonates diagnosed with neonatal sepsis in 2019, and a control group of non-neonatal sepsis cases was taken by random sampling. Chi-square test and logistic regression were used to analyze the data. Results: Statistical analysis showed a significant correlation between one and five-minute Apgar score

Abstract

Coronavirus disease 2019 (COVID-19) pandemic era provides a new habit in form of self-quarantine or physical distancing. The recommendation is to delay visits to health facilities if it is not in the emergency condition to slow down the spread of COVID-19 transmission. This study aims to develop a telemedicine system that integrates three main components, namely doctors, patients, and hospital management in Gotong Royong Hospital Surabaya to minimize the transmission of COVID-19 while still provides comprehensive health services. The development of telemedicine service is divided into 3 stages. In the first stage, emergency screening was carried out by developing Frequently Asked Questions (FAQs). The second stage was continued with prototype testing and the last part was improving the system to create a better referral system. Gotong Royong Hospital telemedicine has been active since June 2021, within 2 months, 45 patients are willing to use telemedicine services. The results showed that telemedicine was easily accepted by patients in internal medicine clinic services (p

Abstract

Indonesia had the lowest position in cognitive skills and educational achievement according to the research of the Economist Intelligence Unit in 2014. Cognitive skills are the brain abilities to reach knowledges and understanding from experiences and information. Parts of cognitive domain are attention and concentration. Attention is the ability to focus on problems. Concentration is the ability to maintain the attention in the longer periode of time. The availibility of adequate nutrition can be achieved by consuming good and various foods, especially at the breakfast. This research aimed to study the difference of attention and concentration abilities between students who had breakfast and those who didn’t in UPTD SMA Negeri 2 Nganjuk. The subjective assessment used questionnaire and the objective assessment used Moca-Ina assessment. This was an analytic research with cross sectional research design applied to 42 eleventh grade students in UPTD SMA Negeri 2 Nganjuk. The sampling technique was Simple Random Sampling by distributing the questionnaire and performing Moca-Ina assessment grade students in UPTD SMA Negeri 2 Nganjuk. The data were processed with version 20 SPSS. The result showed that there were significant differences of the attention and subjective concentration abilities between students who had breakfast and students who didn’t have, based on the breakfast menus (p=0.012), breakfast calories (p=0.012), and breakfast time between who had breakfast at 06.00-06.59 AM and who didn’t have or had breakfast before 06.00 AM (p=0.004). Also, there were significant differences ot attention and objective concentration abilities among students who had breakfast and students who didn’t have (p=0.035) based on Moca-Ina assessment. The researcher could conclude that subjectively and objectively there were significant differences of attention and objective concentration abilities between students who had breakfast and those who didn’t. Respondents who had breakfast had better attention and concentration abilities than the ones who didn’t have.

Abstract

Introduction: Breaking the chain of transmission of infection using an antiseptic solution is an easy, inexpensive and effective method. The incidence of fungal infection is one of the global health problems that lead to severe complications even death, especially in patients with immunosuppressed or immunocompromised conditions and is reported to be increasing. Although alcohol-based antiseptic solutions are frequently used to prevent transmission of pathogenic organisms, these methods are rarely evaluated. Aim: Determine the susceptibility of three pathogenic fungi against alcohol-based antiseptic solutions with and without hydrogen peroxide addition. Methods: To determine the inhibition of the antiseptic solution against fungi, the Kirby & Bauer test disc diffusion method was used. After the petri dishes were incubated at 37° C for 24 hours, the diameter of the inhibition zones were measured using a caliper. The phenol coefficient test was carried out to compare the efficacy of an antimicrobial agent in this case alcohol-based antiseptic solutions against phenol. The phenol coefficient was applied as a test against Salmonella typhi, with a minimum score of 1, using the method according to SNI 1842: 2019. Result: The inhibition zone on Candida albicans ATCC 102231, Aspergillus niger, and Cryptococcus neoformans ATCC 14116 for solution A contains ethanol 80% v / v, glycerin 8% v / v, sterile water add 100% and solution B contains ethanol 80% v / v, H2O2 0.15% v / v, glycerin 8% v / v, add 100 sterile water % all were less than 6 mm which were classified as resistant. The result of the phenol coefficient test for solution A and B were 0.3 and 0.4 , which less than 1, indicated the antiseptic solutions were less effective than phenol. Conclusion: Antiseptic ethanol solution and a combination of ethanol + H2O2 were ineffective inhibits of pathogenic fungal growth. Further studies are needed to form a more potent antiseptic solution in order to improve management of fungal infections prevention.

Abstract

Introduction: Reperfusion injury is a well-known phenomenon that occurred in cardioplegic techniques with cardiopulmonary bypass. Therefore great effort is made to prevent reperfusion injury. Beating Heart Continuous Coronary Perfusion (BHCCP) surgery is one of the alternative techniques to improve an ischemic reperfusion injury in open-heart surgery, either in pediatric or in adult. This technique can be done in complete repair of tetralogy of Fallot (TOF). Purpose: To report our first experience in doing complete repair of TOF using beating heart technique without aortic cross-clamping Case Report: A five-year-old boy came on to ER on January 6th, 2012, with a history of cyanotic since birth. His echocardiography and catheterization concluded a tetralogy of Fallot with a McGoon ratio of 1.95 (the diameter of RPA and LPA were 9.14 mm and 8.79 mm, respectively). A complete repair of TOF was done using beating heart surgery without aortic cross-clamping technique. One month post-surgery evaluation, there weren't any neurological or motor disorders. Echocardiography results showed no residual pulmonary stenosis, but 0.2 mm residual VSD was found. Conclusion: This heart rate technique is an alternative method of myocardial protection in cardiac surgery. In several cardiac centres globally, this technique is relatively safe and can be used during surgery, especially in the total correction of TOF.
